- #11588864May 15, 2024LANE DEPARTURE: BLIND SPOT DETECTION, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, UNKNOWN OR OTHER
1. My 90 year-old mother-in law got trapped inside the car in very bad weather because my phone key wouldn't open the door after I got out and walked her elderly friend to the door of her house about 20 feet away, and then walked back to the Tesla. I've been able to reproduce the problem repeatedly, of course without my mother-in-law in the car. 2. Several near-misses because, unlike most other cars, the Tesla's blind-spot warning system, and many other basic control functions such as adjusting the windshield wipers, opening the glove compartment, etc, require you to take your eyes off the road. For example changing lanes requires you to look away from the lane you're about to move into to check the blind spot display on the touch screen. 3. Several near misses because easing up on the accelerator slows down the car forcefully 98% of the time, but does nothing 2% of the time. Specifically, accidentally flicking down on the right-hand control stick, which turns on the windshield wipers in most cars, activates cruise control and gives minimal feedback to the driver that cruise control has indeed been activated. In this circumstance, easing up on the accelerator pedal doesn't slow down the car at all. This is very frightening and unexpected and could easily cause an accident.

The left and right turn signal indicator buttons gets stuck sometimes. But I noticed on my vehicle it is the left turn indicator button is the one gets stuck most. When it is stuck I cannot make left turn signal. Before I asked Tesla to fix it the issue happens every time on my 8 mile trip. After Tesla attempted to fix it it happens a lot less now but this could mean it can go worse as the component wears out. This can put me and others at risk as sometimes I get panicked and depending on the situation.

The Tesla model 3 from the factory does not have the option to get the trailer mode safety software. Aftermarket suppliers make and sell class 2/3 hitches for the model 3. And while the model 3 is not rated for towing. People still have the tow hitches installed and they tow trailers. The model Y with the tow package has what is called “trailer mode” which enables safety features for towing that allows the vehicle to do several things for safety while towing: - it adjusts the wheel braking and speed stability. - It routs to “pull through” charging stations for safe charging without having to unnecessarily and repeatedly unhook trailers. - auto steer, automatic steering, emergency braking, and following distance is all adjusted to account for the trailer and its weight. - light check, checking for functional trailer lighting. The Model Y that does not get the factory tow hitch does not get this tow package software. If you get the tow hitch added afterwards on the model Y or aftermarket, Tesla does not provide this safety enhanced software. The model 3 does not get this safety software regardless. And yet people are able to install after market tow hitches I believe Tesla should be required to offer the towing software to the Model Y base and Model 3 regardless if the vehicle it tow rated or not because the safety of the vehicle when towing is a necessity. I don’t think it should matter if they charge money or offer it free so long as you can have this trailer mode software option added for safety. Because people are going to tow with their cars. A lot of people don’t follow manufacturer recommendations. And U-Haul trailer rental among many other providers offers tow hitch installations and rents trailers to people with Tesla’s. So the easiest thing is to require Tesla to provide this safety enhancement to every vehicle and not just the model Y “if it gets the tow package from the factory.”

- #11588047May 10, 2024VEHICLE SPEED CONTROL, LANE DEPARTURE: ASSIST
After the recent 1-month free trial of FSD, my basic autopilot does not keep speed and will reset the programmed speed. This causes my car to slow rapidly and unexpectedly. This erratic change of speed creates a safety issue on the road due to the car slowing with no warning and without reason. Tesla has evaluated the vehicle remotely and has said it is performing normally despite this behavior just starting with the FSD trial. The previous 2 years of ownership have had no issues. I am told that this is due to different levels of autopilot having different performance and if I upgrade to the FSD this won't be an issue. No warning lamps are present.

- #11587341May 6, 2024VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
When using cruise control, my 2023 Tesla Model 3 will aggressively brake without warning or apparent cause. The braking is near maximal - as though the vehicle is attempting to prevent an imminent collision. This defect poses a hazard to occupants of vehicles behind mine as well as myself and my passengers as sudden braking could cause a rear-end collision and cascading loss of control. Inclement weather would have exacerbated the problem. This problem is documented. From my brief research it appears Phantom Braking has been the subject of a NHTSA investigation previously but for prior model years. The issue persists. When I questioned Tesla Service regarding this life and limb threatening problem their staff was aware and well versed at requesting the customer report the issue as though it were a minor software glitch. This is a significantly more serious problem necessitating an appropriate response.

My Tesla sets off false front collision warnings multiple times a month, resulting in distraction, but more importantly, it increases my auto insurance rate through Tesla Insurance. I am not sure if Tesla is working on this "bug" or if it is intentional to keep driver safety scores below a certain threshold. I have video proof from the car itself indicating that there was no imminent collision threat.

I was driving behind someone when a suv came over from left lane into the right lane in front of the cars that were in Front of me and made a sudden turn into a business. I was already applying the brake and was the 3 second rule behind the car in front of me, however the car did not stop as I slammed on the brakes as hard as I could. There have been other times that the car will automatically slammed brakes on because it feels that it need to…when it’s not necessary at all. But the other day when I needed the brakes to help me stop abruptly…they failed to do so. I hit the car in front of me and was cited as well. If I had been going faster..this could have been way worse. This definitely needs to be evaluated on Teslas. I have a dahcam video that I’m unable to upload.

I have my phone mounted to the back of the computer screen, and I also have the phone power cord running from the center console 12V plug up to my phone. I have found that if the power cord comes in contact with the computer screen, it can inadvertently put the car in Neutral (it acts like a finger touch). This could be extremely dangerous if, for example, the car is making a left turn in front of oncoming traffic and the car unexpectedly goes into Neutral. I think consumers should be aware that a phone power cord can cause changes on the computer screen if there is contact, especially if it switches the car to Neutral.

- #11586244May 1, 2024SERVICE BRAKES, VISIBILITY/WIPER, FORWARD COLLISION AVOIDANCE: WARNINGS
Tesla ADAS detects non existent objects and issues collision alerts and phantom brakes randomly if there is no cars in front of the Tesla, braking abruptly and almost getting me into rear end accidents. Tesla ADAS freaks out when making lane changes randomly, shaking violently and giving the driver and its passengers a fishtailing simulation. Auto wiper triggers randomly while it's sunny, distracting the driver and forcing an interaction on the center console to stop it. Auto wiper cannot be turned off as it reengages upon using Tesla ADAS. The rear view camera lags. It stutters and shows me a delayed view. Compounded by the Model 3's very small rear view mirrors and side mirrors, this is incredibly dangerous as I have no way of gaining sufficient visibility of the back of the car. Tesla service told me this is all normal.

The deemed to be totaled/flooded from driving through a rainstorm on 03/09/2024. Luckily the car did not shut off until about 3 hours later of being parked where the rain completely turned off the car. The car was taken to a dealership service center but deemed unable to fix due to the liquid reaching the electrical wires. Tesla insurance has been notified of this incident and as of 2 months later, deemed to take any action besides totaling the car. There has been issues in my service records for this car where water was leaking in and the parts were replaced free of charge but not to this manner. There was no warning or lamps regarding liquid being in the car or anything. Cannot get in touch with anyone at Tesla or Tesla insurance except for Tesla emergency roadside where they were unable to assist or reach anyone from Tesla / Tesla insurance in this manner.

The accelerator pedal broke off completely while driving on the highway (this is the second time this has happened). Accelerator pedal felt as if it were stuck when going from full self driving to manual and pedal broke off when more pressure was applied. The car began coming to a rapid stop on the highway due to regenerative braking and I was forced to quickly get to emergency lane, narrowly avoiding other vehicles in the process.

I have a 2021 Tesla Model 3. The current owners manual shows how to open the rear doors in the event of loss of power using an emergency latch at the bottom of the door pocket. Our car doesn’t have that latch in the rear doors (though the front doors each have one). This concerns us and we feel it is unsafe for the back seat passengers.

- #11585444April 26, 2024FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The adaptive cruise control of the Tesla Model 3, like other Tesla models, rely on the cameras for determining speed. While driving on the freeway the Tesla Model 3 began to abruptly break almost causing a car to rear-end me. This has occurred multiple times since the full self driving was released to everyone. I discovered that the software reads the freeway's "Minimum Speed Limit" signs as the maximum speed limit. I reached out to Tesla and was told that there would be a firmware up-date but one has not come out since.

While the car was on the full self-driving it hit a metal block on the road. After the accident there was leakage behind the left front wheel of blue liquid and was suspected to be battery coolant. The car was not drivable due to safety concern. The car functions were degraded and air conditioning was not working. Sensors were on and off. The self-driving could not identify block and did not slow down. The self-driving system has major defect which is not safe to use at all.

- #11584446April 22, 2024FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
While using the Full Self Driving Mode (FSD) the vehicle will randomly and without warning slam on the brakes at highway speeds and decelerate to 30-50mph on an active highway. If the highway is multilane, the car will slow and attempt to change lanes. It appears the vehicle thinks that something is in the road, even if the road is clear. This happens multiple times on my longer road trips, and is very concerning. It did this yesterday with a semi-truck closely behind me. The vehicle has gone into service and I did bring up the issue, but the service team was unable to assist on this issue (screenshots attached). The software currently installed in my vehicle is 2024.3.15, the latest available from Tesla. This issues has been ongoing from every software update I have had in the car however.

Right turn signal gets stuck and doesn't allow me to press it. It is very dangerous when it happens out on the road. First attempt to fix it was April 5th 2024. According to Tesla service invoice the steering wheel was replaced and software was updated. As of April 19 2024 the problem has returned. As I attempted to make a right turn and couldn't signal. I also have videos showing the issue I am having. After reviewing online forums it seems this is a common problem for 2024 Tesla Model 3 which is extremely dangerous.
