At random the following would come on when driving or when starting the vehicle up. It would also reset itself at random. Very scary when you have small children in the van. 1.Collision Mitigation Braking System (CMBS) 2.Road Departure Mitigation System (RDM) 3.Lane Keeping Assist System (LKAS) 4.Adaptive Cruise Control (ACC) 5. Auto head lights The vehicle has not been in any collision. This started after a required update was installed. The code reader scan code does not match what is being stated. When you are using the cruise control then it would turn off. Brake warning will not work. The cable harness for the audio for the entire van was redone last Summer 2023

I am writing to urgently bring to your attention a critical issue plaguing the driver assist systems in Honda Odyssey 2018+ vehicles. Despite meticulous maintenance and adherence to manufacturer guidelines, I have encountered a persistent and alarming problem that appears to affect numerous other vehicle owners, as evidenced by online forums and discussions. The issue initially arose during a rainstorm, where I received a warning message stating, "Some Driver Assist Systems Cannot Operate: Radar Obstructed." Despite the weather clearing and thorough cleaning of both the sensors and the windshield camera, the warning persisted. Subsequently, a series of related problems emerged, including: Illumination of the BRAKE SYSTEM LIGHT Malfunctioning adaptive cruise control Faulty collision mitigation system Inoperative road departure mitigation system These errors not only pose a significant safety risk to myself and others on the road but also indicate a systemic issue with the vehicle's safety systems. As my vehicle is just past the three-year mark and out of warranty, I am apprehensive about incurring substantial costs for dealership repairs, especially considering the widespread nature of these complaints among Honda Odyssey owners. Extensive research reveals that I am not alone in experiencing these issues. Multiple reports and discussions online highlight similar complaints from other Honda Odyssey 2018+ owners, particularly following exposure to adverse weather conditions. Therefore, I implore the National Highway Traffic Safety Administration to conduct a thorough investigation into this matter. It is crucial to identify the underlying cause of these recurring issues and hold the manufacturer accountable for any design flaws or defects contributing to the malfunction of critical safety systems. Furthermore, I urge NHTSA to prioritize public safety and take immediate action to ensure affected vehicles are promptly recalled or repaired to prevent further incident

It is available for inspection on request. Lost blindspot info, lost use of turn indicators, rear door hatch malfunction, several warnings flash up while driving. I was in a busy area and didn't have a way to signal my movements, screen flashing warnings was terrifying, the van felt like it might turn off at any moment. Before the most recent incident, we had brought the van to Honda dealer for repair, for similar issues that occured at start-up, they found water in the power control unit in rear left area of van that caused corrosion and short-circuit. No body damage or alterations to vehicle. Replaced unit and after rainy weather problems recurred at start up and power down, then while driving, they tested the battery and it was almost dead (the battery was only 1 month old) replaced battery. Problems recurred and water found again. significant leak suspected but not found yet as the service department won't evaluate the seals, and told us it's a body-shop task. summary: intermittent left-side passenger door and rear hatch door malfunction, blind spot system warnings/loss, loss of turn indication, brake system warnings, other system warnings, difficulty starting the car and turning the car off, brand new battery drained while the car was parked and turned off. This manufacturing defect should be addressed by Honda, seals should not fail when exposed to regular conditions. Water should not enter the vehicle and short out an important component leading to several malfunctions that could cause loss of important systems and rear-door latch while driving.

NHTSA RECALL NUMBER: [XXX] for fuel pump but they tell me NOT my VIN number. I had to replace this item last January 2023 for $1500.00 I would like my money back. I have filed with Honda to escalate my issue. [XXX] AC has been working off and on for the past year. Runs warm. They are recommending replacing the compressor assembly with o rings, evac and recharge with dye. $2600.00 Never had to replace a compressor with the 05 06 08 09 10's in our family INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11582393April 11, 2024ELECTRICAL SYSTEM, ENGINE, VEHICLE SPEED CONTROL
The spool valve had a complete seal failure leaking oil onto the alternator causing the alternator to fail. As result the entire electrical system failed, shutting off all propulsion of the van. This occurred while the vehicle was in the high speed left lane on [XXX] @ the [XXX] exit. I had to coast the vehicle across two lanes of 75 mph traffic, cars and large tracker trailers. I was passed by several cars and tracker trailer while coasting across the two lanes of traffic as there was no shoulder on the left side of [XXX]. There were no warning lights prior to the event. American Honda Motor Co. issued Service Bulletin [XXX] on [XXX] to Honda Dealers/Service about the potential for the spool valve casket to leak and fail so that Honda could check for this failure and replace the defective spool valve. The service bulletin covers five (5) different Models and for some Models six (6) model years. The part number identified is 15810-R70-A04, Front Rocker Arm Oil Control Valve (Spool Valve). The correction action is to inspect the front rocker arm oil control valve for leaks and, if needed replace it. The possible causes are the gasket on the front rocker arm oil control valve has deteriorated or the gasket groove is out of SPECIFICATION. Service personnel at Jeff Wyler Honda, 949 Burlington Pike Florence KY 41042, admitted to replacing this spool valve on my Honda Odyssey model several times a month. They admitted that the failure of the spool valve seal can leak oil on to the alternator causing the alternator to fail causing a complete loss of the electrical system and propulsion, seen reports of complete engine fires due to the spool valve gasket failure, noted that the the service bulletin was not attached to my vehicle VIN as did Honda of Jackson 1408 U.S. Highway 45 ByPass Jackson TN 38305. Because America Honda Motor Co. failed to attached the service bulletin to my VIN, my vehicle was not inspected properly for this safety defect. The contact owns a 2019 Honda Odyssey. The contact stated that while accelerating from a complete stop, the vehicle failed to respond. The contact stated that upon depressing the accelerator pedal down to the floorboard, the vehicle lurched forward while shifting into gear. There was no warning light illuminated. Additionally, the contact stated while driving at various speeds, the vehicle started losing motive power. The contact stated that upon depressing the accelerator pedal down to the floorboard, the vehicle lurched forward while shifting into gear. The check engine warning light was illuminated. The vehicle was taken to an unknown dealer, where it was determined that the failure was related to the transmission. The vehicle was then taken to a transmission specialist, where a transmission flush was performed; however, the failure persisted. The contact received notification of NHTSA Campaign Number: 23V858000 (Fuel System, Gasoline) which the contact related to the failure; however, the part to do the recall repair was not yet available. An unknown local dealer was contacted. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the failure. The failure mileage was approximately 53,000. Parts distribution disconnect.

* An engine failure occurred due a connecting rod bearing seizure. The engine was replaced at Bryan Honda in Fayetteville NC. Unless the dealer is required to save the parts I don't have them for inspection. * The engine failure occurred on the travel portion of [XXX] stalling the vehicle on the highway. Luckily we were able to move the vehicle off the traveled portion of the highway but it was not safe to be slightly of the road until a tow arrived. * The problem with this type engine is known and subject to recall number [XXX] although Honda says my specific VIN isn't covered when I submitted a claim, possible the recall could be expanded. *The engine and components were inspected at the Honda dealer. *The check engine light started flashing just as the engine lost power. I started the vehicle and it displayed a message on the infotainment system screen that the system had lost power. I believed that this would only affect the radio functionality which I was not concerned about immediately. I placed the vehicle in reverse and I accidentally backed up into a parked car. At no point before the accident did I realize the rear camera did not work and the collision assistance sensors did not function. I believe if the collision avoidance system was working correctly my accident would have been prevented. I have come to depend on the safety features of this vehicle. I know that this is not an issue with my car battery as I recently had a Honda dealer install a new battery prior to this accident and the vehicle started normally.

The car appears to power up on its own even after it has been shut off. I have noticed that the car was on even after I turned it off, so I have been even more diligent about making sure it was turned off. One day I turned off the car. About an hour later, I heard the speakers in my car blaring. When I was finally able to reach the car the sound was off and the car was black. I didn't even get a chance to open the door and turn it off. It just shut down on its own. Yesterday, when I went to start my car, there was a message that the battery was low. As if the car had been on for a while. But when I got in the car it wasn't on. This appears to be a known problem from other Odyssey owners. Specifically I am seeing issue with 2018 models. However mine is a 2023 model.

- #11581985April 9, 2024ELECTRICAL SYSTEM, VEHICLE SPEED CONTROL, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
We bought a used 2020 Honda Odyssey, and a few months after buying this vehicle, the infotainment screen blanked out and the vehicle information screen also blinked out. This occurred at the same time with no warning whatsoever. I just turned on the car, and none of the screens came on. These screens are the ones that show the mph, our mileage, cruise control, etc, and also the screen that shows the back up camera, in car calling, back up alert system. None of that works anymore. We made an appointment to take it to a Honda dealership and a few days later it flickered back on and so we did not keep the appointment. Just a month and a half later, it happened again and this time it has not come back on for over a month. We took it to the dealership and were charged $200 for them to tell us it was something wrong with the Honda computer system and the whole thing would need replaced and we would be charged $1800. This is on a barely 4-year-old vehicle & we did not have the money to replace a defective system. So we have been driving without a mph display. We put our phones up on the dashboard and try a GPS MPH app, but it is wildly inaccurate, and it is a major safety concern because we have no idea what speed we’re going and we have to guess. This is a car with low mileage and barely 4 years old!! We paid a lot of money for the back up camera, in car calling, and extra features like that specifically for the safety of those features. And now we’re being told that basically they can’t find the problem and we have to pay $1800 for those rather high-priority safety features like knowing how fast you’re going. I also have a newly licensed teenage driver and another who will get her temps in 2 months. I can’t even let them drive our vehicle because they certainly don’t have the experience necessary to drive with no speedometer. We live in a state that doesn’t allow handling a phone in any way. This vehicle helped me comply with that & now I must use a phone to show my mph

Prior to this recall, we had noticed an acceleration issue with the car. One day, during our travels, we encountered a severe stall, which could have been a dangerous event. Just as we were grappling with this situation, news of a recall reached us, but unfortunately, the fuel pump required for the fix was unavailable. It was only when the HondaLink app suggested that the dealer could replace the fuel pump that we found a glimmer of hope. Immediately, I secured a reservation for March 15th, 2024. However, upon reaching the dealership, we were informed that the required part was not in stock. Frustrated, I lodged a complaint with Honda, outlining our predicament. Today, on April 4th, 2024, I received a response from them, stating that they would address the issue once the part became available. However, this leaves me in a precarious position. Who will be held accountable if an accident were to occur due to this fault? Neither Honda nor the dealer seems willing to shoulder responsibility. Certainly: In their response, Honda has essentially washed their hands of the matter, stating that it is up to me to pursue further action. They suggested that I could either escalate the issue with the NHTSA or explore legal avenues. Despite this, they have chosen to close my query, leaving me feeling abandoned and unsupported in this ordeal. Feeling increasingly uneasy about driving a car with a known faulty component, I am left with no choice but to consider reporting this matter to the NHTSA for serious intervention. I implore Honda to take decisive action, whether it involves replacing the vehicle or compensating for the trauma we've endured during this time. Each time I get behind the wheel of this car, I am acutely aware of the risk posed by the defective fuel pump. It is imperative that this issue is addressed promptly and responsibly.

The passenger side second row seat will not lock. It has been diagnosed by a Honda dealer that the seat back frame part number 81326-TK-A22 needs replaced. A recall has been made for the drivers side second row seat in bulletin 18-026. The part number is 81726-TK8-A22. It is the seat frame as well. Honda said they would assist me in paying 1/3 of the bill, but I was responsible for the remainder of the balance. But I see it that there was a seat frame issue with the drivers side second row seat and now I also have a seat frame malfunction on the passenger side second row. I believe they should cover the passenger side as well because of it being an important safety issue.
