On 15-Jan-26 (approximately 5 weeks after the vehicle was serviced for the NHTSA #25V274) while doing highway speeds the vehicle suddenly started huffing and loosing propulsion. After the dealership did their initial inspection found that lifters in at least one bank were damaged. The suddenness, the randomness and severity of this unprovoked failure (I wasn’t towing anything or going uphill) combined by the fact that the dealership previously gave this vehicle a clean bill of health makes me have no confidence in the road worthiness of this vehicle.

This report documents two separate safety incidents involving the same vehicle that occurred on different dates. Approximately one month prior to the most recent incident, while driving my young children to school, the vehicle experienced a sudden and severe loss of braking effectiveness while in motion. I was unable to stop normally at stop signs and had to roll through and roll to a stop. I was required to press the brake pedal fully to the floor to slow the vehicle, and even then braking response was significantly reduced and not functioning as designed. The engine remained running and the vehicle could still accelerate. After stopping and restarting the vehicle, braking function temporarily returned. On a later date, while again transporting my children, the vehicle experienced a complete loss of power while driving. The vehicle abruptly shut down while in motion. I was unable to accelerate and experienced loss of power assisted braking. The vehicle only came to a stop after coasting. Following this shutdown, a burning or melted plastic odor was present. The vehicle was later towed and inspected by a dealership. The battery was replaced; however, this did not resolve the issue. After the battery replacement, the vehicle later hesitated to start again. The exact cause of these failures remains unknown. These incidents occurred without warning and represent serious safety critical failures involving braking and propulsion systems. Both incidents placed my children, myself, and other motorists at risk. Had either event occurred on a highway or heavily trafficked roadway, there would have been little to no ability to safely slow or stop the vehicle, creating a significant risk of collision or serious injury. Due to these events, I no longer feel safe operating this vehicle and have stopped driving it. The vehicle remains available for inspection.

In 2024, while the vehicle was still under warranty, we brought it to our GM dealer due to an air suspension fault. The problem typically occurs in extreme cold weather, and following that visit, we did not experience further issues during the remainder of that winter as it was never that cold again. However, the following winter the same issue returned and was diagnosed yet again, and this year we are again experiencing the identical problem with the same diagnostic code which has been diagnosed by 2 dealers and a private mechanic in the last week. It has since become clear that the underlying issue was never properly repaired during the 2024 warranty visit. We recently learned that approximately 3.5 weeks after our 2024 service appointment, GM released a service bulletin outlining the correct repair procedure for this exact issue. We were not notified of this bulletin by the original servicing dealer. This information only came to our attention when we brought the vehicle to a GM dealer last week, where the issue was properly diagnosed and now they are refusing to fix it without us paying $4300 even though it was brought to their attention in 2024 while under warranty. A failed air suspension presents a safety concern, not merely a ride-comfort issue. When the system fails, the vehicle may sit unevenly or at an incorrect ride height, which can affect vehicle stability, steering response, braking geometry, and traction—particularly in cold weather and at highway speeds. Because the failure is intermittent and temperature-related, it is unpredictable and increases the risk of loss of control. The vehicle is not operating as designed when the air suspension is compromised. GM knows about this issue as they did indeed issue a bulletin that covers our vehicle and this was done when my car was still under warranty, but it was not brought to our attention that it existed until this year after the car's warranty was up. 2 seal kits, air springs and the struts!

I had the 25v-274 recall serviced at my GM Dealer and was never provide with any paperwork on the extended warranty that was part of the recall. as you can image, not having anything in writing is a big risk as these engines have been catastrophically failing. GM was supposed to provide a extended warranty and service manual update.

While parked, my [XXX] daughter was in the third-row bench seat. The seat belt became wrapped around her body/neck area. The seat belt entered its 'Automatic Locking Retractor' (ALR) mode and continued to tighten as she moved, pinning her against the seat. The locking mechanism would not release, and the belt became so tight she was unable to breathe. We attempted to manually release the buckle and create slack, but the retractor remained locked and continued to tighten. After several minutes of unsuccessful attempts to free her, we had to use a cutting tool to sever the seat belt to save her life as she couldnt breath, she has brusing where the seat bealt was tighten. The vehicle was stationary in a parking lot at the time. The car has not being inspected , there was not warning. 2022 GMC Yukon AT4 was brought into the dealership for the service recall on the engine. No issues and received the 0W40 "fix". Approximately 4-6 weeks later, I was traveling on the interstate at approx. 75MPH when the truck completely shut down mid-passing with a code stating "conditions for not for shifting" and I was lucky enough to get the truck to the shoulder of the road without being struck by another vehicle as the truck "shifted" to neutral and would not do anything. Once the truck was in park, it shut off and I was unable to get it started again. Received multiple codes (key fob battery needs replacing, battery low, etc.) while stuck on the side of the interstate for several hours until I was able to get it towed (at a charge of $500) to my dealership. It then sat in the parking lot for 3 days for diagnosis pending TAC deciding whether to tear the engine down or replace it. It was determined to meet the recall qualifications and the dealership has ordered a replacement engine and was told they would start at a estimated 4 week repair time but would probably take closer to 6-8 weeks. Dealership has provided a rental at no cost to me but did not refund my towing costs associated with the repair.

Vehicle can be completely off and windshield wipers are still going. So you have to pop the hood and pull fuse out. If you want to use them ( it starts raining, dirty windshield) you have to stop pop the hood and put fuse in. Has been in shop almost weekly repaired every part they could possible switch and still not repaired.

The engine has started using large quantities of oil. Not dripping oil, burning oil. I have been doing oil consumption studies with GMC for months now. It burned 7 quarts in 5,000 miles. The dealership tells me it is not part of the current recall on others with the same engine, make and model. It is doing the same thing as what the recall is for and my VIN number should be added to GMC’s recall list and the motor replaced. It is a serious safety issue for me and my children,

NHTSA Team, I am the current owner of a 2021 GMC Yukon Denali (VIN: [XXX] ) with the 6.2L L87 V8. At approximately 148,239 miles, I experienced a catastrophic lifter failure. My mechanic diagnosed a failed AFM lifter and metal contamination, requiring full engine replacement. This is documented under GM Case #XXX. CARFAX revealed that prior to my ownership, lifters were replaced twice (~10,469 miles and ~25,572 miles). The dealership I purchased from did not disclose these prior repairs, as they used an alternative vehicle history check. Only upon this third lifter failure did my mechanic identify this recurring defect. I submit this to NHTSA’s investigation into post-repair failures of the GM 6.2L engines. The pattern shows prior remedies were insufficient. Supporting documents (CARFAX, mechanic report) are available. Please advise if further details or inspection of the failed engine components are needed. the Engine started running rough and the engine light came on. The dealer told my wife to drive there in limp mode (~15 miles), she did so on back roads. The diagnostics said that cylinder 7 was not firing and teh fuel injector control circuit was bad. they had to replace the Engine Control Unit.

I have a 2024 GMC Denali that had an engine service appointment in September 2025 for the engine recall where they did an oil change & put in performance grade oil & scoped my engine. Rochester Serra GMC said they sent all the engine pictures to GM and my engine was okay according to them. After that appointment in November my engine light went on and my Denali was leaking oil in my driveway. We took the truck back to Serra GMC Rochester and they replaced the oil filter that they said was defective & an oil change was done after my husband requested that just filling up the oil was not okay. They also told us the engine light didn’t show up on their computer system and it must have cleared out, so nothing was done for that. Today 1/25/26 now about a month later my engine light is back on and has remained on with multiple starts. This is not acceptable and needs to be investigated and I want this escalated. I do not feel comfortable driving my truck with this engine light on with a known engine propulsion recall that has no actual cure. The oil replacement and cap change has done nothing and engine light remains on. I just don’t want to hear the same thing that the computer must have cleared the error code. My engine light is on for a reason and I’m asking for a thorough diagnostic test to be done by a certified technician and a get a valid explanation of why my engine light keeps going on & not just changing my oil or waiting for something catastrophic to happen to me or my family. GM is putting me and my family at risk by saying that my truck’s engine is fine when clearly the engine light keeps repeatedly going on for 3 months in a row now since November 17, 2025.

I purchased a Yukon Denali 6.2L in March of 2022 brand new from Bob Johnsons in Rochester NY. On [XXX] at 12,577 miles the engine went through a catastrophic failure, while driving, the vehicle began to make very loud noises. The vehicle was taken to Bob Johnsons GMC 4389 West Ridge Road Rochester, NY 14626. The repair notes #3 main bearing thrust portion had come apart. The vehicle was out of service from 4/3/2023 to 4/19/2023 for engine replacement. On [XXX] at 50,244 miles on the vehicle, I was driving down a busy highway during holiday traffic and the vehicle went into neutral and would not start, we immediately turned the flashing lights on, unrolled the windows and began to show distress to ensure we were not hit by passing cars. We made it to safety and called Onstar for assistance, on this occurrence due to the vehicle not starting we were unable to get the vehicle back in to the neutral position to be able to roll the vehicle. The vehicle was towed to Bob Johnsons GMC due to the Thanksgiving holiday, the vehicle was brought in 11/28/2025, and deemed to need another engine replacement. On November 28th I called to file a claim with General Motors and was called back by Stephanie on November 29th and was told they would get back to me if they had any options as now this is the 3rd engine replacement. On December 3rd, a new woman called also named Stephanie and advised me they were still undergoing options, as I requested a Buy Back with Trade Assistance. On December 9th, my final call from General Motors advisor Stephanie, I was told I do not qualify for a buy back because I placed too many miles on the vehicle between engine failures. The vehicle was out of service from 11/26/26-12/29/26 for engine replacement. On [XXX] 2 weeks and 4 days after the vehicle going back into service, the engine failed again, only driving approximately 245 miles. I am currently waiting on the 4th engine replacement with no updates. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6

While driving approximately 63 mph on a busy roadway, my 2023 GMC Yukon experienced a sudden failure of the brake system, including loss of normal braking capability along with failures of the Electronic Stability Control (ESC) and ABS systems. The vehicle displayed warning messages stating “Brake System Failure” and “Service ESC” and entered a reduced-speed mode. Prior to this incident, there were no warning lights, messages, or symptoms indicating a problem. During the failure, the brakes became largely unresponsive, placing me, my two minor children who were passengers in the vehicle, and surrounding motorists at serious risk of a high-speed collision. After extended deceleration, a minimal amount of braking power partially returned, allowing the vehicle to eventually come to a stop. The vehicle has not yet been inspected by a dealer, manufacturer, police, or insurance representatives, and the issue has not yet been reproduced or confirmed by a service center. The vehicle remains available for inspection upon request and has not been repaired. This incident occurred without prior warning and involved a sudden and dangerous loss of braking capability at highway speed.
