I own a 2018 Ford F-150 3.5L EcoBoost with approximately 93,000 miles. I am experiencing a repeated cold start cam phaser rattle. This is a known issue addressed by Ford in several TSBs, including 19-2365, yet no recall has been issued. It has been "repaired" once at approximately 40,000 miles only to have the same issue reoccur at which time I was told that rattling at startup is "normal" and "since it doesn't persist it is not an issue". No IC gasoline engine should rattle on startup, and I believe this defect poses long-term reliability concerns and will lead to engine damage and failure during operation. Ford should be held responsible for a widespread design flaw affecting thousands of vehicles. I urge the NHTSA to investigate this recurring issue and hold Ford accountable for consumer safety and financial impact.

The contact owns a 2015 Ford F-150. The contact stated that on several occasions while driving at slow speeds, the transmission unexpectedly downshifted into second gear. The check engine warning light illuminated for a while before the power train warning light illuminated. The vehicle was not diagnosed or repaired. The contact received notification of NHTSA Campaign Number: 26V237000 (Power Train). The vehicle was taken to a dealer for the recall repair; however, during the repair, the dealer burned the PCM twice, and the recall repair was not performed. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue, and a complaint was filed. The failure mileage was approximately 60,000.

Pirelli Tires on F150 prematurely cracking & failing w/only 16,700 miles on the tire. Tread barely has any wear. On Saturday evening XXX Left rear tire wall blew out & PSI on tire went from 35 to 0 in less than 1 second. Upon closer inspection, all tires have premature cracks in the walls & the concern is that they could also fail and explode at any moment. We were not traveling at a high speed when the rear tire wall failed, but things could have been very different if we had been a highway - without doubt a major accident would have occurred. Brought the truck immediately to Coggins Deland Ford on [XXX] on Monday morning XXX where I purchased F150 brand new about 30 months ago, there is only about 16,700 miles on the truck. I am well within the tire warranty that Ford offers for the tires & well within bumper to bumper. All 4 tires should be replaced per the Ford tire warranty as they show signs of defect. The dealer has only approved the replacement of the tire that blew out so far & instructed me to contact the tire manufacture about the other tires that are cracking at 16K miles - I contacted Pirelli and was told since the dealer is giving me a hard time, Pirelli provided me with a case number and told me to tell the dealership to file a claim which they already know to do. Ford should provide the tire warranty on the tires per their own policy that is clearly stated on [XXX]. The dealer at first tired to tell me the tire only has warranty for 12,000 but it is not true, Ford provides prorated warranty on new tires up to 36,000 miles - when I showed the tire warranty chart from [XXX]- the service manager agreed with me & told me the service rep didn't inform me correctly & apologized. I just want defective tires replaced per the warranty that I was promised when I purchased my truck, I want to feel safe driving. Ford should not risk my life or my families life- the cracks are visible. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On August 20, 2026, while driving my 2023 Ford F-150 Lightning, the vehicle suffered a sudden, complete, and catastrophic loss of propulsion. The truck stopped abruptly while in motion on an active roadway. This immediate loss of power created an extremely hazardous situation, exposing myself, my passengers, and oncoming traffic to an immediate risk of a collision. The vehicle was towed directly to Williams Ford of Binghamton (Vestal, NY), where technicians diagnosed the issue as a major component failure tied to an outstanding manufacturer safety recall. Ford recall 24S12 Positive HV battery junction box. The vehicle has been grounded at the dealership for over a week awaiting replacement safety parts from the manufacturer. This unexpected, abrupt stop constitutes a severe safety risk that renders the vehicle dangerous to operate on public roads.

When shifting into reverse to pull out of my garage, with my foot firmly on the middle of the brake pedal, the truck suddenly accelerated, squealing the tires until I pumped the brake to make it stop. It was a very frightening experience. I drove it around town, including stopping and reversing and could not replicate it. It happened on a cold engine and it hadn’t been driven in three days. I’ve seen several other reports on Facebook and online of the same thing happening to other people. This is obvious a dangerous problem that needs to be addressed by Ford!

Daytime running lights (LED) in headlights burn out/quit working. Currently it is only the pasenger side of my vehilce but its a common issue and eventually both passenger and driver side DRL's quit working. there is no repair for this, only option for the owner is to replace the entire headlight assembly and that is extremely costly.

XXX, my Ford F-150 began experiencing serious and unpredictable transmission shifting problems. The truck would downshift unexpectedly while traveling at various speeds. At times, it would not downshift properly when coming to a complete stop, and it would sometimes shift back up into third gear from a complete stop at a stoplight. On one occasion, while traveling approximately 65 mph, the truck unexpectedly downshifted. The sudden change in the vehicle's behavior was extremely unsettling and made me concerned about maintaining control. What made the situation even more frightening was that I had no center lane or safe area to pull over without potentially becoming a hazard to the traffic around me. I had nowhere to simply get out of the way and figure out what was happening. That is an extremely unsettling feeling when you are traveling at highway speeds. Another incident was even more frightening. While traveling approximately 55 mph on the highway, the truck unexpectedly downshifted while a logging truck was traveling directly behind me. Fortunately, I was able to maintain control of the vehicle and safely pull onto the shoulder. I contacted my local Ford dealer and was told that its a Software repair, was not yet available and that I would need to wait for Ford's software fix. Extremely concerned about continuing to drive a vehicle that was behaving this way, I contacted Ford directly and asked Is it safe to continue driving the vehicle? Response -Ford was not advising owners to discontinue operation of the vehicle. Took place on XXX, at approximately 8:11 a.m. with Ford representative [XXX]. On XXX, I bring the truck to the Ford dealership for the software update. I received the vehicle back the same day, expected the problem had finally been corrected. Like I never had the truck "software" fix. Back to dealership - they redid the software fix, not fixed. No codes, and still no fix for the transmission shifting. On XXX, my [XXX] son was driving this 2017 Ford F-150 when the transmission suddenly downshifted while traveling on the road. The downshift was severe enough that the rear tires lost traction and the vehicle fishtailed, nearly resulting in an accident. The vehicle was taken to an authorized Ford dealer on 8/18/2026. We reported the sudden downshift, but the dealer was unable to duplicate the condition. The dealer performed Ford Recall 26S28/NHTSA 26V-237 and campaign 19G01, and the vehicle was returned on XXX. On XXX, only two days after the recall repair, the engine unexpectedly shut off while the vehicle was being driven. It restarted afterward. On XXX, the engine again unexpectedly stalled while the vehicle was moving. When the engine stopped, power-steering assist was lost and the driver had to coast the vehicle safely to the side of the road. The engine restarted immediately. When attempting to drive away, the transmission appeared to be starting in approximately third gear rather than first gear. After the vehicle was moving, the transmission eventually began operating normally again. Ford Customer Relationship Center was notified on XXX and formally documented these incidents. The vehicle is available for inspection. I am concerned because the original sudden downshift closely resembles the safety defect addressed by Recall 26V-237/26S28, and serious intermittent stalling and abnormal transmission operation have continued after the recall remedy was performed. I am requesting that NHTSA document and evaluate these post-recall safety symptoms and whether the recall remedy adequately addresses the underlying defect. Driving back from tri cities and when we were on I5 the transmission abruptly shifted down and went to a very high RPM. Was able to limp off the freeway by UW, and then drove home slowly. Had it trailered by the dealership i had just bought it from and they brought it to a shop where they said the transmission needed replaced.

The contact owns a 2018 Ford F-150. The contact stated that while accelerating from a stop light, the vehicle failed to properly accelerate and jerked and hesitated. The contact stated that the transmission upshifted from 1st to 3rd gear and then from 3rd to 7th gear. During the failure the WRENCH symbol was displayed. The vehicle was taken to the local dealer to be diagnosed, and it was determined that the transmission needed to be repaired, and the CDF drum need to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was 96,000.

In late July 2026, my son received a Ford safety recall notice (Recall #26S28) for his 2017 F 150 requiring a PCM software update. I brought the truck to Crown Motors in Redding on August 11th for the recall service. Before this appointment, the truck had no performance issues. Immediately after picking up the truck, a noticeable pinging sound occurred under acceleration. I returned the vehicle on August 12th, where Crown Motors confirmed the issue and advised spark plug replacement. This did not resolve the problem. I was also told that continued driving could damage the engine, so the truck remained at the dealership. Over the following days, Crown Motors worked with Ford engineering and repeatedly attempted software-related fixes. Crown staff informed me they believed the recall update caused the issue, and the first internal Ford team reportedly agreed. However, on August 25th, Ford’s second internal review determined the software update was not responsible and stated that all repair costs would fall on me—despite the clear timeline showing the problem began immediately after the recall service. Customer Service provided no assistance and refused to escalate my concern. As a result, my truck is currently non-operational, Crown Motors does not have a fix, and Ford will not cover repairs triggered only after Ford’s own safety recall. According to NHTSA’s info, it seems Ford is obligated under Motor Vehicle Safety Act (49 U.S.C. § 30111 et seq.) to repair the vehicle. I am requesting that Ford acknowledge responsibility for the recall related issue and cover the necessary repairs. I am deeply concerned at the lack of support and accountability shown throughout this process from Ford. I am resubmitting this info, not sure if my first attempt worked. I had an issue with my internet browser.

The contact owns a 2018 Ford F-150. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 26V373000 (Exterior Lighting) and Customer Satisfaction Program: 25N17 (Rear View Camera Replacement If Required After Dealer Evaluation); however, the failure occurred after the repairs were performed. The contact stated that while driving at an undisclosed speed, the "Parking Brake Engaged" message displayed, and the blind spot sensor became inoperable. An unknown warning light was illuminated. Additionally, the TPMS was inoperable. The vehicle was taken back to the dealer, but the vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 86,000.

After receiving Recall#26S28/NHTSA Rcall#26V237, I made appointment for the transmission module remedy upgrade. After getting upgrade, I didn't go a mile when it stuck in 3rd gear and wrench light came on..I took truck back to Ford garage and had to leave it there.A few days later I picked truck up and made it about 5 miles.I took it back ,as they wanted to drive it and try to get correct codes to come up, but all it did was keep coming up as a different code that wasn't warranted...We had a rejection from Ford on a conference call between Ford,Ford garage, and myself...they would only replace "lead frame"if the down shifting code came up...well, they already upgraded that but will not replace "lead frame" that shows a bad realy molded on it...I have truck back after 2 weeks sitting at their shop, but cannot drive it .When I take off from a stop it will do a hard shift into 3rd, wrench light and tranny fan comes on and I have to find a spot to pull over as it will not shift higher.Once shut off, I can restart and continue, but it usually does it several times before I can continue and shifting returns to normal...Recall from Ford shows if software upgrade doesn't rectify problem, they will replace "lead frame"- Ford garage wants me to drive it until code comes up that they need to warranty "lead frame", but its not safe to drive as it now has up-shifting problem.

The factory Pedestrian Alert System on my 2024 Ford F-150 Lightning produces an exterior warning sound at low speeds that is insufficiently audible to pedestrians in normal parking-lot conditions. On multiple occasions pedestrians have walked directly into my vehicle’s path without apparently hearing my approaching vehicle, requiring me to brake abruptly to avoid hitting them. I’m each instance, my vehicle is operating at very low speeds where the electric drivetrain produces little conventional vehicle noise. I am concerned that the factory pedestrian warning sound is inadequate to accomplish its intended safety function under FMVSS 141. I have not modified or disabled the pedestrian warning system. I believe GM had similar issues with the Chevy Equinox which was remedied via a recall. I am very concerned because several Ford F-150 Lightning owners have noticed this issue. I previously brought mine into the dealer, but they did not have a remedy for it. I am concerned because this is a 6600 pound pick up truck that seems to be in stealth mode at low speeds, which in my opinion is a recipe for tragedy.

The rear window of my 2024 Ford F-150 XLT violently exploded without warning. I was actively entering the truck when the explosion occurred. There was no collision, or object that stroked the glass. The extremely loud explosion terrified me, and because I initially did not know what had occurred, I contacted police. I have a police report of the incident. The failed component was the rear window/rear glass assembly. The shattered glass and vehicle were available for inspection following the incident and was towed to Ford. The sudden failure presented a serious safety risk because I was at the driver's seat when the glass exploded. I could have sustained significant lacerations from the glass. Other occupants could also potentially be injured. The failure has not been independently reproduced. The failure was directly observed by me, and the police were contacted immediately following the incident. I have reported the incident to Ford. Ford has been notified of the failure, but I have not received an adequate explanation establishing why the rear window suddenly exploded. I have subsequently researched similar reports involving Ford F-150 rear-window failures and discovered other consumer complaints describing sudden or unexplained rear-window shattering, including reports raising concerns regarding the rear defroster and/or associated electrical components. I did not use a rear defroster - the car was off. This raises concerns about whether Ford has adequately investigated a potential recurring defect and whether consumers should have been warned. The vehicle had had no warning lamps. This incident occurred on a vehicle purchased new in 2024. I have also experienced another major problem with the vehicle. I am requesting that NHTSA investigate whether sudden rear-window failure in the Ford F-150 represents a safety defect, whether Ford had prior notice of similar failures, and whether additional investigation, consumer notification, or a safety recall is warranted.

The CDF drum is going bad cause the transmission to slip and downshift while going at highway speeds. Many many many owners report this and Ford created a new CDF drum for owners to put in. However this costs thousands of dollars to replace when they know the CDF drum is faulty.
