as of [1/26/26], Recall 23S65 Is not complete Ford’s repair order states the vehicle is NOT SAFE TO DRIVE and cannot be repaired due to parts unavailability. This is a warrantable, safety‑related defect with no remedy available, rendering the vehicle unusable. Fords inability to repair a known safety defect do to parts shortage. Parts are back ordered for 8-12 months. parts delay ,a safety recall, no repairs performed NHTSA Complaint — Vehicle Information •Year/Make/Model: 2022 Ford F 150 4WD SuperCrew •VIN: [XXX] •Mileage at issue: ~31,851 miles (as of 01/26/2026) Component/System : Power Train → Axle / Driveshaft (Rear Axle Half Shaft & Hub Assembly) Recall ID : Ford Recall 23S65 (rear axle half shaft & hub assembly replacement) Incident Description (Narrative): On January 26, 2026, I presented my vehicle to an authorized Ford dealer (Greenwood Ford, Hollister, CA) to have Safety Recall 23S65 remedied. The dealer generated a Repair Order stating: “RECALL 23S65 NOT COMPLETE. PARTS ARE NOT AVAILABLE. UNSAFE TO DRIVE.” The same document notes “8–12 months before parts are available.” Because of this, my vehicle has an open safety defect that Ford acknowledges but cannot repair for an extended period, and the dealer explicitly documented the vehicle as unsafe to drive. I am unable to safely use the vehicle and have been advised to wait months for parts with no firm ETA. I’m filing this complaint so NHTSA is aware of Ford’s recall remedy parts shortage and the associated prolonged safety risk to owners who cannot obtain a timely repair. Safety Risk / Symptoms Observed: •Manufacturer safety recall with no available remedy for an estimated 8–12 months. •Dealer’s written designation that the vehicle is “UNSAFE TO DRIVE.” •Loss of safe operation and use of the vehicle until the safety defect is remedied. Dates & Attempts: •01/26/2026: Repair Order issued; recall repair not completed due to parts unavailability; dealer notes “UNSAFE TO DRIVE.” •Ong INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (F

- #11721633March 3, 2026ELECTRICAL SYSTEM, EXTERIOR LIGHTING
Driving home last night looking in my rear view mirrors I noticed it looked dark at the back of my F150. Once home I noticed I had no rear running lights, no brake lights, no rear turn signals. Not sure related but 2 days earlier I noticed that about 5 indicator lights on my dashboard were activated all relating to driver assist lane changing, backing up assist, hill parking assist, traction control and so on.

My truck’s brakes failed unexpectedly. I know there’s been a recall on similar year and engine models for brake issues. Even though my VIN isn’t showing an open recall, I’d like this inspected because the failure feels identical to the recall problem.

In September 2025 while existing parking garage 2nd in line at stop sign. After 1st car pulled out I accelerated to pull up to stop sign and truck missed gear and engine revved up then gear engaged caused hard shift and throwing me through stop sign into incoming traffic. No issues until Feb 2026 when transmission started shifting hard. Again at stop sign but first in line. When accelerated through stop sign truck transmission missed gear with engine revving and went into gear causing truck to jump across road. Im being told defect is under ford tsb 22-2428.

I know there is an ongoing investigation for F150s years 2015 to 2017 with the six speed automatic transmission. I just wanted to report that I've been having similar issues with my transmission, especially when 4 x 4 is enabled, it seems to worsen the problem. I've had unusual and unexpected shifting shifting. It's almost as if the gears are hiccoughing. I do remember they were having one issue on the highway upon accelerating where it dropped a few years, almost lags, but then came back. Like I said, it worsens in 4 x 4 and especially during hard accelerations, that is worth, I would emphasize the most. My biggest issue is upon hard accelerating. The gears slam, is the best way I could describe it. It's a noticeable and loud, slamming noise that happens when the gearshift from 1st to 2nd into third I haven't tried pushing it past fourth on hard acceleration I'll usually let off because it's such a loud slam, and noticeably shakes the vehicle.

- #11721508March 2, 2026UNKNOWN OR OTHER, VEHICLE SPEED CONTROL
When parking the vehicle at Auto Zone, at a very slow speed, the vehicle suddenly accelerated. Without the accelerator being pressed, the vehicle suddenly accelerated on its own. Notes: Vehicle has One-pedal always activated. It is a new vehicle. Only has 3k miles. I was taken once to the shop for maintenance. Malfunction components: Maybe Speed related components and safety features. The airbag did not deploy and non of all the safety features the vehicle has did not activated. The safety risk was extremely serious. The vehicle went over the concrete parking stopper, tore it off, and then crashed into a concrete column at AutoZone. If anyone had been walking in front of the vehicle, the outcome could have been catastrophic. Despite the impact, the airbags did not deploy and none of the vehicle’s safety systems activated. After the incident, I went to the ER due to pain in both hands, shoulder pain, back pain, and pain in my feet. I was evaluated for these injuries as a precaution. A police incident was done. We have a number. Because it was on private. property there is not a report, but they gave us a police incident number. No warnings, symptoms prior to the incident. It was a dry day, not rain. The incident occurred last Saturday. We tried calling the Ford dealer to have the car towed, but we were unable to reach them. Because of safety concerns, we contacted a towing company and had the vehicle towed to our home instead. We plan to call Ford today, but wanted to report the incident first

The contact owns a 2024 Ford F-150. The contact stated that while the vehicle was at an independent mechanic to be inspected, related to NHTSA Campaign Number: 25V512000 (POWER TRAIN). The vehicle was taken to a dealer, where it was diagnosed, and determined that the vehicle was damaged and was rubbing because the recall repair was not performed. The vehicle was not repaired. The contact reached out to several dealers to inquire about parts availability, but parts were not yet available, even though the recall remedy stated that parts were currently available for the repair. The manufacturer was notified of the failure. The failure mileage was 14,282.

The moonroof on our F-150 exploded going down the interstate. Nothing was hit and it sounded like a gunshot. The headliner remains intact with no damage and glass blew out at an upward angle. We submitted to Ford for a warranty claim but they denied it. Based on my research this is a known problem from Ford and they are not addressing it.

I had my 2021 F-150 into the local ford dealership not even a year ago due to a shifting issue. I was on the interstate and passing a car in the passing lane on the interstate and my truck shifted so hard I thought I lost something and then kinds lost power for a minute. The dealership ran a test on it, and it failed. It was some type of valve that was bad I guess ford and GM are having the issue. Now I am starting to have the same issue on now it is down around 4th to 3rd or 3rd to 2nd gear, but when it shifts once in a while it is a very hard shift and it scares the hell out of you. Now once in a while when I go to shift from drive to park or drive to reverse the shifter will not move. Like I said I am getting ready to get ahold of the dealership again and get it in to be looked at I am just waiting for winter to be over.

The vehicle’s original engine began producing excessive white smoke during startup. The vehicle was diagnosed by AutoNation Ford, and it was determined that the passenger-side cylinder head was cracked near the exhaust port. This defect allowed coolant to intrude into the passenger-side turbocharger. Due to the extent of the damage, the engine required full replacement. The engine was replaced with a used motor under a third-party warranty on August 21, 2024. The replacement engine is now experiencing the exact same failure symptoms. Specifically, the vehicle produces white smoke at startup consistent with coolant intrusion. Bozard Ford performed a diagnostic evaluation, completed on February 27, 2026. Their findings determined that the replacement engine has the identical defect: a cracked passenger-side cylinder head near the exhaust port allowing coolant intrusion into the turbocharger. This repeated failure raises significant safety concerns. Coolant intrusion into the turbocharger and combustion chamber may lead to engine misfire, loss of power, potential engine damage while driving, and possible sudden engine failure. Additionally, burning coolant may damage emissions components and create hazardous driving conditions due to reduced visibility from exhaust smoke. The fact that two separate engines in the same vehicle have experienced the exact same cylinder head failure suggests a potential design or manufacturing defect specific to the 2.7L EcoBoost engine. I respectfully request that NHTSA investigate this issue to determine whether this is a broader safety-related defect affecting other 2015 Ford F-150 vehicles equipped with the 2.7L EcoBoost engine. Please let me know if additional documentation, repair records, or diagnostic reports are required.

- #11721317March 1, 2026POWER TRAIN, WHEELS, SUSPENSION
The vibration occurs consistently above approximately 40 mph and becomes more pronounced at highway speeds. It is felt in: • Gas pedal • Underbody near shifter • Steering wheel • Seat Despite numerous repair attempts and replacement of major drivetrain components, the vibration has not been permanently resolved. Truck keeps throwing road force on brand new tires as well. Repairs and attempts have included: • Tire pressure corrections • Multiple tire balances and road force balancing • Replacement of all four tires • Subsequent rebalancing of the replacement tires after road force values continued to fluctuate • Additional tire replacement when road force variation repeatedly returned despite proper balancing • Replacement of both front hubs • Replacement of both front axles • Replacement of front steering knuckles • Replacement of front differential ring and pinion bearing kit • Replacement of right front axle • Replacement of transfer case • Replacement of front driveshaft • Diagnostic tire swapping from another vehicle for comparison At one point, test tires from another truck were installed to isolate the concern, which resulted in the vibration spreading further into the steering wheel and seat. The issue affects drivability and confidence in the vehicle at highway speeds. Current visit beginning January 7, 2026: Vehicle has remained at the dealership continuously with no return to me and no confirmed repair completion (now exceeding 40 consecutive days). The cumulative days out of service significantly exceed 30 days. Despite substantial drivetrain component replacement, the vibration concern persists. The persistent vibration affects normal use, drivability, and confidence in the vehicle at highway speeds. Persistent drivetrain vibration at highway speeds raises safety concerns. Repeated axle, differential, transfer case, and driveshaft replacements indicate unresolved mechanical instability.

Ford F-150 Rear Axle and Hub Replacement: It has been 130 days since I received Safety Recall Notice 25S82 / NHTSA Recall 25V512 from Ford stating parts were available and to contact my local dealer for repairs. I did so and they told me they do not have the parts I have followed up with them three times and I am still waiting for parts. Should these part fail on the highway it could leave me stranded or worst case scenario I could lose control and crash. I need NHTSA to put pressure on for to get this done!
