See attached document for complaint. I write this letter to you requesting your assistance regarding Ford Motor Co. recall 23S65 / NHTSA Recall 23V896. I received an initial recall notice in May, 2025. For your convenience, I am including a copy for your quick reference. In addition, I have received a subsequent recall reminder from Ford Motor Co. in November, 2025 noting that I have not addressed the safety recall repairs as of yet. I was urged to seek necessary repairs from my local Ford Dealer as soon as possible. A copy of this letter is also included for your reference and records. Please be advised that I have been in contact with my local Ford Dealer located in Montrose, CO on numerous occasions in an attempt to have there service department make the necessary repairs to my 2022 Ford F-150, most recently on January 2, 2026. I have been instructed that parts are still not available for them to fix the recall on my truck. It has been (8) months ago that I received an initial recall notice from Ford Motor Co. A few weeks ago I called the Ford Recall Assistance Center at 1-866-436-7332 in hopes of getting a resolution and timeline for a repair work to be completed on my Ford F-150. Unfortunately, this too was simply a waste of my time and energy as nothing was done to complete the necessary recall work to my vehicle. This has been extremely frustrating. I recognize there are safety issues that make it dangerous to drive my Ford-150 without the necessary recall repair work being completed. My wife and I rely solely on our F-150 for transportation as we are a single vehicle family and we are concerned about a potential future accident or vehicle breakdown. It is difficult for me to comprehend how Ford Motor Company can be so negligent and incompetent in getting this recall notice addressed in a timely fashion. In my opinion, Ford Motor Co.’s inaction in dealing with my F-150 recall is nothing short of being totally unacceptable. Thank you in advance for your consideration and assistance in this matter. I am reporting a serious safety issue with my 2021 Ford F-150. This truck has already been subject to multiple recalls, yet the problems have continued. On several occasions while driving, the truck has suddenly shut off without warning, causing a complete loss of power while the vehicle is in motion. This has happened while my family and I were in the vehicle and has nearly resulted in a serious crash. Losing power in traffic without warning creates an extremely dangerous situation and has put my family’s lives at risk. A vehicle shutting off while being driven is a major safety hazard. Despite multiple visits for service and ongoing recall issues, the problem has not been resolved, and the truck continues to fail in a way that could easily lead to a catastrophic accident.

I received an "important safety recall" notice 25S82 from Ford motor company in August, 2025. The notice from Ford states that replacement parts are available and to schedule an appointment with a local Ford dealership to replace necessary parts and address the safety recall concerns. Since August of 2025, I have contacted my dealership several times and they continue to indicate that parts are still not available from Ford to address the safety recall / concerns. I called the number on Ford's recall notice again today and they either don't answer or immediately hang up on customers like me when the call does go through to speak to an agent. Nine (9) months have passed now and neither Ford nor their dealership network has done anything to address the manufacture defects / safety concerns associated with my vehicle. My dealership has indicated there is a nation-wide back order on the replacement parts but that conflicts with Ford's written communication to consumers and it's no excuse for inaction that could put public safety further at risk. Please take necessary actions to increase Ford's focus on and resolution to this persistent risk. Thank you.

I am writing to express my serious concerns regarding the unresolved rear axle recall on my Ford F150. Although a temporary repair was performed last year, I remain deeply worried about the safety of my vehicle and, more importantly, the safety of my family. I have contacted my local Ford dealership multiple times to address this recall, but each time I am informed that there are no parts available for the necessary repairs. Given that this recall has been outstanding for some time, I am troubled by the apparent lack of urgency from Ford to resolve this critical safety issue. To add to my concern, the axle bolt has already broken once, which poses a significant risk while driving. I urge the NHTSA to intervene and provide assistance in expediting the resolution of this recall. Thank you for your attention to this matter. I appreciate any help you can provide. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

2013 Ford F‑150 Limited – VIN [XXX] On [XXX], my 2013 Ford F‑150 Limited experienced a sudden and dangerous transmission failure involving the transmission conductor plate (lead frame). My son was driving the vehicle when the dashboard abruptly shut off, multiple warning lights illuminated, and the truck lost engine power. Shortly afterward, while braking, the engine attempted to accelerate on its own, and the vehicle resisted slowing down. This created an immediate and life‑threatening situation. These symptoms match the known failure mode of defective 6R80 transmission lead frames, which NHTSA has previously identified as a serious safety hazard due to sudden loss of power, erratic shifting, unintended acceleration, and dashboard failure. The vehicle became undriveable and could not be safely transported to a Ford dealership. It had to be repaired at a nearby independent shop (Auto Stop Elite of Purcellville, VA). The conductor plate was replaced on February 12, 2026, at a total cost of $1,689.59. Despite this being a well‑documented safety defect affecting many Ford F‑150 owners, no recall or service campaign was ever issued for my VIN. No warnings appeared on Ford’s website, Carfax, or through any Ford communication. Other owners have received reimbursement from Ford for the same failure, yet Ford provided no notice or remedy in my case. This defect placed my son’s life in danger and represents a clear safety issue that should be addressed. I am requesting that NHTSA review this incident as part of the ongoing pattern of conductor plate/lead frame failures in Ford’s 6R80 transmissions. We are informing you because we have contacted Ford and they have done NOTHING to resolve this. We had to have it repaired and were never aware that there was a safety recall. We expect to be reimbursed in full for this repair by Ford. To date - we are being ignored and will now pursue with the Virginia Attorney General's office as well. Excessively hard transmission gear changes int eh 2, 3, 4 gear range. Happens in cold weather. Happens when accelerating and when down shifting. Gear shift is so violent it rattles the vehicle. Vehicle still operational, but long lime viability is not good. Feels like changing gears in a standard transmission but without using the clutch.

Windshield wipers are only working on the slowest setting, regardless of the position of the speed. Safety is put at risk as it cannot keep up with the rain. The dealer has reproduced the issue. The vehicle has not been inspected by the manufacturer, police, or insurance. There are no warning lamps.

[XXX] Noticed hard shifting while ascending a 13% grade at [XXX] , while ascending for a brief period I lost forward power, throttle was unresponsive. I noted the shift dashboard fluctuating between various gears as though confused as to which gear it should be in. [XXX] Hard shifting between 3rd-4th and 3rd-5th becoming more consistent and noticeable. [XXX] I've been driving this vehicle a lot less but today while driving to my storage room with items loaded in my bed (should be noted, the items were not a weight concern) on the 10 freeway west bound at highway speeds, the truck throttle became unresponsive and once again, seemed to fluctuate between 1st and 10th gear. I found myself in between several big rigs, I was in the #1 lane passing a semi to my right, in between 2 more semi's one in front, one to my rear. The truck slowed down on the freeway, throttle became unresponsive. For that moment, my drivetrain was completely unresponsive. Roughly 20-30 seconds later the transmission decided to land in 7th gear with a loud "thunk" noise accompanying a sudden shudder in the vehicle as I applied the throttle to resume highway speeds. Although at that point, I was very tender with the truck, for the rest of the trip I experienced no further issues other than the 3-5 hard shift. On or about [XXX], I moved the truck from the old house to my new house, approximately 30 miles away. For most of the trip the transmission only exhibited the 3-5 hard shift but at the last mile or two, the drivetrain, became completely unresponsive for more than a minute, shifting between multiple gears, at one point, locking up briefly before resuming. Fortunately I was only driving around 30mph and in a residential neighborhood. At this point, I'm afraid to drive it. I've made an appointment with a Ford dealership on 2025-03-09. They will provide a full inspection report, if possible, I will upload here if I am allowed to update this report. Since before my 60K warranty expired, I have experienced clunking when automatic transmission is espeically shifting down from about 4 to 3 of the 10 speeds. It has gotten worse, and it actually has good and bad days just like you and me. The other day it felt so severe that it seemed the entire truck would implode and then I would be sitting duck, or worse, hit something out of control or from quickly losing power steering. I know this issue is known, and there are now lawsuits. Date below is first onset. Dealerships have told me all sorts of crazy options to solve it over the years, from changing transmission fluid, to replacing transmission.

On March 3, 2026, I went in for two oil leaks. I received a call saying my truck was ready. I went to pick up my, drove off into the freeway. Everything was running smoothly until the anti-braking system turned on, cutting off power to my brakes. I was in the second to the last lane, luckily I was able to get to the last lane. I was almost hit and almost caused a huge accident. The dealership has offered to review the truck again. I paid close to six thousand dollars and it wasn’t for my brakes. I want my money back.

While driving at highway speed (60mph), the truck suddenly downshifted to 1st gear causing the rear wheels to lock up until the vehicle slowed to roughly 45mph. At the time of the incident, the truck was not loaded, not towing and weather conditions were cool and dry. Since that incident, the truck hunts for gears, shifts erratically and harshly and will occasionally get stuck in a random gear.

Problematic transmission shifting with lower gears causing significant hard shifting and jerking motion while driving. The problem was confirmed by my local Ford dealership on 11/26/2025, who recommended an overhaul to the transmission and tear down to determine if further damage is present or if a new transmission is needed. There were no warning lamp indicators showing any earlier related matters.
