STRUCTURE:BODY
Rear passenger door experiences intermittent safety defect where it fails to latch or seal closed, bouncing back open when shut. This structural failure poses severe, immediate operational hazards: • When the rear passenger position is occupied: failure of the door to properly latch or lock creates a high risk of the door flying open unexpectedly while the vehicle is in motion. This exposes the occupant—potentially a child—to a direct risk of ejection from the vehicle, as well as catastrophic injury from side-impact collisions where the structural integrity of the passenger cage is compromised due to an unlatched door. • When the rear passenger position is not occupied: Even when empty, an unlatched door can drift open during sudden acceleration, cornering, or braking. This creates a severe distraction for the driver, blocks visibility, and presents an immediate aerodynamic hazard. A door swinging open into adjacent lanes of traffic or bike lanes poses a catastrophic risk to external vehicles, pedestrians, and cyclists. Issue first appeared on or about April 2026 and has recurred in July, August and October. Prior to and concurrent w/ the latch failure the vehicle exhibited hidden system symptoms. Deep electronic diagnostic readout of the vehicle error memory explicitly links the physical latching failure to active, cascading electronic communication failures across the vehicle's Controller Area Network (CAN) bus infrastructure. The "Basic central platform" module reports multiple concurrent communication timeouts of door electronics and microswitches: • CD1408: StatusMicroswitchPushButtonPassengerDoorRear: Timeout • CD1407: StatusMicroswitchPushButtonDriverDoorRear: Timeout • CD1409: StatusMicroswitchPushButtonPassengerDoor: Timeout • 930709: outside door button: ... Blocked • CD1404: power distribution... Video: [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

ENGINE, POWER TRAIN, FUEL/PROPULSION SYSTEM
BMW car states suddenly loss of power and entering turtle mode. Then car suddenly stops or loses speed while driving. Battery potential appears to degrade and starts with inability to deceleration. Very dangerous conditions for driving. The car will lose battery potential and then shutdown reduce speed suddenly and/or completely stop. The BMW dealership has replaced 2 cells in the modular battery that restored some service so the sudden loss of power is not constant

SERVICE BRAKES, FUEL/PROPULSION SYSTEM, POWER TRAIN
While driving under 45 mph on 9/17/2026, the vehicle suddenly lost drive propulsion and would not respond to accelerator pedal input. Power spontaneously restored after roughly 1 minute. BMW Proactive Care immediately pushed automated vehicle alerts: "2472: Drive carefully. Chassis stabilization" and "29: Cont. driving possible: Drivetrain". On 9/27/2026, while stopped in live traffic on a 45 mph road, the vehicle completely lost throttle, failing to accelerate from a stop. Warning chimes and dash alerts sounded. After multiple vehicle power cycles over 5 minutes, power slowly resumed in a severely degraded state limiting speed to 21 mph on a 45 mph roadway before restoring. BMW Proactive Care pushed escalated alerts: "2472: Chassis stabilization", "53: Automatic Hold", and "29: Drivetrain". Sudden loss of motive power and extreme speed caps in active traffic lanes created severe rear-end and intersection collision hazards. The vehicle has 6 dealer service visits and 30+ cumulative days out of service since July 2026 for unresolved electrical and charging defects before escalating to drivetrain shutdown. Prior to dealer intake, an independent OBD scan (BimmerLink) identified root faults in the Combined Charging Unit (CCU): Code 224183 (pedal sensor 2 short to ground) and Code 224102 (HV battery coolant pump overcurrent). Authorized BMW dealer scans (Global Imports BMW) confirmed Code 224183, Code 224F89 (restricted acceleration), Code 4809F7 (driving stabilization unavailable), Code 480A1D (parking brake unavailable), and multiple wheel-slip torque limitation faults. Subsequent authorized BMW diagnostics (United BMW) confirmed permanent Check Control shutdowns: CC ID 00170 (Drivetrain failure), CC ID 00636 (High-voltage deactivated), and CC ID 02261 (Steering and braking restricted). The vehicle is grounded at an authorized dealer under BMW Corporate Engineering (TSARA case) and is available for inspection.

STEERING
On 11/11 I was driving my vehicle and for the first time ever I received a error message in the vehicle of reduced driver steering. It said driving was still possible but take the vehicle to a dealer for evaluation when possible. This message popped up and went away the rest of the day and first half of the following day. I researched the message and apparently my make year and model have a known issue with the steering rack design that causes it to fail prematurely. There were many reports online of people getting this message and upon taking it to their local BMW they were told it was a known issue and the part would be replaced. So I scheduled an appointment at my local dealership for early December which was their first available. The following afternoon I went to drive a quick errand and go a new message saying steering greatly reduced. The power steering was completely gone. The car was nearly impossible to turn the wheel it took all the strength I could muster to do so as turning was nearly impossible. I drove it to the dealer as it was no longer safe to drive. The dealer told me that there was no recall on the part and because I'm 3 months and change out of warranty the replacement would not be covered. I am extremely upset. This is a known issue with this part, they have a new redesigned part that addresses the premature failure. I should absolutely not be responsible for having to pay for a part that was knowingly faulty.

STEERING
Intermittent and sudden "Changed Steering Effort" warning on dash every 2-3 vehicle starts. Steering effort is very noticeably increased. This is a known issue among all BMW iX owners, easily inspected, and BMW has swept this problem under the rug for too long and quietly replaced all the steering racks when the warning pops up. Fortunately for many, they're under the factory warranty. Unfortunately for me, I have had my car less than 2 years, and barely over the factory warranty on mileage. However, it is VERY concerning as many owners are getting their steering racks changed out at even under 10k miles. Yes, this can happen suddenly on the freeway at speed and I do believe is a safety issue. The problem can easily be reproduced and is a very typical problem on BMW iX's all over the US and globally as many owners on forums have been asking for help on this issue. The vehicle has been inspected by the dealer and their fix is to replace the entire steering rack. Yes, intermittent warnings. I hope NHTSA can get to the root of this and have BMW issue a recall for this steering rack issue, as our cars are very heavy and fast and if there are any sudden failures, it can be dangerous and fatal at highway speeds.

STEERING
The steering rack on my under 2 year old BMW iX with ~21k miles needed to be replaced. The vehicle displayed an intermittent warning about increased steering effort and to contact my service center. Upon taking it in they diagnosed it as a steering rack issue. Before taking it in, internet searches suggest many other owners of 2022 BMW iXs are having similar failures for the last several months. Steering failure could easily cause a crash and significant injury or death if it happened at the wrong time. Having the rack fail on a car this new is worrisome.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS
I am writing to report a serious safety concern regarding the 2022 BMW iX, specifically related to the malfunction of its safety system due to the software provided by Green Hill Software. This issue has resulted in multiple instances where the safety system of the vehicle locked up, putting drivers, passengers, and other road users at significant risk of accidents. On several occasions, while operating my 2022 BMW iX, I have experienced sudden and unexpected failures in the safety system. These failures have occurred during critical moments of driving, where the vehicle's safety systems are relied upon to prevent accidents and protect occupants. The incidents have almost led to accidents, causing severe distress and endangerment to myself and others on the road. Upon investigation, it has been identified that the fault lies within the software provided by Green Hill Software, which is integral to the functioning of the vehicle's safety systems. Despite attempts to rectify the issue through software updates and dealership interventions, the problem persists, posing an ongoing threat to the safety of BMW iX drivers and the public at large. I urge the NHTSA to investigate this matter thoroughly and take appropriate action to address the safety concerns associated with the software provided by Green Hill Software in the 2022 BMW iX. It is imperative that swift action is taken to ensure the safety and well-being of all motorists and pedestrians affected by this issue. Furthermore, I request that NHTSA liaise with BMW and Green Hill Software to implement a comprehensive solution to rectify the software-related safety issues in the affected vehicles promptly. This may involve recalls, software updates, or other corrective measures necessary to mitigate the risks posed by the faulty software.

UNKNOWN OR OTHER
I a not certain how to report this incident but I received a text alert from a local car dealership that there was a vehicle recall of my front grill due to failure of a device that is supposed to unfreeze ice from that part which is experiencing a failure. Since this is a major BMW Dealership, I did not question the validity, I merely scheduled the repair. When I arrived there was no promised loaner car and I was angered that I had cleared my busy morning and there was no way I could also clear my afternoon to wait for my car so I ordered a Lyft to take me home. Then, I became suspicious when I was informed that they did an inspection of my 2022 vehicle and presented a list of repairs, I did no sign any authorizations allowing them to do anything to my vehicle. I told them to do only the recall repair and then I looked up my vin and became aware that there was no safety recall outstanding on my vehicle and never anything concerning icing of the grill. Now I need to pick my vehicle up and I am i fear of what they may have done to my vehicle.

ELECTRICAL SYSTEM, FUEL/PROPULSION SYSTEM, LANE DEPARTURE: BLIND SPOT DETECTION
Power loss, dash went black and loss of drive power. This failure has occurred twice and I more fully documented each failure to BMW. When was the last time this occurred? August 28th 2. GPS error, vehicle speed limit showed 25 under all conditions while actual speed showed correctly. When was the last time this occurred? Early August 3. Side collision system failures. Either fails entirely and indicates no vehicles on either side or will randomly indicate the presence of vehicles when they are not there. Other times it will function correctly. This has happened multiple times now. When was the last time this occurred? October 7th 4. Driver assistance errors pop as a multiple page error on the infotainment for no apparent reason a. On 217 an error message was displayed on the screen and heads up display saying Driver Assistance Limited. The message was longer but that’s all I could read driving in traffic. Conditions could not be more ideal and literally just left service with a freshly washed vehicle so not sure what the failure could be. I think this message is meant to occur if the cameras aren’t working or are obstructed by dirt, extreme weather etc. This message will display if the system has anything that prevents parameters from being met, this is something that will come up from time to time. 5. Assisted driving turned itself on and set a speed of 37 MPH When was the last time this occurred? August 5th 6. Automatic window stopped working despite the activation point still showing setup and active. Checked settings and tried it a few times and it just stopped working but settings look correct. It had previously been working. When was the last time this occurred? In the last week. 7. BMW assistant failures on about 3 of 4 commands. Only the mic button will activate the assistant as the voice activation stopped working. Making calls, navigation, turning off steering wheel heat, turning on massage seat all failed 1-3

FORWARD COLLISION AVOIDANCE
The contact owns a 2022 BMW IX. The contact stated that the wife started the vehicle while parked in the garage. The wife exited the vehicle to retrieve an item and left the front driver's door open. Moments later the vehicle drove in reverse out of the garage and across the street and crashed into the neighbor's parked vehicle. The contact indicated that during the failure the collision avoidance system did not activate. No injuries were reported. The vehicle was later taken to the local dealer who was unable to determine the cause of the failures. The manufacturer was contacted regarding the failure. The failure mileage was 16,000.

POWER TRAIN, ELECTRICAL SYSTEM, ENGINE
I was traveling on 6/12 at approximately 4:30 AM at around 65 MPH when I received a warning message something to the effect of “emergency system malfunction - vehicle will now stop”. I lost all throttle and the vehicle slowed to a stop. Due to the lack of other vehicles present at the time, I was able to get to the side of the road before the vehicle completely stopped. I restarted the vehicle and it seemed to operate normally, but I took the vehicle to the service center to diagnose the problem because it could have caused a serious accident in heavier traffic. When I checked the vehicle in, the service advisor stated that it seemed like a serious issue and they would want to get BMW involved directly, but when they had it “ready” for me two days later, all they had done is attempt to pull trouble codes and couldn’t tell me if it was safe to drive or not. They did provide me a loaner at the time and I have attempted to get the dealership to diagnose this issue, but to date they have not provided me with any information about it after having the vehicle for about one week. I contacted BMW North America Customer Relations and explained the problems with the dealership and asked about the mechanical failure and whether I should be concerned/take it to another dealership/etc. They stated that they don’t get involved with the dealership service departments and were generally unhelpful. I stressed that this could be a serious safety issue, but I didn’t get the sense they were interested in getting to the bottom of what happened and whether it was in fact a serious safety issue.

VISIBILITY/WIPER, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
1. The rear wiper has a frame made of plastic, with a simple T shaped connector. I have found it laying in my driveway 4 times now, and have only owned this 2022 BMW iX since November. If there is any resistance at all when you turn the wiper on, like some snow or frost, the T shaped connector will un-click, the wiper will tip sideways, and fall off the car. This could happen at speed if you engage the wiper with ANYTHING on your back window and could fly into the car behind you (have not experienced this yet...). 2, The Adaptive Braking can engage in very weird situations. Driving south on Rt 3 in New Hampshire (the Everett Turnpike, I think it's called) after dark between exit 5 and exit 4, I was in the middle of Rt 3's three lanes with NO traffic around me at all. The iX decelerated suddenly by about 15-20 mph, so thank god no vehicles behind me! Realized at that point what had happened - Rt 3 curves left at this point as you are heading south. At this spot, the Exit 4 ramp goes straight off the road at an angle. The ramp is short and quickly comes to a set of traffic lights. This traffic light is right at the height of Rt 3, as the ramp dips somewhat for this exit, so the traffic lights are at the height of car brake lights at this point. The traffic lights were red and there were 6 or 7 cars braked showing red brake lights. I believe the BMW iX adaptive braking incorrectly engaged at this point, thinking this was traffic that was on Rt 3 and that the BMW iX needed to stop for it. This traffic was actually 300 - 400 yards down the ramp, but the adaptive braking engaged strongly to 'avoid a collisioin'.

SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS
This vehicle was involved in a collision, none of the safety features worked. The front end collision warning did not go off. The automatic emergency braking did not engage. The Car jolted forward after the collision and did not stop, potentially risking another collision. The SOS or emergency collision BMW assist was not activated, there was no call from anyone from BMW after the accident. No warning lights or collision indicators went off prior to collision as they should have. This vehicle is puts the safety of others at risk if it fails to stop after a collision (regardless of the speed of impact) as well as quick acceleration post collision.

It took them 6 weeks to replace the battery component that was part of the recall. My car was unable to be driven during this timeframe. Under my state lemon laws I am eligible for a car buyback from BMW as it was out of service over 30 days. I registered a request for this through BMW corporate and it was denied by them.
