I was driving to work when after running across a minor pothole I heard a clunking sound. It turns out it's the top cap for the front coil-over Bilstein shocks had broken allowing the shaft and the bump stop to protrude through the upper mounting plate. The coil did its job of keeping the vehicle suspended, but the shock piston has no ability to alleviate any stresses put upon the coil. Dealer states that because there was an outside contribution, i.e. a pothole, which had been driven around and over many times before, wasn't warrantable. There was no more than standard bump and sway than when driving around as usual. There was no shudder as though I was going too fast for the defect in the road. I dont know if there was pre existing stress damage from a recent collision(opposite corner of truck) or if the was a defect with the part. The top cap is made out of aluminum rather than steel, which I find disturbing after learning this is not the first to have broken. This vehicle has only been off pavement in gravel driveways and parking lots. I hope you can investigate this thoroughly.

The vehicle is a 2024 Toyota Tacoma SR5 that was later traded for a 2025 Tacoma SR5 due to a serious safety defect involving the rear brake lines. The problem is the same one described in Toyota’s recall: the rear brake hoses can be damaged by debris, resulting in brake fluid loss and potential brake failure. The safety risk was significant. The vehicle was used to transport a young child, and because the brake lines could fail without warning, the truck could not be safely operated for daily driving. Multiple franchised Toyota dealerships confirmed that they had no remedy parts available for several months and that the repair could not be performed. This left the vehicle without a functional safety repair for an extended period. The manufacturer’s public statements indicated that a fix existed, but dealerships consistently stated the opposite. No alternate transportation, loaner vehicle, or temporary replacement was provided during this period. The defect was not reproducible by driving because of the risk involved, but the safety issue was acknowledged by dealerships who stated that the recall was active and unresolved. The vehicle was effectively unusable for normal safe operation. No warning lights appeared on the dashboard; the hazard is related to the physical brake line design and exposure to debris rather than an electronic sensor. The vehicle was inspected by multiple Toyota dealerships, all of whom confirmed that no repair could be performed at the time. The component was not replaced, repaired, or made safe before the vehicle was traded out of due to the unresolved safety defect. The situation caused substantial safety concerns and financial hardship and appears to reflect a wider problem in remedy availability for this recall.

I was driving on the highway when my vehicle suddenly lost power. The engine light and traction control light came on, and I lost power, and the engine started sputtering. I had to pull off to the side and put on hazards. My Tacoma would only drive about 25 MPH after this happened. Based on a quick search, this is a very common problem in 3rd-generation Tacomas!

The vehicle was being operated in 4-wheel-drive, mode 4H, and when turning to the left at low speed, there was a grinding noise from the front of the vehicle and it would not proceed. The vehicle had to be put back in 2-wheel-drive, mode 2H, put in reverse, backed up a short distance, and then put back in drive, and then it was able to proceed. This is likely the CV joint issue reported in other 2024 and 2025 Toyota Tacomas. The dealer has been notified, and the vehicle will be taken in for service.

On November 14, 2025, at approximately 0200 hours, in Fresno, California, I was driving southbound on Highway 99 at approximately 70 mph using cruise control. It was raining heavily at the time, and visibility was significantly reduced. I was traveling in the middle lane when I suddenly felt what I believed to be an impact on the driver side of my vehicle. Due to the weather conditions, I was unable to visually identify what struck the vehicle. Immediately prior to this event, I noticed faint headlights in lane 1 (fast lane), but I could not determine whether that vehicle made contact with mine. At the moment of the perceived impact, the side curtain airbags deployed unexpectedly. I initially thought the deployment was a collision with another vehicle because the force felt like a physical impact. Once the curtain airbags deployed, I immediately lost control of the vehicle. Steering became impossible, and the vehicle veered off the roadway, ultimately coming to a stop on an embankment adjacent to the highway. California Highway Patrol officers responded to the scene, took my statement, and inspected the vehicle. They reported no exterior body damage consistent with a collision. The following day, I visited the tow yard and conducted a daylight inspection of the vehicle, confirming that there was no visible impact damage on the body panels or exterior surfaces. After the on-scene investigation, the vehicle was towed, and the responding officer provided me with a courtesy ride. As a result of the incident, I sustained minor injuries, including whiplash, road rash on my left shoulder and upper forearm, and soreness in my neck. This event involved unexpected deployment of side curtain airbags without any confirmed collision, leading to loss of vehicle control, roadway departure, and driver injury. I am submitting this complaint for investigation into potential airbag or sensor malfunction.

The multimedia head unit and Apple CarPlay system are malfunctioning. The screen randomly disconnects, goes black, and often fails to connect to iPhones. It shows “Retry,” and sometimes Apple CarPlay will only reconnect through the Bluetooth settings instead of normal wired or wireless CarPlay. Both of my new-generation Tacomas are available for inspection upon request. This issue creates a safety risk because the screen disconnects or goes black while driving, causing loss of navigation, hands-free calling, and vehicle information. This forces me to look down or try reconnecting the phone while the vehicle is in motion, which increases the risk of distraction and a possible accident. I have taken both of my Tacomas to Toyota for inspection, but the dealer was unable to reproduce the problem. The issue happens repeatedly during normal driving and occurs on multiple iPhones, so it is not a phone-specific issue. The vehicles have only been inspected by Toyota at the dealership level. No inspections have been done by the manufacturer, police, or insurance. The problem is ongoing and the vehicles are available for inspection at any time. The symptoms include the screen showing “Retry,” disconnecting without warning, going completely black, switching to Bluetooth-only connection, and failing to recognize iPhones at startup. These issues appear randomly on different days and with multiple iPhones. No other vehicle warning lights were present.

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The multimedia head unit / Apple CarPlay system is malfunctioning. The screen randomly disconnects, goes black, and frequently fails to connect to iPhones. The system displays “Retry,” and at times Apple CarPlay will only reconnect through Bluetooth settings instead of normal wired/wireless operation. Both vehicles are available for inspection upon request. 2. How was your safety or the safety of others put at risk? The failure creates a safety hazard because the screen disconnects or goes black while driving, causing sudden loss of navigation, phone functions, and display information. This forces the driver to look down, attempt to reconnect the phone, or search through Bluetooth menus while the vehicle is in motion, increasing the risk of distraction and a potential collision. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? No. I have already taken both of my new-generation Tacomas to Toyota for inspection. The dealership was unable to reproduce the malfunction, even though the issue happens repeatedly during normal driving. The problem occurs on multiple iPhones, confirming it is not phone-specific. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The multimedia system has only been inspected by Toyota, but no police, insurance, or manufacturer inspection beyond the dealership has taken place. The issue remains ongoing and can be inspected at any time. 5. Were there any warning lamps, messages, or other symptoms prior to the failure, and when did they first appear? Yes. The multimedia screen will: show “Retry” when attempting to connect disconnect mid-drive without warning go completely black switch to Bluetooth-only connection fail to recognize iPhones during startup These symptoms occur randomly, across different days, and on multiple iPhones

The contact owns a 2021 Toyota Tacoma. The contact stated that while driving 10 MPH, after depressing the accelerator pedal and then attempting to depress the brake pedal to make a left turn, the vehicle failed to respond as intended. An independent mechanic was contacted. The contact pulled over into a church parking lot. No warning light was illuminated. The vehicle was taken to an independent mechanic to be diagnosed; however, the diagnostic result was unknown. The vehicle was not repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 47,000.

The 3rd generation Tacomas rear cabin vents (2 vents) allow water to enter the cabin because they are not tight to the body or sealed properly. The water leaks down the back wall and forward onto the floor soaking the carpet and padding underneath. The water in the carpet/padding has yielded mildew/mold throughout (water goes all the way to the front of the truck). The water also leaks through a floor grommet into a section where the cab mount bolts are and significant rust develops. Not only is the exposure to mold a health hazard, but the corrosion/rust on the cab mount bolts is a significant structural concern. There are many online posts with this problem, and i have just recently found this on my Tacoma. My vehicle has had the 3rd brake light recall, and this issue is completely separate from that recall. Toyota has a history on these vehicles not sealing them as they should. 1. The 2 rear cabin vents fail to keep water from entering the cab. I am happy to provide the vehicle for inspection. 2. Exposure to toxic mold is a health concern. Significant structural damage to the cab mount bolts is a concern if the vehicle were to be in an accident. 3. Numerous people online have confirmed this issue, and they have indicated dealerships are well aware of the issue. I put a garden hose to the back of my truck myself and saw water entering the vehicle. 4. Vehicle has not been inspected by manufacturer, police, insurance, etc. 5. Unknown when water leak first started. In the past 2 years i have started to notice a mildew smell after a rainy day. I am happy to provide additional photos, but the internet is full of this very issue. Please see the following sites with photos/videos/descriptions of other Tacoma owners having the exact same issue. Thank you very much for your time. The engine in my 2019 Toyota Tacoma TRD Off Road suffered internal damage that appears linked to Toyota’s low-pressure fuel pump recall (issued 10/28/2020, repaired 581 days later on 6/10/2022). While out on personal lease, the truck was driven over 16,000 miles with the defective pump, which can cause lean running conditions and elevated combustion temperatures. The result was metallic contamination and a damaged wrist pin in cylinder #6, requiring full engine replacement. All failed components remain available for inspection upon request. The fuel pump defect and resulting internal engine damage continues to place my safety and others’ at risk by creating potential for loss of power, engine stall, or total failure while driving. The problem was confirmed by two separate Toyota dealerships, both of which diagnosed the need for engine replacement. The vehicle has been inspected by multiple Toyota service centers and the Toyota Brand Engagement Center, which opened and later denied my case. No warning lights or messages appeared prior to the ticking noise that first appeared around 62,000 miles. In fact, no warning lights have ever illuminated on the dash. Despite clear evidence that the recall-related defect likely caused the damage, Toyota has refused to provide any assistance or coverage toward repairing or replacing the failed engine, leaving me with a $21,000 repair for a vehicle that was otherwise maintained and operated responsibly.

I’m filing this report because the dealership where I purchased and have my vehicle regularly serviced failed to notify us of an active safety recall related to the brake system on our Toyota truck. The dealership is Classic Toyota – located at 2301 W Mercury Blvd, Hampton, VA 23666. The recall was issued in February 2025, yet the vehicle has been serviced by the same Toyota dealer multiple times since then, including an oil change and a state inspection. The dealer never mentioned the open recall or offered to repair it. I bought the truck prior to the recall in July of 2024. We only learned of the recall recently through the Toyota app, not from the dealership. This is unacceptable given that brake recalls pose a serious safety risk. I am requesting an investigation into this dealership’s failure to notify or perform the recall repair as required by federal law.

My 2025 Toyota Tacoma has an ongoing issue where Apple CarPlay frequently disconnects or fails to connect altogether. This happens both at startup and randomly while driving. The problem occurs across multiple iPhones, different cables, and even after ensuring that both the phone and the truck’s software are updated.

- #11698110November 7, 2025BACK OVER PREVENTION: SENSING SYSTEM: CAMERA
I was traveling home from work. Upon leaving work the backup camera on my truck worked as expected. Once I arrived home the back up camera and all other camera would not appear on the screen and the screen was blank. I attempted to put the truck from drive to reverse several times and the back up camera would not appear. I attempted to use the "view" button located on the console and the cameras would not appear. I then resulted in parking the truck, turning the truck off, starting the truck, and then testing which this time the cameras did work. Things of interest that may play a role. I did use android auto for the journey. I did also check that the built-in system appeared on the screen after disconnecting my phone. I do have a video of the experience if there is a way to upload that.
