1. The rear underbody air conditioning (AC) lines and their supporting structural mounting brackets have failed due to severe corrosion. Specifically, a heavy steel mounting bracket has rusted excessively, causing localized galvanic corrosion at the point of contact with the adjacent aluminum AC lines. This structural and material failure caused the aluminum line to erode, crack, and leak high-pressure chemical refrigerant into the environment. The vehicle, failed components, and detailed photographic evidence of the galvanic corrosion and rusted bracket are fully available for inspection upon request. 2. The failure presents multiple safety and health hazards: •Environmental and Inhalation Hazard: The high-pressure leak of mobile air conditioning refrigerant (R-1234yf or R-134a) releases a hazardous chemical compound directly beneath the cabin. •Defrost and Visibility Failure: The rapid loss of system pressure completely disables the vehicle’s climate control and window defrosting/demisting capabilities, severely compromising driver visibility during inclement or humid weather. •Structural Degradation: The severe rust and erosion of the underbody mounting bracket mean structural components are failing prematurely on a 2021 model, creating a risk of loose or dragging underbody debris at highway speeds. 3. The physical failure (loss of refrigerant and severe physical corrosion) has been visually confirmed. However, the Toyota dealership service center actively refused to document the cause. However, photographic evidence clearly reveals the distinct, localized galvanic corrosion occurring exactly where the rusted steel bracket contacts the aluminum line. Further, Toyota’s highlander model had this same problem which was addressed through T-SB-0021-26. 4. The vehicle was inspected by an authorized Toyota dealership which acknowledged the leak but refused to provide a written technical breakdown. 5. Rust, corrosion, and loss of cooling ability.

- #11744995June 18, 2026STEERING
My 2015 Toyota Sienna has “Sticky” steering. When on a curve, like an highway curved ramp, the steering locks in place. I have to pull the steering wheel back to keep the car on track. When it sticks I can literally let go on a curve and it will hold it’s position. Even when driving straight this occurs. The steering wheel will stick in place requiring me to turn it to get the steering wheel to come back to the desired or correct position. I’m seeing that this is a regular problem for Toyota steering across multiple models. Having your steering get stuck in place has the potential to be a very serious safety issue. This may cause an accident and resulting death. The complete repair for this safety issue to the consumer is over $3000.00. This is ridiculous! This is my first Toyota. I think their reliability night be really over rated.

- #11745053June 18, 2026VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, SERVICE BRAKES
Less than 1000 miles on my car i do not remember exact dates hence i have added todays date for incident date. All three incident happend in three different days. Scenario 1 two times. Scenario 2 one time --- SCENARIO 1: Curved Road / No Lane Markings --- Conditions: ACC set at 25 MPH on an undivided two-way road with no lane markings. Parked vehicles present at curves; oncoming traffic at safe distance. Issue: On two separate occasions, the vehicle applied sudden, significant braking without driver input while navigating curves. No imminent collision risk existed. The abrupt speed reduction created a rear-end hazard from following traffic. Reported to Toyota support: YES --- SCENARIO 2: Michigan Left Turn Lane / 40 MPH --- Conditions: ACC active at 40 MPH on a multi-lane road with a dedicated Michigan Left turn lane. Forward path in my lane was clear. Issue: A vehicle ahead moved into the Michigan Left turn lane and began decelerating. As my vehicle accelerated (path now clear) and was still slightly behind — approaching but not yet parallel to — the turning vehicle, the system applied brief unexpected braking. The turning vehicle then moved further forward to complete its Michigan Left maneuver ; my vehicle resumed normal speed when some diagonal distance had been seen. Reported to Toyota support: NO ACTION REQUESTED: Investigate whether Toyota Sienna's PCS/ACC (1) miscalibrates object detection on unmarked curved roads and (2) fails to distinguish lateral vehicles from forward obstacles. Software update or remedy may be warranted.

My rear lift gate was misaligned and gets stuck. The dealer just fixed and wouldn't honor the warranty, but I believe this shoddy build quality could impact the safety of my family in an accident as it didn't appear the hatch was properly secured

The center console between the front seats has a design defect. When small objects such as a credit card, key fob, or coin are left on the flat surface of the console, they can slip through a gap into the internal storage compartment. Once inside, the object becomes lodged and physically blocks the console lid from opening, locking the owner out of the storage compartment. The console must be partially disassembled to retrieve the object. This is a design flaw, not operator misuse. The flat surface visually appears usable, and the owner's manual does not warn against placing objects there. This defect is widely reported in the "Toyota Sienna 2025+" Facebook group (the largest online community of current Sienna owners), where multiple owners report the same problem. The defect is therefore not isolated. The vehicle was purchased new on 01/22/2026 from Nourse Toyota. The dealer declined to repair and referred me to Toyota. Toyota Customer Experience (Case ID XXX, contact: [XXX], 06/04/2026) declined to offer a remedy. On 06/10/2026, the Toyota Brand Engagement Center (Case #XXX) confirmed in writing that no recall or Customer Support Program covers this issue, and that the diagnosis does not indicate a manufacturer defect. No warning lamps or messages appeared. The defect is mechanical. I request NHTSA review this defect for investigation and potential recall action. I am the original owner of a 2024 Toyota Sienna Hybrid (VIN: [XXX]). The vehicle has required two significant engine repairs within the first 57,000 miles of ownership. Both repairs were performed at authorized Toyota dealerships under warranty. INCIDENT 1 — ENGINE REPLACEMENT (10,000 MILES): At approximately 10,000 miles, the engine developed severe knocking. The dealership determined the engine was defective and performed a complete engine replacement under warranty. INCIDENT 2 — OIL LEAK (57,000 MILES): At approximately 57,000 miles, the engine developed an oil leak between the oil pan and the timing belt cover. The dealership repaired the leak under warranty. The dealership informed me that this repair carries only a one-year warranty on the work performed. The fact that the engine required complete replacement at only 10,000 miles indicates a manufacturing defect. The second engine issue at 57,000 miles suggests a pattern of powertrain reliability problems. I contacted the Toyota Brand Engagement Center (Case #XXX) and spoke with both an advocate and a supervisor. Toyota declined to offer any extended powertrain coverage and would only agree to review future issues on a "case-by-case basis." I am concerned about the safety and long-term reliability of this vehicle's powertrain given the recurring nature of these engine failures. A complete engine failure at 10,000 miles on a brand-new vehicle is not normal wear and tear — it is a defect. I am reporting this so that NHTSA is aware of potential systemic engine issues with the 2024 Toyota Sienna Hybrid. We experienced a high whine for a couple of months. No codes were being read by any mechanics. Then the transmission started slipping between 2nd and 3rd gears, and then we couldn’t reverse. Now it’s not switching gears at all. There is a recall for our year and type of transmission, for this issue, but our vin number is not included in that recall.

June 3, 2026, approximately 5:30 PM EDT, I entered my 2025 Toyota Sienna to move it from the street beside my home, to my driveway. Upon closing the driver's door, the rear liftgate window exploded into the vehicle. This occurred without any warning or adverse action by myself or anyone else. The vehicle has been in no collisions, and has only 6429 miles on the odometer. I phoned Toyota's "Customer Experience Center" to report the incident and to seek warranty service for this problem. I was told to drive the vehicle to my Toyota Dealer, which is located 115 miles away, for inspection. I explained that it is unsafe to drive a vehicle filled with broken glass and with no rear window 115 miles. If I vacuum up the glass, there is nothing for the dealership technician to inspect. If I tape plastic over the opening, it could come loose at highway speed, and cause an accident, and the tape adhesive could damage the vehicle finish. The Toyota agent advised me that the vehicle must be inspected at a Toyota Dealership, and if the vehicle is unsafe to drive to the dealer, we could have it towed. But in a subsequent call to the "Customer Experience Center", my wife was informed that Toyota would not cover a tow of that distance in their warranty, and it was our fault for buying a car and taking it to an area that far from a dealer. The phone number we called is located inside the vehicle owner's manual cover under a heading, "If there is not a Toyota dealer near you, please call the following number:" In the morning, once they are open, I will call the nearest dealership, 115 miles distant, to seek further guidance. Based on my background in both mechanical engineering and automotive failure analysis, it is my opinion that the window and its support system was not designed to withstand the prolonged sun and heat exposure experienced. The outside temperature reached at least 82 degrees F where the vehicle was parked.

- #11741894June 3, 2026SERVICE BRAKES
My 2021 Toyota Sienna Hybrid has an intermittent and dangerous brake failure where the brakes suddenly release pressure during normal braking. When slowing down—especially at lower speeds or approaching a stop—the brake pedal briefly loses resistance and the vehicle surges forward as if the brakes let go. I must immediately re‑apply hard pedal pressure to avoid rolling into traffic or another vehicle. The vehicle as a result can stop up to six feet past the intended stopping point in some instances. This issue is unpredictable and has occurred multiple times. It creates a serious safety risk. Toyota performed T‑SB‑0047‑24 (Brake ECU Reprogramming) on my vehicle, whereafter the brakes seemed to function normally for several months, but the problem resurfaced and occurred threee time in one week in May of 2026, which prompted me to bring it backk to Toyota for another look. The dealer has been unable to reproduce the issue and therefore has not repaired it. The veicle has been returned to me, and continues to have the problem intermittently at a random frequency. Toyota corporate Brand Engagement Center has been notified, and a case was opened (Ref # 260528002416). This case was closed however after telling me that the dealership connot reproduce the problem, therefore they cannot repair it. This appears to be a brake control defect involving the hybrid brake‑by‑wire system. Because the failure is intermittent and leaves no diagnostic codes, it is extremely difficult for the dealer to diagnose, but the safety risk is severe. I'd like to advise that I have owned and driven earlier hybrid models (2012-14) that never exhibited this type of brake failure. I'd also like to point out... an internet search of this problem reveals that this problem seems to be prevelant in newer Toyota hybrid models. This was confirmed by the service manager at my local dealership. I have not been informed of any continuing engineering efforts given the failure of the TSB.
