I am reporting a safety issue with my 2026 Tesla Model Y. The rear seatbelt system has experienced three separate mechanical failures, each requiring service and replacement. The vehicle was purchased new from Tesla, Inc.. Due to these repeated failures, I have had to adjust seating arrangements for my two children in booster seats, including temporarily relocating one child to the middle seat when a side seatbelt became inoperable. The issue has since recurred on the opposite side. This pattern represents a significant and ongoing safety concern.

I believed Tesla full self driving (FSD) was in effect. My Tesla proceeded at 35 miles an hour toward a stop sign and I had to manually stop the vehicle to prevent blowing the sign. It turns out that Tesla sent an email 2 days ago (April 7 2026) that notified me that FSD had been cancelled because my credit card was declined (I had to replace a card on April 7 due to a lost card). I did not see this email. There was no notification in the vehicle that FSD had been deactivated. When I thought I was engaging FSD, I was in fact engaging an autosteer mode with much lesser capabilities, essentially only acting like adaptive cruise control and lane guidance. Engaging this lesser mode is done in EXACTLY the same fashion as FSD, and any driver using it in my situation would always assume FSD was active. This problem, if not corrected will absolutely cause accidents and potentially fatalities. Tesla needs to inform the driver via the head unit that FSD is not active in an obvious, unmistakeable way.

The contact stated that they rented a 2022 Tesla Model Y. The contact stated that the vehicle was supervised in autonomous mode when it suddenly turned left while driving at approximately 45 MPH, causing it to hit an electrical box nearby. The vehicle then steered itself back onto the road and shortly after, turned off the road towards the right and crashed into a tree, where it came to a stop. There were no warnings or notifications that were displayed during the incident. The air bags did deploy. The vehicle did not allow the driver to correct the direction of the vehicle or disable the autonomous mode setting. The vehicle had caught on fire. The contact and the other passenger, sitting in the front passenger seat of the vehicle at the time, were able to exit the vehicle. The fire department was called and extinguished the fire. The police department was contacted, and a police report was filed. The vehicle was towed and totaled. The contact received a broken left wrist, a sprained right foot, and seat belt bruising to their lower stomach and chest area. The passenger in the vehicle received seat belt bruising. Medical attention was needed. The contact rented the vehicle through Turo and informed the person they had rented from. The contact was not sure if the incident was reported to the dealer or manufacturer by the owner. The contact stated their attorney contacted the manufacturer and informed them of the incident in an attempt to retrieve data information from the vehicle at the time of the incident. The failure mileage was unknown.

While performing a low-speed reverse turn into a driveway, a loud clunk was heard and the driver side front wheel completely separated from alignment. The vehicle became immediately undrivable. Upon inspection, the ball joint retaining nut was found to be absent from the front driver side suspension assembly. No torque verification mark was present on the fastener. The vehicle is currently at an authorized Tesla service center for repair and is available for inspection. Safety risk: Complete loss of wheel alignment and directional control. Failure occurred at low speed in a private driveway. Had this occurred at highway speed the consequences could have been catastrophic. Confirmed by dealer: An authorized Tesla service advisor confirmed that a Technical Service Bulletin exists in Tesla’s system for this exact condition. No owner notification was issued prior to failure. The advisor confirmed the retaining nut is installed by an automated robot assembly process with no human torque verification step and no verification mark applied. Prior warning symptoms: NONE. No warning lamps, no messages, no audible symptoms prior to catastrophic failure. The absence of a retaining nut on a safety-critical suspension fastener with no quality verification process represents a systemic assembly defect potentially affecting all vehicles produced under the same assembly process.

Then while still at light which was red the vehicle unexpectedly accelerated toward [XXX] on its even with feet on brake, I attempted to maneuver to avoid colliding with vehicles traveling on [XXX] , then made a sharp right turn on sidewalk of [XXX] drove striking a "Bee Line Bus" sign located in front of [XXX] . The bolts attaching the front passenger wheel's lower lateral link to the body fell out. Luckily this occurred while pulling into a parking lot. Once both bolts had fallen out the vehicle was not steerable. There have been many instances of this reported online, but usually they occur with far fewer miles and I was not experiencing the symptoms. The day prior there was an alert stating that an alignment was needed but it was still okay to drive. I could tell there was a slight alignment issue while driving. The vehicle was towed to a Tesla Service Center and is currently being repaired.

Vehicle: 2023 Tesla Model Y Long Range Complaint: I am reporting a serious safety issue involving a vehicle sold by Tim Dahle Nissan Murray in Murray, Utah. Approximately three weeks ago, I purchased a 2023 Tesla Model Y Long Range from this dealership. After purchase, I discovered that the rear seat belts were not functioning. I have evidence indicating that the dealership was aware of this defect prior to the sale. This is a critical safety issue, as seat belts are required safety equipment under federal motor vehicle safety standards. The vehicle should not have been sold in this condition. Additionally, the vehicle had an issue with the right rear wheel/tire losing air. When I returned to the dealership, they failed to properly inspect or repair the issue. Instead, they reduced the air pressure in the other tires to match the leaking tire, rather than addressing the defect. After leaving the dealership, the tire again lost air. Upon inspection by another service provider, the wheel rim was found to be bent. When the tire was inflated to proper pressure, it began leaking air immediately. This created an unsafe driving condition, as the vehicle could not maintain proper tire pressure. I was forced to have the vehicle repaired by Tesla at my own expense ($1,390). The combination of non-functioning seat belts and an unsafe wheel condition placed my family, including my newborn child, at significant risk. I am requesting that this matter be investigated as the dealership knowingly sold a vehicle with defective safety equipment and failed to properly address additional safety-related issues. Also a police report was filed but I do not know how to get ahold of that it was filed with the Murray police department

Car was in FSD mode, backing out of a parking place very slowly when it accelerated and crashed into a post and wall. No other cars or people were involved, but no alarms, beeps, or warnings were sounded and the screen/monitor/camera systems went blank simultaneously. This caused over $5000 worth of damage per the estimate provided by the only body shop in the area allowed to fix Teslas. There is also a gap in saved data per our retrieval from the USB and camera installed in the vehicle.

The contact was informed that the repair was not covered. The vehicle was not yet repaired. The manufacturer was notified of the failure. The failure mileage was approximately 52,000.

Getting message on screen since october 2025. That time total milage on my car was less then 30,000 STEERING ASSIST REDUCED and LANE DEPARTURE AVOIDANCE FEATURE UNAVAILABLE. I did reset / restart the vehicle / soft / hard reset, software update. and This both message got disappear and then again after sometimes its keep coming back. So i did appointment to rocklin tesla center and they gave me cost estimate $5770. Thats way too much so i decline but they said those message might keep popping up because of LOW VOLTAGE DISTRIBUTION AND CONTROLLERS. So i accepted that service and paid $359.68. But i am still getting same both messages on my tesla screen. Sometimes it does comes and sometimes it does not. I did online research and people with same problem stating that because of TESLA's poor manufacturing steering wheel and some kind of faulty circuit board causing this issue. My car never been to any accident and its very low on milage. Please look for other tesla owner who has same problem and tesla has to fix this problem without charging this much amount. Thanks

On December 2, 2025, my Tesla was charging in the garage of my residence in [XXX] , when a fire appears to have originated in or around the vehicle area. Before the fire, I smelled smoke/burning and also heard popping sounds. The fire spread rapidly and resulted in the destruction of my home, creating an immediate and severe risk to the safety of all occupants and anyone nearby. This incident exposed those present to danger from flames, smoke inhalation, toxic fumes, and structural collapse, and it caused emergency evacuation and displacement. The fire department identified the garage or vehicle area as the origin. The exact component or system that failed is unknown at this time, but the circumstances indicate a possible failure involving the vehicle, charging system, battery system, or related electrical components. I am not aware of any prior battery, charging, or electrical warning messages before the incident. Emergency responders and insurance representatives were involved after the fire. I previously attempted to notify Tesla of this incident, but I did not receive a response. Because this event involved a catastrophic fire and total residential loss, I believe it should be reviewed as a serious vehicle safety matter and that all relevant vehicle data, charging data, components, and related evidence should be preserved for inspection. I was parking my vehicle in my driveway, turning left to park in front of my garage and heard a loud pop. I got out, looked underneath the car and didnt see anything unusual. I later investigated by moving the vehicle and noticed my steering wheel was turning without any driver input, and heard the tires scrubbing more than normal. I got out of the vehicle and noticed my tires were not aligned, having one tire pointing to the left, and the other tire pointing straight. Then I noticed a large bolt on the ground. I called Tesla and they told me my car was 2000 miles out of warranty and would have to pay for towing and the repair. I feel the steering should still be covered under warranty.

Incident Summary: On April 1, 2026, while operating a 2025 Tesla Model Y at highway speeds (approx. 65-70 MPH), the front hood (frunk) spontaneously unlatched, flipped upward, and struck the windshield. The impact completely shattered the windshield, obstructed 100% of forward visibility, and caused structural damage to the hood and hinges. I was able to safely navigate to the shoulder by relying on side-view mirrors and lane-keep assist, but the incident constituted an immediate and life-threatening safety hazard. Failure of Safety Redundancies: Lack of Digital Warning: At no point prior to or during the drive did the vehicle’s software provide an "unlatched," "open," or "warning" notification on the primary display or via audible alert. The vehicle’s sensors failed to detect a compromised latch state. Secondary Latch Failure: The mechanical secondary safety catch, designed to prevent the hood from fully opening if the primary latch fails, did not engage or hold. The hood bypassed all mechanical and digital failsafes. Spontaneous Activation: The vehicle had not been manually accessed in the frunk area immediately prior to this trip, suggesting either a mechanical fatigue of the latch assembly or a software-commanded release error. Year/Make/Model: 2025 Tesla Model Y License Plate: [XXX] Previous Recalls: This failure appears identical in nature to NHTSA Recall 24V-554, which affected 2021-2024 models. It appears the remedy or part quality issues addressed in that recall may persist in the 2025 model year production. Nuna Exec Next car seat. I have the older version of this car seat for my older child and it works great. The new version ("Next") has an issue where the tigthening/loosening strap folds over and gets stuck, thus making it impossible to loosen the straps and remove the baby from the seat which is very dangerous for the child, especially in case of an emergency. This is not the way the item should work. I contacted the company, they sent me a full replacement. The replacement seat acts the same way, they are willing to send me another shell replacement. This is clearly a production/design issue - not a one-off seat issue.

Right rear passenger seat belt malfunctioned after owning the vehicle for one month. When I called the service department to discuss the issue, they said the part would take 5 days to come in. They denied me a loaner car to drive while I waiting for the part. The malfunction was confirmed by the dealership the following week and required replacement. I still cannot believe that a company who values safety, expected me to continue to drive my children in a car that had a malfunctioning seatbelt, until the part came in.
