Turn signal does not always activate. If I hit the turn signal stalk so it goes past the first detent (for three blinks) but not hard enough to get to the second detent (on until cancelled or turn completed), it does not come on at all, either the three blinks or constant blinking. This is dangerous because the turn signal does not activate predictably, especially when changing lanes on the highway. I should be able to tap the turn signal to get the 3 blinks to come on and then switch lanes, but when used this way it does not always come on. I had this inspected by a Tesla Mobile Service technician and he reproduced what I reported but said this is working as intended because it is detecting an "accidental bump" and not activating. My vehicle is available for inspection upon request. There are no warning lights or messages.

The car was delivered to me today. It has 15 miles on the odometer. The brakes do not work. I almost crashed into a home. Extreme safety concern. The next device appointment is July 20th. I will wait for them to diagnose the issue. Very concern this auto manufacturer is delivering faulty brand new products to consumers.

Was at a drive through and took foot off the brake and barely put any pressure on accelerator the car went out of control — sudden acceleration.. First time this has happened with this car. Was definitely not working properly. Less than a second was at full Speed. Had to swerve to try and avoid the car ahead of me. Entire left side of car is crushed and torn apart.

While driving at a normal speed and turning around at an intersection in our neighborhood, the car suddenly went out of control, causing the car to hit a fire hydrant in front of a home, smashed a tree before crashing into the siding of the home. Insurance deemed the car as "totaled". We were reimbursed by insurance so this is not about monetary losses, but a report to protect other drivers and their families. The Tesla database records mentioned user acceleration and error, but this doesn't sound like a reasonable cause. The airbags didn't open and neither did the automatic emergency braking or forward collision warning work. I was unable to brake and control the car to stop. This accident is captured in the security system video of the impacted home.

The adaptive cruise control (or emergency braking system) repeatedly braked for no reason. There were not other vehicles in the immediate area. I was driving on Interstate 90 at 70 MPH and the vehicle emergency braked down to about 50 MPH before recovering. This is a recurring problem and has happened at least a dozen times since I've owned the vehicle. I no longer trust the feature and hesitate activating it. The danger is risk of a rear end collision if another vehicle is close behind when the problem occurs. I have read that this "phantom braking" issue has been reported 700+ times. The manufacture (Tesla) has not inspected my vehicle. I believe a warning message was displayed but I am not sure. I was too busy reacting to the issue to notice any displayed messages.

Driving on major interstates across Idaho, Wyoming, and Utah, there were several instances of so-called “phantom braking” — while using Adaptive Cruise Control, with no other cars or obstacles in front, the brakes were rapidly applied, decelerating the car from roughly 75 MPH down to around 60 before I could manually press the accelerator pedal to override the behavior. Not only is this dangerous for myself, but could easily cause an accident if people are following closely at highway speeds. If Tesla’s camera-only system (as opposed to radar) can’t do the job, they should at least provide an option for a non-adaptive cruise toggle that would hopefully work around the issue.

Every two weeks or so, the screen goes blank. When parked, you can do a vehicle reset and after 2 or 3 tries the screen with turn back on and the car is able to be driven. Sometimes, the screen will go blank while driving. This will require the driver to pull aside, power down the vehicle, and try to reset it. This is a huge safety issue when the screen goes blank when driving. Tesla says there's nothing wrong, or try a reset, or the next update will solve it. When there is a bug like this on an EV it is a huge safety issue.

We just completed a 6500 mile round trip road trip from So Cal to Toronto Canada and back. 5/24/22-6/12/22 During the trip we used the "Auto Pilot" with and with out "Auto Steering" Not Full Self Driving. I lost count of how many times we encountered "Phantom Braking" with the car at highway speeds. My guess would be at least 50 times. It got to the point where I would not use Auto Pilot especially when there was heavy traffic at full highway speeds. My deduction on why this is happening is that the Tesla cameras are "Infrared" sensitive and it seemed that the car was applying the brakes when it would see the heat waves off the hot road surface. Also in early morning one day the sun was directly behind us and the cars shadow was directly in front of us on the road surface and it kept applying the brakes thinking it was another car. In slow moving bumper to bumper traffic going 30MPH or less does not seem to initiate the "Phantom Braking". But at highway speeds it is super scary and dangerous when this happens. ;(

Auto dimming and bright headlights do not dim for oncoming traffic consistently. Unsafe for other travelers at night. I’ve owned two Tesla’s model 3 and model y over the last 4 years. Both had issues with the Auto Bright/Auto dimming headlights. The automatic feature takes too long to turn on the high beams and takes too long to dim for oncoming traffic. The auto dimming function also reacts to reflective signs and turns off high beams when unnecessary. The auto dimming feature works best when you’re approaching a curve where the travel path is to the right where oncoming traffic crosses the field of cameras. Auto dimming rarely works when turning to the left or straight for on coming traffic since cars are to the left of field for the cameras or head on traffic but Auto dimming feature fails. The bright and dim will go on and off as cars approach further causing issues with people flashing lights at you in their direction of travel. The auto high headlights also react to reflective signs and will go high low high low further causing safety issues when they should remain on.

I am reporting 3 incidences of Phantom braking with adaptive cruise control on, and 2 incidences of phantom vehicle displays on the central console while parking into a garage. 4/17/2022 ~ 1pm. on RT 101 CA between King City and Camp Robert Rest Area/Bradley. Traveling at 75-78 mph on widely separated divided straight highway, with no trees, only low grass, and no cars nearby, we (my wife driving) experienced two sudden jarring braking slowdowns. This is a hazard if there is a car tail-gating us or if the following driver does not expect us to brake for no reason. 4/18/2022 ~ 11am on Rt 10 CA between Palm Desert/Indo and Desert Center, We were traveling at 76 mph on a widely separated divided straight highway, with no trees, and only low grass. A Tesla car in front of us suddenly braked for no apparent reason and shortly thereafter pulled over to the shoulder and stopped. A few minutes later we experienced a sudden braking action in our Tesla car. In May/2022 we observed two separate incidences of a phantom van displayed on the Tesla central console while parking our car into our garage. A phantom van appeared once on the right side and once on the left side when the Tesla car was half-way into the garage. We park in the center of the garage which is relatively empty with 2 plastic tool boxes on the left side and a Traeger grill on the right side. The garage is 19' by 19' with a 16' wide garage door opening.

On June 28th 11:50pm with 174 mile range, my Tesla Model Y LR died while driving on fwy 5 with my 10 year old. I barely had time to exit, it gave me warning “PULL OVER SAFELY” “VEHICLE SHUTTING DOWN”. I barely made it to off-ramp, my car completely shutdown at the end of off ramp. Could not put it to neutral, me and my 10 years old had to exit the car immediately, called tow truck via tesla app. Tow truck showed up in 20 mins, can was not drivable. This could have been much worst, if I was not able to exit and my car stuck on middle of the freeway at night. This is a huge safety concern.

My vehicle brakes suddenly for no apparent reason. Happens usually on highway, usually when on self driving or auto navigation mode but also in cruise control rarely. It will go from 65 or 70 mph to 35-40 mph all of a sudden without warning without any vehicle close by. I notified Tesla multiple times. They said “it happens”. They asked me for exact date time location, I gave it, it still happens so I gave up and don’t use self driving much I cannot now tell you exact date time but if it happens next time I can keep records

Purchased a Tesla Model Y approx. 3 weeks ago and within the first day experienced sudden, aggressive braking on the freeway with nothing around and no apparent reason for braking at all. This has happened at least ten times in the three weeks we’ve owned the vehicle. It’s very scary when it happens and we’re very concerned it will cause an accident with my family and small child in the back. It happens when the car is in auto pilot and adaptive cruise control. The car gives no alert and no indication that it’s going to brake hard all of a sudden in the middle of the interstate at 70 mph it will instantly drop speed by as much as 15mph or more. Thankfully nobody has been behind us yet. For us this is an extreme safety concern and we have expressed this to Tesla and I have sent out a maintenance technician today who took it for a simple test drive and said there’s absolutely nothing wrong with the car and that’s the safest car in the world and that what we were experiencing is normal behavior for a Tesla and to be expected. He said when it happens to stereo on the accelerator to get it to stop which is ridiculous. This is a known problem generally referred to as phantom braking and I was assured by the sales rep before accepting the car that this doesn’t happen in auto pilot and nothing to be concerned about. Now here we are stuck with a dangerous vehicle that they can’t fix and won’t take it back. Please help get these cars off the road until they fix this huge safety issue. I am shocked there has not been a recall. I will also file a complaint with the state of Washington.

I had the Tesla on autopilot on interstate I-80, it was raining. The car was in the middle lane. It suddenly started to slide to the right and hit a truck in the right lane. I tried to regain control of the vehicle and hit the brake hard, but it did not seem to work. Tesla clipped the truck and continued to slide off into the guard rail.

I was traveling on a highway when my Tesla Model Y abruptly slammed the brakes for no reason while Autopilot was engaged. The vehicle slowed down very aggressively, this could have caused an accident if there was another vehicle following close behind me. After the first incident, I blamed a dirty windshield/camera lens blocking the systems ability to see what was ahead. But it happened two other times on different days. At this point I was convinced that there is an issue with Tesla's Forward Collision Warning system. On a different occasion, I was driving under an overpass and the shadow on the pavement (overpass shadow) triggered the collision warning and once again aggressively applied the brakes. It is very unfortunate this is happening on this vehicle. I love my Tesla Model Y, but I am very hesitant to use Autopilot due to these issues. I personally like Tesla because they are using advanced technology to make driving a lot safer for everyone on the road. However, it is our responsibility to report these issues so the manufacturer can investigate the root cause. I hope this comes to a solution soon because it is a very dangerous situation that can cause harm to me and my family as well as others on the road. I will point out that I have been fortunate there were no close vehicles in behind me during the three separate times this occurred.

-Auto pilot (adaptive cruise control). Yes it is available for inspection. -Vehicle unexpectedly slowes down on highways (when using cruise control) at an unsafe rate when there is nothing in the path of the vehicle. This has cause multiple instances of vehicles almost rear-ending me due to the rapid deceleration. -I have asked a Tesla Service Center representative about the phantom breaking and they stated it was a known issue and that was being worked on. -No, the vehicle was not inspected by the manufacturer since we were advised it was a known issue. -There is no warning lights or sounds before the vehicle suddenly brakes. I suspect that hills, shadows, on coming traffic on two lane highways are some of the causes for the phantom braking.
