Numerous times we have experienced phantom braking when the cruise control is engaged at highway speeds. We think it occurs when you are close to cresting a hill and a vehicle is detected in the distance. The vehicle is quite distant, perhaps 500-1000 ft, but the auto braking system only see a short distance of lane lines (because they are out of sight on the other side of the crest) and interprets the vehicle to be much closer. This generally occurs about once every 100 miles of highway driving. Because of this, we are reluctant to use the cruise control. The date provided below is just the latest occurrence.

When we were driving on the highway at 75 mile speed with autopilot engaged, the car breaker suddenly from 75 to around 50 instantly. This happened twice in one hour. Fortunately, there were no vehicles behind us at that time.when complained to Tesla, they came back and said they did not see anything from their remote hardware diagnostics.

I was driving highway on 2/13/2023 around 11:12am. Traffic was clear, weather was good, I was using autopilot with Tesla model Y. The car decided to break at highway speeds for no reason. The breaking was very aggressive and I was very scared. There was no accident but something is not right with autopilot of Tesla. The car is new and using latest available software update.

When autopilot is engaged the car slows down without warning and there isn't anything in front of the car. We had close calls with cars behind us when the car applies the brakes for no reason. The wipers would come on for no reason and as long as the autopilot was on it couldn't be turn off. So we had to disengage the autopilot and turn off the wipers and drive another 300 miles without autopilot for our safety.

After having strapped our kids to their car seats, my daughter had her feet resting on the rails of the driver seat in front of her and her feet were trapped and squished really hard when the driver seat started to automatically adjust between my driver profile and my husband's profile. I am 5'3" and he is 6'3" so his profile has the seat further back than mine. My daughter started to scream and cry immediately as we tried to quickly reset the driver profile to mine so she could free both of her feet. She had slight bruising and soreness after the incident. This is reproducible and the only way to prevent was to delete my husband's driver profile to avoid having his phone trigger the self adjusting mechanism when he opens the driver door. The vehicle has not been inspected for this issue by Tesla or any one else. We believe this is a feature that is by design but it poses a risk to anyone sitting in the back and unable to rest their feet on the floor such as a kids in their car seats. There are no warnings, messages about not resting your feet on the seat rails and the seat doesn't stop and doesn't detect if something is on the rails. I couldn't find a way for the driver profile to be locked to a single driver and be manually triggered for any other driver. I tried to search how to report this to Tesla and I couldn't find an obvious way to do it. The number listed in the manuals doesn't have an option for this. I couldn't find an option to report via the Tesla app.

Car braked aggressively and unexpectedly on highway in full traffic with autopilot (lane keeping cruise control) for no reason. Car in front was over 10 car lengths away and not slowing. Almost caused collision with car behind me but I took control immediately and hit accelerator. Night time driving - approximately 530pm on route 64/1 headed west in Raleigh nc. My car is “vision only”. Extremely unsafe.

Failure: rear motor disabled. Verified customer concern. Inspected rear drive unit and found inverter had internal issue causing failure. Replaced rear drive unit inverter and verified vehicle now operates as expected. Safety: car speed reduced to 68mph max and the regenerative braking barely worked. All functionality like cruise control/adaptive were restricted Yes - reported to Tesla dealer/service center and had verified and fixed under warranty No warnings prior. Driving down the road I heard a loud clunk and then a warning appeared saying power reduced, rear motor disabled. Ok to drive.

A backup camera is required on all new cars. Tesla has a backup camera system comprising 3 views. When reverse gear is engaged, camera views should appear every time on the instrument display. On the first drive of a given day, the camera views are not properly visible until the car has been driving for a few minutes. Specifically, “night mode” on the infotainment display (effectively the instrument cluster) becomes stuck on dark colors and unreadably low brightness. When reversing during daylight hours, the screen is not bright enough to view the backup cameras. Additionally, audible cues for turn signals are inaudible. After about 1/4 mile of driving, the display “catches up” to day mode and responds with appropriate brightness. I believe the dysfunction of the system is a safety hazard to vehicle occupants, other drivers, and pedestrians. I have previously contacted Tesla service about this issue. I was told that the issue is related to software; nothing can be done; it will be fixed at a later, indeterminate date via software updates. The issue persists consistently and has no fix aside from a workaround.

The high-beam headlights are randomly turning on while city driving. The auto high-beam function is turned off, so high beams should not turn on. This affects the safety of the other cars around me. I typically drive in the city and using the high beams is not necessary because of dense traffic and available overhead street lights. This issue has not been confirmed by a dealer, service center, manufacturer, police, etc. There is no warning before the high-beams turn on. When they turn on, the blue high-beam indicator on the driver display does illuminate.

SYSTEM MALFUNCTION: Emergency braking ("phantom braking") while set to automated driving/adaptive cruise control. OUR SAFETY PUT AT RISK: On a 2,000 mi. road trip from Denver CO to Tucson, AZ and back, the vehicle slammed on the brakes at 75-80 mph speeds over two dozen times. All incidents occurred either driving south on I-25 or west on I-10 going to Tucson, or on I-10 east and I-25 north on our return. By far the most dangerous event occurred when we were driving east on I-10 in SE Arizona on our return to Denver on 29 January. Driving in the right lane at 75 mph on a perfectly clear day with excellent road conditions, the vehicle suddenly slammed on its brakes. Following closely behind us was a semi-tractor-trailer, that also slammed on its brakes, blasted its horn, and very nearly rear-ended us. The truck driver then followed us at very close range for the next ten miles, If we slowed down, the truck slowed down. If we speeded up, the truck speeded up. We disengaged cruise control so we wouldn't involuntarily brake hard once more. Finally, the truck drive honked once more and dropped back as we sped up. In our view, this was an evident case of road rage on the trucker's part that might have ended tragically. REPRODUCTION OF PROBLEM: We have a 2/27/23 appointment with a Tesla service center here in Colorado to check out the automated driving/cruise control/camera and sensing system. However, this problem only arose under quite specific topographical, and road surface/light conditions on I-25 and I-10 in rural areas, so I'm doubtful Tesla service techs would be able to replicate the conditions. WARNINGS: There were no warnings emitted by the vehicle either audibly or visually (on the display screen) prior to any of these incidents.

Windshield easily to crack by road debris while driving. Dropped off my vehicle for replacement windshield and tesla agent informed me that there was numerous vehicle ahead of me waiting for replacement. Also wipers activates when is not raining or won’t stop after it stops raining while in Auto causing distraction to the road.

After leaving the vehicle plugged in and coming back to it after an extended period of time, The vehicle’s camera is stuck on black screen with spinning circles and gps stuck on last location. Only resolution is to walk away and the vehicle goes into a state of deep sleep. This is a major safety issue because the rear windshield is smaller than a typical standard vehicle, so without the cameras, visibility in the rear is almost non existent especially during inclement weather. Vehicle was brought to service several times to address the issue. Today will be the third attempt to remedy the issue. Issue is ongoing and has been existent since vehicle possession.

While driving on the highway at 70mph, on two separate occasions while using autopilot and cruise control, the car suddenly braked for no apparent reason. There were no vehicles immediately behind me at the time, however this was terrifying because I was scared that I was going to be rear ended at a high speed. I can no longer trust this safety system due it has happened twice randomly now.
