FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
We had 2 phantom breaking occurrences while in Autopilot traveling at 75 MPH within 10 minutes. The first time we were passing a pickup with trailer on the right and the in the second a pickup was passing us on the left. in both cases there were no other vehicles or objects on the road in front of us. If there was a vehicle following close behind us there could have been an accident. We did not come to a complete stop but I am not sure if that was because of Autopilot or my reaction. There were no other warnings.

Auto wiper doesn't turn on for low or medium. Defrost doesn't clear windshield in bad weather conditions. Above conditions make it difficult to operate in bad weather conditions due to poor visibility. As per manufacture, everything working as designed but problem exists. Vehicle brakes suddenly while driving even when there are no cars or obstacles of any kind in front or even in the distance. This has led to several events where I was almost rear-ended because of how strongly and violently the car was breaking on the highway. Last example was on March 5th 2023 at 1:19 p.m. while driving on interstate 90 in Ohio going westbound, there were no cars in front or in the distance in the lane. The car suddenly applied the brakes in a very strong fashion in the middle of the highway. There were at least five examples just like this one which happened prior.

I was driving on East on I-8 and was East of Yuma, AZ. I was using the enhanced auto pilot feature. I was traveling 70 miles per hour (5 below the speed limit) There were no autos in front of me and nothing on the shoulder. There was a semi truck behind me (I am not sure how far back.). My car suddenly put on the brakes and rapidly slowed down. (I am not what speed) The de-acceleration was so severe that my coffee cup too flew onto the floor. The semi truck was able to move to the right lane but blew his horn loudly. It was extremely scary and dangerous for both of us. I have done some research and this seems to be a problem that many Tesla owners are experiencing. This issue needs to be fixed immediately.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
This has been an ongoing problem with our car. While using cruise control it will randomly slam on the brakes for no reason. Nothing is around us it just instantly takes the car from 75 to 60 without warning. It happened multiple times to us over the past few days. Luckily I am nervous about using cruise control in this car so I had my foot on the gas but it did happen with a semi behind us. I was able to manually correct the problem myself quickly but it could have been very bad. These cars are going to kill someone if Tesla doesn't fix this problem soon. So far they have not cared about fixing this problem at all.

Hello, I was taking my car reverse and my car door was open slightly and it hit my garage door, and that was my mistake. My only problem here is, the driver side rear window broke and it broke into MILLIONS OF PIECES and shattered all over inside.the car. Not sure what the technology is, and it should not break into millions of tiny, tiny pieces. It's a brand new car and struggling to clean it because they are no minute. Thanks

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Multiple "phantom braking" events while using adaptive cruise control (no full self driving, just basic adaptive cruise). First time while going at interstate speed and crossing a bridge, vehicle saw a shadow on the road and slammed on the brakes thinking it was a vehicle. Obviously this is a startling and dangerous situation especially for vehicles behind you. This has also happened on the highway, flat terrain, roads were clear of any obstacles and it slams on the brakes thinking there is something in the road. Every time it does this, there appear to be shadows on the road (from street signs, light poles, trees, etc.). The sudden braking is of course bad, but there's also nothing in the vehicle that tells you why, no chime, it just hits the brakes. It still appears that cruise control is active, but it won't resume speed after an event like this until you turn cruise off and back on again. I know this isn't an isolated incident and it absolutely will cause a crash, if it hasn't already.

LANE DEPARTURE: BLIND SPOT DETECTION, BACK OVER PREVENTION: WARNINGS, VISIBILITY/WIPER
The Tesla Model Y Blind Spot warning system does not accurately detect the position of cars in the lanes next to the car. The display shows the car as behind the vehicle while it is actually at the rear quarter panel. There is only an audio warning if the turn signal is activated. The car is in the blind spot yet the system does not warn the driver. I took the car in for an adjustment to cameras and Tesla said they are functioning as designed. Tesla removed the radar sensors that aid in parking at low speeds. This was intended to be replaced with Tesla Vision. Tesla Vision functionality is not yet available. Although a low-speed collision with an object may not result in injury to a person, it is a safety feature that has been removed. The windshield wiper's speed cannot be adjusted or even started on the stock. To change speed or turn on the wipers you must look away from the road, locate the control panel and then select the desired speed. This clearly is a safety hazzard.

It seems like this is a common problem if you search "tesla phantom braking," which very obviously needs a solution. I commonly experience the phantom braking when there is a shadow like passing under a bridge. The car will slam on the brakes thinking the shadow is another car, which is obviously extremely dangerous especially on the highway. A simple solution would be to add a legacy cruise control option, while fixing the phantom braking. This would cause a lot less wrecks and deaths.

On February, Friday 17th, 2023 around 10:15pm on 287 exit ramp towards rte 78 west , a truck with a flatbed suddenly went from right to left but the car failed to detect that and we almost collide into the back of it. Collision was avoided only by driver reaction. All collision warning systems failed to detect the situation. Still this is the same behavior we have seen since we bought the car. There is no awareness of close proximity objects. Tesla is acknowledging that critical ultrasound sensors were removed from the car.

Driving 70mph on the highway, car abruptly clunked and three warning messages appeared. APP_W009 Automatic Emergency Braking is disabled; DIF_A062 Power reduced; and BMS_a170 Acceleration and top speed recuded. Car immediately dropped to 40mph and gave me enough time to exit the freeway to the shoulder. After putting in park, the car would no longer shift into any gear. Soft reset and full reset, exiting and re-entering the car didn't fix the issue. Tow required. Failure of rear motor (inverter) was cited in the service center. Seems to be a common issue across Model 3 (as far back as 2017), Model Y (deliveries in Nov/Dec 2022) and Model S (the last 3-4 years). Very dangerous if someone had hit us when it decelerated hard on the highway. Traveling with a 6yr old in a car seat. Only 500 miles on the car.

On February 20, 2023, my infotainment system console and screen completely failed as I was backing up to a Supercharger. This started with the typical lag I have filed service complaints for before, the camera occlusions, and then nearly caused an accident with the barrier pole in front of the Supercharger. This is the eighth near miss in over a year of ownership and the 27th occurrence of issues related to the infotainment system console and screen. Often, it is burning hot, other times it is woefully underpowered to accomplish anything in a timely manner (if at all) when these incidents occur. I have been to the Tesla service center twice about this very issue. Each time, I was told software update would be issued that would fix the problem(s). That has never been the case. With everything so reliant upon that center-mount display and its associated computer in this model of vehicle, it HAS to be considered a must-not-fail component to the extent humanly possible. I am taking this vehicle in for a third time for this and multiple other issues that seem to some degrees associated with the issue. After badgering the Tesla helpdesk personnel with demands to replace or fix the issue, they have told me on February 23, 2023 that they will be replacing the computer next week (whether this is to evaluate the device for issues or to hide evidence to their culpability in an incompetent design flaw, I cannot be sure).

The vehicle heating/defrost system is not functioning in cold weather. It only functions in my heated garage. In cold weather it cannot defrost causing visibility issues, driver distraction (trying to get the heat/defrost to work while driving) and a reduction in blood flow to the extremities. The heating system blows freezing cold air into the cabin when on. I have written a lemon law letter to Tesla, demanding the vehicle be replaced under Alaska law, and they have not done so after 80+ days. The vehicle’s supermanifold was damaged the day we took delivery. Tesla has not honored Alaska law and I’ll be filing paperwork with the state’s attorney general’s office. Tesla should not be allowed to operate in the State of Alaska due to committing an unfair trade act in the State of Alaska and endangering the lives of their customers.

I have experience multiple instances of "phantom braking" where the car suddenly brakes when there is nothing in front of the car. The most concerning incidents are when traveling on interstate highways while Autopilot is engaged. The car will brake suddenly, rapidly, and unexpectedly. I experienced this problem that first day I picked up the vehicle and continue to experience the problem on extended interstate drives of several hours. The car is available for inspection upon request. The biggest safety concern is that a closely following car could rear end the rapidly braking Model Y. Another concern is that the car could loose control on ice or snow while rapidly braking. Despite three requests, Tesla has declined to inspect the vehicle. During these incidences, none of the cameras have indicated that they were obscured nor were any other warnings indicated that I am aware of.
