1. Phantom Braking occurring often and no release date for firmware update. Not sure how to avoid rear end collision even when attentive. 2. Inaccurate speed limits, forward collision warnings, inaccurate green light chimes. 3. Autowiper which is working for other manufacturers over decade is beta in tesla. Previous 5 service center visits confirm as working and in 6th visit they say as beta. This is cheating customers. Using display screen to control frequency is distraction in wet roads.

I feel Tesla is building cars with non-standard parts and structural issues. 1.I bought a new car and started hearing brakes noises from 6000 miles where as previous car brakes rotors were good till 70k miles. 2.Within 9000 miles, driver side door, A pillar, B pillar, rear seats, display panel has to be either insulated or replaced. I am not sure how this effects car value or vehicle integrity going forward.

The contact owns a 2021 Tesla Model Y. The contact stated that on various occasions, while driving at various speeds and making turns, a squeaking noise was heard in the front of the vehicle. No warning lights were illuminated. The vehicle was taken to a local dealer, where it was diagnosed that the braking system needed to be adjusted. The vehicle was repaired; however, the failure persisted. The contact stated that the squeaking was also heard while exiting or entering the vehicle or if pressure was added to the hood. The vehicle was taken back to the dealer, where it was diagnosed that the front lower lateral link assembly needed to be replaced. The vehicle remained at the dealer awaiting the repair. The contact became aware of NHTSA Campaign Number: 21V835000 (Suspension); however, the VIN was not included in the recall. The manufacturer was made aware of the failure and advised to file a complaint with the NHTSA Hotline. The approximate failure mileage was 47,500.

The plugs and door latches on both rear doors have repeatedly failed, endangering the safety of my family and other occupants of the vehicle. I am concerned about the safety of drivers and passengers and believe that this issue requires your immediate attention. The plugs and door latches on both rear doors of my Tesla Model Y have failed repeatedly. The failure of the door latches and plugs has repeatedly endangered the safety of the occupants, particularly children who may be unable to exit the car in an emergency situation. The door trim falls out of place, locking the occupants in the back of the car. This problem has been reproduced and confirmed by the dealer, as evidenced by my car's multiple visits to the service center for the same issue. Attached are the documents, they are all for the same issue - though tesla classified them inconsistently. The vehicle has been inspected by the service center, but the problem has persisted despite their attempts to fix it. There were no warning indicators prior to the failure, making the problem even more dangerous as it can occur without the driver's knowledge. I am deeply concerned about this safety issue, particularly given that the 2021 Tesla Model Y is a popular vehicle that is widely used by consumers across the United States. It is vital that action is taken to address this issue and ensure that all affected vehicles are recalled and repaired. I strongly urge the NHTSA to investigate this issue and require a recall of all affected vehicles. The safety of drivers and passengers must be the top priority in the design and manufacture of all vehicles, and it is clear that the Tesla Model Y's rear door latches and plugs are failing to meet necessary standards of safety.

Around 3:54 PM EST, I was traveling on US 1 Route 1 North Bound, and I tried to turn right into Target Complex (4196 US-1, Monmouth Junction, NJ 08852) from Route 1. I slowed down to turn into the complex, and my vehicle had Sudden Unintended Acceleration while I am turning into a parking location. It jumped over the curbs and hit a tree. Airbags got deployed, and the car was damaged badly. Insurance deemed it a total loss. I saved videos from the crash from the USB. I can forward or upload them.

I had the car for exactly one week. I was driving the vehicle in the street, and my vehicle jerked and stopped in the middle of the intersection and a warning sign came on “rear motor disabled”. Then stated it two or three more times, stating the vehicle is safe to drive under 50 miles, and no steel grades. I do not feel safe driving this vehicle at all at this time, and feel that Tesla needs to replace my vehicle as the vehicle is a week old, and the motor isn’t working. This is a huge safety concern as it could’ve caused a major injury accident, especially if I was on the freeway.

The incident happened on 4/9/2023 at 2:30 pm. I drove in I-10 from Phoenix to California with Cruise Control and Autosteer ON. The speed was set to the highway speed limit. There were three other passengers with me. This seems to be related to the known issue of "Phantom Breaking" in Teslas. I was driving on a 4-lane road with no traffic, then the car suddenly reduced its speed from 65 mph to almost 30 mph, causing a sudden jolt to everyone in the car. There was nothing in front or reason for the car to break. No incoming traffic. It does not seem to have detected a "ghost" obstacle as an emergency. As there was no beeping or emergency message on display. No warning of any type. Luckily, no car was following me; otherwise, it could have caused a rear-end collision.

While using the cars adaptive cruise control, or enhanced autopilot as Teala calls it, the brakes would engage randomly and unpredictability while driving highway speeds (phantom braking). There were no cars, objects, or any other hazards around to warrant this "phantom braking". This has happened dozens of times while taking road trips.

On a multi day drive across the country I had multiple episodes of automatic breaking while on autopilot and using Traffic aware cruise control. In all instances there were no vehicles in either lane ahead of my vehicle. Braking episodes did not result in an accident but made the features essentially unusable.

Car making sudden braking moves while on trip using cruise control. I tried several different lanes and actions but braking persisted and cruise control discontinued. No damage or injury. Tesla has known phantom braking problem braking. We were at risk to be rear ended. I will contact Tesla next.

Severe phantom braking incident - 12:18 CST, March 12th 2023, followed by less severe braking problem at 1:16, 1:23, 2:18 on the same day and road. No forward traffic, no hazards, just bright sun. The car with a less experienced driver applied hard braking nearly causing a rear end collision with a truck approaching from the the back. The truck moved into the shoulder and passed to avoid hitting the car.

I was using the advanced cruise control (autopilot without steering assistance turned on) on a four lane highway with light traffic on a clear partly cloudy late afternoon. The speed limit of the highway was 75mph which was my set speed. For no reason at all my system slammed on the brakes, hard. Tesla refers to this as "phantom braking" according to online reports. This almost caused an accident as the car behind me had to very quickly swerve out of my lane. This is unacceptable behavior for an ADAS system to push the brakes so hard without warning or reason.

- #11516203April 9, 2023SERVICE BRAKES, ENGINE, STEERING
FATAL TESLA BRAKE FAILURES. The brake of Tesla suddenly failed while I was driving, it has happened multiple times even after the service center claimed the issue was fixed. When the issue happened, the brake was not effective and made huge noise while I pressed it. Meanwhile the steering got pretty heavy, other components failed(front motor disabled, traction control disabled and stability control disabled) at the same time as well. Tesla Service Center has tried to fix this issue once, but the issue happened again after ONLY ONE WEEK since they fixed it. Luckily I was driving slowly on a local road both times, but it was apparently fatal if driving on a highway. I've uploaded the error message on Tesla screen and repair invoice as attachments.

I was driving on free way with Autopilot (Adaptive Cruise Control) set at 79 mph. Without warning, the Model Y suddenly quickly decelerated. There was no vehicle or barrier in front of me. I was able to quickly response and started to step on the Accelerator and noticed the speed dropped to about 70 MPH. There was no sound alert during this process. It was about 8:20 in the morning and I was driving east towards the sun. My daugher was with me in the vehicle and experienced this scare. There was no vehicle closely behind me, otherwise this may cause collision. The vehicle functioned normal afterwards.

At highway speed, using the autopilot feature for speed control only, the car applies severe braking when there is nothing obstructing the road or requiring brakes. This has happened dozens of times over the first several thousand miles we have owned the new car. We filed a report with Tesla and they said this is not a problem and is expected behavior. We are concerned this will occur with a vehicle behind us resulting in being rear ended.

Suspension has a defect from factory that prevents proper alignment. Per alignment shop the car cannot be aligned to factory specifications. Premature tire wear is potential safety hazard.
