I have reported to Tesla how dangerous it can be long term to have one pedal driving and to use the accelerator as a cancelation mode to it’s many errors, see uploaded example picture. Also, having lighted notifications on mirrors causes people to subconsciously ignore actual blinkers of other vehicles; the only thing that should be on a side mirror is an adjustable fish eye mirror that allows to see the entire side lanes and tire placement.

Recently I had three back to back near miss head on collisions on highway early morning still dark. These three along with a previous near miss the vehicle had zero reaction to the imminent collision with roughly half a second to spare. It happened so fast that I must have let go of the record button as there was no saved video. I am working with lemon law firm and class action firm, get your crap together because these recent accidents reported in AZ are also your fault for not reacting appropriately to previous complaints.

While driving home, my Tesla was operating in Full Self-Driving (FSD) mode and initially performed normally, stopping at a red light and proceeding smoothly when it turned green. Moments later, the vehicle suddenly accelerated on its own, lost control, and began spinning uncontrollably before sideswiping a parked car and coming to a stop on the wrong side of the street in oncoming traffic. I was fully awake, alert, and did not manually steer or accelerate at any point. The incident appears to have resulted from a malfunction in the vehicle’s FSD system and/or its steering and acceleration control mechanisms. There were no warning lights, error messages, or alerts before or during the event—it occurred without warning. This malfunction created an immediate safety hazard that endangered my life and posed a serious risk to other drivers, pedestrians, and surrounding property. The situation could have easily resulted in a fatal collision or injury. I promptly notified the police, and the incident was reported to Tesla Insurance, which serves as my coverage provider. Despite my repeated efforts to have the issue inspected and addressed, Tesla Insurance has refused to acknowledge fault, open a claim, or cover related damages. As a result, the malfunction has not been reproduced or confirmed by Tesla or any authorized service center. The vehicle has since been inspected by Collision Works, a Certified Tesla Collision Center in Long Beach, California (500 E. Anaheim St, Long Beach, CA 90813), which provided a preliminary repair estimate. I am currently responsible for repair costs out of pocket pending resolution with Tesla Insurance. The vehicle remains in my possession and is available for inspection by Tesla, investigators, or safety authorities upon request. The complete and unexpected failure of the FSD system to regulate acceleration and steering represents a serious safety defect that warrants immediate investigation to prevent future harm.

On [XXX] at approximately [XXX], I was driving northbound on [XXX] in Phoenix, Arizona in my 2025 Tesla Model 3 using Tesla's Full Self Driving (FSD) (Supervised) mode, when the incident occurred. I was paying close attention while driving in the left-most, non-HOV lane coming up to a bend just before the [XXX] overpass, when I noticed traffic ahead slowing down and coming to a stop. I monitored FSD as it went through the act of braking; however, it seemed to wait a little longer than I would have expected before it actually started to brake, and when it did, the FSD system had to brake more aggressively than I've previously experienced. As it neared the end of the braking event (while the car was still moving), the FSD system disengaged itself without any input from me (neither by manually applying the brake pedal, applying torque to the steering wheel, or pressing the FSD button). When the FSD system disengaged itself, my vehicle continued forward. Had I not been paying attention, my vehicle would have collided with the rear end of the vehicle in front of me. Since I was paying close attention, I immediately slammed on the brakes after noticing that FSD had disengaged itself. I have dashcam footage of this event from the TeslaCam system; however, it does not include audio or telemetry data. Even without that, you can see in the video that my car noses down while FSD applies the brakes, then noses up when FSD disengages itself, and then noses down again when I apply the brakes manually. (The dashcam footage exceeds the 20MB limit; however, I can provide it via other means upon request.) It should be noted that I am an experienced Tesla FSD user, as I used it for over a year in my prior vehicle (2018 Tesla Model 3, which had been upgraded with the HW3 FSD computer), and this is the first time I have ever experienced a near collision event while using FSD or an event in which FSD disengaged on its own. The 2022 Model 3 Tesla does not have readily available emergency rear door release. Tesla's automated response to me was to locate the bolts and screws on the underside of the door armrest, then I need to remove the bolts and screw. Once that is done I can then remove the panel under the armrest so I can then reach inside and pull on the manual release cable. This could not be done during an emergency, you would need to have tools and in an emergency people panic and they would have to do everything by feel if they are trapped in the backseat of the Model 3 Tesla. This is a life safety hazard and needs to be addressed by Tesla to provide a fix that will allow easy access and not require tools to open the door if there is no power to the door release. This needs to investigated and corrective actions taken to make Tesla comply with changes for safety.

My family and I were in my Tesla Model 3 2022 and heading from Charlotte to Orlando on [XXX]. The car was in Full Self-Drive (FSD) mode while commuting through Daytona Beach. The rain was heavy in the area. While driving in Daytona Beach, FSD caused the car to change lanes and go to a lane that had standing rain puddles on [XXX]. After FSD caused the lane change, the car immediately hydroplaned out of control. The car's short clearance from it's bottom and the ground which traps water that exacerbates hydroplaning, along with its generative breaking feature made matters worse and uncontrollable. The car spun into the median three times and settled in the middle of danger interstate. The car is totalled. My family and I require medical attention and counseling due to the trauma. The push type turn signals for the model 3 almost caused when accident when I had to enter and exit a roundabout. The orientation wasn’t clear and I was trying to signal to not exit instead - another car misunderstood my intent. this miscommunication lead to a close call. I’ve been driving for over 20 years - this is an unsafe implementation of something basic.

My Model 3 was T-boned in an intersection, hitting both the front and rear doors, as a result of a vehicle running a red light. The driver that hit us was speeding and didn’t hit brakes. The impact pushed the Tesla almost in a complete 360 on an adjacent street where the driver side of the car jumped a curb and stopped after hitting a pole slightly on the driver side. Red light etc all substantiated by police report. Airbags deployed on the passenger side from the seat and upper panels. Front airbags failed on both sides. There was also a delayed airbag deployment from the back of the driver seat that only ripped the seat but never deployed fully. The smell of fire / burning and debris was all that happened from the driver seat. I requested a crash report from Tesla which outlines the speed and airbag deployment. The report says that knee level airbags deployed but they never did. I crossed referenced crash test speed data and various complaints about Tesla airbags. The data reviewed seems to indicate that I experienced failed equipment which is currently under review. I’ve also reviewed various sources of information about 7 2024 Model 3 driver side airbag known issues, where Tesla only recalled one vehicle. I’ve been told that other recalls for this issue are u see evaluation and to keep checking my vin for details. I share this because serious injuries can be caused by this failure, especially when hit by an SUV. The vehicle was totaled, not drivable and towed from the scene. Please be cautious if you have a 2024 model 3 and demand that Tesla look into the airbag manufacturing issues promptly and issue relative recalls. The collision alert also failed.

The contact owned a 2023 Tesla Model 3. The contact stated that while driving at an undisclosed speed on the third floor of a parking garage, the vehicle suddenly accelerated to full speed unintendedly, and the steering wheel and the brake pedal were inoperable, causing the contact to lose control of the vehicle. The vehicle crashed into a wall and crashed into the vehicle in front, and the vehicle stopped. No further information was available. No warning lights were illuminated. The front driver's side air bag deployed. The vehicle was towed to a tow lot and was deemed a total loss by the insurance company. A police report was filed. There was no injury sustained. The manufacturer was not notified of the failure. The failure mileage was 66,000.

I was horrified to read of the deaths of three beautiful California girls in a Tesla Cyber Truck, that was involved in a crash, was on fire, and the electrical system malfunctioned resulting in them not being able to get out of the car. Tragically, they burned to death.(Recall too [XXX] ' incident as well as other incidents). I went to search the Internet for Model 3 (2023) to see if there were manual overrides in the event of crash/fire/electrical malfunction (even if related to dead battery) and found a You tube video (NOT information provided by Tesla mind you) that showed that on the front seats, there should be a lever not far from the door handle that you can pull to activate a manual release of the door. In the video, they said 'models' are different as to the rear seats but there 'should be' a lever. I could not find any manual override for the back seat passengers. So, I drove to the Tesla Dealer/Service department to have them show me how to activate the manual override for back seat passengers. The technician admitted that in many Teslas, including mine, THERE WAS NO SUCH CAPABILITY!! He admitted Tesla was acutely aware, they do not prioritize safety and that the back seat passengers would be stuck. He agreed that it will take a major class actions and major fines, and/or jury verdicts for them to do anything about it. Appalling and not acceptable. I would request that NHTSA mandate that all Teslas (probably models before 2025) lacking the device necessary to allow passengers to escape death due to failed electrical system be recalled to modify the car at Tesla's expense to comply with the reasonable expectations of consumers. This is a major design defect, unreasonably dangerous and presents a very real threat to the consumers who have no knowledge of the defect, much less where to look for the override (if it exists) because that information is not provided when you purchase the car. Full self drive 14.1 removes the max speed setting that allows drivers to control the speed of the vehicle while under computer control, leaving no way to prevent the car from speeding when the speed profiles choose to drive over 28+ mph over the speed limit. The lack of ability to control the speed of the system is fundamentally unsafe by design.

The turning signal buttons (this car has buttons instead of a typical stalk) often becomes stuck. It’s like all of the buttons on the left side of the steering wheel are already pushed down, and therefore, I cannot indicate whether I’m turning by pushing that turning button.

On [XXX], I was driving my new 2025 Tesla Model 3 from my home in The Woodlands, TX, to San Antonio for a weekend trip. At approximately [XXX], while traveling on [XXX] a few miles before Exit [XXX], the vehicle suddenly behaved abnormally. It appeared to take control without my input, causing the entire car to wobble violently. I completely lost control of the vehicle for several seconds. There were no obstructions, debris, or road conditions that could explain this behavior. I have video recordings from the car’s cameras clearly showing the incident. After pulling over safely, I contacted Tesla, who guided me through some reboot steps. I decided to cancel my trip due to safety concerns and began returning home. However, at approximately [XXX], near Exit [XXX] on [XXX], the same issue occurred twice more. I immediately pulled over again and arranged for the car to be towed to Tesla Service. This was a terrifying and potentially fatal malfunction. It appears to involve a serious defect in the traction control, steering, or drive system.
