I was involved in a traffic accident on 6/29/23 involving myself and my [xx] year old son, both in the front seat. We were struck in the back by one vehicle, spun out of control, hit another vehicle on the passenger side where my son was sitting and spun out of control again. Driver side did not strike anything and side airbags deployed (both front seat and back seat), passenger side airbags never deployed. My son's safety was put at risk as the damage was extensive and the car was totaled. He suffered two fractures in his shoulder and a broken clavicle. The problem has not been confirmed by the dealer. The vehicle has been inspected by my insurance carrier, Progressive. There were no warning lights, and no lights confirming that the airbag was turned off while my son was sitting in the passenger side. “INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).”

The low engine oil warning light keeps coming on, and there’s appears to be no visible leak. I add about a quart each time this happens and it does fine for a while. The last time I took it to the dealership, I told them about it. They just said they didn’t see a leak and not to worry about it. Ultimately the issue is how will this impact the longevity of my engine?

- #11532740July 17, 2023SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
This is already related to Mazda's recall #23V-275 BRAKE PEDAL TRAVEL CONCERN. My car was already involved in crash which I firmly believe that it was related to the brake pedal issue since the brakes didn't hold off the car and the brake assist system was never activated during the accident. Mazda is claiming that my ABS system serial number is a good part, and their dealership claimed that the problem couldn't be reproduced after they held the car for 11 days! I made several attempts to contact Mazda USA but I am not getting tractions for weeks.

- #11530708July 6, 2023FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
I'm reporting significant safety issues. All are related as they indicate that the error rejection for the sensors are not properly calibrated. These false positive errors are dangerous. It affects the automatic emergency braking, forward collision, blind spot, and cross traffic warning systems. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The sensor responsible for detecting road impediments and cross-traffic/blindspots. I'm not sure how to provide it for inspection. How was your safety or the safety of others put at risk? YES - when safety systems frequently have false positive indicators (auditory and visual). This has caused the car and driver to respond to a threat that is not real. This can also cause ignoring true positive indicators (alarm fatigue) and uncertainty of the systems ability to respond to true negative events. Has the problem been reproduced or confirmed by a dealer or independent service center? No, but they are incompetent. I was told the issues might be: bugs on the sensor, maybe a plastic bag floated by the car, maybe the road was shiny, maybe there was something and I didn't see it. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not to my knowledge. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, this is a brand new vehicle (2023) and we are the only drivers. These issues have been happening since we started the lease. The cross/blind spot false positive happen daily. The braking/front end collision have only happened ~4 times.

- #11526712June 12, 2023ELECTRICAL SYSTEM, ENGINE, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
There have been 3 different ongoing issues. The car was ordered and delivered brand new in April 2022. At 4,000 miles and again at 12,000 mile is would display low oil and require 1 quart of oil. The dealership acknowledged that it has an oil consumption issue, unresolved. I have to bring it in every time it is low prior to its required oil change time/miles. At 19,742 miles the cars battery died. They replaced it an have no reason why it died. Every time I'm driving in the mountain pass the transmission will not advance above 3rd gear. The engine will whine and I have to put it into manual and move it to 4th and or 5th gear. When I coming down the other side of the mountain, I have the same issue with it only advancing to 3rd and not proceeding to 4th or 5th. Then when I reach the bottom and on flat road the message- safety system and deactivated. This last for apx 10 minutes. The dealership has no idea why it does it.

- #11526142June 9, 2023TIRES
On June 07, 2023, I was driving on the highway when I noticed a rumbling noise from the front driver-side tire (215/65R18). I got out and inspected the tires and observed nothing wrong with the tires. The next day (06/08) my wife was driving the vehicle on the highway (60 mph) and the vehicle started to shake. She pull the vehicle over and got out to check the vehicle. She observed the front driver-side tire sidewall was completely torn open. My wife never hit anything or ran over anything. The tire shop (Kost) could explain what happened to the tire. The shop person stated that it looked like a complete failure of the sidewall. The tire had approx. 9000 miles on it. In all my years of driving I never had a tire fail this bad.

- #11525939June 7, 2023VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
After setting the adaptive cruise control, and overriding it with the accelerator pedal when approaching a vehicle in front of me to pass it, my vehicle briefly accelerated hard while switching lanes after clearing the vehicle in front of me. This happened even though I had taken my foot off the accelerator already. My current speed was also slightly higher than the set cruise speed. E.g. I set it to 75, I was driving 77 and it jumped up to 80 for a short while before settling back to 75. This acceleration happened right as I cleared the vehicle in front of me and I was in the other lane. You can feel the sudden acceleration. It is as if the radar noticed the vehicle wasn't in front of it any more so it accelerated disregarding the current speed or accelerator position.

- #11520927May 8, 2023FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
I've used cruise control for decades and use it even on city streets, so I can concentrate on the road rather than my speed. The concept of adaptive cruise control is great, but the execution is very flawed on my Mazda CX30. The system fails to recognize lane separation and turn lanes where lines do not exist. The result is the Mazda CX30 adaptive cruise frequently brakes to slow the car for vehicles not in the lane it is in. At some point this is going to result in a rear end accident, because of other vehicles not allowing proper distance and my Mazda CX30 slowing for no valid reason. I shared this issue multiple times with dealership service, with Mazda via their website and with Mazda via emailed surveys. Dealership service says everything is working correctly. Mazda has not responded. If I could turn this functionality off, and have it stay off, I would do so and not report it here, but it turns back on every time the car is started. I was puzzled by this being an issue for me, on my Mazda CX30, but not for friends with other vehicles. What I learned talking to friends is that they rarely use cruise on city streets, which is the primary location for the issue. I discovered a Cheverolet Traverse, I rented, only beeped at me when it detected issues at locations where the Mazda CX30 slowed.

- #11520797May 7, 2023VEHICLE SPEED CONTROL, SERVICE BRAKES
It happened twice already when I step on the break pedal the car goes accelerated feels like you step on the gas pedal that is the problem the first incident I stopped and parked next to the gate because I need to put the code of the gate to open then when I come back to my car and step on the break to release the transmission from P parking 🅿️ then the car goes accelerate too fast almost hit the gate good thing I release the break pedal then hit again or step again the break pedal. The second time I was with my wife she’s parking backward then the car goes accelerated backward. She just stepped on the break pedal. And if we’re driving in the freeway if we break it didn’t stop right away it will still slide for how many meters

"low oil" indicator came on after less than 5000 miles after oil change. Found oil to be 1 quart low. Scheduled oil changes are 10,000 miles apart, which suggests engine has defect which allows oil to be consumed within the engine. No signs of oil leakage.

- #11516324April 10, 2023SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
On March 9, 2023 - car brakes failed at a red light and vehicle collided with an oncoming car coming from the adjacent direction. Vehicle proceeded not to brake for an additional 30 sec minum at which the brakes finally engaged. There was no braking alert - the smart braking system did not engage and there was Forward Collision warning. It is available for inspection upon request. My safety and the safety of oncoming passengers were at risk which could have been fatal Insurance representatives have not yet inspected. Absolutely no warning lots engaged.

- #11514744March 31, 2023SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I was driving my car on the highway. The car automatically deployed the emergency braking system with no objects in front of the car. The car automatically braked then flashed the brake warning indicator. Car mad itself come to a complete stop. I began researching and found that others have had issues with “phantom braking” when the car has collision avoidance technology. This needs to be seriously investigated as it is a safety hazard.

Involved in an accident low speed on 3/17/23. Upon impact with other car passengers seat belt did not activate until passenger hit dash with her head and then it apparently engaged and threw passenger back with tremendous force injuring passenger. Upon inspection at the body shop they found the following code: U0452..invalid data received from restraints control Panel. History. They will be calling Madza for inspection. This needs to be inspected as soon as possible before someone gets killed.

I picked up my certified pre owned CX-30 just 2 days ago and noticed the windshield appears distorted or wavy which results in eye strain and headaches. This seems to occur in the left side of the windshield. It is difficult to focus for long periods of time. I asked another person to drive the car and they confirmed it nearly instantly. I recently learned this is a known issue with the vehicle.

- #11511941March 15, 2023FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The contact owned a 2021 Mazda CX-30. The contact stated that her husband was driving 60 mph on cruise control and while approaching a traffic light he depressed the brake pedal but failed to deactivate the cruise control and stop the vehicle. The contact stated no warning light was illuminated. The driver steered to the right and crashed into another vehicle. The contact stated all the air bags did deploy. The driver sustained leisure on his back and both legs due to the air bag deployment. The contact sustained leisure on her head, her nose, her left ribs, hips, and both legs due to the air bag deployment. The contact stated they both sought medical attention at a near hospital. The contact did not know in detail the injuries of the other individuals but did know both individuals sought medical attention. A police report was filed. The vehicle was not drivable. The insurance company deemed the vehicle a total loss. The vehicle was towed to an unknown location. A dealer was not contacted. The manufacturer had not been informed of the failure. The failure mileage was approximately 14,000.

- #11505472February 3, 2023POWER TRAIN, ENGINE, UNKNOWN OR OTHER
On December 26, 2022, after running errands for an hour or so and parking outside in parking lots, I restarted the car and pulled out of a parking lot. 3 dashboard warning lights flashed, one after the other: Engine malfunction, have vehicle inspected; Dynamic stability control malfunction, have the vehicle inspected; Safety and driver support systems partially disabled, drive safely. Vehicle was stuck in high gear and would not shift to lower gear as I pulled into the driveway (fortunately I was close to home), forcing me to have to floor the gas pedal to go up the very slight driveway incline. Dealership thought they fixed the problem, exact same thing happened on 12/28/22 (same dashboard warnings, stuck in high gear) when we were on the highway. Car was barely able to make it up the exit ramp in heavy traffic, could easily have been rear-ended. We were able to get the vehicle home by driving in Manual. It would still not downshift, even in manual, but we managed to get home (about 20 miles) in high gear with slow acceleration. Once again dealership thought they fixed the problem. Exact same thing happened (same dashboard warnings, stuck in high gear) for the third time on 1/31/23. Fortunately I was close to home and was able to pull up the driveway by flooring the gas pedal once again. Car is currently at the dealership service center again. Vehicle has 8,156 miles.

Suddenly, for no known reason, the dash displayed a warning that the seat belts and air bags are not properly operational and to get the vehicle inspected. Note there is less than 3000 miles on the vehicle. The vehicle has not been involved in any collisions and has not been damaged. I have scheduled an inspection with the dealer, and an internet search revealed this is a problem with new CX-30s.
