I am reporting a severe electrical fire and system failure hazard resulting from unauthorized, dealership-installed aftermarket wiring modifications on a brand new 2026 Mazda CX-30. At the point of sale, Walker's Renton Mazda ([XXX]) spliced invasive, unauthorized aftermarket tracking/security wiring (suspected LoJack or similar) directly into the factory CAN-bus and main wiring harnesses. These modifications were done without consent, actively concealed during the pre-purchase inspection, and compromise the integrity of the vehicle's primary electrical architecture. Specifically, the physical installation bypassed standard safety protocols and compromised the vehicle's firewall insulation. As a military electromechanical engineering veteran, I recognize that unauthorized taps into a modern vehicle's high-speed data network and main power lines introduce immense risk of short circuits, signal logic failure, parasitic draw, and most critically, catastrophic electrical fires. Despite providing physical evidence of the tampered wiring, the dealership's management has denied the modifications exist and refused to remediate the safety hazard by unwinding the compromised vehicle. Mazda North American Operations (MNAO) has also refused to intervene under Case #XXX. The vehicle remains in my custody, but it is fundamentally compromised and unsafe due to the dealer's tampering with factory safety and electrical standards. I am requesting NHTSA investigate this dealership's practice of covertly installing unregulated electronics into new factory wiring harnesses, as it presents an unreasonable risk to motor vehicle safety. - #11758126August 20, 2026EXTERIOR LIGHTING
The high beams would turn on by themselves, and no matter what I did, they wouldn't turn off. Each time the high beams turned on, I would have to fiddle with the control stick for a few minutes until the high beams turned off. Sometimes they would turn off for a second and then just turn back on. This is dangerous as it can blind other drivers. This has happened ever since I bought the car a few month ago. On August 11th the control stick broke off while I was trying to turn off the high beams. Now I have no control over the turn signals or the lights. I brought it to the dealer, but they refused to repair it for free. This is a brand new car which is under warranty.

1.5 years after purchasing new my passenger side front door would not open from the inside. Mazda states car is out of warranty at 36837 miles indicating a cable had come disconnected. Approximately 1.5 years later at 59934 miles the front passenger door again would not open from the inside. Mazda states that this door is used frequently (it is not) and after troubleshooting replaced the door handle and lock mechanism on June 22, 2026. Other than having out of pocket expenses of $738 I feel fortunate that I wasn't exposed to a life threatening situation whereby leaving the vehicle through the passenger side was my only option. Searching forums online show that this failure is currently occurring to others. My hope is that Mazda will formally recognize this issue with the passenger side front door before someone is seriously injured.

- #11757325August 16, 2026VEHICLE SPEED CONTROL, POWER TRAIN, ENGINE
I was driving my 2026 Mazda CX-30 in a parking lot and was pulling into a parking space at a very low speed. I lightly pressed the accelerator to move the car forward, but the car suddenly surged forward much faster than I intended or expected based on how lightly I had pressed the pedal. The sudden acceleration resulted in a collision and property damage. There were no warning lights, warning messages, or other dashboard alerts before or at the time of the incident. I do not know what specifically caused the car to accelerate this way. I am concerned that there may have been an issue with the accelerator/throttle response, engine controls, transmission, PCM, or another part of the powertrain. The vehicle has not yet been inspected by Mazda or an independent engineer specifically for this issue, so the cause has not been determined. It is currently as a collision center. Before the accident, I had also noticed that the vehicle's acceleration and pickup sometimes did not feel normal, although there were no warning lights or messages indicating a problem. I am reporting this because the car's unexpected acceleration at such a low speed created a serious safety concern. I would like the incident and vehicle to be investigated to determine whether a defect or malfunction contributed to the sudden acceleration. The vehicle is available for inspection.

Reporting Product Defect in Mazda Vehicles I am writing to bring to your attention a recurring design flaw affecting current Mazda vehicles, including models such as CX30, MX30, and potentially the Mazda3 series. Following heavy rain or high humidity conditions, there is a consistent issue with the tire pressure sensor alarming and the smart key being unable to unlock the car doors. I have checked my key fob battery, RF function, car battery - none of these are the root cause. Despite consulting local dealerships and various Mazda forums, no one seems to have a definitive solution to this problem. Therefore, I am reaching out to report the existence of this product defect. This issue appears to be more prevalent during summer in Arizona - after the thunderstorms, this issue came up every time. Even with the heat here, it takes from several hours to 2 days for the affected part to dry out and function again.

When I first got my Mazda CX30 2024 New lease ,my Breaks were very weak & didn't perform as new. Only 2 years later w 9K miles on it, Mazda states my breaks need to be replaced. My rotors need to be shave. A whole list of services on a car thats 2 years old. My concern is, break service at 9K confirm my car wasn't new and that Mazda put my Safety at risk w Old Breaks... Mazda is hiding somerhing about my breaks playing games...in the mean time I feel my safety is in jeopardy with defective breaks that maza should fully cover under warranty. Which they won't honor. Putting my safety in jeopardy

I am the original owner of a 2023 Mazda CX-30 AWD (VIN: [XXX]), purchased new on XXX from Mazda of Clearwater, [XXX]. This vehicle has experienced four separate warranty failure events involving critical safety systems within approximately 32 months and 44,530 miles of ownership. In June 2026, at approximately 44,500 miles, my 2023 Mazda CX-30 AWD exhibited sudden RPM lock and inability to shift, with delayed and jerky acceleration. Dashboard warning lights activated including transmission malfunction indicators. DTCs P0756 (Shift Solenoid B Stuck Off) and P0780 (Shift Malfunction) were set. The vehicle was taken to an authorized Mazda dealership. The dealer confirmed the concern and contacted Mazda's regional tech specialist. The valve body was replaced first per Mazda's direction; however, the vehicle continued to exhibit rough and delayed shifting on test drive. Mazda then directed the dealer to replace the full transmission assembly. The repair required 35 days at the dealership. This failure did not occur in isolation. The same vehicle had previously required warranty replacement of brake components (disc, caliper, pads) at approximately 26,400 miles due to a driveability concern, and battery and drive belt tensioner replacement at approximately 29,100 miles. An independent service record five months prior to the transmission failure confirmed transmission fluid was at correct level, ruling out neglect. The pattern of cascading safety-system failures — braking, electrical, and transmission — within 32 months on a low-mileage vehicle suggests a potential systemic manufacturing defect. The RPM lock and inability to shift at highway speeds represents a direct safety hazard to the driver and other motorists. All repairs were confirmed and performed by an authorized Mazda dealer under factory warranty. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11753153July 27, 2026VISIBILITY:DEFROSTER/DEFOGGER/HVAC SYSTEM:COMPRESSOR
The climate control system suffered a sudden, complete failure due to an engineering defect documented in Mazda Technical Service Bulletin (TSB) ID: MC-10203760-0001. The factory evaporator core leaked refrigerant rapidly starving the A/C compressor of essential lubricating oil. This caused the compressor to overheat and complete seize. This failure occurred during extreme summer heat exceeding 100 degrees Fahrenheit, creating an immediate health hazard inside the cabin during my daily 40-mile highway work commute. Additionally, the complete loss of climate control functionality compromises the vehicle's defogging and defrosting capabilities, creating a severe visibility hazard during sudden weather changes on the highway. Mazda customer service corporate teams refused to cover or reimburse the repair because I had to utilize an independent repair facility (Firestone) within walking distance of my home to safely remedy the immediate environmental hazard. The manufacturer is actively denying financial accountability for a well-documented manufacturing flaw that leads to premature secondary component failure. No warning lamps, messages, or other symptoms of the problem prior to failure. First appeared 7/23/26.

- #11751249July 18, 2026WHEELS
The rear wheel bearing failed at approximately 50k miles, causing grinding noise, humming, vibration, steering pull. It was replaced by Mazda under warranty in 2025. The wheel bearing failed again at approximately 59k miles. I have seen reports from other CX-30 owners describing the same recurring wheel bearing failure. This is a known issue that Mazda has not been resolving for the owners and there need to be a recall for this issue.

The wiper settings cannot be adjusted every speed is high- (meaning, the wipers always go super fast) unless, its on auto. I dont alwaus want to use auto. My dealership tech now has "refused" to work with me anymore- or has encouraged me to find another dealership bc of all the random little things ive brought up aboht the car. Seems to frustrate him. Just 2 days ago- the car started leaking water thru the top of the liftgate when going through the car wash. (I do opt for touchless- btw)

- #11744720June 17, 2026VISIBILITY/WIPER, VISIBILITY:DEFROSTER/DEFOGGER/HVAC SYSTEM:AIR HANDLER/CHAMBER, DUCTS, AND VALVES
The vehicle experienced a premature internal A/C failure, resulting in a total loss of climate control refrigerant. Because the A/C system failed, the vehicle's windshield defroster completely stopped removing humidity and moisture from the cabin air. This caused the front windshield to instantly and severely fog over while operating the vehicle completely obstructing forward driver visibility. The manufacturer has issued Technical Service Bulletin (TSB) MC-10203760-0001 for factory evaporator defect on 2021 models, acknowledging low refrigerant charge due to component leakage. However, the manufacturer's current TSB VIN cutoff is too narrow. My vehicle is a 2021 model built at the same facility experiencing the exact same component failure past the arbitrary cutoff, proving the dangerous defect extends into later production runs. The current parameters must be expanded to protect public safety and a recall needs to be initiated to fix the issue.

- #11744391June 16, 2026SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
What component or system failed or malfunctioned, and is it available for inspection upon request? The automatic braking turned on while I was accelerating. My car is available for inspection. How was your safety or the safety of others put at risk? I could have been rear ended because my car automatically braked at an unpredictable time in rush hour traffic with someone very close behind me. Has the problem been reproduced or confirmed by a dealer or independent service center? It has happened to me maybe 4 times before but the dealer hasn't been able to reproduce it. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? It first happened soon after I bought my car new.

- #11743896June 13, 2026SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS
On June 3rd, 2026, about 1:15 PM, I braked in the rain because there was a stopped car in front of me. The brakes did not work; the Automatic Emergency brakes did not work; the Forward Collision Warning did not work and I hit the car in front of me causing a 3 car accident. As I hit the car I saw the Forward Collision Warning come on and then go out. Unfortunately, the brakes worked after that and the Mazda Collision Center cannot find anything wrong with the brakes. I note that the 2022 and 2023 models of the same car did have recalls related to the brakes. The claim is still open and I have not yet received the final damage report. Yesterday I was told the revised estimate is $16,000 and my car and the middle care with front and rear and damage are considered totaled.

2025 Mazda CX-30 experienced a sudden complete loss of power while driving at highway speed with no prior warning lights or indications of malfunction. Immediately before shutdown, steering became extremely stiff due to loss of power steering assist. Vehicle was steered towards the shoulder during highway speed of 65mph and was electrically inoperable while rolling, this includes the loss of hazards, power steering and electronic brake lock engagement.

Following a recent inspection at Perry Mazda in Santa Barbara, my vehicle was diagnosed with a failed clock spring mechanism causing airbag malfunction warnings. Clock spring failure under 70K miles on a 2021 vehicle is premature and suggests a defect rather than normal wear. I contacted Mazda North America and it has been over a week without a response. I have maintained the vehicle responsible and have no history of collisions or modifications that could have contributed to this failure.

[XXX] fuel door assembly has come loose from body. It looks like the clips that hold it in place have become worn or loose by the weight of the gas nozzle or by the fact that the gas door will not close correctly and car wash machine has made it looser and it appears to not shut all the way, and the body colored panel has fallen off as a result and when you pump gas, the assembly moves may be possibly causing it to leak gasoline if the seal or gasket becomes dislodged because of the loose clips that hold assembly to the body panel. The contact owns a 2023 Mazda. The contact stated that the vehicle was braking randomly while driving. There was an unknown malfunction code that flashed quickly. The steering was stiff and failed to turn. The steering wheel adjusted and guided the vehicle into the wrong lane. The RPM was sporadically reeving. The RPM increased, and the vehicle stalled without acceleration. There was smoke emerging from the hood. There was oil leaking. The vehicle failed to turn off and on. The fuel gauge and infotainment center were not working properly. The contact stated that while leaving the local dealer, the vehicle stalled. The battery was replaced. The vehicle was towed to another unknown local dealer who determined the battery needed to be replaced. The vehicle was replaced, but the failure recurred. The dealer determined that there was an oil leak and needed to replace the transmission, serpentine belt, and battery. The dealer informed me additional testing was needed. The vehicle was not repaired. The manufacturer was contacted regarding a buyback, but no assistance was provided. The failure mileage was approximately 37,000.

- #11734610April 29, 2026FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Unexpected Automatic Emergency Braking Activation at Highway Speeds in Cold Temperatures. Description: I am reporting a safety issue with a 2023 Mazda CX-30 involving the collision avoidance / automatic emergency braking system. While driving at highway speeds, the system activates a forward collision warning (audible alert and dashboard indicator), followed almost immediately by automatic braking, even when there are no vehicles, obstacles, or pedestrians ahead. Conditions When Issue Occurs: * Ambient temperature: below ~40°F (4°C) * Road conditions: cold, often icy or snowy * Speed: highway speeds 45-75 * Environment: clear roadway, no visible hazards * visibility : clear System Behavior: * Audible warning and visual alert activate * Automatic braking occurs approximately 0.5–1 second after the warning * No obstacle is present to justify intervention Safety Risk: This presents a serious safety hazard because: * Sudden braking at highway speeds creates a high risk of rear-end collision * The warning-to-brake interval is extremely short, leaving little time for driver override. * Occurs in low-traction conditions, increasing risk of loss of control. * The event is unexpected and not predictable by the driver. Frequency: * Occurred multiple times under similar cold-weather conditions. * Appears repeatable based on temperature range. Attempts to Resolve: * Sensors/camera appear clean and unobstructed. * No prior collision damage to the vehicle. * Issue persists despite normal operation otherwise. * Similar reports observed from other owners in online discussions. This is not a one time occurrence, in the section below, i noted the last event but this is a daily occurrence in temperatures below 42F.

While driving, on approach to a stop signal, at an intersection the car stopped moving. The display said engine malfunction, have inspected. The electronics and engine continued to operate, but the car would not move when gas pedal was depressed. I shut off and restarted my vehicle numerous times, with no effect on the problem, the car still would not move. The vehicle behind me had to brake unexpectedly, and I could have been hit by others in the intersection, there could have been an accident.(I put car in neutral and the driver behind me, pushed my vehicle, manually, out of the intersection) AAA towed vehicle to the dealer I purchased it from, they were unable to reproduce the problem or retrieve codes to identify problem.( AAA did something under the hood before towing and I was able to drive vehicle to nearby parking lot to be towed) No other inspection has been conducted. Over prior week I noted intermittent engine revving while driving for very brief seconds and this occurred just prior to the incident that resulted in the inability to move the vehicle.
