I was trying to park behind a car. Slowed down the car. Just about 20 feet behind the car, the car suddenly started to accelerate. The speed was super fast. I had to step on the brake some hard to stop the car. Fortunately the car stopped just in time. No crash happened.

This is a potential safety issues that will put my passenger at risk and it should not take this long to find a remedy.

1) Electrical malfunction with warning of locking malfunction transiently appearing 2) Locked inside the car and the manual door release would not work after the above. 3) Finally got out after using key fob and multiple attempts to use the door handle, closed the door, and the car locked and could not open it with either the key fob, Lexus application, nor with the emergency physical key (it would turn but not unlock the door). 4) After 1st malfunction, had a smell of burned electrical component but NO smoke or fire. The hybrid system was fully charged however, it is probable that after speaking to Lexus while inside the car with the ignition on to trouble shoot a charging scheduling issue, the 12 volt battery was partially discharged. The car has 441 miles and after Lexus service arrived, they were able to get it drivable HOWEVER, check engine light remained on and "system malfunction...visit your dealer" was also on the entire 7 mile trip to the dealer with the tow truck following me all the way. I kept the window open despite the rain so that I could get out if something else happened.

The contact stated while driving 35 MPH on a rainy day, the brake pedal was depressed; however, there was an abnormal squishing sound coming from the brake pedal, and the vehicle failed to stop as intended. The contact stated the vehicle went through the stop light and up onto a curb before the vehicle stopped. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. Additionally, the contact stated that the failure had occurred previously. The vehicle was taken to the dealer; however, the dealer confirmed the abnormal squishing sound when the brake pedal was depressed. The vehicle was not repaired. The contact was informed that the manufacturer had declined to repair the vehicle. The manufacturer was made aware of the failure and a case was filed. The contact was informed that both actuators had been replaced previously. The contact referenced Technical Service Bulletin: L-SB-0005-21; however, the contact was informed that the repair was not covered under warranty or any special programs. The failure mileage was approximately 31,600.

I am writing to report a serious safety issue involving my Lexus RX350. While driving on the freeway with my [XXX] child, I suddenly heard a loud explosion-like sound coming from the roof. To my shock, I discovered that the sunroof glass had shattered into pieces. Fortunately, the sunroof cover was closed, preventing glass from entering the cabin and potentially causing severe injuries. When I contacted my Lexus dealership, they acknowledged that another GX had come in just the previous week with the exact same issue. They insisted that a rock was the cause. However, I find it extremely concerning that a small rock could cause such catastrophic failure, indicating that the glass material may be dangerously flimsy. This incident has left me deeply shaken, as it could have resulted in severe injury or even loss of control of the vehicle. I am afraid for my safety and that of my family, knowing that this could happen again. I urge NHTSA to investigate whether this is a defect in the sunroof design or material quality, as it poses a significant risk to drivers and passengers. We bought the brand new Lexus (RX450H) back in 2022 (Vin [XXX] ). As of today, February 03, 2025, the mileage on the car is 20,125 miles. The OEM tire size was 235/55R20, all the 4 tires were Bridgestone Ecopia H/L [XXX] . Recently, one of the wheels exploded when it passed a pothole, the tire was damaged and the rim was bent/cracked as well. I brought one tire, same brand and size, and installed it to the new Lexus rim. The installer told me that these tires were "Recalled" and suggested to replace all 4 tires ASAP. When I start to coast to come to a stop the engine will rev, even though the Energy Flow screen shows the gas engine is disengaged and running only on EV. The engine will rev, even with me using the brake. But the EV light is off and the rpms are rising. Eventually the EV light will come on and the gas engine will cut off and the vehicle will switch to EV mode. My mpg has dropped to 36.2, so something is not right. When I was at the dealer for my 10K service, they told me this was the car "shedding" the extra energy, but the battery was not fully charged, so I'm perplexed and frustrated.

Unintended acceleration. After complete stop at signal light behind another car electric motor was keep pushing car forward with feet on break and no feet on accelerator. Car was at complete stop and then after complete stop car started moving so press breaks more harder and car started moving up and down due to electric power pushing car forward and break system trying to stop car. Below is the email that I sent to both the manufacturer and the dealership: I hope your Christmas Eve was better than mine. My son and I were driving up to the Poconos [XXX] to go skiing and I noticed the sonar detection light come on. When we were driving back from the Poconos, the transmission would not catch around the second or third gear. The RPMs would shoot up. The dash / warning lights lit up. I pulled over to the side of the road and I called Lexus dealership, service center, and road side assistance. However, due to the fact that it was [XXX], the offices were closed. I managed to make it to my parent’s home, but it was a very dangerous ride on [XXX]. The odometer stated that we were going 50 mph, but we were crawling and the RPMs were 4000. The RPMs at rest were always over 1000. Additionally at times, the gear would not shift into second or third and/or bottom out. Obviously the transmission is beyond malfunctioning. Below are pictures including: 1.) check your engine: A malfunction in the electric control of the engine, throttle or automatic transmission has been detected... 2.) a malfunction in the Intelligent Clearance, Sonar System has been detected 3.) traction control turned off 4.) Secondary Collision Break System malfunction, 5.) System Malfunction 6.) Drive-Start Control Malfunction. The vehicle has just over 3000 miles. I am beyond distraught. My phone number is [XXX] . However, I think it's best if we communicate through email at this point. I planned a family ski trip to Killington, VT for the New Year. I would like to request a loaner vehicle so I can salvage this holiday. I look forward to hearing from you and your service department.
