The contact owns a 2022 Ford F-150. The contact stated that while driving at approximately 5 MPH the braking system and power steering suddenly failed causing the driver to lose control of the vehicle and crash into a concrete light pole pillar. During the incident, the vehicle was severely damaged. The driver sustained a right-hand laceration and chest and leg injuries. The front passenger sustained both leg injuries. Both occupants required medical treatment. The cause of the failures was not determined. A police report was taken at the scene and the vehicle was towed to an undisclosed location. The contact indicated that the failures had also occurred prior to the crash. The local dealer was notified of the failure. The manufacturer was not yet contacted regarding the incident. The failure mileage was 10,100.

Issue #1. Forward collision sensors not working warnings keep appearing and flashing on dash. Vehicle has 12000 miles. 2. Hood popped open at speed of 90 MPH without a warning or manual operation of lever. Hood was not opened during a 500 Mile trip so operator error is ruled out. My 2021 F150 XLT with 19,000 miles has never had functioning trailer brakes. Initially I was plagued with constant trailer wiring fault / trailer disconnected issues, and zero slide or pedal brakes, when trying to tow a travel trailer. Two separate dealerships initially dismissed my concerns because it passed their trailer simulator test. Eventually, Ford replaced the trailer wiring harness under warranty. After that, I had intermittent slide brakes that would work, but I never saw any brake output when using the pedal brakes, regardless of gain setting. Ford then replaced the trailer brake controller, which worsened the issue: once again I am getting trailer wiring faults. My slide brakes are extremely hit or miss, and when they do engage they catch and release several times. I still have zero brake output when using the pedal brakes regardless of gain. The dealership consulted a Ford engineer and assured me that my truck is perfect, I should not expect to see brake output for pedal brakes, and all is working as designed. They blamed my trailer. I demanded to test my trailer with a different F150 from the dealership and it works flawlessly - brake output on both slide and pedal brakes, no error codes. It is horrifyingly obvious that I have never had ANY trailer braking functioning with my pedal brakes now that I have compared the braking distance in the loaner truck to my truck. If I had believed Ford, I would be towing a 7,500lb trailer with zero idea that I did not have any trailer brakes. Dealership and corporate are both kicking their feet on a potential fix to this horrifyingly dangerous trailer brake problem.

Park/Emergency Brake system light illuminates and system engages when vehicle is placed from drive to park, and when system is shut down the light illuminates and engages. Also, when vehicle is started the park brake light shows to be engaged. When the vehicle is put in reverse the light is still illuminated while the vehicle is backing up. The light stays on while the vehicle is in backward motion. Safety concerns for me and my family are: 1. What if the Park Brake engaged while driving at normal speed - what could happen? 2. With vehicle parked and brake system engaged driver should not be able to put vehicle in reverse and back up without disengaging brake lever. The vehicle was taken to the local dealership service center twice to try to get resolution of the park brake light issue. They were unable to duplicate the issue and stated that according to Ford's maintenance policy there was nothing more that could be done. We have tried to contact Ford through their main customer service phone number. We literally spent hours on the phone trying to reach someone who could help us with the issue, but no one had an answer for us. A case file was made by Ford Customer Service and marked "RESOLVED". It has not been resolved. As stated, the park brake light started showing up in February 2022. As of this time there has not been an incident, but I can't rule out that something might happen. I think it should be addressed by the dealership and/or Ford engineer with more interest than just a dismissal attitude.

My pickup was at the dealer getting an oil change and tire rotation (23140 miles) and I was informed that the right rear axle bolt had sheared requiring replacement of the axle assembly and that it was not safe for me to drive the vehicle off the lot. This is the second time that the right rear axle bolt has sheared. The first time, the axle assembly was replaced at this same dealer at 14482 miles. I no longer have the original bolt that sheared, I will request the second sheared bolt so it could be inspected if needed. I do not know what the safety ramifications are except to say that both times, Ford said it was not safe for me to drive the truck off the lot. There are numerous reports of rear axle bolts shearing in 2021/2022 F-150s with the Max Tow package (apparently, this package has a different axle configuration) on the Internet. It is currently being held by the dealer for the second repair. There were no warning lights, noises or any other indications that the axle bolt had sheared. When the first bolt sheared, I heard a rattle from the right rear tire and I expected to find a broken lug bolt or loose lug nut but instead found the sheared axle bolt. No noises were heard when the second bolt sheared.

While driving with sound volume at normal setting (volume 8), speakers started to pop and static erratically, which startled me, almost causing an accident. The issue will randomly go away, no warning lights on screen or dashboard. The static and pop sounds still continued at volume setting one. The erratic static and pop goes away then it happens again 20 min later. My ears are ringing from the popping sounds

The 10 speed transmission used in Ford vehicles is plagued with problems to include, skipping gears while shifting, very harsh upshifts and very harsh downshifts. It's just a matter of time when a failure of this transmission will cause a serious accident. My 2021 F150 exhibits this behavior on a frequent basis yet the Ford dealers tell me this behavior is normal. Every Ford F150 owner I have spoken too are experiencing the same problems. Why isn't NHTSA doing anything about this? Or does it only matter when an accident occurs where is loss of life?

I took my 2021 Ford F-150 VIN [XXX] to a 3rd party mechanic for a TX state inspection as required by law. The mechanic told me he could not preform the inspection because my truck would not communicate with his emissions testing computer. He said this has happened many other times with other newer F-150s. As can be seen with NHTSA ID number 10209217 and Manufacturer Communication Number: SSM 50602. It appears Ford via their software and emissions test is only allowing these vehicle’s to be serviced at Ford dealerships which is wrong. Took delivery of a new 2023 Ford F-150 on 2/18/2023. Upon inspection once at home I discovered excessive Rust/Oxidation in the underbody area. Specifically, there was excessive rust and oxidation in the following areas: Rear Differential, Rear Axle Casings, Rear Axle Hubs, and portions of the front Frame Rails. This amount of rust/oxidation should not exist in a new vehicle and has the potential to lead to a driving performance/braking incident/accident that could damage the truck and or injure vehicle passengers. Research indicates Ford Motor Company has known about these issues for a couple of years. Furthermore, after contacting Ford, I received a boilerplate email stating that they would look into this problem without indicating whether or not they would issue a Recall or CSP (Customer Service Program) to remediate this problem. This is without question a safety issue and should be investigated by NHTSA. Pictures of the affected areas and a copy of the email from Ford area attached to this filing.

I have been having exhaust fumes coming out of my dashboard ventilation. These carbon monoxide fumes that affects me and my family while driving. Upon starting the vehicle there was an extremely loud electrical static/popping sound coming from all speakers. Sound was extremely loud, in the hearing damage range - estimate over 100db. After a few seconds the sound stopped. I have seen reports online that this is happening to multiple customers driving late model Ford F150 with the B&O speaker system. If this had happened while driving it would be extremely startling and could lead to accidents. The sound is so loud it could cause hearing damage.

I took my truck in for an oil change and tire rotation one 3/18/23 and currently has ~13,500. Upon pulling the rear drivers side rear tire the axle bolt fell out as it had been sheared off. There was no notice of the issue and as I understand it could have resulted in my right real wheel to have broken off while driving. The dealer immediately took it out of service and currently is searching the fix.

On multiple occassions the speaker system has had a deafening blast of static noise that actually made my ears hurt. luckily my daughter wasn't in the car on either occassion but the sound was so loud that I actually had ringing in my ears after I figured out how to make it stop. I told ford about it but they said there's no fix and they don't know when one will be coming out. The Cabin recirculation button turns off by it self, lets air in from outside.. ie. smoke, car and truck exhaust.. cold air. this has been going on since I owned the truck and Ford says it is designed to do that.. window anti fog design.. I will get in my truck in the morning and turn heat on to take chill off... low air speed and 2 clicks past half way on the heat and it will only stay on for 1 -5 min and turn off on its own. I drive 1 hour to work on interstate and everytime it turns off I smell exhaust from semi's and other cars around me in traffic. so I have to keep turning it on to keep from getting choked out by the smells from traffic. Ford says no way to turn off.. This is a design flaw.. I know of no other car manufacture that does this.. I have videos of it turning off..

I need to address a safety concern! On 3/13, I was driving back home. my 3 year old daughter was sitting in her car seat (behind the driver seat). She pulled the seatbelt from the middle seat and wrapped it around her neck choking herself. I had just got home and thank god I did because this would have been a totally different story. I pulled up to my mailbox to get my mail. I happen to look back and saw she was purple on the verge of blue. I had no scissors in my car to cut the belt. I drove up my long driveway ran into the house came out with scissors and cut the belt as it was locked and there is no emergency unlock button. Her neck was very red and has broken blood vessels. I have pictures to prove this! No where on the belts does it say “choking hazard” I thought seatbelts were supposed to SAVE LIVES! My seatbelt can be replaced however, my daughter life cannot! Thankfully, is ok!! I called a ford dealer to get the belt fixed. Was told it was $1,000!!!!!

Windshield wipers would not turn on during a rain storm. Water with still squirt now but I had no action in the wiper blades and stuck in the rain. Next day windshield wipers were working fine acted like there was a short in the system ! Forward for months and windshield wipers suddenly stopped working in the middle of a blizzard white out conditions! Dealer finally says that the windshield wiper motors gave way !! I looked into this the first time it happened, and there was a major recall due to these motor defects ! My Vin number was not associated with the recall that they had my local Ford company put it by the wayside and they still are because the Soonest they can have it serviced and new fire put in his two months out!! Ridiculous
