While driving 70MPH on the expressway the dash randomly sent a message saying “Pull over vehicle shutting off” and then it put itself into limp mode. Thankfully we were able to coast to the small shoulder and put on our hazards. This happened at 2am with 4 kids in the car. Nothing in the car would work, it wouldn’t turn on, shift, nothing. We had to sit on the side of the expressway for hours waiting for a tow. While waiting it had the check engine light on, limp mode light, and messages to service hybrid system & plug in battery for reconditioning. It is waiting to be serviced, but there is a very limited amount of techs that will service it due to being a hybrid.

The van was going about 10 mph and the side wall just burst and went to flat right away and tire pressure warning came on. Yokohama had a recall for this same issue in 2022 but this DOT was not included. Nothing came in contact with tire or van when the issue occurred. This would have been a major issue if going full speed. Discount tire in Southaven MS stated the tire just gave out with no indication of anything hitting the tire. Yokohama refused to do anything as this is a BraunAbility handicap van which shows them as owner 1 to do the conversion through Chrysler. These Vans cannot be purchased through Chrysler directly. Yokohama did now even offer to report this issue internally as a complaint on the tire. I kept the tire for further inspection and investigation. I also have pictures and video of the tire prior to replacement. Tire Yokohama AVID S34 (S34RV) 236/65R17 104T. The P style is not indicated on the tire. I have 3 other tires remaining on the van with same tire/DOT (DOT 4UL8-PKX3321

I rented this car from Budget on Saturday, May 31, 2025. When I drove it on Sunday, June 1, 2025. The check engine light came on and the car gave a warning that it would shut off. The first time I saw this notification I was stopped at a stopped sign at a 4 way intersection. The car proceeded to shut off after some time (maybe 20 seconds or so, not sure). I was at a full stop and the car was in "Drive". When the car shut off it went into "Park" automatically. I turned the car back on and put it back in "Drive". The car would not move. I check to make sure the parking brake was not turned on and it was not. I put the car in "Park" and back into "Drive" and the car still did not move. I turned the car off a few more times before it switched back into gear normally. I was unable to return the car at that time. About an hour later as I was driving on the interstate in the left lane, the same warning light came on. I had to immediately move from the left lane of a 6 lane interstate all the way to the right shoulder. There was no exit or left shoulder. Once I got there the car idled for a moment and shut off; I am uncertain how long it was before it shut off. I waited a couple of minutes since I was also shaken up at having to move so many lanes on a busy interstate with so many lanes. I got to my destination after about 30 minutes. On the way home about an hour in to the drive, the same thing occurred. I moved onto to the right shoulder of a two lane interstate at this time. This time I shut the car off myself and waited a couple of minutes to turn the car back on. I then drove another 45 minutes home. Then this morning, same warning in my driveway before I left to return it. There were issues with the blind spot warning not working some of the time as well as the display screen would flicker on and off or just become disabled but still visible. I have reported this to the rental company but I want that car off the road.

My wife was driving our 2022 Pacific PHEV Limited with 4 kids on board suddenly went into limp mode while driving northbound south of [XXX] on [XXX]. Turtle signal was on dash with just seconds to get off freeway on limp mode. Pacifica had to be towed to Woodhouse Chrysler in Sioux City where it remains to this day. Changed PCIM and remains in limp mode. Chrysler approved hybrid transmission repair under hybrid warranty but dealer found not repairable and needs new transmission. Second row passenger seat headrest does not lock into place. After folding the seat down to put in car seats in the third row, the headrest will not lock. This would cause severe trauma if one of our many children were to be seated in the seat during a crash. The issue has not yet been evaluated at a service center, but I intend to contact them and see if the headrest can be replaced through warranty.

First of all, I would like to say that I received an "IMPORTANT SAFETY RECALL" notice citing that "certain 2022 Chrysler Pacifica vehicles failed to perform" However, my car's problem, though similar, is not quite the same as the problem mentioned by the Federal Motor Vehicle Safety Standard" - rear visibility. My car's DRIVERS' SIDE VIEW mirror is the one that poses the problem. When I put the vehicle in reverse, the right-side view mirror adjusts so that I can see the right backside of the vehicle. This, unfortunately, does not occur with the left sideview mirror. I can only see the ground. This an obvious determent to the safety of not only myself, but others as well. When I reported this irregularity to Hunter Dodge, I was told that was just the way the mechanism worked. I was told that there was nothing they could do. No one bothered to check it out. No one seemed concerned. This has been an issue since I purchased the vehicle. I don't know if anyone else has this same issue; but I feel that I am not the only one. This functionality necessary for the prevention of serious injury to pedestrians and/or property. Thank you, in advance for receiving this information.

The contact owns a 2021 Chrysler Pacifica. The contact stated that while driving at various speeds, the vehicle hesitated while accelerating with the accelerator pedal depressed. The check engine warning light was illuminated. The vehicle was taken to a local dealer where it was diagnosed and determined that the rocker arm, lifters, and the right intake camshaft needed to be replaced. The vehicle was not repaired due to parts not being available. The manufacturer was not made aware of the failure. The failure mileage was approximately 101,000.

[XXX]- bought new 2025 Chrysler Pacifica w/ 29 mi at Troncalli Chrysler Dodge Jeep (TCDJ). [XXX]- 1st time -I was driving the car when it made a “clunk” sound when turning left. All the lights on the dashboard came on and the ABS system turned off.The car safety features disabled and it did not recognize we were in the seats (seat belt lights came on).I was able to drive into a parking lot.Turned the car off and then restarted it after a few min and the lights on the dash were gone. Decided to cont driving, figured it was a computer error.Went 2 mi and had to turn left again. All dash lights came on including engine, ABS.Pulled over and lights turned off after car was turned off/on again. Tried to get the car home.Turned it on and no alerts.Drove <1mi and the car began to stutter and dash lit up.Lost the ability to accelerate, and the steering seemed stiff and hard to control.Lost ABS system.Pulled over into parking lot barely moving.Had to leave the car in the lot and have it towed to TCDJ 3/29- Towed to TCDJ service depart..Total miles on car:283. 3/31- TCDJ states it's the batteries that caused the malfxn. Explained I did't think the batteries were the issue.They replaced both batteries. [XXX]-Serious safety issue. Picked up car @ TCDJ.We did not make it home from TCDJ.Trip from TCDJ to home is ~27 mi. At mile 22, turned left and ~0.5 mi later, while driving ~ 15-20mph, the car shifted on its own, throwing itself into park at a major intersection.All of the dash lights came on.The Emergency VAS, ABS system and Shifter Disabled displayed on the dash.The windshield wipers turned on at high speed.The head and tail lights began to flash.The car shifted so abruptly w/o warning we were almost hit from behind.It did not slow to a stop or brake it just threw itself into park.Police were called. Car wouldn't turn off or move. Could not put in neutral manually. Still @ TCDJ now >30days. Alternator and ABS replaced. Continues to break and need a tow when TCDJ test drive it. I received a warning to park safely, engine will shut down soon. This is the third time it has happened. One time coming down a mountain with no place to pull over safely with a van full of family members. Second time at high speed on an interstate, this time on a high speed local road, but had enough time to pull over before losing power. The van has been in the shop and/or waiting for repairs for over 10 weeks during my 53 weeks of ownership. Chrysler has delayed accepting the recommended repairs that their dealer states is necessary. I have spoken to other Pacifica owners that has experienced the same issue. Is Chrysler waiting for folks to get killed before accepting responsibility?

In 2024, our vehicle displayed a critical warning stating: “Stop safely. Vehicle will shut off soon.” While driving on the highway, the vehicle began losing engine power—a terrifying and dangerous experience. We immediately took it to the dealership, where they diagnosed a transmission failure and proceeded to replace the transmission. Unfortunately, we are now experiencing the same issue again this year. The vehicle continues to lose power and shut off while in motion on the highway, creating an extremely hazardous situation that nearly resulted in a serious accident. This ongoing malfunction poses a significant risk to the safety of drivers and passengers. Chrysler must take immediate responsibility by recalling these vehicles and replacing them with safer alternatives to prevent further danger to human life.

Letter sent to AVIS after the incident: The image included is an example and not associated with incident Concern Regarding Recalled Rental Vehicle - Avis Rental Agreement [XXX] Dear Mr. Tucker, I am writing to share a concerning experience my wife and I had with an Avis rental. We rented a Chevrolet Pacifica Hybrid for a drive from Atlanta to Chicago. About an hour into our trip, the vehicle displayed a warning message: "Stop Safely. Vehicle will shut down soon." Following the instructions, we immediately pulled over, activated our hazard lights, and called Avis emergency roadside service while consulting the owner's manual. We then decided to move to a nearby gas station parking lot for safety. While still on hold, we researched the warning online (see link) and discovered it was a known recall for our vehicle. The recommended temporary fix—turning off the car, opening and closing the driver’s door, and restarting the engine—allowed us to continue. However, the warning appeared three more times during our trip, requiring us to stop and repeat the process. Though we managed the situation, it caused unnecessary stress and could pose a serious risk for other customers. After 15 minutes on hold, we disconnected from emergency road service as we no longer needed assistance. We called back later to inquire about switching vehicles, but chose to continue rather than delay our trip even further. This raises important questions: Why does Avis place vehicles with outstanding recalls into its rental fleet? What is Avis’s general approach to managing manufacturer recalls? If such vehicles are rented, why not inform customers in advance and provide a workaround? While I have contacted Avis/Budget about customer service concerns in the past, this is a safety issue. I urge you to escalate this matter to your management team and clarify how Avis plans to prevent similar situations. I would appreciate a formal response outlining your approach to recall management. My car has the 03A recall, and had the software update in March 2024. In Summer 2024, I got the red turtle, "Stop, pull over. your car is going to stop" warning, my car stopped moving, etc. I was able to get it restarted and CEL came on and has been on ever since. I took it in for the open recalls (66B), and state inspection, as well as the liftgate recall that was performed by the dealership but the liftgate has never worked since then. They stated that all the recalls except 66B were completed and when I asked why I had gotten the STOP NOW message after the software "fix" was done, they had no answer. I provided them with research stating that Chrysler had issued an unlimited lifetime warranty for these transmissions, since that is where the short is located that is causing the cars to stop. They have insisted since then that the transmission isn't the problem; that my hybrid battery is the problem. I have had no issues that would indicate the battery is the problem, but plenty to indicate the transmission is the problem. My car's been in the shop since March 17, and it is April 27. The new battery was supposed to be in on April 10, but there is still no ETA. I have a STAR case and a case open with Customer Cares, who has been absolutely no help in getting this problem actually fixed. Despite my providing the dealership and Customer Cares the necessary information stating that the new transmission is the only repair that will stop the car from stopping randomly, endangering myself and my family and other people on the roads, they continue to encourage me to "trust the engineers" and let them replace the battery. They are REFUSING to fix the issue, and I will not drive that car until it's properly fixed. This transmission issue is creating death traps and there is a known fix that even Chrysler has acknowledged by issuing lifetime warranties on the transmissions affected by this recall, yet the dealership and Chrysler Cares refuses to perform the fix for it.

The vehicle, without warning, flashed the message “Stop Safely, Vehicle Will Shut Off Soon” and suddenly went totally unresponsive. It had shut down within 60 seconds. This was on day 4 of ownership and less than 1000 miles on the vehicle. We were unable to restart, shift, or get the vehicle to respond in any way. Upon reading further, this is the exact issue that over 67,000 of these models were recalled for in years 2017-2023 with transmission replacement as the required repair.

We were driving on the freeway, 80 MPH, 0% battery life. Warning came on the dashboard stating something like “ Pull over safely, engine will stop”. Engine had already stopped and we luckily avoided impending doom and pulled off to the side of the highway. Turned car off, waited a couple min, and started the car again. Car turned on with check engine light. Able to slowly get to the next exit. Next morning, took to dealer. Say there were not able to see any issues. Van is around 28,000 miles.
