I have a 2021 chevy silverado and in october of 2023 I lost oil pressure and took the truck to a mechanic shop and they determined that the engine had spun rod bearings and crankshaft bearings inside the engine block and needed to be rebuilt. I had them make the repairs and upgraded the engine with aftermarket parts which cost me $15,600. When the recall came out this year it came with a form for reimbursement. I collected all the documents required to submit for reimbursement and I was denied because I did not have the work performed at a gm dealership. I didn't take it to the dealership because of the time they required to fix the truck and it was already out of warranty because of the mileage and I really didn't want to replace the engine components with more of their faulty parts. Just because I had the repairs done at a 3rd party shop does not excuse the fact that they sold me a truck with a faulty engine.

The transmission is making a clanking noise during the lower gears. The local dealer said the transmissions is "done for". The truck has around 125,000 miles on it. The truck is available for inspection. When checking for recalls, i saw the GMC were being recalled for the same issue.

VIN: [XXX] Component: Powertrain / Transmission / Recall Remedy Description of the Problem: Recall remedy applied in June 2025 failed. Transmission subsequently failed. On August 25, 2025, a Mangino Chevrolet technician confirmed in writing that the failure was “related to the recall that was previously performed.” My truck was previously in GM’s care at Van Bortel Chevy (Nov–Dec 2024), after the recall officially opened on 10/24/2024, but the remedy was not applied at that time. I was forced to continue driving thousands of miles with the defect still present. Despite the written admission that the failure is recall-related, GM Customer Care has only offered a 50/50 split on the transmission replacement (my portion ≈ $3,950.34 + tax) and is now attempting to reframe the issue as a maintenance problem. Transmission fluid service intervals are not the cause here — the federally mandated recall remedy failed to prevent the very failure it was supposed to address. This situation has left me without a safe, reliable vehicle. GM has acknowledged the recall remedy failed but is refusing to take full responsibility. I am requesting that NHTSA investigate GM’s handling of this recall remedy failure. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

See attached document for complaint. When contacting my local dealership, Dick Hannah Chevrolet located in Sandy Oregon the Service Representative BXXXXX explained to me that they could not complete service in the form of inspection. BXXXXX recommended that I defer my request for a month. This is an example of Chevrolet, and the local dealership combining in a course of action meant to deceive their customers. While on the one hand they indicate an interest in activating the recall on the other hand they refuse to provide service. The customer, me, is left in no mans land with a serious recall condition unable to seek resolution. My request, investigate both the Company, Chevrolet, and the dealership and pursue resolution as the recall is intended.

I am filing a formal complaint regarding a serious safety concern with my 2021 CHEVROLET SILVERADO, following the completion of recall N252494001 performed by Galleria Chevrolet in Dallas, TX. Prior to this recall work and related inspections, my vehicle operated without any abnormal engine noise. After the recall inspection and a subsequent fuel injector repair performed by the dealer, my vehicle began producing a bottom-end engine noise that had never been present before, and ultimately we have catastrophic engine failure. What is highly concerning is that the dealership staff previously documented the situation in writing as follows: “We performed the recall inspection and the truck passed. After performing an injector repair now there is a bottom-end noise when it's running. Did the failed injector possibly affect the validity of the test? That may be the case. We know that the truck did not experience the noise before the inspection; is it possible that the steps we took affected the truck in a way that got the noise to show itself? That’s what we need to determine.” Despite this acknowledgment, the dealership is now denying any responsibility, classifying the damage as unrelated “wear and tear”. This reversal appears highly suspicious, as it contradicts their own observations and written notes. It suggests that the dealership and GM may be refusing to fulfill their obligations under the recall and repair warranty, leaving me with a vehicle that has a blown engine and a significant safety risk. Given the timing of the noise, occurring directly after recall work and injector repair, and the internal documentation from the dealership itself, I request NHTSA investigate whether: This issue is part of a broader trend affecting other vehicles under the same recall. GM and its dealers are properly honoring their obligations to fully repair vehicles impacted by recalls. Consequential damage resulting from recall work is being systematically denied coverage.

On 6/4/2025 while driving my truck, I noticed smoke coming out of vents of the front passenger side of vehicle. I got out of truck and went to open passenger door to investigate and the engine ignited. A neighbor called 911 and the local fire department responded. I contacted my insurance company. They had the truck towed so their investigators could examine it. The cause was classified as indeterminate and the truck was declared a total loss. The truck had approximately 51,000 miles at the time of the incident and was well maintained, with new tires and no known mechanical issues or warning lights prior to the fire. The fire occurred while driving on a road seven miles from my home.

The contact owns a 2024 Chevrolet Silverado 1500. The contact stated that while driving at 40 MPH, there was a chirping sound coming from the engine. A dealer was contacted. The contact pulled over to the right side of the road and waited for tow truck assistance. The vehicle was towed to the residence and then towed to the dealer the following day to be diagnosed. The contact was informed that the engine needed to be replaced. The vehicle was not repaired. The contact stated that the failure had occurred at least two or three times per week. The contact related the failure to NHTSA Campaign Number: 25V274000 (ENGINE AND ENGINE COOLING); however, the VIN was not included in the recall. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was 27,900.

- #11686370September 10, 2025ELECTRICAL SYSTEM, STEERING, VEHICLE SPEED CONTROL
Truck was intermittently losing power steering and shutting off/reseting itself. Was a loss of electrical and was throwing Stabilitrak Service warning. Would not stay on due to truck resetting itself but loss of power happened typically when I was in parking lot or driveway. Could have easily been on the highway which could have been deadly. Dealership found that the battery cables and module were not getting enough power to truck. Replaced these and it seems okay now. Total cost was ~$1200. I see several recalls for battery cable issues but nothing pulling on my 2018 Chevrolet Silverado 1500 LTZ crew cab.

The vehicle was a service loaner provided to me but it was factory new with the sticker still on the window. The dealer told me they would still sell the vehicle as new. When I started it, I had a display screen that indicated it was in a Demo mode. Taking it out of Demo mode would have required me to confirm that I had purchased the vehicle. The volume control and power control for the audio were not working at all. The volume was probably at 30% where I could hear but not clearly and also just loud enough that I couldn't hear sounds outside the vehicle well. This condition was not affected by the audio source. I could no change the volume in Bluetooth, AM, FM, XM, or Phone modes. I also could not turn off or mute the audio by pressing the Power button. There was a responding fire truck behind me on the road and I went to turn off the audio to hear it clearly and I was not able to. I took a video of this condition happening multiple times as photographs don't help to demonstrate much. I did notify the dealer by sending the Service Manager a text message with the video and said he would notate the condition. This persisted through multiple days and multiple restarts. After approx. 4-5 days, the expected functionality returned with no action or updates that I am aware of.

- #11686381September 10, 2025FUEL/PROPULSION SYSTEM
I was slowing down when coming upon another vehicle. When I accelerated there was no power and I realized the engine had stalled. I was in the outer lane of 3 lanes of heavy traffic and was able to coast into a parking lot and stop. Luckily I was in the outer lane and in the right place at the right time to be able to get out of traffic. I got a text message instantly from OnStar saying to have the engine and transmission serviced immediately. The engine would not start. I called my dealership and told them what happened. They gave me a phone number for a tow service who towed the truck to their dealership garage. I had no prior messages or symptoms that the engine was going to fail. The dealership removed the faulty fuel pump module and replaced the fuel pump control module. I feel very fortunate that my wife and I were at a place where we could exit the highway and get help quickly.

We have been calling and trying to get this recall taken care of. Every time we call they never have the parts and "will let us know". Its getting frustrating getting these notices from different sources and not being able to take care of it. Please help us to het this done by directing us to a repair shop that has the parts available.

- #11686004September 9, 2025LANE DEPARTURE: ASSIST, LANE DEPARTURE: WARNING, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
When I bought the truck it was working fine. Went through Chevy and got all the inspections done before buying the truck and it worked fine. About 2 months later my front collision automatic breaking, lane keep and assist, and automatic high berms stopped working. I’ve talked to many other Silverado owners and this is happening to them too. Even when trying to reset it. With the amount of times this has happened to all these Silverado’s a recall should absolutely happen because having to pay for this is crazy when it’s happening to so many vehicles

My forward collision light what on sp i took the vehicle in to have it fixed under warranty. Upon having that part replaced the dealership informed me my OnStar module (unsure of technical name) was bad and that the vehicle would be inoperable until it is replaced. The part is on backorder with no ETA. My safety is a major concern as this module controls several safety braking features and with me unaware of its malfunction a collision would have caused severe injury or death.

While driving "No Oil pressure" alarm went off on my 2019 Chevrolet Silverado. I had to immediately pull off to the side of the road, upon inspection the Oil cooler line separated at the crimp joint. This is the exact failure mode as described in "N212326940 - Customer Satisfaction Engine Oil Cooler Lines Detaching from Crimp". I have my truck at a GM dealership and they said my VIN is not included on the TSB. I also contacted GM Customer Service and was told the same that my VIN was not effected by this issue. My failure is the same mode as the documented issue, it has to be related to part quality issue as described on N212326940. The GM dealership is charging me $2000 to replace the line.
