I was driving down my neighborhood road about 20mph there were no other cars around on the road at all only mailboxes and cars parked in driveways. The car suddenly slams the brakes hard enough the tires were squealing and threw us forward thankfully me and my child had our seat belts on. It scared the heck out of us cause there was no warning and definitely not expected.

While operating the vehicle on both interstate highways and local roadways, an intermittent malfunction occurs involving the All-Wheel Drive (AWD) system. The instrument panel displays a warning message stating, “Service All-Wheel Drive System.” When this warning appears, the vehicle automatically disengages from AWD and reverts to Two-Wheel Drive (2WD). Once this occurs, the system does not allow the driver to manually return the vehicle to AWD. This issue has occurred on more than three separate occasions. Notably, there have been instances where the warning indicator illuminated immediately upon starting the vehicle, prior to any extended operation. When the malfunction is present, the vehicle exhibits abnormal driving behavior, including a noticeable jerking or hesitation that feels consistent with a transmission-related issue. The failure is unpredictable in nature, with no clear pattern or triggering condition identified. This creates a significant safety concern, particularly when operating the vehicle at highway speeds or in conditions where AWD capability may be necessary for proper traction and control. An appointment has been scheduled with an authorized dealership for diagnostic evaluation; however, at the time of this report, the vehicle has not yet been inspected or repaired, and the issue remains unresolved.

It appears that both driver's and passenger's inertia locking mechanism are defective. They do not lock keeping driver and passenger from going forward into dashboard. I had to make sudden stop the other day, and wife (passenger) went flying into the dashboard and hit her head. After assuming the seat belts auto locked when in use, we were surprised. Since then we pull the seat belts all the way out and release them and have to repeat this step 8-10 times before it catches, and then a slight move forward unlocks the restraint. SOmething isn't right about this, and I'll bet numerous cars are the same way, you only know once you have to stop quickly. I reported to the dealer, but the want $200 approximately just to look at it, and maybe $400-$800 each to fix something that should be covered by GM.

Car started fine, pulled it out of the garage, shut it off, went out to start it up two hours later, it started after cranking a bit but stalled, started for a quick second again but stalled again right away, just kept cranking as I tried it, the check engine light on, I put a code reader on and code told me it was the code for fuel pump module, my vehicle is a 2022 equinox and only has a little over 9300 miles on it. Definitely not something that should’ve happened with that low of miles but yet somehow my VIN was not included in a 2023 recall, I was told it wasn’t anyways!

While driving on the freeway, my vehicle displayed a “Service All-Wheel Drive System” warning and unexpectedly switched into two-wheel drive. This created a serious safety concern due to sudden loss of traction and control. Additionally, on multiple occasions while reversing, the vehicle abruptly applied the brakes without any obstruction present, displaying a message that automatic braking had engaged. My foot was on the accelerator at the time, and this created a sudden and unsafe situation. The vehicle has been taken to the dealership for repair, but due to the unpredictable nature of these issues, I no longer feel safe operating the vehicle.

I am experiencing multiple unresolved defects with my 2026 Chevrolet Equinox affecting drivability, braking behavior, and vehicle structure/fit and finish. The primary issue is a persistent vibration, shimmy, or wobble during moderate acceleration from a stop. This condition is felt through the steering wheel, seat, and floor, and occurs consistently during low‑speed launch and normal acceleration. In addition, the vehicle produces a snapping or grunting noise during hard braking, as well as rough and loud movement noises from underneath the vehicle during normal operation, which raises concerns about braking and drivetrain integrity. There are also body and interior fitment defects. The front passenger door does not sit flush with the rear door panel and appears slightly open when closed. The dealership service manager stated that correcting this condition would require removing and pulling the rear door panel outward so both doors align. The glove box door also does not sit flush when closed, and the service manager advised that replacement might make the condition worse. These concerns were reported to an authorized Chevrolet dealer. The dealer confirmed that General Motors has acknowledged the acceleration vibration condition in a Preliminary Information service bulletin (PIT6466) affecting 2025–2026 Equinox vehicles and advised that GM engineering is investigating the issue and that no repair is currently available. The combination of drivability issues, abnormal noises during braking, underbody movement sensations, and body misalignment affects the use, value, and safety perception of the vehicle. The vehicle has very low mileage, and several of these issues have been present since early ownership. I am submitting this complaint to document these conditions and request that NHTSA monitor potential powertrain, braking, suspension, and vehicle structure concerns if a defect trend exists. All of these defects have been acknowledged by the Dealership Service Dept

This issue appears to be related to how the bumper is attached (clips, fasteners, or design), not due to any collision or damage. A loose rear bumper creates a potential safety hazard, as it could detach while driving or pose a risk to other drivers. The vehicle first experienced a serious malfunction on October 12, 2025, while driving on an expressway at 65 mph, and the same issue recurred on March 8, 2026, while driving on a 6-lane, busy highway at 45 mph. The condition involves the vehicle unexpectedly shutting down, rendering the acceleration pedal useless and causing the car to glide to a complete stop in traffic, creating a significant safety concern and rendering the vehicle unreliable for normal use. The vehicle has been at the Chevrolet dealership since March 8 2026 with them unable to replicate the issue. There were no warning lamps, messages, or other symptoms of the problem before the failure. I called OnStar both times it happened immediately after the incident, with neither time resulting in any negative results on the report.

This is a 2025 vehicle with 25,000 miles and the Check Engine Soon light stays illuminated due to a faulty PCV Valve or possibly a poorly designed crankcase. The engine code is P04FB and is commonly found as an issue when you do a search. As a safety precaution, the vehicle is programmed to not allow the remote start to work as designed when the engine light is on so this issue affects the overall functionality of the car. Having spoken with the dealership, they report seeing that engine code often and that the part is on backorder. The dealership seems to think it's the crankcase causing the issue and not the PCV Valve as the fix they have been ordering for them is the case. It will potentially go away as the weather is hotter, but if a chilly night or morning comes up, then the light will come back on because there is moisture buildup on the PCV Valve when the temperature drops that causes the light to come on. With all of the complaints I have read online for this very issue, I would like to see NHTSA step up and make Chevy fix their problem. Thank you.

Sometimes when I am backing up, the breaks will slam on. Back in 2025 first I noticed, but thought it was human error, just wasn’t sure. After it happened more than a couple of times in 2026, I realized it wasn’t me and must the safety warning system? I googled after it happened this month and (phantom breaking) popped up for the Chevy equinox. I Have an appt. With Chevrolet to check it out in 2 weeks. Scary when it happens. I’ve only noticed it so far when backing up. If it were to happen other than in a parking lot, that could be dangerous, especially if I am going faster.
