I am reporting a serious safety issue with my vehicle, purchased from Ed Morse Cadillac. The vehicle has unexpectedly shut off while I was driving on multiple occasions (3 times to date). When this occurs, the entire infotainment and dashboard screen goes completely black, and most alarmingly, the brakes become unresponsive or extremely difficult to engage. This creates a highly dangerous situation where I am unable to control or stop the vehicle safely, putting myself and others at serious risk of a crash or injury. This is not a one-time occurrence—it has happened multiple times under normal driving conditions. I have contacted the dealership, but they have ignored my attempts to resolve the issue. This problem clearly involves critical vehicle systems and should be investigated as a potential defect. I am requesting that the NHTSA open an investigation into this issue, as this vehicle poses a danger to public safety.

AIR BAGS, SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I am submitting this complaint to report a significant safety system failure in my 2025 Cadillac XT4 during an accident that occurred on April 12, 2025, in Dallas, Texas. During the incident: •The Anti-lock Braking System (ABS) did not activate, and I was unable to stop in time to avoid a collision. •Neither the driver nor passenger airbags deployed, despite sufficient impact that should have triggered deployment. •OnStar Automatic Crash Response did not activate, nor was I contacted by any emergency response service through the vehicle. None of the vehicle’s safety systems functioned during this event, despite being standard factory-installed features. My XT4 had no warning lights or diagnostic messages indicating any malfunction prior to the crash. This failure closely resembles previous complaints noted in NHTSA filings (e.g., ABS sensor failures, OnStar module failures, and the 2025 XT4 passenger airbag recall). I am extremely concerned that this may point to a broader defect within the model line’s safety infrastructure, including collision sensors, electronic communication systems, or airbag deployment logic. I respectfully request that NHTSA investigate whether a defect exists in the braking or airbag systems, particularly where no safety features activate in a crash. I would be willing to provide repair records, crash photos, and allow the vehicle to be inspected if needed. Thank you for your attention and for helping to ensure driver safety through proper oversight.

While driving the car rapidly decelerated along with loss to steering control.The engine essentially shutdown and would not accelerate past 30 MPH with full force. Oil temperature was 280/300 and rising. Warnings came on the car stating service all wheel drive system,service safety restraint as airbags will not deploy, service transmission, service power steering along with the check engine light.I could not maneuver the car nor was the car drivable.Car was towed as it could not be driven.Car put myself and others at extreme risk.If I was on a highway myself or others would have been killed.Car was bought on [XXX].Incident occurred on [XXX]. Car was towed to Bergeys Cadillac Dealership in Plymouth Meeting PA and dealership proceeded to say nothing is wrong with the car.There is a class action suit against this exact issue with the same car and engine and dealership failed to acknowledge the issue and failed to let me return the car.Dealership has continued to say nothing is wrong with the vehicle. My 2023 XT4 had a fuel module failure. I was driving along a busy highway at 50 MPH when i lost complete power. Now i see on the Cadillac forum, many people are having problems with the Cadillac XT4. According to the towing company it happens all the times. I also read on the forum other people were told this too by towing companies. This is Dangerous and you could be left in the middle of a intersection and be T-boned. Shame on Cadillac for not caring. We have another XT4 in the household and now are afraid to drive. My car is currently being repaired. so at this time i do not have a repair invoice. Its still at Sheehan Cadillac Pompano Beach fla.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I was driving with my husband and daughter, when all of a sudden, the vehicle braked out of nowhere (twice) with no one in front of us and the Emergency brake notification flashed on the windshield. Thankfully no one was directly behind us or else my daughter, who was in the back seat, could have been seriously injured. We had the vehicle towed to the dealership and they said this was not common but in fact it is. This vehicle is not safe to drive.

FUEL/PROPULSION SYSTEM
Fuel pump module failed twice. There was no warning lamps, messages or other symptoms prior to the failure. The first time I was driving the second time the car was parked and did not start. See below for details. Safety: Thankfully i was not driving 70+MPH down a busy highway when the car shut down. Given the past recall, the number of Cadillac owners experiencing the problem and that it's happened to me TWICE within three years, it's only a matter of time before serious injury or death occurs. First time: [XXX], Milage: 15,063 i was driving down a small-town highway doing 55 when the car stopped working. All power went off, no acceleration, difficulty steering. I was able to safely get to a side road. Had it towed to local Chevy dealer for repair. They diagnosed the issue: Bad fuel pump driver control module. Second time: [XXX], Milage: 34, 6112 The car was parked at my house and failed to start. Had it towed to local Cadillac dealership for repair. They diagnosed the issue: Bad fuel pump driver control module. Additional information I've found a previous recall on the fuel pump module from 2021 on Cadillac SUVs [XXX] I've found many forum posts of Cadillac users experiencing the same problem. This is just one link for XT4 and the last post from a user with the exact same problem was March 6, 2025, and another on Feb 4, 2025. STEERING
Starting auto on20Feb2025 I was unable to turn the stearing wheel.Turned auto off, restarted same problem. Called Cadallac for repare appointment. Re sstarted the car later operations normal. Car deliverd to Cadalac service dept. scan tool indicated problem. Cadillac repare report -cleared all codes. Road tested no issues.

SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
The AEB system engaged when it should not have with no obstructions on [XXX] . I violently jerked, I thought I hit something but did not. The safety of myself and a person following me would have been been compromised. If someone was closer to me in the rear they would have hit me. My GM Onstar light turned to red and Onstar contacted me to see if I was ok. The problem has been confirmed by Faulkner Cadillac in Bethlehem, Pa. False Automatic Emergency Braking-Bulletin [XXX]. When the occurrence happened the Automatic Emergency Braking warning came on in my Driver Information Center. This is my second Cadillac XT 4 in two years and both have experienced the same thing. As I'm driving and come to a light to stop, the entire vehicle system goes black and cuts off. This has happened to me know 4 times, were the vehicle becomes disable in the middle of the road, during heavy traffic and almost hit several times from the rear. Each time I was in tears, thinking I was going to die and would call OnStar for assistance. The vehicle would not start, keep making this clicking sounds as if it's trying. The vehicle would automatically go into 'Park' and will not release for any other gear. I was told the Fuel Management System, that I was told was not suppose to happen in the 2024 model like it did in my 2022 model. My app also reports a "Transmission System Failure" for a brand new vehicle almost a year old & 7700 miles on it. I can't deal with GMC & my life over their products. I want to get rid of the vehicle and never purchase another one of their products. Highly disappointed & the lack of concern from GMC about their customers.

VISIBILITY/WIPER
Backup camera does not function at night. The issue appeared on day one, the car was leased in this condition in July 2024. The problem has been reported to and known by the dealership and witnessed by the sales manager at King O’Rourke dealership in Smithtown NY. The first record of written correspondence is shared in a photo of an email September 2024. If it’s dark outside without bright street lights it is literally black on the screen. It has also been inspected by the service department and the recent email is attached . They gave me my car back without fixing it the first time. I brought it back again and they changed the camera and updated the software. They said saw the dark screen at night. GM is telling the dealership to test another car of the same make and color. The dealer stated that in the meantime they cannot give me another loaner while they figure out how to fix the problem because GM said it’s functioning normally (photo of email attached). I am currently driving the car and live on a very dark street. This is VERY unsafe and GM is not taking this seriously.

SERVICE BRAKES
I purchased this car used in early 2024. Shortly after buying it, the brakes began squeaking while backing out of the garage in the morning. This is the only time day this occurs. It has been a constant complaint since. I took it to Suburban Cadillac in Troy, Michigan where I purchased the car. They said it is a characteristic of the car and GM is aware of it. Their only suggestion was to spend $1,800 to replace all four brakes and rotors. This causes anxiety as the noise is the same as if the brakes are worn out. The shop at the dealer checked the pads and they all had 8 to 9 mm left. Also, the mileage on the car is about 22,000 miles.

Driver's and passengers' front windows. When driving in sunlight the windows are hazy/milky. Inspected by dealership. There are no warning lamps, messages or other systems 1st appeared July 29, 2024 Have brought the vehicle on numerous occasions to the dealership for this issue. When driving during daytime in sunlight both driver's window and front passenger window are hazy/milky. Dealership states they cannot replicate the issue. There is nothing to replicate, the haze and milky substance are on the windows.

ELECTRICAL SYSTEM, SERVICE BRAKES, BACK OVER PREVENTION: AUTOMATIC SYSTEM BRAKING
On four occasions now, the car slammed on the brakes as if the collision detection system was going off. Someone did almost hit me from behind as they needed to slam on their brakes due to my cars collision system going off when not supposed to. This has been well documented with Cadillac and the leased vehicle has been back in the shop 4 months out of the year we owned it. It is a major safety issue and am very concerned it will happen at a time that may cause a bad accident, injury or loss of life if something is not done. Cadillac has replaced many electrical parts and tells us it is fixed when in fact it is not and it happened again this week and will be going back to the shop for the 5th time next week.

The contact owns a 2021 Cadillac XT4. The contact stated that while stopped at a traffic signal, the signal turned green; however, when the accelerator pedal was depressed, the vehicle hesitated before responding as needed. The contact stated that the vehicle lunged into motion. The contact stated that the failure was intermittent at first but became a recurring failure. The contact stated there was no warning light illuminated. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the cam shaft and transmission needed to be rebuilt. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 45,000.

On June 13th 2024, and on July 30th 2024, while driving on highway my Cadillac XT4 suddenly jerked forward as if I had been struck from behind by another vehicle. The vehicle seemed to have lost all power and appeared to stall in the lanes of the highway. This created a very dangerous situation, causing vehicles behind me to swerve in order to avoid a collision. Be advised that the vehicle did not turn off, because I didn’t have to restart it, however it felt like it did. This situation could have caused myself, and passengers to be injured or killed, especially if a large truck was behind me and unable to stop. Luckily I was able to steer the vehicle onto the shoulder off the highway.

Driving to a local car wash, I stopped at a traffic light. When I tried to accelerate the transmission warning and power reduction waring flashed on HUD. I could not drive or place the car in reverse, the car had lost all its mobility. I had to let the car coast downhill to pull the car to the side of a busy street. I was fortunate this did not happen on a congested highway, but regardless the inability to move a car off a busy street is an extreme safety hazard. The vehicle needed towed, and after 31 one days was returned to me, four days later while on the highway bolts were falling off the undercarriage of the vehicle. Upon further inspection the rear exhaust bracket had fallen off and was missing 3 bolts.

The contact stated that prior to the complete failure, there were several error messages displayed on the screen. The contact stated that the only function on the instrument panel that was operating as needed was the speedometer. The vehicle was taken to a dealer and diagnosed with failure within the radio unit. The manufacturer was not made aware of the failure. The vehicle was repaired; however, the failure persisted. The failure mileage was approximately 3,000.

The contact owns a 2021 Cadillac XT4. The contact stated that while his wife was reversing and exiting the residential driveway, the back over prevention sensor activated. The contact stated the back over prevention warning light illuminated. The contact stated that there were no obstacles behind the vehicle. The vehicle was taken to the dealer, but the failure could not be duplicated. The contact stated that the vehicle was retrieved and shortly after exiting the dealer's lot, the failure reoccurred. The vehicle was taken to another local dealer where the failure was unable to be duplicated. The contact also stated that the vehicle was taken to Auto Zone and the failure was unable to be duplicated. The vehicle was then taken back to the original dealer. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 32,000.
