The car allows you to set favorite channels and change them from the left scroll wheel on the steering yoke/wheel. It works for FM radio, but stopped working last November for XM radio. After 9 contacts with Tesla, they have not corrected the defect. In order to change the favorited channels, it requires 3 or 4 taps on the screen - which requires removing your eyes from the road for 4-6 seconds. When it works properly, tapping the scroll wheel left or right changed the channel to next or previous favorited channel. I neatry went off the road last week trying to change the XM radio to the next favorite. This did work on both my previous model Tesla sedans from 2013 until 2023. It worked on this car for several months, but stopped working in November 2023. Nine requests to Tesla for repair have have not been successful in any action.

The yoke steering wheel had multiple issues ranging from cosmetic to skipping sometimes while turning. To the best of my knowledge, based on talking to Tesla Service professionals, Tesla is aware of this problem and has internally created a document to change these yokes, but it is not doing as regularly as it should. The service staff refuses to clarify the conditions of warranty repair for this faulty part, and I suspect a major incident can happen with these Yokes failing.

Subject: Urgent ADAS Issue: UI_a114 Error in Teslas with EAP/FSD in East Oahu I'm reporting a critical, location-specific safety concern affecting Teslas with Enhanced Autopilot or Full Self-Driving in East Oahu, particularly when heading westbound. This widespread issue, known as error UI_a114, disables all Advanced Driver Assistance Systems (ADAS) without warning. It impacts essential safety features, such as Adaptive Cruise Control, Lane Keeping, and Emergency Braking, presenting significant risks. Tesla Support in Honolulu acknowledges this as a multi-year issue among East Oahu Tesla owners with EAP/FSD, regardless of activation status. This suggests a software-related problem or environmental interaction unique to this region, necessitating urgent investigation and resolution to ensure driver and public safety. Further details are provided in the attached PDF. Your attention and action on this matter are crucial.

The vehicle consistently displays the message "Rear right safety restraint system fault" on the dashboard since its delivery in January 2024. This indicates a potential malfunction in the airbag or seatbelt system, posing a safety risk to passengers occupying the rear right seat while the vehicle is in operation. Despite multiple inspections and diagnoses by Tesla, the dealership has attributed the issue to a firmware or software glitch without providing a definitive solution. Consequently, the warning message persists on a daily basis, necessitating urgent resolution to ensure the safety and functionality of the vehicle's safety systems.

The yoke steering wheel only had a small horn button on the right side, only accessible by right thumb. If you are in the middle of a turn that requires more than a quarter turn, then your hand cannot possible be in the right place to use the horn. The lack of a horn in the center of the steering (like every other car) is unsafe. On many occasions I have found myself needing the horn but unable to press it fast enough and have had to take other evasive actions to avoid other vehicles.

This wasn't just one incident, but many incidents since I got the car on [XXX]. The horn is not in the center of the steering wheel where it is on every other car. It is a tiny button on the steering wheel that is impossible to find if you are turning the wheel and even harder to find when someone is going into your lane and is about to hit you or when they are backing out of a space and you have no time to look for the tiny button. I've come close to getting in many accidents because of this. Tesla has since changed the steering wheel/Yoke so the horn is where it should be (in the center of the steering wheel) but they are not replacing the old dangerous ones. Seat belts in rear seats are not installed correctly. The tongue of the seatbelt does not fit into the buckle properly/ correctly. The service center was notified of this issue. A service apportionment was set up. The service technician identified that they see the problem. The car was left to be serviced and the problem to be corrected. However when we were notified that the service of complete and the car was ready to picked up, upon picking up the car there was no change. The seatbelts are the exact same way as when we dropped it off. We were told that seatbelts function as expected. Our children carseats are in those seats and we are concerned for their safety. These factors, coupled with the volume of recall notices we’ve received on the vehicle so far, give us significant pause and raise major concerns about the composition, fidelity, and safety of this Tesla Model X.

Horn is not in the center and rather a button. This makes it very hard to hit in an emergency. Please make them replace all past and new steering wheel with this issue.

My steering wheel doesn't have a usable horn functionality. There is a subtle button on the side of the wheel, which isn't even a physical button, and it's very small in size. My wife and I had a few situation already where we couldn't do a horn signal quickly enough and we barely escaped an accident in those cases. The existing horn button is very hard to use, it's placed non-intuitively, it's impossible to find without looking at the steering wheel, and it's very hard to use when the steering wheel isn't positioned straight. All other cars I driven over my lifetime had a horn right in the center of the steering wheel, which is the most obvious and usable placement, given the urgency of the situation when drivers are supposed to use it.

While operating my vehicle, it is required to use turn signals to indicate turning intention to other drivers. Even after thousands of miles of use, the 'Capacitive touch' buttons on the steering wheel that Tesla has are unsafe, unusable, and infuriating. These buttons are difficult to locate, difficult to engage with and impossible to use quickly or reliably. Every Tesla without turn signal stalks should be recalled. I often press the wrong button, no button, or just have to simply give up on signaling because it is impossible to safely or reliably use the buttons provided. Turn signal stalks are easier, faster, more reliable, and thus safer. Please help end this nonsense by requiring tesla to fix this. thank you.

Turn signal buttons do not work for short periods of time while driving. It is available for inspection upon request. I am unable to signal for merging and turning which is unsafe. The problem was not reproduced, but was confirmed. Yes the vehicle has been inspected by the manufacturer and corrective action was taken. However the issue has not been fixed. No other warning lights, symptoms, or messages.

On an occasional basis, and during a critical event, it is imperative that the operator of any vehicle be able to quickly, and reliable use the horn. This is needed to alert other drivers of dangerous situations, or bring attention to those around the vehicle. After driving this vehicle several thousand miles, I have determined that the horn 'button' is not something anyone can commit to memory or use safely when needed quickly in an emergency. Muscle memory of all cars is 'push center for horn'. The 2021-2023 Tesla Model X uses a button that is touch sensitive and nearly impossible to operate quickly or reliable in an emergency situation. Tesla has corrected the issue with the 2024 Model X, however that leaves thousands of cars not repaired. Please require Tesla to correct this asinine oversight.

Center steering wheel horn is not included on these vehicles, which should be considered a significant safety issue as we are trained from a young age to hit the center of the steering wheel/yoke in the event of an imminent accident. I was recently driving when another vehicle ran a stop sign, and I was barely able to avoid them; however, when hitting the "horn" to alert the other driver, it didn't work as the horn button is not in a typical location. This should be considered a significant safety risk. Elon Musk himself indicated a software update would fix this issue; however, it has not. Tesla now indicates it requires a new wheel/yoke and has not yet made it available or covered under warranty.

- #11578455March 20, 2024LATCHES/LOCKS/LINKAGES, ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE
The contact owned a 2023 Tesla X. The contact stated that while driving at 25 MPH, the contact became distracted by an incoming phone call and crashed into another vehicle. The air bags deployed upon impact and the contact became locked inside the vehicle. The whereabouts of the other driver were unknown. The police were called to the scene but were unable to assist with the contact's removal from the vehicle. After close to an hour, the contact finally found a latch to unlock the vehicle. The contact sustained a headache as a result of the accident but did not seek medical attention. A police report was filed. The vehicle was towed from the scene to a Tesla service center. The contact stated that the forward collision avoidance feature failed to operate as needed which should have prevented the accident. Upon investigation, the contact discovered that the vehicle was included in NHTSA Campaign Number: 23V862000 (Latches/Locks/Linkage; Electrical System). The manufacturer was informed of the failure but offered no assistance. The vehicle was later totaled without the contact's knowledge. The failure mileage was approximately 1,000.

It is commonly known that the drive shafts on the Model X fail. This is not a wear component and should be a recall. The suspension is also part of this issue and there are after market solutions to fix the suspension that is causing the failure of the half shafts.

A couple of days ago, when I was dropping off my child at school, I parked in a spot that required parallel parking to save time as I was in a rush. After reversing into the spot, I just needed to move forward a bit by 1-2 feet. I naturally slide the gear shift up on the screen. Unfortunately, it didn't slide into place, and the car was still in reverse (R), but I didn't realize it. I was already very close to the car behind me. When I pressed the accelerator, the car continued to back up. Luckily, I slammed the brake, avoiding a collision. Just wanted to share that even after a year and a half of driving a Model X, mistakes can still happen. After reading what happened to [XXX] , I posted what happened to me on a social platform and someone else shared they had a similar experience that needed up with a crash. So I decided to report mine and encouraged them to do the same. I like the minimalist design but I need to stay alive to enjoy it. As Tesla is moving away from stalkless design in more models, I hope they can do something to make the current shift more noticeable. The horn in the Tesla Model X is very hard to operate in real world driving environments because it is a haptic button that’s is placed amount other haptic buttons. Several times over the past few months I have been unable to safely operate the horn in situations where another driver was coming into my lane or otherwise I required the use of the horn in an emergency situation. I had to take my eyes off the road in order to look for the haptic button and remember where it was placed. In my opinion, Tesla over-engineered the horn and made it unsafe to operate for the driver
