My steering suddenly got extremely stiff on the highway while driving. There was a warning that assist will be reduced but no assist was present. I had to use all my strength to turn the steering and pulled into a parking lot with great difficulty. Then I reset the main computer and the power steering was functional again. I was lucky since the traffic was moving slower than usual but it could have been very dangerous and was a major safety concern.

The contact owns a 2022 Tesla Model 3. The contact stated that while operating the vehicle, abnormal crackling and bubbling sounds were heard coming from underneath the rear seat. While driving later, an electrical arcing sound emanated from the same area. While at a local charging station, the contact discovered that the battery failed to charge when connected to a high-voltage charger; however, the battery could still be charged at the contact's residence using a 110-volt charger. The vehicle was driven to a local dealer, who was unable to identify the problem, and no service was performed. The vehicle was driven back to the charging station, but the high-voltage charging failure persisted. The vehicle was then towed to a local service center, where it was diagnosed and it was determined that the high-voltage battery required replacement. The high-voltage battery was replaced, and the vehicle was repaired. The manufacturer was notified of the failure. The failure mileage was 38,200.

Vehicle: Tesla Model 3 Issue: Power trunk failed to detect obstruction Date of incident: July 5, 2026 Location: Surprise, AZ During normal use of the vehicle, my child pressed the “close trunk” button while I was leaning into the trunk area. The powered trunk began closing and did not stop when it made contact with my head. Instead of reversing direction, the trunk continued applying downward force until I moved out of the way. This indicates a failure of the trunk’s obstruction detection system, which is supposed to prevent injury by stopping or reversing when an object or person is detected. This malfunction poses a significant safety risk, especially to children who may accidentally activate the trunk or to adults loading items. A powered trunk should not be able to continue closing on a person’s head. I request that NHTSA investigate whether this is a defect in the Tesla Model 3 trunk sensor system and whether similar incidents have been reported.

After disengaging Full Self-Driving (FSD), a mandatory questionnaire pop-up now stays on screen until manually answered, instead of auto-dismissing after a few seconds as it previously did. This forces the driver to interact with the touchscreen immediately after taking back manual control — a moment when full attention should be on the road, not a pop-up. The prompt blocks part of the instrument/display screen until it is tapped, and in some cases requires a minimum 5-second voice response to dismiss (a previous workaround allowing a near-instant voice note to clear it was removed). The pop-up has also been observed persisting even after the vehicle is shifted into Park. This creates a distraction risk during an already safety-critical moment (immediately following a system disengagement, which by definition means FSD failed to handle the driving task). This behavior has persisted across at least 3 software updates. Multiple attempts to raise this with Tesla directly have not resulted in a fix or response. Other FSD users have reported the same complaint. No collision has occurred as a result of this specific issue, to my knowledge, but the design creates a foreseeable distraction hazard during takeover events, which are often already high-risk moments.

Lane keeping assistance turns on with every drive no matter how many times I set it to off. Where I live there are many potholes and poorly paved roads, along with poorly marked roads. As I am swerving around obstacles the car will fight the steering inputs I am giving it, trying to steer me back into whatever I was avoiding. This is a safety issue, and is not a problem on our older Tesla model 3 performance, as once that vehicle is set to off, it stays off. Now, as I start every drive I am navigating through a couple screens to turn off this "safety" feature.

On July 1, 2026, the vehicle was serviced by the manufacturer for GPS and camera malfunctions, and the vehicle computer was replaced. On the morning of July 2, 2026, during the first use of Autopilot after that repair, the vehicle experienced repeated serious driver-assistance malfunctions over a short period while traveling on a highway. While Autopilot was engaged, the vehicle repeatedly moved/steered toward the center of the road and near the path of a truck in an adjacent lane. The truck nearly struck the vehicle. The driver had to manually intervene to maintain control and avoid a potential collision. Video of the incident is available upon request and shows the truck and manual intervention. The vehicle was returned to the manufacturer service center the same day. The service center reviewed logs, performed an inspection/test drive, and stated that no hardware faults or issues were found and that the test drive was normal. However, the incidents were intermittent, occurred during highway Autopilot use, occurred the morning after a computer replacement related to GPS/camera systems, and were captured on video.

Tesla Full Self-Driving (Supervised) repeatedly ignored the “Avoid Toll Roads” navigation setting and attempted last-second lane changes into Express Toll Lanes at freeway speeds. This has occurred approximately 10 times. I was repeatedly forced to take over steering immediately to avoid entering the toll lane, creating a serious safety risk due to sudden maneuvers in heavy traffic. On several occasions, I could not safely intervene before the lane change was completed. Tesla Service Center inspected the vehicle, found no hardware or camera faults, and stated the behavior appears to be software-related.

On May 24, 2026, at approximately 8:28 AM, I activated Actually Smart Summon on my 2025 Tesla Model 3 using the Tesla mobile application to reverse the vehicle from my garage. Before this incident, the vehicle displayed no warnings, alerts, messages, or other indication of a problem, and I had previously usd Actually Smart Summon without similar behavior. Shortly after activation, the Tesla mobile application became unresponsive, and the vehicle did not stop despite my attempts to stop it through the app. I attempted to open the vehicle door during the incident but was unable to do so while the maneuver continued. Because I could no longer stop the vehicle through the app or access the vehicle, I placed my phone in my pocket so I could use both hands to try to stop the vehicle and prevent further damage. At that point, I was no longer pressing or holding the Actually Smart Summon control in the application. As the vehicle continued reversing, I repeatedly repositioned myself behind it and pushed against the rear of the vehicle with my body multiple times. Despite this physical intervention, the vehicle continued reversing and applying rearward force, forcing me to repeatedly move out of its path to avoid being pinned between the vehicle and my neighbor s garage. The vehicle pressed into my neighbor s garage with enough force that the impact transferred to the parked vehicle inside the garage. It did not stop upon the initial impact and continued pressing into the garage afterward. This sustained movement is clearly visible on the preserved TeslaCam footage and independent home-security recordings. I narrowly escaped being crushed between the vehicle and the garage while attempting to stop the vehicle and prevent further damage, sustaining a muscle strain in the process. The incident created a serious risk of bodily injury in addition to substantial property damage. I do not know which specific component or system failed. Based on the sequence of events, the incident

My 2023 Tesla Model 3 (VIN [XXX]) has a recurring safety defect in its driver-assistance system involving two unsafe behaviors. First, “phantom braking”: while driving with Autopilot/FSD engaged, the vehicle decelerates suddenly and without warning when there is no obstacle or threat in its path, creating a rear-end collision risk. This was first reported within the vehicle’s warranty period and has recurred. Second, on the most recent occurrence, with a Tesla service technician riding along to witness the issue, Full Self Driving (Supervised) failed to proceed safely through the intersection of [XXX] and [XXX]. The vehicle stopped mid-turn while another vehicle approached from the opposite direction. I had to take manual control to complete the turn and avoid a hazard. The Tesla technician witnessed this directly, and the service summary states FSD “did not proceed through the intersection properly.” The vehicle has been to a Tesla service center twice for this defect. On [XXX], at approximately [XXX], I was traveling northbound at the intersection of [XXX] and [XXX], with the vehicle’s self-driving feature engaged. As I approached the intersection, the vehicle unexpectedly proceeded into a left turn despite the traffic signal displaying a red left-turn arrow. I immediately recognized that the vehicle was not stopping and had to take control by forcefully jerking the steering wheel and slamming on the brakes to prevent the vehicle from running the red light and potentially causing a serious collision. There were no actions on my part that would have caused the vehicle to continue through the red left-turn signal. The weather was clear, the roads were dry, and there was no rain or any adverse weather conditions that would have affected visibility or vehicle operation. I was driving today in California, to TopGolf. I had to cross a railway crossing around [XXX] I think, or [XXX] - one of those 2, between [XXX] and [XXX] pm.. I was using Tesla's FSD driving to go on a local road. I was not distracted or occupied - the vehicle was going at a fairly high speed like maybe 35 or so, and in a couple of seconds, it broke the red and white safety gate at a railway crossing, inspite of it being down and the train being on the tracks - I WAS IN SHOCK. The car completely missed the red and white gate, and just blew past it, damaging my windshield completely to smithereens and throwing glass all over the car. I was in such SHOCK and TRAUMA that I literaly wanted to exit the car and sit down somewhere, but i gathered my composure and drove quickly to the nearest safest place I could stop, and got into some office parking garage. I then called the cops and reported it. This was such a shock, and thank the Almighty God that I wasnt hurt and that the train didn't just hit me. This HAS to be recalled, and there is no way this should be allowed on the roads. I used FSD a Lot and i will never use it again. The contact depressed the accelerator pedal immediately to continue driving. The vehicle was taken to the dealer three times. The dealer reset the mirrors and sensors; however, the failure persisted. The vehicle had not been repaired. The dealer suggested that the contact purchase another vehicle. The manufacturer was not made aware of the failure. The failure mileage was unknown.

Vehicle was operating under Tesla Full Self-Driving (Supervised) on [XXX], on [XXX]. While FSD was engaged, the vehicle struck a large object/debris that was present in the travel lane. The system did not appear to detect or avoid the obstacle. The impact caused severe damage to two wheels and two tires, and the vehicle required towing. Tesla Service later confirmed that the damage was caused by road debris and replaced the damaged wheels and tires. I am submitting this report because I am concerned that the FSD system failed to identify or respond to a significant roadway hazard. On June 7, 2026 an accident occurred on Overlook Parkway, Riverside southbound (near chabot ct intersection) involving my 2024 model 3 that was in FSD mode (Full self driving). The car be veered from the street and hit a light pole as the road was curving to the left. Airbags were deployed, and the car was a total loss, but nobody was hurt. The carhad automatic emergency braking and lane keeping assistance and despite this in FSD Full self driving mode managed to hit the lightpole and the accident occured. Can you investigate if this is a defect in the safety features of the car, or Tesla's FSD software or Automatic emergency braking and lane keeping assistance software? Thank you.

A fatal crash occurred involving a 2023 Tesla Model 3. The vehicle departed the roadway, impacted a guardrail, struck a utility pole on the driver’s side, rolled down an embankment, and came to rest upright. The crash resulted in catastrophic intrusion into the driver’s compartment and a fatal injury to the driver. I am requesting that NHTSA investigate the crashworthiness and occupant protection performance of this vehicle, including the structural integrity of the occupant compartment, airbag deployment and effectiveness, and the performance of any advanced driver assistance systems, including collision warnings and automatic emergency braking. I am also requesting review and preservation of all available vehicle data, including Event Data Recorder (EDR) information, vehicle telemetry, driver assistance system status, warning notifications, braking data, steering data, and any other information recorded before and during the crash. At this time, I do not know whether a vehicle defect or system malfunction occurred. My concern is whether the vehicle’s safety systems and occupant protection systems functioned as intended and whether additional vehicle data may help determine if any safety-related issues contributed to the severity of the crash.

This is my fourth high voltage battery failure on this vehicle. Battery replacements occurred at approximately 13,000 miles, 60,000 miles, 96,000 miles, and now at 121,000 miles. The first failure occurred at 13,000 miles on a nearly new vehicle. During the second replacement a Tesla service technician verbally stated that 'the refurb process has vastly improved since your last battery' before retracting the statement when questioned, strongly suggesting refurbished units were installed in a vehicle under original warranty. Tesla has declined to cover the most recent failure citing warranty expiration, despite this being a documented recurring defect that originated well within the warranty period. The vehicle is currently inoperable due to high voltage battery failure. I was around 40 days outside of warranty with this last failure.
