Sunroof glass failure that occurred on my 2024 Mazda CX-5. While driving to work on May 9th at 11:16 pm going 67 miles per hour on route XXX in [XXX] , I suddenly heard a very loud popping/explosive sound from above the vehicle. Upon safely pulling over, I discovered that the sunroof glass had shattered unexpectedly. There was no known object strike that I or my two passengers observed, no prior cracks or chips visible before the incident, and no warning signs of damage beforehand. The glass appeared to have exploded outward/ upward and appeared curved upward. Most of the glass we believe hit the vehicle behind us as it was gone by the time we were able to safely pull over. It was approximately 58 degrees outside with no inclement weather. The vehicle currently has approximately 53,000 miles and has been properly maintained. The incident was extremely startling and created a significant safety concern while driving. Luckily, the interior sun visor was closed when this incident occurred, preventing the glass from entering the vehicle and injuring myself or my two passengers. Because the vehicle is relatively new and the damage appeared sudden and unusual, I would sincerely appreciate Mazda reviewing this matter for possible warranty or goodwill assistance. I have always valued Mazda’s quality and reputation, and I am hopeful the company can help support resolution of this unexpected issue. I have attached photographs of the damage and would be happy to provide any additional information needed. I would also appreciate the dealership documenting whether any definitive impact point or external strike evidence was identified during inspection. My 2022 Mazda CX-5 Signature began producing a loud whirring/rattling noise from the driver-side front headlight assembly. After investigation, the noise was identified as coming from the internal cooling fan located inside the LED headlight unit. The cooling fan, which is integrated into the bi-LED headlamp assembly, appears to be failing prematurely due to bearing wear. This is a known widespread defect on 2019–2023 Mazda CX-5 models. Mazda has already issued Technical Service Bulletin (TSB) 09-015-23, which acknowledges the noise issue and instructs dealers to replace the entire front combination light assembly under warranty. However, no formal recall has been issued. The root cause appears to be Mazda’s design decision to use the full LED headlight beam as the Daytime Running Light (DRL), which keeps the headlight cooling fans running continuously whenever the vehicle is in operation — even in daylight. This accelerates fan bearing wear far beyond normal expectations. The headlight assembly costs $1,200–$1,700 per side to replace out of warranty. Owners whose warranty has expired are facing repair bills of $2,400–$3,400 for both sides. This constitutes a significant safety risk as headlight failure reduces nighttime visibility. I have scheduled a warranty repair at an authorized Mazda dealer under TSB 09-015-23.

On October 16, 2025, while slowly pulling into a parking space, my vehicle experienced sudden and unintended acceleration without driver input. I did not depress the accelerator. Despite attempting to stop the vehicle, I was unable to do so. The vehicle mounted the curb, entered the sidewalk area, and struck a community mailbox, a tree, and two legally parked vehicles. My vehicle and the two parked vehicles were declared total losses. At the time of the incident, pedestrians, including toddlers, were present on the sidewalk. This loss of vehicle control created an immediate and severe risk of serious bodily injury or death. Post-incident diagnostics indicated multiple system errors, including but not limited to: •Powertrain Control Module •Anti-lock Brake System (ABS) •Instrument Cluster •Smart Brake Support / Mazda Radar Cruise Control Of particular concern is that Mazda, through its authorized dealer Rosenthal Mazda of Gaithersburg, Maryland, had prior notice of abnormal vehicle behavior. On two separate occasions before the crash, I reported that the vehicle would jolt suddenly, as if struck from behind, and at times felt as though it was attempting to accelerate without input. On each occasion, I was informed that the dealer could not duplicate the issue. No corrective action was taken, and the vehicle was returned to service. This event constitutes a serious safety failure involving unintended acceleration and apparent braking or electronic control system malfunction in this-model vehicle. Given the involvement of advanced driver assistance systems and braking components, this matter warrants immediate and thorough review.

This vehicle was purchased two months ago. In that time the windshield has sustained multiple significant chips on multiple occasions during routine highway commuting. Some of these are in the line of sight of the driver, causing distractions or visibility issues. Prior to purchasing this vehicle I owned a 2012 Toyota Corolla, which traveled the same route every day and did not receive any significant windshield chips over a period of years. It seems that the windshield of the Mazda is particularly fragile and/or brittle compared to other windshields and suffers chips and cracks too easily.

Life-threatening, unoccupied rollaway vehicle with personal injury. Possible and/or suspected issues: "Not-P" switch circuit malfunction; "false park"; lack of cross-module redundancy; lack of BCM and PCM redundancy; Diagnostic Blind Spot; lack of Occupancy-Brake interlock; systematic safety failure culminating in unintended, driverless, runaway Zombie vehicle with personal injury. Background Info: * Low mileage, mainly in-town errands driving. * Not previously reported quirks, possible latent defects: liftgate opening that was hit-or-miss requiring multiple unlock trunk-unlock doors + get out of the car manual attempts; intermittent off-shutdown double-tap issue to power off car; ill-timed "rear seat alert". * Reported "engine won't start" resulting in three key FOB battery replacements and a locked key FOB. Day of Incident: * Auto-lock and the EBS were not used. * Driver intent was to park and unload, then permanently park. Car was in-park twice before ultimately placed in reverse and backed into position, facing downhill on a slight slope and along a left-side stone retaining wall. * Driver thought vehicle was fully in Park. * Attempted open liftgate via "open trunk, open door" routine, which did not work, so I exited the vehicle to try the manual liftgate button, which also did not work. * Three back-and-forth "unlock trunk/doors + manually press liftgate button" did not work. * Note: Upon opening the door or anytime after, I did NOT hear an audible warning that the car was not fully in Park. Nor did the car move/creep in any direction. * Deducing I must need to shut off the car to open the liftgate -- I leaned in and depressed the on/off button. * This caused the car to move forward, driverless, door open and scraping the stone wall, across a secondary road, over an 11" curb, and down a hill before stopping due to a tree. * I was injured. Pinned between a narrowing door and the car, I tried to turn the wheel right before being knocked down/falling

2021 Mazda CX5 AWD- Non Turbo Engine. Sunny Day, Dry. My Car was clean. No Precipitation. Some shadows of the interstate light poles across the road 200-300 feet in front of me, but they were thin shadows. Sun Was to my Left. I was driving South on May 01 2026 about 10am on the Interstate going Highway Speeds, with adaptive cruise control "on", in the 2nd lane of a 4 lane interstate. Thankfully no other traffic ahead or behind me. Car slammed on the brakes suddenly, pretty hard. I counteracted and added more gas pedal to keep in going. I turned off the Cruise Control. After a while I re-engaged the Cruise Control and had no issues. This could have been a huge accident in busier traffic. Will get the car checked out at Mazda Dealership Service this week.

Sudden acceleration even. Date April 10, 2026, at around 2:30 PM on a clear sunny day. Car make and model Mazda CX5 bought on 7/4/2023 VIN [XXX] , mileage around 10,000, only one owner. While attempting to park at BJ’s Wholesale Club ,when driver started to make a turn to a parking space, vehicle suddenly accelerated, applying the brakes stopped the forward motion, then driver put the car in reverse to try to properly park in a parking space and the car accelerated in reverse, this sudden motion could have killed or seriously injured any person passing in back of the car, when that motion stopped driver put the car in drive to be able to finally park the car and it sped uncontrollably, brakes didn’t stop the vehicle and it crashed against two vehicles parked nearby, damage to the Mazda vehicle was mainly to the left side, the front grill was not damage because driver was not able to control the rapid forward motion. In addition, numerous other 2026 Mazda CX‑5 owners are reporting the exact same problems across multiple online forums and owner communities. These include: • Complete center display blackouts • Random system errors • Telematics failures • Driver‑assist malfunctions • Connected Services not working at all Owners are actively seeking help from one another because Mazda is ignoring customer reports, providing no guidance, and offering no official communication or support. This widespread pattern strongly suggests the issue is not isolated, but rather a potential systemic defect affecting the 2026 CX‑5 platform.

The air conditioning compressor clutch suddenly would not engage, leading to reduced visibility out of the windshield during humid weather in Oklahoma springtime and extended times for windshield to defog. Temperatures inside the vehicle also rose to very high levels without air conditioning available, even with windows down while driving. No crash or injuries but this presented an unsafe situation for a vehicle with only 39,000 miles and purchased new from the dealership. There were no indicator lights, messages, or warnings, other than observation that air conditioner vents were no longer blowing cold air when selecting the AC button after problem occurred. Problem was verified by both an independent mechanic and our Mazda dealership. AC system was overcharged from the factory per diagnostic results, leading to internal AC compressor short and early component failure.

Both LED headlights died within two months of each other on this relatively new vehicle. Outside the three year warranty, but only 27,000 miles (well within the 36,000 warranty period). As it's outside of the three years, the warranty already expired regardless of mileage. One headlight was grinding (fan) while the other's low beam simply died. Each headlight cost $2800 to replace. You can't simply replace a burned out bulb with this headlight design. The entire unit must be replaced at a cost of almost $3000 depending on your dealer. This is a major safety issue when headlights die while driving at night. This is not a user maintenance issue. It is not normal wear and tear. Almost $6000 dollars to replace poorly designed headlights within two months of each other on a new-ish vehicle while being a safety concern constitutes reason enough to issue a recall, and to reimburse owners for previous replacements.

The vehicle exhibits unintended acceleration behavior when the brake pedal is applied. Instead of slowing down, the engine revs and the vehicle does not decelerate as expected. On March 16, 2026, while merging onto a highway and applying the brakes, the engine suddenly revved to approximately 3,500 RPMs while the vehicle failed to slow down. There have been previous issues with the vehicle not slowing when breaks are being applied, nut this is the first instance when it was noticed that the RPMs increased while breaking. This issue has also been reported to the dealership, with service documentation noting that when applying the brakes, the engine revs and the vehicle does not slow or accelerate properly. This creates a dangerous condition where braking input does not result in expected deceleration and may instead cause acceleration-related behavior. The issue has not been resolved. This issue significantly increases the risk of collision and loss of vehicle control.
