Vehicle was using an off ramp and shutdown in the middle of a lane. It was 10:00pm and all systems were shutdown, including lights. Vehicle refused to restart and could not be placed in neutral, the vehicle could not be removed from the interstate off ramp. This issue has been brought to the attention of Honda, the started assembly was replaced on March 5, 2026. This vehicle has been delivered to Honda for similar electrical/starter issues 4 times since October 27, 2025. The first report was filed with Honda on October 27, 2025 with all lights on dashboard lit when vehicle was active. Had to wait until a tow truck arrived before vehicle could be safely removed from the scene.

- #11760283August 28, 2026AIR BAGS, SEAT BELTS
On XXX while traveling at approximately 50 mph on a smooth highway, both front airbags and the seatbelt pretensioners spontaneously deployed and exploded without warning. There was absolutely no collision, crash, road debris, or pothole impact. There were no vehichles in front or behind me. The vehicle continued smoothly for 100 yards post-blast. The vehicle's exterior and undercarriage remain completely undamaged. COMPONENTS FAILED: The Supplemental Restraint System (SRS) electronic control unit failed catastrophically. The explosive blast shattered the windshield, locked the seatbelt assemblies, and inflicted immediate blast-induced acoustic trauma/severe tinnitus. The vehicle is completely disabled but safely secured on private property and is available for immediate physical inspection. MANUFACTURER AND DEALER CONFIRMATION: The manufacturer (American Honda Corporate) has officially cross-referenced this vehicle and confirmed that the safety remedy for the known SRS control unit defect (NHTSA Recall Campaign 13V-412) was previously performed on XXX, at an authorized dealer (Billy Wood Honda in [XXX]). This incident represents a catastrophic failure of a certified manufacturer safety remedy. American Honda Corporate has opened a high-priority Level 1 Product Liability Investigation under Corporate Case Number XXX. RISK AND WARNING SYMPTOMS: Driver safety was put at extreme risk due to an unprovoked explosive detonation at highway speeds, causing temporary blindness from white smoke and sudden hearing deprivation. The SRS warning lamp was confirmed illuminated immediately following the zero-impact deployment. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

At 64,000 miles both rear shock absorbers failed (rusted), one completely the other on the verge. Driving the car was dangerous as the car became unstable at higher speeds when going over normal bumps and road holes. The repair including parts, labor, and necessary four wheel alignment was $1,051. The dealer did not acknowledge this being a common issue nor did they deny it. A google search indicates this is a common issue with Honda Odyssey Vans for the 2021-2024 manufacturing years. There were no warning indicators other than experiencing the effect of the shock absorber failures while driving the vehicle. The vehicle passed PA State Inspection earlier in 2026 (March).

COMPONENT THAT FAILED & AVAILABILITY: The vehicle's primary electrical architecture and data communications network (CAN bus) failed during a safety recall service for the SRS ECU and rearview camera. After attempting to complete the recall work, the vehicle is no longer able to start. SAFETY RISK: The manufacturer's federally mandated safety recall remedy compromised the vehicle's critical safety and control infrastructure. This rendered a previously reliable family vehicle entirely inoperable, destroying its ability to communicate, start, or turn on. It poses a systemic safety risk if the corporate recall procedure itself causes severe electronic failures in consumer vehicles. REPRODUCTION & DEALER CONFIRMATION: Yes. The failure was induced directly by Williams Honda ([XXX]) during service on Thursday, XXX. The dealership's Service Manager explicitly confirmed to me that their technicians compromised the electrical architecture while performing the SRS ECU and rearview camera recall work. The vehicle has remained dead at their facility ever since. INSPECTION BY OTHERS: The vehicle has been actively inspected by dealership technicians in coordination with Honda Engineering headquarters. It has not been inspected by police or insurance representatives, as no crash occurred. I have requested an inspection by a Honda Corporate Field Technical Specialist (FTS). WARNING LAMPS & PRIOR SYMPTOMS:NONE. Prior to this appointment, the vehicle was in perfect electrical and mechanical working order. There were absolutely no warning lamps, messages, or symptoms of any starting or electrical problems. The vehicle had just safely completed a seven-hour road trip immediately prior to being dropped off for the recall work. My 2025 Honda Odyssey shut off in Auto Idle Stop while at a busy intersection. The car was unresponsive and would not engage the motor. After five minutes of trying to turn the car off and on, it eventually restarted and the motor came back to life. This issue put me at great risk of bodily harm. Given the fact the car was unresponsive in such a surprising way, I was very lucky not to have cars crash into me from behind as they few by for the 5 minutes I was stranded. This is the first instance and has not been reproduced as I am too worried to drive the car now. No inspection of the vehicle has been completed as this incident just occurred. No waring light prior to the incident, but the battery warring light did come on while the car shut off and unresponsive.

- #11760455August 28, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, LANE DEPARTURE: ASSIST, LANE DEPARTURE: WARNING
We were backing up the vehicle and the children started playing around acting like they were pushing the vehicle. The rear camera showed they were on the back corners of the vehicle and I think also the rear camera. As I pulled away all the safety notifications started to appear - adaptive cruise control, lane keep assist, high beam automation and a brake system error. The vehicle was taken to the dealership in Richmond, Kentucky who advised that the issue was the front camera that is in the rearview mirror mount area. I asked how this could be the front camera when everything happened in the rear of the vehicle. They advised they didn't know how but that is what their code readers are recognizing as the issue. They also stated that they have several individuals reporting this issue to them each month but there are no recalls due to this. This seems to be a safety defect as I cannot see how children playing around at the back of the vehicle affects the 'front' camera in turn affecting so many safety items.

Safety Complaint – 2019 Honda Odyssey Elite Front Camera Failure I own a 2019 Honda Odyssey Elite with approximately 60,000 miles. The front windshield camera used by the Honda Sensing system failed. When the camera failed, the vehicle displayed approximately 11 warning messages/errors involving multiple safety-related systems, including the Brake System, VSA, EPS, Electric Parking Brake, Brake Hold, Hill Start Assist, and Honda Sensing features such as ACC, LKAS, and CMBS. This was concerning because one camera failure caused multiple safety systems to become unavailable or display warning messages. The vehicle’s driver-assistance and safety functions were significantly affected. I took the vehicle to a Honda dealer, and the dealer diagnosed the front Honda Sensing camera as defective and replaced it. The total cost was approximately $2,600, including the camera, parts, labor, and calibration. I believe this is an unusually expensive failure for a critical safety component, especially on a vehicle with only approximately 60,000 miles. The front camera is not a normal wear item and is essential to several Honda safety and driver-assistance systems. I am also concerned that similar Honda owners have reported failures involving Honda Sensing cameras. I would like NHTSA to investigate whether there is a recurring or widespread defect involving the front Honda Sensing camera on Honda Odyssey vehicles, particularly model years 2018–2020. I respectfully request that NHTSA review complaints and repair records related to this issue and determine whether further investigation or a safety recall is warranted. I am submitting this complaint because I believe owners should not have to pay approximately $2,600 to repair a potentially recurring defect in a critical safety component.

The central front adjuster lever (the release mechanism located at the front bottom of the car seat shell) has suffered a catastrophic mechanical failure. The lever is physically stuck in the "open" position and fails to snap back down or engage. Because the lever remains stuck open, the internal metal locking teeth cannot clamp down onto the harness adjustment webbing strap. Consequently, the child restraint harness completely fails to lock or hold tension. In a vehicle collision, sudden braking event, or even under standard manual pressure, the harness straps pull entirely loose. This defect prevents the seat from securing the child, posing an immediate risk of severe injury or ejection during a crash. The manufacturer (Graco) was contacted regarding this critical component failure under reference case #[XXX]. They refused a structural replacement or shell exchange, offering only a minor $40 purchase rebate, effectively leaving the vehicle occupant unprotected unless a new seat is purchased out-of-pocket. - #11760040August 27, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, LANE DEPARTURE: BLIND SPOT DETECTION, LANE DEPARTURE: ASSIST
The monocular camera (multipurpose camera secured under the rear view mirror) has failed. This disables the following" Collision Mitigation Braking System (CMBS): Detects obstacles or vehicles ahead and helps brake automatically if a collision seems likely. Forward Collision Warning (FCW): Works with CMBS to send visual and audible alerts if you get too close to a vehicle in front of you. Lane Keeping Assist System (LKAS): Adjusts steering torque to help keep your van centered in a detected lane. Lane Departure Warning (LDW): Alerts you if you drift across detected lane lines without using your turn signal. Road Departure Mitigation (RDM): Applies steering help or braking if you drift off the paved road entirely. Adaptive Cruise Control (ACC): Maintains a safe, set following distance behind a vehicle traveling ahead of you. These systems are helpful in keeping you safe while driving. The problem with camera failing cause these system warnings to populate on the dash. This is distracting. This has been confirmed with independent service center. Has not been inspected by manufacture. Started August 21. quoted $2200 to replace and recalibrate.

The car on a weekly basis or almost every time it is driven. The car jerks when driving at low speeds. This is a safety risk when driving for vehicles in front or behind me, as the car jerks forward. This has been mentioned to the dealer, but they stated a charge of 180 USD will be incurred for diagnoses. If you look at the NHTSA complaints database there are numerous complaints similar to what I am experiencing from other owners stating the same issue of jerkiness on the honda Odessey. I am surprised there is no recall on this. I put the date in down below but it happens almost every time the car is driven.

What component or system failed or malfunctioned, and is it available for inspection upon request? - Fuel injectors - They can be inspected in needed How was your safety or the safety of others put at risk? - This issue can cause issues with the engine and ultimately make the vehicle unsafe to operate. Has the problem been reproduced or confirmed by a dealer or independent service center? Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? This is a known issue with honda vehicles including the odyssey for 2019. There is a Service Bulletin 21-010 that mentioned the issue but only covers some VINS but this is an issue with other VINS that the dealer refuses to cover. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? A Check engine light will come on.

the problem only happens after driving for more than 30 minutes. when I stop at a light, the engine starts shaking and stalls. the starter seems cranking normally, but it takes about 3 attempts and about 40 seconds to restart. One day, the engine stopped while I was driving. There is no Check Engine Light. I took it to Honda dealer. but they keep saying they cannot replicate the problem. It is so dangerous to drive this car.

Our 2025 Honda Odyssey Touring has repeatedly stalled or nearly stalled while driving. Of at least three dealer visits, the stalling issue was first formally documented on XXX, when we reported the engine shutting off twice while driving and the brake warning light activating twice. DTC 15477 ("failing starter") was recorded, and the starter was replaced. XXX, my [XXX] daughter was driving alone when the van lost power at a red light and would not respond to the accelerator once the light turned green. She was unable to shift gears or shut off the engine, and the van remained unresponsive in live traffic for several minutes before power returned on its own. A bystander helped restart the vehicle and move it to safety. I believe this defect is the subject of open NHTSA Engineering Analysis EA25004 (opened XXX), covering 2018–2025 Odyssey and related Honda/Acura models for Auto Idle Stop failing to restart after a stop. On XXX, the dealer confirmed the software was current, the battery was healthy, and replaced both relays, completing every step of Honda's documented repair bulletin. Hours later, on the drive home at 27,942 miles, the van stalled again at a stoplight. The dashboard instructed the driver to depress the brake and press the switch to continue driving. The driver powered the vehicle off and restarted it to regain control. Video of this incident: [XXX]. Dates of incidents: XXX – Two engine shut-off events and two brake warning light events reported; starter replaced. XXX – Daughter's incident described above. XXX – Software, battery, and relays completed per repair bulletin; vehicle stalled again the same day at 27,942 miles. Repair history: Full documented scheduled maintenance and bulletin remedy applied (starter, software, battery, relays) across three or more visits; vehicle stalled again hours after the final step was completed.
