Power System warning message appeared on the dash. The vehicle lost power and went into park. While trying to get a safe place, the vehicle cut off again while other cars were behind me. Multiple indicator lights and messages began to cycle on the dash. The car would not go into neutral to be pushed. The car was towed to the dealership as it was inoperable. The problem has been reproduced at the dealership.

The center infotainment touchscreen registers inputs on its own when no one is touching it, opening menus, changing selections, and switching screens and radio stations. This distracts the driver while the vehicle is moving and can interfere with the backup camera display and navigation. On October 3, 2026, Safford Honda Arlington (RO #201229) performed a master reset with no fix and told me this is a known issue on 2018–2022 Honda models with no remedy available yet. I have opened a case with American Honda customer service.

My vehicle underwent Honda’s fuel-pump safety recall repair at an authorized Honda dealer on April 1, 2025. After the recall repair, the vehicle developed a sensor warning directing me to return to the dealer. On February 17, 2026, the dealer inspected the vehicle. Its repair record states “HIGH CIRCUIT VOLTAGE” at terminal #2 and “FOUND PIN MELTED IN FUEL PUMP ASSEMBLY.” The dealer replaced the complete fuel-pump assembly at no charge. A dealership representative later told me their mechanic had accidentally caused the melted-pin condition during the earlier recall work. Since then, I have experienced recurring instrument-cluster/display corruption with vertical lines/banding that can make portions of the digital display unreadable. I have photos/videos documenting multiple occurrences. I am concerned because important vehicle information can become unreadable while driving. I have also observed estimated full-tank range decline from historically about 425 miles to approximately 373–395 miles. I have documented a fill of 7.438 gallons followed by a displayed range of 373 miles. I do not know whether this is related. I do not know whether the cluster malfunction or range concern is related to the recall repair, documented high-circuit-voltage/melted-pin condition, or subsequent fuel-pump replacement. I opened a case with American Honda. Its case manager characterized this as a “workmanship issue” and said he would contact the original dealer’s service manager to have it addressed. I have not received that promised contact. I ask NHTSA to investigate whether the recall repair and subsequent electrical and instrument-cluster problems present a safety defect requiring further action.

The Honda infotainment screens have a history of "Phantom Touches". I have replaced my infotainment system once under warranty. The problem is that while out in sunlight the vehicle's infotainment system will randomly beep and change screens and functions on its own as bands of sunlight enter via the windshield or side windows. It is extremely distracting and things like switching music souces from blutooth source to radio, cutting in and out, swapping to the default screen saver, vehicle settings screens in the middle of turning in traffic are inherently hazardous. The Insight's windshield is already factory tinted at 70% VLT. Adding an IR rejecting window film will reduce the light passed below my state's ([XXX]) legal threshold. 90% VLT window film is not widely available and would still drop my VLT to 63% effective. This is an ongoing problem across models and years for at least 8-9 years. The infotainment center is already set at the 'normal' touchscreen sensitivity (vs. the more sensitive option). Covering the infortainment center screen with a piece of cardboard eliminates the distraction and phantom touches, but it also blocks safety warnings and my backup camera screen. I purchased a new 2022 Honda Insight from a dealership. The vehicle began shaking when I was driving and even stopped in the roadway several times. Honda has inspected and attempted to repair the vehicle but multiple people have indicated that the issue cannot be resolved and was sold to me as a faulty vehicle. Honda, specifically, the mediation department ([XXX]), has indicated that they would not be resolving the issue in any way. It is important to note that this occurred regardless of whether the check engine light was on or off. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I was driving to Pilates class and everything was normal but as I reached a railroad crossing the car suddenly and for no reason stopped dead on the tracks. Luckily for me there were no vehicles in front or behind me and there were no trains coming. I was terrified and wasn’t sure what to do but I pushed the Drive button and the vehicle went ahead as if nothing happened.

The car was last driven and parked at about 8:30pm local time. At around noon the next day, we noticed that the dashboard in the vicinity of the passenger airbag was melted with an approximately 12-inch hole. There was soot staining the interior of the vehicle, and the windshield was burned and cracked above the hole in the dashboard.

- #11747521June 30, 2026UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: WARNINGS
The vehicle unintentionally accelerated while pulling into a parking space. I was depressing the brake to slow down while making the right turn into the space. The engine suddenly revved up (as though you were depressing the gas and brake at the same time) and it accelerated forward (a though you suddenly released the brake while depressing the gas pedal) and the car lept forward, jumping a 3 foot long curb, striking the vehicle parked in front of me. The tires kept spinning as though the gas pedal was being depressed, even though the vehicle could no longer make any forward progress since it had run into the front of the vehicle parked in front of me. We’re it not for the vehicle in front, I feel that the car would have continued accelerating forward and could have caused injury or death to customers in the shopping center and/or damage to other vehicles or structures. The vehicle was towed and is currently in the body shop for repairs, but I don’t think this addresses my concern for the sudden acceleration. I don’t want to drive it in case the problem reoccurs and could cause further damage. I’ve reached out to Honda to see if I can tow the vehicle to their service repair center to try to diagnose the problem by reading data from the car’s computer, but they have not responded. I checked the floor mats and they were flat and properly installed. If I had mistakenly depressed the gas pedal (not likely as I was already braking as I approached the parking space), I would have thought that the collision avoidance system would have braked the vehicle. Airbags also did not deploy in either vehicle.

The A/C evaporator failed due to a refrigerant leak caused by corrosion. The component is available for inspection upon request at the dealership where it was diagnosed. The failure resulted in complete loss of air conditioning. In hot weather conditions, particularly during highway driving and stop-and-go traffic, loss of A/C poses a risk of heat-related illness to the driver, especially in high-temperature climates. The defect was confirmed by a Honda dealership technician during an inspection in May 2026. The service advisor explicitly stated that the evaporator is manufactured from the same defective material as the A/C condenser, which Honda already acknowledged as a manufacturing defect under Service Bulletin 21-016. A Honda corporate customer relations representative independently confirmed the same information — that the evaporator shares the same material and is subject to the same corrosion issue. No warning lamps or messages appeared prior to failure. The first symptom was warm air blowing from vents, which progressively worsened until A/C stopped functioning entirely. Honda repaired the condenser under Service Bulletin 21-016 at no charge but refused to cover the evaporator repair, quoting approximately $2,600 out of pocket despite acknowledging the same root cause. I am requesting NHTSA investigate whether a recall or additional service bulletin covering the A/C evaporator on 2019-2021 Honda Insight vehicles is warranted, as other owners are likely experiencing the same defect.

At approximately 148,000 miles the steering would have an intermittent notchy, or sticky, feeling while driving straight. While driving straight at cruising speed, I noticed that the resistance on the left side of the steering wheel was greater than the right side, then the resistance would decrease suddenly (release), and the steering wheel would rotate slightly to the right, but less than 1 degree, so it would be hardly noticeable on video. The issue seems to only present itself after the vehicle is driven 20-40 minutes and when the temperature is 47 degrees or higher. I have noticed it at low speed (15-20mph) and at highway speeds. If I recall correctly, the issue first presented itself when we had our first 60 degree day and I was driving on the highway with the wheel straight. Then as I accumulated miles throughout the weeks, I would notice that it didn't happen on colder days but then happened more often on warmer days. Now that the vehicle has ~148,800 miles on it, and the air temperature is 67 degrees, the issue presented itself this morning (4/1/2026) after 3 minutes of driving and during the first sweeping left turn on my commute and at approximately 40 mph. Then the issue continued to happen throughout the drive, eventually becoming nearly constant by the end of the 45-minute commute. I do have an appointment with the Honda dealer for diagnosis, but the service advisor and I suspect that the steering rack is failing.
