The contact researched and became aware of an unknown recall; however, the VIN was not under recall. The vehicle was taken to a dealer where it was diagnosed and determined that there was an intermittent drive motor position sensor failure inside the transmission, and the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 70,000.

After the inspection, they stated they could not determine the cause of the warning light and refused to complete the recall repair. The vehicle is available for inspection upon request. The recall involves a safety issue that could put me, my passengers, and others at risk. Because the recall has not been completed, I do not feel safe operating the vehicle. In addition, I have been unable to use the vehicle for work because it has not been repaired. Yes. The authorized Chrysler dealership confirmed that the warning light was present and performed a diagnostic inspection. However, they were unable to identify the cause of the warning light and refused to complete the recall repair. They informed me that the case had been escalated to a Chrysler/Stellantis engineer. es. The vehicle was inspected by an authorized Chrysler dealership. The dealership escalated the case to a Chrysler/Stellantis engineer, but after more than three weeks, no solution or repair has been provided. The dealership informed me that a warning light was illuminated and required diagnosis before the recall could be completed. After performing the diagnostic inspection, they were unable to determine the cause of the warning light. The recall repair has been delayed for more than three weeks without any resolution or estimated completion date.

I have been experiencing random engine shutdowns since April 6, 2026. I’ve taken it to be serviced for this on four occasions since this time and it has not been appropriately fixed. There is a warning light that flashed before it happens that says “your car is about to shutdown, please pull over” but it does not allow ample time to pull over safely before randomly shutting down. The dealership replaced the PCM in may 2026 but I had the car back less than 24 hours and it happened again. The car has shutdown a total of 5 times since April 6, 2026.

2024 Chrysler Pacifica completely shutdown during a left turn. Engine died all electrical stopped functioning. Had to restart the vehicle to move the vehicle. This condition happened twice last week, but has not happened since. Fortunately it was a low speed situation rather than at highway speeds. Highway speed could have been a very serious situation with the potential of being rear-ended from high speed traffic. Vehicle has 27,500 miles on the odometer and was driving normally prior to the shutdowns. Chrysler dealer has been notified of the situation, but an appointment is 7-10 days out. No warning lights or messages were noticed.

On July 19, 2026, I was driving my 2022 Chrysler Pacifica Hybrid on E-470 at approximately 75 mph. Without warning, the dashboard displayed "Stop Safely. Vehicle Will Shut Off Soon," and the amber check engine light illuminated. At the same time, the vehicle immediately lost all motor power, began slowing, and did not respond to accelerator input. There were six occupants in the vehicle, including four young children. I was traveling on a busy six-lane highway with little to no shoulder. I had to maneuver across traffic without motor power to reach the side of the road, placing all six occupants at immediate risk. After turning the vehicle off for about five minutes, I restarted it. Only the check engine light remained illuminated, and the vehicle appeared to have normal power. We continued driving at 55–65 mph to reach a safer location. About 10 minutes later, the same warning appeared and the vehicle again lost all motor power. This time we were on a steep, busy exit ramp and had to immediately pull off as the vehicle rapidly lost speed, again placing all six occupants at risk. After waiting about five minutes, we restarted the vehicle, drove it to a safe location, and had it towed to a dealership. The dealership was closed, so the cause of the failure has not yet been determined. This incident created an extremely dangerous situation, and we narrowly avoided a collision on the interstate. A sudden loss of propulsion at highway speeds without warning is a serious safety hazard that warrants investigation and consideration for a recall.

Vehicle repeatedly entered limp mode / lost power at highway speed a few days after the dealer performing the remedy for NHTSA Recall 23V- 010 (FCA Recall 03A, transmission internal wiring connector short). The recall remedy did not correct the defect. Prior to the recall work, the van entered limp mode on 2 other occasions. Timeline: On 7/10/26, Park Chrysler Jeep in Burnsville, MN performed the open recall remedy (Recall 03A / NHTSA 23V-010) on our vehicle. We informed the dealer we were departing on a multi-week interstate family road trip, and the dealer released the vehicle as repaired. On [XXX], about 40 minutes into our trip, the vehicle entered limp mode with a severe loss of motive power while driving at highway speed on [XXX] between Northfield, MN and [XXX]. We were able to reach the shoulder safely. After multiple restart attempts, the vehicle resumed operation. On [XXX], the vehicle entered limp mode 2 additional times under the same conditions on [XXX] in IL, and on [XXX] in IN, again at highway speed and with heavy traffic present. In one case there was barely enough shoulder to pull over safely. Each event created a sudden, significant speed differential with surrounding traffic. On 7/16/26 the vehicle was taken to Bob Chandler Chrysler Dodge Jeep Ram in Columbus, OH. The dealer's diagnosis is that the transmission must be replaced. The repair order ID is [XXX]. The vehicle is at that dealership awaiting parts. Safety concern: The vehicle lost motive power at highway speeds 3 times in two days after the recall remedy was performed and the vehicle was returned to us as repaired. Occupants at the time included 2 adults and 3 children. No crash, injury, or fire occurred, but each event presented a serious collision risk from sudden deceleration in traffic. This experience is consistent with the concern in NHTSA's reopened investigation that the software-only recall remedy does not address the underlying hardware defect. I took the van to get the safety recall done in 2026, Feb 25. For the rear view back up camera. I took it to the same dealership that I bought the car from, Jones Junction. However, upon further complications with the back up camera, this issue has never been fixed or resolved. If anything, the issue has become worse. I am unable to use my camera 98% of the time. It freezes. It goes black or is unavailable to see anything out of. It's a safety recall for a reason. I want this issue fixed. As I have been driving like this with the issue for quite some time. What are my next steps? I prefer not to have to dealer hop if I can help it. But I might have to. Is there a patch they may have to do to re-install the software for the camera? Or would they have to go and flash it again and what ever else? My true gut tells me that I dont think this specific dealership ever did the recall, correctly.

Safety was put at risk due to it being a family car with children. A quick response was necessary to prevent oversteer during correction. Apparently this is a known issue on earlier models and seems to be continuing. No warnings.

Specifically, the vehicle experienced faults with the High Voltage System Interlock Circuit (HVIL), the High Voltage Service Disconnect, and the Hybrid/EV Battery System Precharge circuit. The vehicle is currently located at a manufacturer dealership and is available for inspection. The failure of the high-voltage interlock and precharge circuits compromises the vehicle's electrical isolation safety mechanisms. This poses an immediate risk of a sudden loss of motive power. Additionally, because these faults are rooted in the high-voltage battery system—a system actively under federal scrutiny and manufacturer recalls for thermal runaway—the failure presents a severe risk of spontaneous vehicle fire, endangering occupants, bystanders, and nearby structures. Yes. The failure was initially recorded by an independent diagnostic scan tool, which pulled several confirmed high-voltage fault codes. The vehicle was then taken to a certified manufacturer dealership, where it has been held in the service department for a month. The dealership has confirmed the presence of a severe system failure and initiated repairs costing at least $1800 (with final costs still potentially increasing). Yes. The vehicle is currently in the possession of, and being inspected by, technicians at a certified manufacturer dealership. Returning from [XXX] to [XXX] area Wed. XXX, traveling at ~75 mph on [XXX], I experienced a loss of power on a mild incline and an associated “Stop Safely - Vehicle Will Shut Off Soon” instrument cluster message. I had to move hazardously in traffic while coasting to a stop on the right shoulder. This occurred with ~24,810 total miles on the vehicle since I purchased it new. I turned off the vehicle, waited a few minutes and then restarted it. I continued the drive with the vehicle acting normally until it happened again at 24,897 odometer miles, same message with same behavior. Traveling ~75 mph, I again had to move hazardously in traffic to coast to the right shoulder. Except now I notice the Malfunction Indicator Lamp (MIL) is illuminated. I again turned the vehicle off, waited a few minutes and then restarted it. I was able to drive normally to the Chrysler dealership (Autonation in Katy Tx) where I spoke to a Service representative. He recorded my VIN and took some notes as I explained what happened, but would not plug in a code reader as I requested. Instead he said I would need to leave the vehicle there for at least 24 hours or bring it back later. Since I was still ~55 miles from home, had my wife and cargo still in the vehicle and wanted to know what malfunction code(s) were present, we elected to drive the vehicle home. On the way home, I stopped at both an Advance Auto and an Autozone where both read the only code present as P0A43, see attachment. I also noted that the Emission Ready status was showing "NO". I then drove the vehicle less than a mile to our home where it has remained in my driveway. Mileage is now 24972 on the odometer. I plugged in and charged the vehicle to 100% high voltage battery state. Two days later on XXX, I started the vehicle and the MIL is no longer illuminated, still at 24972 odometer. - #11748855July 7, 2026VEHICLE SPEED CONTROL, SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
We were at a complete stop waiting for traffic to go, I had my foot on the brake pedal and the car lunge/revved up and hit the car in front of me. This is the 3rd time this has happened. The first time we were advised it was probably the front-end sensor that were probably covered with ice. We were advised they are very sensitive. The 2nd time it was winter still, so we took the car through the car wash to make sure no ice was built up. We did not get in an accident because traffic started to move. We took the vehicle to the dealership, and they could not find anything wrong with it. Chrysler Corp got involved and they sent a 3rd party to check the van and black box. They advised nothing was found on the black box (not even an accident) and their investigators found nothing wrong with the vehicle. No warning messages were reported after the crash. Police were called to the scene and insurance was notified. Our insurance company advised that we should file this on here. There were 3 people in the van besides myself. We had a loaner vehicle for almost 3 months and we cannot get any information or paper work from Chrysler Corp regarding the investigation. We were advised the investigation was confidential and proprietary.
