* Primary Component: Forward Collision Avoidance * Secondary Component (if available): Electrical System / Lane Departure Step 3: The Incident Description (Copy & Paste below) > Summary of Safety Defect: > The vehicle is experiencing a critical, recurring failure of the Advanced Driver Assistance System (ADAS). There are two primary life-safety issues: > * Phantom Braking: The vehicle initiates sudden, forceful emergency braking without any external hazard or obstacle present. This rapid, unprovoked deceleration occurs at highway speeds, creating an immediate risk of rear-end collisions. > * Total Warning Failure: The system fails to provide any visual or audible "Pre-Sense" alerts when actual hazards are present. The safety suite is non-functional and fails to warn the driver of oncoming items. > * Context: The vehicle has undergone four repair attempts and has been out of service for over 30 days. As of late December 2025, the manufacturer has been unable to resolve these safety-critical software and sensor malfunctions. >

Vehicle: 2022 Audi Q5 (VIN [XXX] ) Mileage at incident: ~20,000 Component: Powertrain – Engine / Turbocharger – Sudden Loss of Power NARRATIVE: On or about [XXX], my 2022 Audi Q5 experienced a sudden loss of engine power while driving approximately 60 mph. The vehicle began vibrating/shaking severely, jerking on acceleration, and displayed warnings including “Drive system: Please switch engine off” along with a check engine light. The vehicle entered reduced power/limp mode, creating a hazardous situation in traffic at highway speeds. The vehicle was taken to an authorized Audi dealer in Wayzata BLVD Minneapolis who diagnosed catastrophic internal engine failure and documented metallic debris in the engine oil. Audi replaced the engine with a new long block (warranty repair). However, despite confirmed metal contamination in the oil system, Audi directed the dealer to flush and reuse the turbocharger rather than replace it. The turbocharger is supplied by engine oil and may retain embedded metal in its bearing assembly, creating an ongoing risk of turbo bearing seizure/failure and another sudden loss of power at highway speed. I am reporting this because (1) the vehicle already experienced sudden power loss at highway speed, and (2) the repair decision to reuse a contamination-exposed turbocharger may create continued risk of sudden loss of power and unsafe operation. Dealer repair: Engine replaced; turbocharger reused after “flush.” Current safety status: I do not feel the vehicle is safe for highway driving due to risk of turbo failure and sudden power loss. Requested action: Please log and review similar complaints for potential investigation related to sudden loss of power and contamination-exposed turbo reuse in warranty repairs. Unpublished costly catastropic electrical failure Audi of America reviewed the case and declined assistance solely because the vehicle was already totaled and unavailable for dealer inspection. Audi did not dispute the presence of water intrusion, the technical findings, or the cause of failure. This failure occurred without warning under ordinary environmental conditions and resulted in sudden loss of vehicle function. I am submitting this complaint to document a potential defect related to the protection and placement of electronic systems vulnerable to water exposure. I am submitting this complaint to report a potential safety-related defect involving water intrusion and catastrophic electrical failure in a 2018 Audi S5. After a heavy rain event with no roadway flooding or submersion, my vehicle experienced complete electrical failure and would not start. Inspection revealed water intrusion into the trunk area where critical electronic components—including the battery and electronic control modules—are located. The damage was extensive and rendered the vehicle inoperable, resulting in a total loss determination. Attached documentation includes photographs, video evidence, and a technical inspection summary prepared by an independent automotive service facility and reviewed in consultation with Audi-trained technicians. The findings indicate water intrusion consistent with design vulnerability rather than misuse or lack of maintenance.

On December 22nd i got into the vehicle and a few errors popped up which consisted of “loose wheel warning malfunction” “brakes: limited functionality drive with causation” “active front assist and emergency assist unavailable”. I called the nearest dealer and was told it’s ok to drive it’s just a software glitch and if i don’t feel comfortable driving i should take it to the dealer and come back tomorrow for a loaner since service was closed already. So i proceeded to drive home, about 10 minutes into the drive a warning for the coolant came up so i pulled off into a parking lot and as i went to park another warning came up saying coolant too high turn off vehicle and as i was approaching the parking spot the car started to overheat and smoke. This is a BRAND new car with BARELY 1000 miles on it.

Vehicle: 2018 Audi Q5 Prestige Mileage: Approximately 34,000 miles On December 14, 2024, while driving at normal road speeds, my vehicle suddenly lost power and displayed multiple warning messages, including drivetrain and system malfunction alerts. The vehicle became unstable and unsafe to operate while I was in traffic, forcing me to carefully maneuver off the roadway. This occurred without any prior warning or abnormal engine behavior, it was just so sudden. I brought the vehicle to an authorized Audi dealership the following day. After inspection, the dealership diagnosed a catastrophic internal engine failure, identifying a cracked/chipped valve in cylinder 3. The dealership technicians and service staff stated that this type of failure is extremely rare on a vehicle with such low mileage and normal maintenance history. They also indicated that they had not encountered this type of valve failure on a similarly maintained Audi vehicle at this mileage. There was no evidence of carbon buildup or abuse, and no external contributing factors were identified. The failure resulted in sudden engine malfunction and loss of drivability while the vehicle was in motion, creating a significant safety risk. The dealership recommended replacing the engine cylinder head, with an estimated repair cost approximately $9,100. The dealership could not confirm whether debris from the damaged valve remained in the engine or whether the repair would fully mitigate future safety risks. I am submitting this complaint due to concerns that this failure represents a potential manufacturing or materials defect that can lead to sudden engine malfunction or loss of power while driving, posing a safety hazard to vehicle occupants and other road users.

Vehicle rear subframe comes with plastic covers that trap road salt and cause severe corrosion. Issue only presents itself when plastic covers are removed, revealing rust/corrosion and lost material, and a risk of the subframe to split resulting in lost of vehicle control on the road. This was found as a preventative action due to reports from other car owners and is confirmed by Audi Dealers as a common issue. Labor and parts estimate around $6000 (including $2000 in parts)

Audi has a pre-sense mechanism that cannot be permanently turned off. I have almost been hit three times when the Audi brakes FOR NO REASON at all; people almost rear-end me because the Audi comes to an immediate halt. And there is no reason for the pre-sense to engage when it does go off. They are flukes. I am going to get seriously injured.

This vehicle Misrecognizes intersections on two lane highways and causes the vehicle to unexpectedly slam on its brakes without warning in anticipation of an intersection that either does not exist or where my vehicle has the entire right way. this behavior has persisted since the date of purchase and has not been corrected at all. Audi has confirmed that the vehicle is operating as designed however, I have personally almost had two major traffic accidents when this system engaged in the process of overtaking a vehicle, slamming the brakes and almost causing a accident where he and I was rear ended. This is not a vehicle. I would let a child learn how to drive in.

While driving my 2021 Audi Q5, the vehicle experienced a sudden transmission disengagement without warning. The vehicle behaved as if it had shifted into neutral while in motion. When this occurred, pressing the accelerator produced no response — the engine would not rev and the vehicle would not accelerate. This created a serious safety risk as it occurred during normal driving conditions and could have resulted in a collision. After stopping the vehicle and shifting into Park and then back into Drive, the transmission temporarily re-engaged. The issue reoccurred multiple times. Prior to one of the failures, the vehicle hesitated and bucked when starting from a stop. A warning message later appeared stating: “Transmission malfunction. You may continue to drive but will be unable to reverse.” The vehicle was taken to an authorized Audi dealership. Audi of America approved replacement of the transmission control module (mechatronic unit); however, this repair did not resolve the problem. After repeated failed adaptations and diagnostics, the dealer confirmed that the entire transmission requires replacement, citing internal gear set failure. The dealer documented that: Basic transmission settings and adaptations were attempted at least five times The failure consistently occurs at the same gear position A similar Audi Q5 case required full transmission replacement Audi Technical Assistance Center instructed the dealer to replace the transmission Despite this, Audi of America has refused further assistance due to the vehicle being slightly outside the standard warranty period, even though the mileage is extremely low and the failure is safety-related. This appears to be a defect involving the Audi S-tronic transmission, as similar complaints have been reported by other owners. The sudden loss of propulsion while driving presents a clear safety hazard. Safety Impact Sudden loss of acceleration while driving No throttle response during failure Occurred wit

The cooling system failed without warning! As I’m driving on the highway my vehicle began smoking! I smelled a burnt smell! Upon getting off the highway , I noticed my coolant had leaked almost completely out! I check my vehicle every morning before work as I drive through three counties to get to work and my coolant was full as I had just topped it off that morning! Upon inspection it was found from a local mechanic to be the water pump! My truck was fine last month upon getting a n inspection.
