While driving at highway speeds on the interstate, the driver-side sun visor suddenly experienced an internal mechanical failure, snapping at the pivot point. Instead of detaching completely, the visor dropped down into a vertical position, completely blocking my forward windshield view. Because this occurred while driving directly into blinding sunlight, the combination of the obstructed windshield and the sudden, unshielded sun glare caused an immediate and severe loss of forward visibility. This created an imminent crash hazard on a high-speed roadway, requiring an emergency reaction to blindly clear the obstruction from my field of vision while attempting to maintain vehicle control.

Driver side rear passenger inside door handle broke. I do rarely have rear passengers. I can count on one hand the times I’ve had rear passengers. This week I had a rear passenger and the door handle broke. The passenger did not abuse the handle. It was regular force used yet it still broke. Online community groups for my vehicle show that this has been happening alot to other 4th gen Tacomas. Not just rear drive door like mine but drive door as well

- #11769743October 7, 2026ENGINE, VEHICLE SPEED CONTROL
On [XXX], my 2026 Toyota Tacoma Trailhunter experienced an abrupt loss of power while passing a semi-truck on [XXX] in northern British Columbia, Canada. It was raining sufficiently to require windshield wipers but not at full speed. As I accelerated to pass a semi-truck in rain and road spray, the dash gauge indicated turbo boost increased, and then abruptly my truck suddenly lost power and entered what appeared to be a reduced-power or “limp” mode with many dash lights. It started slowing down immediately with no throttle response. I could no longer maintain highway speed and had to pull over immediately. This created an immediate collision risk while passing a large truck in highway traffic. After stopping, I inspected the air filter and intake. The filter was not visibly wet, but I found water droplets inside the intake near the sensor. I have a photograph documenting these droplets. I was approximately a couple hundred kilometers from the nearest Toyota dealer, so I drove at reduced power (max speed limited to approximately 40 mph) to the closest mechanic, who restored normal operation. I do not have the diagnostic fault codes. I suspect rainwater or road spray entered through the factory snorkel during a turbo boost event causing the sensor to detect water and the subsequent loss of power, but the underlying cause has not been confirmed. My primary safety concern is the sudden inability to maintain speed or accelerate during a highway passing maneuver, and the possibility of recurrence during driving in rain and road spray. I request that NHTSA investigate this potential safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11769379October 6, 2026ENGINE, FUEL/PROPULSION SYSTEM
2018 Toyota Tacoma SR 4WD Double Cab, 3.5L V6 (2GR-FKS). At about 87,000 miles the check engine light came on with code P11EC, Cylinder 1 Air-Fuel Ratio Imbalance (Port). Cylinder 1 failed compression. Leakdown and coolant tests had passed earlier; borescope did not show heavy scoring. The Toyota dealer said replacing the direct injectors would fix compression (quote about $4,499). After the dealer injector repair, cylinder 1 was still 124 psi (spec about 142-180 psi), so it did not fix compression. The dealer then quoted $11,778 for a short block (+$500 incidentals) and $14,238 for an engine assembly that was not new (about 45,000 miles on the replacement discussed); adding injectors again put the short-block path at about $16,278. I was steered toward buying a new truck and had the injector work reversed because it did not fix what they said it would. Toyota Brand Engagement Center reviewed the case, called the dealer, took VINs for other Toyotas, and denied goodwill; no recall or TSB. An independent shop later pulled the engine and found the cylinder 1 exhaust valves leaking badly; cylinder wall still looked good. Machine shop valve job, new rings, injectors tested (two bad), all six replaced; repair about $8,000. Truck runs normally after. Consequences: loss of power, failed emissions monitors, risk of further engine damage if driven, high repair cost. Not a crash. This appears to be a known pattern on the 2GR-FKS in 2016-2019 Tacomas (P11EC, cylinder 1, leaking/burnt exhaust valves, often after direct-injector washdown). Toyota has not recalled; dealers stop at injectors or quote a short block/used engine, and owners pay out of warranty. Please log this under 2018 Toyota Tacoma: engine, fuel system, and valve train (cylinder head/exhaust valves, fuel injectors), so the pattern is on record. Servicing dealer: Nashville Toyota North. The rear quarter panel pressure vents failed which allowed water intrusion into the cabin of the truck. Severe mold and smell has occurred due to water damage. There were no warnings from toyota or dealership about these pieces failing and causing damage. To have a dealership replace just the parts, NOT including ruined carpet and pads due to mold, I was quoted $800- $5000 to replace a $60 piece of plastic

All interior door handles have a plastic piece within the door panel that can snap under normal operation, resulting in loss of ability to exit the vehicle while inside. My vehicle, and many other 4th Generation Tacoma owners, have reported this happening. For my situation, my driver’s interior door handle does not engage the door latch mechanism as of [XXX]. The window must be rolled down and outside handle pulled to open the door. Upon taking it to the dealer on October 3rd, the part is to be replaced with the original part that was installed. The issue has been stated by the service department at Wyatt Johnson Toyota (Clarksville, TN) as being a nationwide issue. That part is on national backorder and I am driving this vehicle until that part arrives at an unknown date per the dealership. This poses a hazard because if I am in an accident and need to exit the vehicle through my driver’s side, my life would be at risk if I cannot roll down the window to open the door from the outside as the interior handle is inoperable. If/when the part does break again as it has not been improved by Toyota, then this will be a recurring safety failure that continues to put lives at risk if a driver or passengers cannot exit the vehicle through normal operation. Recommend this gets presented to Toyota, with a reinforced part created by Toyota, and a subsequent safety recall with part replaced on all 4 doors at no cost to the owner.INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

No matter how hard i push accelerator it does not provide power feels almost like seiches to from drive . Worried that loss of power will cause accident since i csn accelerate. Toyoa want me to replicate this condition and is impossible to do. Offed to pay myself to check for metal shavings. I will pay to stay safe if they will deny the issue again. No check engine light

The vehicle's structure (frame) is failing. A recall for corrosion susceptibility was disseminated for the make and model of my vehicle. A coating was applied to the frame, and I was then told by Malloy Toyota that rust will now NOT become a problem for the frame, as "it's been properly treated according to Toyota's standards following the recall." However, upon State Inspection yesterday, 9/21/26, I was told that much Rust Damage exists on the frame. The vehicle is available now for immediate inspection. My safety is immediately put at risk because I'm driving a vehicle whose structure is now compromised by the recall Toyota said they "prevented" and therefore fixed. The problem has indeed been confirmed by an independent service center. The vehicle has only been inspected by the independent service center for VA State Inspection. No warning indicating that the vehicle's frame was rusting. The issue was first brought to my attention after yesterday's VA State Inspection on 9/21/26

The contact owns a 2018 Toyota Tacoma. The contact stated that while driving 65 MPH, the brake pedal was depressed and went to the floorboard, and the vehicle continued moving forward. The vehicle was taken to the local dealer and was repaired under NHTS Campaign Number: 18V888000 (Service Brakes, Hydraulic); but the contact had evidence that the repair was not performed. There was an abnormal fuel odor inside and outside of the vehicle before the failure, which continued afterwards. The manufacturer was informed through brand engagement and customer care. The failure mileage was approximately 8,000.
