SERVICE BRAKES
Completely unexpected, I started my Prius V and the ABS warning light, Traction control and parking brake lights were on. I had been hearing a high pitched thumping sound when stopped at street light or other times when brakes were engaged, but my mechanic was unable to locate the source. After the warning lights came on, The brakes still worked in a limited fashion requiring several seconds to engage. Had I not been an experienced driver using longer distance between myself and other drivers, I am sure I would have wrecked. Took the car to my mechanic. He ran the codes and found C 1391. After researching the chat groups, I found this to be a long term known defect. Disappointed that consumers reports did not mention. Called dealership and they required their machines run diagnostic codes. I decided to call Toyota customer service and talked to several levels of associates and I am stone walled. By the way: I feel for those people in their jobs. The supervisor I talked with kept mentioning the distressing lights. My concern is the wrecks I, and other drivers, could cause. I do see people stating in chat groups that thry have wrecked. And someone mentioned that if the dealership runs the code, the receipt will say “ do not drive” and “Toyota is not liable” if car is driven and a wreck occurs. ( my summary may not be exact.) I now have a fabulous non-functioning car.

SERVICE BRAKES, ELECTRICAL SYSTEM
Bought the car from a dealership in january 2022 with 158,000 miles and heard buzzing noise that I was told was normal with hybrid cars.no codes showing on car and passed dealer inspection.Driving down a highway and heading to a stop brakes are hard to mash and caused skidding. No brake lights or abs lights are on. Couple days later brake lights and traction lights come on and then went back off. Take to Tires Plus and I am told I have to schedule appointment with toyota dealer as they do not have the software to diagnose. Take to toyota dealership to see codes 1391c for brake system. After searching issue there was a recall/customer warranty extension for replacement of this part for 150,000 miles /march 19,2022 whichever comes first. This did not show up on a report for dealer to inspect before selling vehicle and causes a huge safety concern to people that did not own this car before 150,000 miles. If a less experienced driver would not have known to reduce brake pressure applied it could have slid and caused collision or if brakes would have been needed during an emergency situation. Car is now sitting at toyota dealership and advised not to drive as brake pressure was lowered while being diagnosed. After talking with toyota they cannot provide assistance due to car being slightly over the 150,000 mile mark. Also if the brake pedal is mashed for an extended period of time the dash lights for brake system and traction control go off and will not come back on until it is driven for awhile

The contact stated that the check engine warning light was illuminated. Car hesitates after stopping at a traffic light (5/30/21). After a few seconds the car finally engages (accelerate). Happens infrequently. Brakes seems to be stuck after braking and releasing brakes (7/15/21). Took a few second to acceleerate. Happens infrequently. Car switch to neutral after stopping at a traffic light (9/15/21). Car switch to neutral after stopping at a traffic light (10/12/21.) Car switch to neutral after stopping at a traffic light (10/15/21). Warning lights came on (10/15/21) Diagnosted: pressure control chip for the master cyclinder failed. After braking at a traffic light the car switched to neutral by itself and failed to acceleerate or move forward (10/16/21).

Hi my name is [XXX]. Toyota headquarter case # [XXX] Incident day July 6, 2018 I had a big technical problem with my 2014 Toyota Prius V where it caught on fire because one of the valve lifters fell into the engine head and broke the engine block while driving. When that happened, the oil began to leak into the muffler and the engine and hybrid wiring immediately caught on fire. Many of my colleagues who also bought the same vehicle for Taxi business use in San Francisco all have a similar issue as mine. A few of theirs caught on fire, and a few others mentioned to me that the head gasket for their same Toyota vehicle blew out and each of them had to replace it at around 120,000 miles. I believe that’s why they don’t manufacture these same vehicles anymore because of the defectiveness in the engine. I opened a case with Toyota Manufacturer back in July 2018 and they sent a specialist to inspect the vehicle after the fire. He found the piece that fell inside the engine and that damaged the engine. The Toyota specialist admit to me that the engine of my 2014 Toyota vehicle is very defective and must be covered under warranty. My mechanic replaced the engine for me with a used one with 28,000 miles. This entire case was documented under my name with Toyota Motors Manufacture. I contacted the highway safety and I tried to file a claim against Toyota at the time, but I never got a follow up on my case after I left various messages. All they said was that they will investigate but never gave me any information regarding the engine fire situation or honor any warranty. I hope this incident will be investigated by your department and have this vehicle under recall. Thank you. Found car tampered with cut in carpet under driver seat to check battery maintenance per Toyota owner manual where hybrid battery was not receiving charge , was running hot Unknown person impersonated me registered my VIN to enroll with Toyota upgraded software app Entune version not with purchase is a 2021 software version with parental like controls, I took to Toyota dealerships below for assistance , new battery and hybrid faulting losing charge, car does not provide hybrid fault notice , loses charge , uses little gas , I researched on side of road to rest throttle and work around as I was driving it could have lost all power and stalled per recall notice needed hybrid software update Ran dr Prius app and finding recalls serviced at Toyota chambers burg pa are coding J0V issue w/inverter not completed and additional engine & battery codes almost causing my car to not enter fail safe mode hybrid battery almost fully discharged as approaching Bay Bridge Service notes from Fitzgerald day there were no issues specific repair request was for diagnostic report of car hybrid battery and sensor check, they claim no issue Only repair was replaced fob key although car at location 10 days, missing fuses found upgrade causes car to get 2-4mpg and burn battery hot/discharge without any dash or other warnings from car Safety recall notice no. 18v-684 are indicative of car symptoms Since incident Easton Md Toyota Service, Arlington Va Koons, Hagerstown MD Toyota and chambers burg refused to provide diagnostics Chambers burg held car did not perform service claiming no issues but noted cut carpet under driver seat above hybrid air vent battery cool/ intake Problem still an issue no Toyota will run diagnostics -

The brakes failed while I was driving. I had received a "Voluntary Customer Support Program" notice several months before this happened. I called the dealership right away and they told me not to worry about it and to put the notice in my owners manual. After the brakes failed, I had it towed in to the dealership and they determined that it was an issue that would be paid for by Toyota. Now my car has been in the dealership for almost 2 months and the part is still not available. They have no estimated time to expect the part. I have a loaner, which they would not give me at first until I complained to the Toyota customer service number. My loaner is a large SUV, which is a gas guzzler and is much bigger than I am used to. I just want my old Prius V back. Repaired.

The contact stated while driving at an undisclosed speed, a vehicle hit the front driver's side causing damages to the front driver's side door and undercarriage. The contact stated none of the air bags deployed. The contact stated ambulance did arrive at the scene where they took the contact to the hospital. The contact sustained injuries to their head due to the front side window shattering causing a laceration on the left eyelid and small facial fracture as well bruising and swelling of the left eye. The contact stated the other driver in the accident and passenger was taken to the hospital as well but the contact did not know if they were injured. A police report was filed. The vehicle was towed to the tow yard. The contact attempted to reach out to the local dealer but there was no response. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was 76,000.

PRIUS DISPLAYED ALL BRAKE WARNING LIGHTS WHEN CAR WAS RUNNING. TOYOTA DEALERSHIP CONFIRMED THAT BRAKE BOOSTER AND PUMP ASSEMBLY NEED TO BE REPLACED, BUT WOULD NOT REPLACE THEM UNDER CUSTOMER SUPPORT PROGRAM 20TE07 BECAUSE THEY COULD NOT PULL THE SPECIFIED DIAGNOSTIC CODE. DEALER ACKNOWLEDGED THAT THE BATTERY DYING TWO WEEKS PRIOR COULD HAVE ERASED THE NECESSARY CODES, BUT STILL REFUSED TO FIX THE CAR. AFTER CALLING THE CORPORATE CUSTOMER SERVICE LINE AND SPEAKING TO SOMEONE FOR 30 MINUTES, THEY STATED THAT TOYOTA COULD FIX THE PROBLEM, BUT MADE A BUSINESS DECISION NOT TO.

VEHICLE SPEED CONTROL
WHILE SLOWING AND PULLING INTO A PARKING SPACE, IN FRONT OF THE DENTIST OFFICE WHERE MY WIFE HAD AN APPOINTMENT, THE VEHICLE SUDDENLY ACCELERATED. IT JUMPED THE CURB, AND SHE STEERED HARD RIGHT TO AVOID THE BUILDING DIRECTLY IN FRONT OH THE PARKING SPACE. SHE SIDESWIPED THE FRONT OF 2 VEHICLES PARKED NEXT TO THE SPACE SHE WAS MOVING INTO, AND THE CAR CONTINUED TOWARD THE STREET. IT HIT THE CURB GOING OUT OF THE PARKING LOT AND THE CAR FINALLY STOPPED. A PERSON CAME TO THE DRIVERS SIDE OF THE CAR AND ASKED MY WIFE IF SHE WAS OK, AND SAID SHE SHOULD TURN THE CAR OFF - WHICH SHE DID.
