The sun roof suddenly shattered while driving at a high rate of speed on the freeway. A large chunk of glass broke upward and sprayed cars behind me. Shards of glass fell into the passenger area. The was a large bank and whoosh of air startling the driver.

The contact owns a 2026 Lexus NX 350H. The contact stated that the A/C unit failed to operate during extremely hot weather. A diagnostic test by the dealer determined that there was a hole in the condenser, which caused the refrigerant to leak. The manufacturer declined to help with covering the repair of the vehicle under warranty. The manufacturer attributed the damages to outside debris. Upon further inspection, the owner discovered that the A/C condenser protective shield was composed of thin metal, which failed to adequately guard the condenser against road debris. The vehicle had since been repaired. The failure mileage was 1,200.

The Lexus NX 350 has developed a very strong odor coming from the ventilation system. The smell is consistent with standing water, stale moisture, and/or mold. This is a significant health and safety concern, as I am worried there may be standing water or mold, potentially including black mold, within the HVAC/ventilation system. If mold or contaminated moisture is present, it could potentially be circulated throughout the vehicle and contribute to respiratory irritation or allergy symptoms. I brought this concern to the dealership’s attention during a routine service visit. The cabin air filter was replaced, but this did not resolve the odor or the underlying issue. The ventilation system and potential sources of moisture have not been adequately investigated or resolved, however online review do note this is a common issue with this model. Other than routine maintenance performed by the dealership, the vehicle has not been inspected or serviced elsewhere, and I have not experienced any other issues with the vehicle. I am requesting a thorough inspection of the HVAC/ventilation system and any potential sources of water intrusion or moisture to determine the cause of the odor and ensure the vehicle is safe to operate. I am very conserned with a silent health issue, lightly brished off that may lead to lung issues ond other respitory concerns. Please take this matter seriously.

ELECTRICAL SYSTEM
After sitting all night -- Started the car which started normally. But the instrument gauge cluster remained totally black except for the warning symbols in test cycle. The infotainment screen started properly. But the gauge cluster remained black. No speedometer, gas gauge, tire pressure etc. Just black. Tried multiple times, turned on and off, killed the electronics by holding the volume button, got out and opened the hood then opened the fuse box and found nothing wrong (pressed and wiggled the fuses) no loose fuses. opened the door and got back in. Car and all gauges started but the center gauge cluster took longer than normal to boot. That was in mid June 2026 2 months ago and has not reoccurred. In searching for solution discovered that the opening and closing and reentering the car is a workaround (sometimes) so I think it was the fix.

Our 19-month old Lexus NX450h with under 7,000 miles on it was not driven for a week. When we went to start the vehicle this morning, the car would not unlock via the keyfob, credit card key or app. The car required AAA to open the doors and jump start it. This is a very common failure with the 12-volt battery in the Lexus NX hybrids. This could result in a motorist being left stranded somewhere.

I purchased a brand new 2025 Lexus with only 3 miles recently I had a nail in my tire and took my vehicle to a local tire shop who could not removed the tire to put a patch because I did not have tire lock key; we searched the vehicle from top to bottom one was never given to me at the time of purchase. I contacted Lexus of Palm Beach several times and left a message for my service advisor BK and texted BK no response and several messages for the service manager who to this date has never called me back, finally BK told me that “Lexus had a new system and he didn’t received the messages” I reside in [XXX] and BK promised to coordinate with the dealership I provided him for me to obtain a tire lock key, this was June and I never heard back from BK. Last night I had issues with a different tire late at night and because I could not remove the tire I had to have my vehicle towed. However upon requesting Lexus tow company I had to wait 198 hours in for help by myself in a closed business parking lot for AAA called by the Lexus Roadside Assistance. Lexus has placed me in a life endangering situation. I am now stranded at my house with an inoperable vehicle and cannot go to work; the local dealership in Tampa does not have any appointments; this could have been avoided if not for the negligence of Lexus. The interior door latch is electronic with a manual lever in case it loses power. The problem is the electronic latch will occasionally malfunctions, and the door is partially open. When this happens, you cannot get the door to open all the way with the electric door latch or the manual door latch. The only way to get it open is to pull very hard on the door to close it again or get someone to push on the outside of the door. I alerted the dealer about this, and they said I have to push harder when I open the door. This has happened to me many times. The safety issue here is plain. If a person is not strong enough to pull the door closed it will not open.

The contact stated that with the toggle set to max brightness, the instrument panel failed to provide adequate illumination. The contact stated that the failure was a hazard while driving. The vehicle was taken to a local dealer where the vehicle was inspected, and the contact was informed that the instrument panel was operating as designed. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 500.

On 2026-07-05, my car started except the digital instrument panel was blank/black. This meant the digital gas and battery gauges weren't visible, in addition to the speedometer, etc. I tried restarting the car to see if it was a fluke but it did not come back. On 2026-07-06, the car worked fine / as expected, but after a google search, it seems like this happens to other owners as well in the past with no clear reason why. Wanted to report in case it becomes a larger thing.

I started my vehicle and thr complete infotainment screen was black, no display, no backup camera, nothing but a black screen. The volume button still worked and you could touch the screen and her things change but you could not see anything because the screen was black. It is like turning the brightness completely off a touchscreen, it works but you can't see anything. There are current recalls for this for other models but they say mine is not included. But I had the exact same problem with mine. It did not work until I turned my car off, returned and restarted my car, after that it worked normally. My can needs to be included in the recall.

The high-pressure fuel pump and related fuel system sensor failed, resulting in a sudden loss of engine power. The failure was confirmed by diagnostic trouble codes retrieved by an independent mechanic. The vehicle is available for inspection upon request. 2. How was your safety or the safety of others put at risk? While driving at highway speeds, the vehicle suddenly lost power and became inoperable, leaving me unable to accelerate or maintain speed. This created a dangerous situation with surrounding traffic, including a truck traveling behind me, increasing the risk of a rear-end collision or crash. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic retrieved diagnostic trouble codes indicating failure of the high-pressure fuel pump and related sensor. A dealership acknowledged the issue but was unable to reproduce the problem at the time of inspection because the codes had cleared. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an authorized dealership after being towed. No police or insurance inspections were conducted. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no significant warning signs prior to the sudden loss of power. The failure occurred unexpectedly while driving. Diagnostic codes were retrieved immediately after the incident but were no longer present during the dealership inspection.

While driving on the highway, something (likely a small rock) from oncoming traffic flew into the windshield. Upon contact, the windshield cracked and shattered in a manner I have not seen a windshield break before, immediately spraying glass shards across the interior of the vehicle (to include dashboard, seats, and floors) and the driver. From the outside of the windshield, the area of damage is soft to the touch such that it is likely applying any pressure would collapse that area and/or enable you to punch straight through to the other side, so for safety concerns it is undrivable until the glass is replaced. I am concerned the glass could have been defective in some way, as I don't believe this is a typical response to common road hazards like small rocks/other projectiles. The windshield was the original factory installed windshield. The vehicle has not yet been inspected by the manufacturer but the incident has been reported and owner is awaiting contact from the manufacturer regarding inspection. It can be available for inspection until the glass is replaced, but owner will have glass replaced as soon as possible. Additionally, because it is not drivable, it would require inspection at residence or towing. The incident was reported to insurance. There have never been any previous warning lamps/other indicators that the glass was defective.

My vehicle has slowed while driving four times within the last 12 months. I have taken it to the Hendrick Lexus dealership in Charlotte as well as the Scott Clark Toyota dealership in Charlotte. I was told by the Lexus dealership they could not find anything wrong (this was back in July 2025). The Toyota dealership stated (March 2026) that there may be a problem with the fuel pump, but they were unable to confirm this issue and after getting the vehicle back the same thing happened while driving. This has been concerning. It is very dangerous for us to drive the vehicle at this point. I see that there has been a safety recall introduced in the state of Georgia at a Smyrna Lexus dealership, but I have yet to see any other information. I would greatly appreciate an investigation. Thank you.

3 unresolved safety defects on 2024 Lexus NX 450h+ plug-in hybrid leased May 2023: ELECTRICAL/BATTERY: Rear seat occupant sensor produced continuous false alerts since weeks after lease began, preventing vehicle from locking. High-voltage components including on-board charger located beneath/near the rear seat area per dealer representation at time of sale. Service manager acknowledged liability in writing July 2023. Lexus TSB L-SB-0035-24 addressing battery monitor module failure (DTC P162B87) never applied. Recall 25LD01 covering high-voltage floor wire corrosion unverified for this vehicle. Not charged since approximately April 2025 — over one year — due to fear of battery malfunction, fire, or explosion. Operating exclusively on combustion engine, eliminating primary plug-in feature. Reported in writing for 2 years 8 months. Never repaired. GM confirmed in writing February 2026 defect never submitted to manufacturer. DRIVETRAIN/TIRES: Severe premature tire wear from approximately 2,252 miles. At 7,800 miles dealer declared vehicle unsafe for freeway driving. Owner used alternative transportation for court appearance due to safety warning. Dealer's own alignment printout January 2026 shows multiple out-of-specification values. All 4 tires replaced after 72 consecutive days dealer possession, classified as goodwill, never submitted as warranty claim per GM written admission February 2026. HVAC/BIOHAZARD: Persistent mold/mildew odor from HVAC since early in lease. Service advisor initially attributed cabin odor to rat feces in engine bay entering cabin through air circulation — admitting biohazard exposure to occupants. Rodent nesting caused by manufacturer's soy-based wiring insulation. Rat feces constitute hantavirus and salmonella risk. Same defect across three consecutive vehicles from same dealer. Engine bay sanitized and evaporator cleaned during 72 days, classified as goodwill only, never submitted to manufacturer. Documentation available for inspection.

VEHICLE SPEED CONTROL, BACK OVER PREVENTION: WARNINGS, SERVICE BRAKES
Possible malfunction involves vehicle speed control and braking during a low-speed parking maneuver in my 2024 Lexus NX350h. While slowly pulling into a parking space, I was actively applying the brake and creeping forward. While my foot was on the brake, the vehicle suddenly surged/accelerated forward instead of continuing to decelerate. The vehicle ran over a sign and jumped a curb before I could stop it. The vehicle is available for inspection and is currently with the Lexus dealer. My safety and the safety of others were put at serious risk because this occurred unexpectedly while parking. If a pedestrian, fixed structure, or another vehicle had been in front of me, this could have caused severe injury or worse. Thankfully, I was not physically injured, but this was a traumatic event, and I was terrified. The problem has not been reproduced by me after the incident. At the time of this report, it has not yet been confirmed by the dealer or an independent repair facility. I have requested a full diagnostic inspection and review of all available stored data before any codes are cleared. The vehicle has been delivered to the Lexus dealer for inspection. The incident was also immediately reported to my insurance company. I do not know if Lexus manufacturer representatives or other parties will inspect it. I did not observe any warning lamps, messages, or unusual symptoms before the event, and this had never happened before.
