ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, UNKNOWN OR OTHER
Recently my car has had a series lights come on including 4wd fault, pre-colliosion assist not available, service advance trac, hill start assist warning, and something with the brakes when in reverse. I took it to the dealer and they said it's something electrical that will need parts, and further investigation with removing the seats. That will be another day. Today, while driving with my son in the back, I was taking a u-turn on a busy street. The car acted like it switched into neutral (but didn't), and wouldn't accelerate, leaving me inching into traffic at 1 mph with cars going 40mph coming at me. Luckily I was not hit, but easily could have been. I drive almost 60 miles a day for work and this is a safety hazard not just for me, but my family too.

The contact owns a 2026 Ford Explorer. While the contact's wife was driving 60 MPH, the instrument cluster independently turned off. The vehicle was taken to the dealer where it was diagnosed with a software malfunction. The contact was informed that the unknown software needed to be updated. The vehicle was repaired; however, the failure reoccurred. In addition, the contact stated that only the fuel gauge and the speed odometer operated as needed. The vehicle was taken back to the dealer where the vehicle was diagnosed with an unknown software malfunction. The contact was informed that the software needed to be updated. The vehicle was repaired. The contact was informed that the software update was completed, and the system was rebooted. The contact was not satisfied with the repair. The contact researched and inquired about the APIM board being replaced to prevent the failure from reoccurring. The contact was informed that the manufacturer did not approve of the repair. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 1,000.

When i go to accelerate i lost power and then 2 seconds later car would finally go, this has happened several times getting onto freeway or just trying to pass and have almost been rear ended, i took car to dealer and no remedy available still 1 year later, my specific car has had several recalls which is ridiculos, i am not getting any help with my situation

VISIBILITY/WIPER
The panoramic sun roof shade failed on my vehicle where instead of it retracting properly it drops down onto the second row passenger's. This posses a safety risk as it impedes completely the drivers ability to see through the rear view mirror by blocking off the view of the back glass. The issue happened without any notice or noise it just happened all of a sudden I noticed it as I was not able to look through rear view mirror. This also puts at risk others as it impedes the drivers field of view allowing the possibility of an accident of occurring. I like to drive with my sunroof open very often, so now that is a risk I will have all the time I open my sunroof where I will have my field of view blocked. I have called two ford dealership to try to get quotes to repair the issue. First I was shocked with the price for the repair, but I was also shocked when I was told that it is a very common issue with the panoramic sunroof where the shade drops down. I asked do people usually get it repaired or not and they told me that most people don’t fix it that they just decide to deal with it or not use the sunroof at all. One of the dealerships told me the cost of the repair was roughly above $3 thousand dollars and the other told me it would be around $2.8 thousand dollars. A lot of labor is required as they have to bring down the headliner and remove the sunroof glass panels to be able to get it repaired. They also were not able to guarantee that after the repair that it would not happen again as they told me it was a design flaw that it was just a matter of time before it happened again. I believe Ford should be hold accountable for this and provide a permanent fix to this issue that is very common with the panoramic sun roof that creates a safety hazard for vehicle occupants and others. Especially if after customers have to pay considerable amounts of money for repairs and the issue will happen again as it is a design flaw.

I parked my car and when I returned to it the car would not start. Diagnosed as a faulty fuel pump. Had this problem occurred while I was driving it could have caused serious injury or death. There is an active recall on fuel pumps for the 2021 Ford Explorer that does not apply to my VIN number so Ford will do nothing to remedy the situation and parts are impossible to find as dealers scramble to fix the vehicles included in the recall.

SEAT BELTS
On XXX, in the parking lot of O'Reilly Auto Parts (store #2460, telephone number: [XXX]) [XXX], between 12:10 pm and 12:45 pm, my [XXX] daughter was seated in the rear driver's side seat of my parked 2022 Ford Explorer when the unbuckled seat belt unexpectedly became twisted around her body and would not release through normal operation. She was unable to free herself and the belt continued to tighten as we attempted to untangle her. She experienced difficulty hearing, breathing, lost sensation in her arms and nearly lost consciousness. Multiple adults attempted to release the seat belt using the buckle and by manipulating the belt mechanism, but the belt would not release. Because of the immediate risk to her safety, the seat belt had to be cut in order to free her. The failed component was the rear center seat belt assembly. The damaged seat belt is available for inspection upon request. This malfunction placed my daughter at risk of serious injury due to restricted breathing and the inability to exit the restraint. The vehicle could not be returned to normal operation because the seat belt had to be cut to release her. At the time of the incident, there were no warning lights, messages, or other indications that there was a problem with the seat belt system. UNKNOWN whether there were any prior symptoms before the failure. The incident was witnessed by individuals who assisted in freeing my daughter. Two O Reilly employees and my [XXX] daughter who was a passenger. They assisted during the emergency and can verify what occurred. The manufacturer and local dealership have been notified, and a customer service case has been opened. At the time of this report, the manufacturer has not inspected the vehicle or confirmed the cause of the failure. SERVICE BRAKES, POWER TRAIN, BACK OVER PREVENTION: WARNINGS
I am reporting a safety concern involving a delayed transmission gear engagement and delayed rearview camera activation. Intermittently, when shifting the vehicle from Park into Reverse or Drive, there is a noticeable delay before the transmission actually engages the selected gear. During this delay, the rearview camera also fails to activate immediately. Because there is no immediate indication that the requested gear has engaged, I have experienced situations where I believed the vehicle was in Reverse or Drive, but it remained in the previous state for several seconds. In some instances, the transmission engages unexpectedly after the delay, causing the vehicle to move in a direction different from what I anticipated. For example, after selecting Reverse, the vehicle may hesitate before moving, or after changing between Reverse and Drive, the delayed engagement can result in the vehicle moving in the previously engaged direction before finally shifting into the selected gear. This creates a significant safety hazard, particularly when parking, backing out of driveways, or maneuvering in areas with pedestrians or other vehicles. The delayed rearview camera activation further increases the risk because it prevents the driver from immediately seeing behind the vehicle while attempting to reverse. The issue occurs intermittently and without warning, making it difficult to predict. I am concerned that this condition could lead to a crash, property damage, or injury if the vehicle moves unexpectedly due to the delayed gear engagement. I request that NHTSA investigate this issue to determine whether it is related to a transmission, electronic shift control, or software defect affecting the safe operation of the vehicle.

FUEL/PROPULSION SYSTEM
I was traveling on an 8 hour drive after just getting a new battery and fresh oil change, I also had a full diagnostic ran the week before travel and no issues were found in my vehicle. When we arrived at our destination we stopped 1 final time and attempted to start our vehicle to go to dinner and the car would not turn over. There were not any warning lamps or messages that appeared. We used roadside assistance to tow it to the [XXX] dealership and they ran another full diagnostic that showed "replace fuel pump and fuel pump driver". When I called my local dealer back home, Evansville Ford ([XXX]) they pulled the previous week diagnostic and confirmed nothing showed up regarding the fuel pump at that time. 1 week later is when we had it in [XXX], and the fuel pump failed. The light for the right hand mirror blind spot warning came loose within the mirror housing. The light still works and activates when a vehicle is in the blind spot, but the light has fallen to the bottom of the mirror housing and is barely visible (a yellow glow at night).

Purchased brand new vehicle two months ago, only has 2000 miles on it. We took it on vacation 14hours from home. Made it to our destination and started getting “vehicle communication errors” on the screen. Jumped the vehicle, notifications went away. We scheduled an appointment with a local ford dealer who told us to bring it in. On the way to the dealership and while driving on the road, the vehicle screen listed the same “vehicle communication error” indicator and every single vehicle service light was on. We lost all electrical power. No blinkers, no miles per hour indicator, no lights etc, but we were able to continue driving to the service dealership. Drove an additional 15 miles with absolutely no electrical power and did not want to stop as we were afraid the car was not going to turn back on, on the side of the road per previous experience.

EXTERIOR LIGHTING, SUSPENSION
Faint popping and knocking sound coming from front suspension, LOUD clunk coming from rear suspension when going over bumps. Both seem to cause vehicle to be floaty while driving. Also headlights sometime flicker while driving with switch in auto position and sometimes will not shut off after exiting vehicle ( have to turn switch to off position and then back to auto to resolve ) Vehicle has approximately 21k miles on it but off warranty due to time. Will be bringing into local repair shop soon.
