The contact owns a 2024 Toyota Rav4. The contact stated while starting the vehicle, the hood was shuddering significantly. Additionally, the contact stated while driving at various speeds, the hood shuddered and rose continuously while remaining closed. The contact pulled over to the side of the road each time the failure occurred due to safety concerns. The contact stated that the failure had reduced the ability to pay attention to the road while driving. The dealer was notified of the failure and informed the contact that it was a known issue. The manufacturer was notified of the failure and informed the contact that the failure occurred because the hood was no longer made with metal material but with aluminum material. The manufacturer referred the contact to the dealer for assistance. The vehicle was not diagnosed or repaired. The failure mileage was unknown. The VIN was not available.

On [XXX] I discovered and reported the unauthorized and unlawful installation of a wifi screw spy camera in my vehicle to Officer Palamcci from the Danvers Police Station located in Danvers, Ma. The WiFi Screw spy camera is discreetly installed in my vehicle. Which was recently purchased at IRA Toyota Danvers, M. The WiFi screw spy camera installed by a Toyota employee has the presence of a LED in the center of the black WiFi screw which is currently screwed inside the mount of the rear view mirror. This is in violation of Toyota’s Code of Conduct safe practices and procedures and privacy laws and regulations. I have submitted the criminal complaint to the FBI, Danvers Police station, Woburn Toyota and Woburn Police Station. The contact stated that when she had parked the vehicle on a road with an upgrade with the front of the vehicle facing up, she observed that water was leaking out from the bottom of the door. The contact stated that when it rained, the noise and water leak were more noticeable. The contact had not taken the vehicle to a local dealer or independent mechanic. The front passenger seat is unusable as the OCS sensor has turned off the airbags for that seat. Every time the car is started, or begins driving after sitting idle for a couple of minutes, it will beep and alarm constantly for several minutes, warning me of the passenger airbag. It is a serous distraction, and now prevents me from transporting a passenger. This has been going on since June 2024, and dealer refuses to look at car due to recall. I was even willing to pay myself to make the repair. No one will touch it. The remedy was originally set for the summer time, then end of 3rd quarter 2024. Now, it’s almost a month after the end of the 3rd quarter. Thank you for your time in reviewing this.

- Passenger side rear tire failed - Tire is at dealer and available for inspection - Extremely dangerous situation, tire failed on a section of interstate highway under construction. Only 2 lanes available and they were narrower than normal width. Part of the shoulder was converted to driving lane so almost no shoulder to pull off on. The incident happened on a Sunday afternoon and interstate was extremely busy with weekend traffic. My wife was extremely lucky not to be involved in a serious accident. - Tire has not been inspected by anyone other than the car dealer - If the tire pressure warning light came on it wasn't noticed. There were no symptoms or warnings prior to the failure.

The contact was also informed by the dealer that the failure was a result of the vehicle's design and that no other repairs could be made to address the failure. I've seen this error message (voltage abnormality steering power low) a few times whenever we are at a stop light and begin to go. This only happens whenever the Start/Stop function is active so the engine is stopped while not moving. Then once we start going, it will show up for a second and then disappear. We have taken the car to a local shop where they said that the alternator was beyond normal use and we received a quote as they believed it was worth replacing. When we went to Toyota dealership as the alternator is still under warranty they said they didn't see anything, they would not allow us to drive the car or hold onto the break in their shop. Problem continues to persist.

This incident occurred on the morning of [XXX]. I was driving my new, 2024 Toyota RAV4 hybrid on the highway (it has less than 20,000 miles on it). There were no vehicles or other obstacles or road debris of any kind near me. I suddenly heard an incredibly loud, explosive sound. It was like a gunshot or as if a body had fallen on my roof. I then heard the noise of wind. My sunroof had exploded. Fortunately, I had the sunroof and the interior screen closed, so glass did not fall into my car. Once I was able to pull over, I examined the sunroof to find it had shattered and there was no indication of anything falling on or colliding with it. I took two photos and proceeded slowly to the Toyota dealer (Ourisman Toyota in Richmond, Va.) where I purchased the car brand-new/off-the-truck in February 2024. The dealer stated that they would not cover glass, including a spontaneously exploding sunroof on a new car, and so I am required to pay out of pocket. Although fortunately I was able to keep control of my car and was not injured, this was a serious and dangerous event. (Note, I was unable to upload the photos below, but have them available.) There is a recall on our passenger side air bag - NHTSARecall #23V-865. Toyota recall#23TA15 Bellingham Toyota verified this - and we have an indicator/problem light that is on. I am very upset becuase we were told a fix would be available in 3rd qtr 2024 and now it is 4th qtr 2024. When I called Toyota headquarters, they have no answer as to when there will be a fix. They offered a loaner - however they prohibit the use of that loaner in Canada. Additionally, we would have to pay the cost of a rental up front and then submit receipts later for reimb but they CAN't guarantee reimb. We are very low income and so we cannot front that money, NOR take a chance that it would not be reimbursed. We live right on the US/Canadian border and we go to Canada a lot to visit family - two of which are currently in the hospital/his mom and my sister. My husband is a quadriplegic and we only have this one vehicle to go back and forth with our 2 daughters. There is nowhere near enough room with his wheelchair/walker for when we all want to travel. Additionally, when it's just him and me, he can't hear me speaking to him when he has to sit in the back seat all the time. At this point, we would like Toyota to take our current vehicle back in a trade and provide us with a new car that has none of these problems.

The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The contact stated that while driving at approximately 35-40 MPH with traffic ahead slowing down, the vehicle registered the traffic information as an immediate stop ahead and the braking system engaged along with the contact depressing the brake pedal to slow the vehicle down. Upon coming to a stop, a pick-up truck rear-ended the vehicle 3 - 4 times. No air bags deployed. The driver's front seat belt failed to retract during the impact to safely secure the driver. The contact sustained injuries to her knees, shoulders, and arms. The front passenger sustained bruises from the seat belt. The rear passenger sustained head and back injuries. All 3 occupants received medical attention via ambulance to the Emergency Room. A police report was filed. The vehicle was towed due to it being undrivable. The insurance company was currently inspecting the vehicle. The vehicle was not diagnosed or repaired. The dealer was not contacted. The manufacturer was not made aware of the failure. Brand new vehicle recently purchased 9/28/2024 at the Dealership, Toyota of AA, Michigan. While driving at highway speeds greater than 70 mph, the flimsy hood balloons/bubbles up and starts flexing visibly and violently with almost 2 inch amplitude waves and looks like it wants to become unlatched and slam into the windshield. Pretty scary! This is definitely a safety risk that raises the fear in me of a potential serious crash. Vehicle dropped back at the Dealership 10/7/2024 for inspection.

I was pulling into a parking space at work and went to break to park and my car accelerated up a hill. After I released my foot from the break,and tried pumping the break the car eventually decelerated and I was able to come down off the hill to park. I had damage to the passenger corner panel and had to file and insurance claim for repairs. I called the dealership and they said they had not heard of any type of issues with breaks on this model and to file an insurance claim and they would inspect the vehicle for default.

While driving at about 30 mph on [XXX] on Sunday, [XXX] at [XXX] the moonroof suddenly exploded. It sounded like a gunshot and startled me. I was not going under an overpass, there were no people nearby who would have thrown anything at the car, and I was not following another vehicle. I believe the glass shattered as result of mechanical defect. The moonroof was closed at the time so I was not hit with flying glass, but the situation could have been quite dangerous had the roof been open. I also think the loud sound of glass exploding could cause other motorists to jerk the wheel in shock at the loud noise. Naturally, there were shards of glass flying off my vehicle afterward, which creates something of a road hazard for anyone following behind. I have had the moonroof inspected by the Toyota dealer. They advise that glass is not covered under warranty, which is nonsense because at the least it violates the warranty of fitness for a particular purpose. But, it is hard to prove a manufacturer's defect so I will be submitting a claim through insurance. In my opnion, Toyota should cover the repair costs as I did nothing wrong and was not hit by a foreign object. Under braking conditions and hitting a bump on a paved road, the vehicle will have a 1 to 3 second momentary loss of braking. At the same time, a grinding noise, similar to (if not the same as) the ABS system is heard, and the VSC light flashes. The occurrence of this problem is unpredictable, the severity is inconsistent. It happens across various ranges of brake pressure/deceleration rates and speeds. No accident has occurred to date with this vehicle, but we have had close calls. In some cases, our vehicle has inadvertently entered controlled intersections due to loss of braking. Dealer has inspected the problem but has been unable to replicate or resolve to date. Based on internet research, this appears to be a common problem. Very concerned this could occur at the wrong time and lead to an accident, causing injury or death due to behaviors of the vehicle's braking (and loss of) being outside the driver's control.
