While driving in traffic, my 2023 Toyota Mirai suffered a sudden and total loss of motive power. The vehicle became completely unresponsive while in 'Drive', presenting a severe safety hazard. The dashboard displayed multiple cascading warnings, including 'FCV System Malfunction', 'Pre-Collision System Malfunction', and 'PKSB Malfunction'. The vehicle had to be towed to Dublin Toyota (Repair Order #653103). The dealership retrieved critical diagnostic code P1DC500 (FC AIR PRESSURE TOO HIGH) and verified the vehicle would not go into 'Ready' mode. The dealer opened Toyota TAS Case #TA26079027. However, the dealership performed no mechanical repairs to address the root cause of the sudden power loss. They simply cleared the safety code, test drove it, and explicitly wrote on the final invoice: 'ADVICE CUSTOMER TO DRIVE VEHICLE UNTIL PROBLEM OCCUR'. Furthermore, they refused to document my stated concerns that the vehicle is unsafe to drive. Returning a vehicle to a customer with an unresolved, life-threatening defect that causes total loss of power at speed is incredibly dangerous.

The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired since the extended warranty had expired, and the vehicle had a Salvage Title. The vehicle was taken to another local dealer, where it remained for further investigation. The manufacturer was made aware of the failure. The failure mileage was approximately 48,000.

The contact owns a 2022 Toyota Mirai. The contact stated that while driving approximately 60 MPH, the vehicle started losing motive power. The accelerator pedal was depressed, and the vehicle continued to decelerate. The contact stated that while driving approximately 15-20 MPH, several warning lights were illuminated, and the stability control symbol was displayed, and a yellow flashing warning light was illuminated. The yellow flashing warning light turned red, and the vehicle stalled. The instrument cluster turned black, and the steering wheel became difficult to turn. The contact attempted to restart the vehicle but was unable to shift to park(P). The contact stated that a Police Officer attempted to jumpstart the vehicle, but the vehicle failed to start, and a tow truck company was called for assistance. The vehicle was towed to the residence. The contact replaced the battery and was able to start the vehicle and the a “FCV” and the shift to park(P) message was displayed. The contact called a local dealer and was advised that the vehicle needed to be towed to the dealer for a diagnostic test. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 45,000.

On January 26, 2026, at approximately 6:50 AM, I experienced a serious safety-related malfunction in my 2023 Toyota Mirai while entering traffic on Inglewood Avenue in Lawndale, California. I was pulling out of a parking lot into active traffic at low speed. At the moment I was entering the roadway, the vehicle suddenly lost propulsion/power without warning. Simultaneously, the vehicle’s infotainment/center display went completely black, consistent with a system reboot or electrical interruption. After a brief pause, the display rebooted and turned back on, and immediately afterward the vehicle unexpectedly surged forward. At the time of the power loss, my foot was lightly on the accelerator pedal as I was merging. When the vehicle’s systems reinitialized, the car applied power consistent with the pedal position, resulting in a sudden forward movement. The accelerator was not fully depressed, and this was not a full-throttle event, but the surge occurred without warning while the vehicle was in active traffic. The sequence of events — loss of propulsion, apparent electronic system reset, followed by unintended propulsion upon re-engagement — created a hazardous situation while entering traffic and raises serious concerns about the vehicle’s electronic control and power management systems. The vehicle’s behavior was unpredictable at a critical moment when controlled acceleration was required, which could have resulted in a collision or injury. There were no warning lights or messages prior to the event. I am reporting this incident because it appears to involve a safety defect related to loss of propulsion and unintended surge while in motion. I am scheduling a service inspection and will request a full diagnostic scan, including history and freeze-frame data from all control modules. 1) Electrical display goes off on its own. While car in motion and driving,Distract me while I’m driving. 2) car does not start. AAA came to jumpstart, but no luck main battery under the car not responding defective battery. 3) fuel system not working properly cannot fill up the car all the way with hydrogen, it’s giving me 180 miles to maximum of 200 miles to drive ,Car slowed down on it’s own in the freeway lose power very unsafe and unreliable vehicle. 4) Blindspot, not working all the time as I am backing out it does not see other cars or pedestrian coming from left or right side or sometimes stop on it’s own with no car or pedestrian on either side. I have took this car to dealer several times and all they’re telling me they cannot duplicate the issue and I don’t believe it. this is a LEMON vehicle and very unreliable and unsafe this vehicle need to be recall. This car is now parked in my driveway I am afraid to drive this vehicle due to a safety issue .

Summary of Issue: I am the owner of a Toyota Mirai that suffered a sudden loss of power due to contaminated hydrogen fuel. The incident rendered the vehicle undrivable and raised serious safety concerns, as loss of propulsion in a fuel-cell vehicle can occur without warning while in motion. The vehicle was taken to Toyota of [XXX] for diagnosis and repair, where Toyota confirmed the presence of contaminated fuel. Toyota’s Stance: According to Toyota, the contamination issue was addressed by flushing the contaminated hydrogen from the system and refueling the vehicle. Toyota has stated that this process has restored power and that the company does not accept responsibility for any damage caused by the contamination. They have not disclosed any data on the type or level of contamination found, nor have they provided a written service report detailing inspection results or long-term corrective actions. Owner Concern: Independent research and fuel-cell engineering references indicate that contaminants such as sulfur compounds, carbon monoxide, or heavy metals can permanently degrade or “poison” the fuel-cell stack. Flushing alone cannot reverse such chemical or structural damage. As a result, there is a risk of future performance loss or total stack failure. Without transparency from Toyota regarding contaminant analysis or verification of system integrity, I cannot consider this vehicle safe or reliable to drive. Request for Resolution: I am requesting that Toyota: Provide complete written documentation of the contaminant analysis and testing performed on my vehicle. Disclose the long-term plan for ensuring the fuel-cell stack and related components have not been compromised. Offer either a full buyback/repurchase under applicable consumer protection laws or a complete fuel-cell stack replacement verified by engineering inspection. The warning that is supposed to appear every time the car is started that the car will auto shut off after one hour of idling never once appeared. As a result, my husband did not know the auto shut off feature existed on his car. He fell asleep in his car on a hot summer day and when the car auto shut off after an hour, he died of heatstroke. The lack of a warning coupled with the car auto shutting off after an hour was reproduced and confirmed by an independent service center. The car has not been inspected by anyone else. There were no warnings, messages, or other problems prior to my husband’s death. He was not informed of the auto shut off feature at the dealership at the time of purchase and the dashboard warning never once appeared to alert him such a feature existed. He thought it was safe to sleep in his running car. The auto shut off feature does not work properly as there was no warning on the dashboard and does not safeguard against people or animals being in the car at the one hour mark. Toyota needs to know this is a deadly feature and make some serious changes.

In 2021 I found a Toyota Mirai Hydrogen Fuel lease take over opportunity on [XXX] I saw the car in person and applied to take over the lease from Toyota Financial. A month and a half later I was able to take possession of the car. I thought I am going to get the $17,000 fuel card with the car that Toyota blocked. This was a used car that I got with over 4,000 miles on the car. Toyota financial Never told me that I cannot use the fuel card, when I applied to get the car? Issues with the car: Not finding Fuel, I live in West Los Angeles/BH There were Hydrogen Fuel station in Van Nuys, Studio City, Topanga, East Hollywood, Santa Monica, all very far from me, the west Hollywood Hydrogen fuel station which was closer has been under construction. Each Gas Station that offers Hydrogen fuel, usually has only "One" Hydrogen Fuel Pump, that none of them is serviced by the gas station, they all have a separate owners which are renting space from the gas station, which huge lines of cars waiting and praying so they can get Hydrogen fuel. There has been many times that I had to drive from one Hydrogen Station to another, to get fuel, and been unsuccessful in getting Hydrogen for my Mirai. There was once that I ran out of fuel while trying to find Hydrogen Fuel. So I had to tow the Mirai to a gas station in Thousand Oaks to get fuel. I constantly have problems with the car. There have has been many times that Hydrogen was not available, the pump did not work, it did not accept payment, it only gave a few dollars of fuel at a time, the pump did not disengage from the car (there has been times that the pump did not seperate from my gas tank for half an hour, I have it on film) Every Mirai owner I have met at different gas stations are sick and tired of dealing with all the fuel issues associated with the Mirai. Before I took over the Mirai lease, I went to the Santa Monica Toyota dealer, they told me that every Mirai comes with a $17,000 fuel card! I am writing to express my deep disappointment and frustration regarding the malfunction of the Teammate™ Advanced Drive package in my recent purchase of a 2022 Mirai Limited edition, with the Vehicle Identification Number (VIN): [XXX]. The faulty performance of the system resulted in an accident and subsequent damages to my vehicle. On November 5, 2022, while operating the vehicle with the Teammate™ Advanced Drive package engaged, the car failed to stop as intended and collided with a parked car, causing significant damage. Understanding the gravity of the situation, I immediately contacted my insurance provider and followed their recommended course of action. What component or system failed or malfunctioned, and is it available for inspection upon request? Car suppposed to stop itself as it has all sensors. Still car rear ended other car. How was your safety or the safety of others put at risk? Jeopardy. Since accident occured in residential area my family was okay. What if it was in Free way? Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet, they are not providing me alternate transportation during inspection so not ready yet from my side. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Vehicle was inspected by Insurance as normal accident. Not sure if there were further investigation. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warnings
