I was driving during heavy rain when I heard and felt a big thump. I wasn't sure what happened as I did not hit anything in the road. I was in my neighborhood driving at low speeds (<30mph). When I got home, I realized my fender liners and lower splash guard of my car were missing. I went back out to retrieve these pieces of my car, and it appeared the fender liners and the splash guard got torn off my car without impact with anything, as nothing was in the road near it. I was going to attempt to reconnect them but the plastic clips on the front bumper also ripped off, making this a very expensive repair requiring bumper replacement and sensor recalibration. This obviously could put others at risk of running over a large number of components that were left in the road. I read many other instances of other people with the same vehicle (as well as with the Solterra) having an issue where the felt fender liners can become saturated with water, sag, and get caught in the wheels, ripping them off. I was one of those unlucky people to lose both sides and the splash guard all at the same time. The dealership / body shop inspected the vehicle. No warnings at all - just sudden ingestion of the fender liners into my drivetrain.

While operating my 2024 Toyota bZ4X at very low speed on the road , I observed a stationary shipping container directly ahead and applied the brake pedal with maximum force approximately 4 seconds before contact. I held the brake pedal at full pressure continuously. I never touched the accelerator. The vehicle came to what appeared to be a complete stop then continued to advance and made physical contact with the container despite my maintained maximum brake input. The vehicle's own ECU-recorded Freeze Frame Data (retrieved by an authorized Toyota dealer using PCS Data Viewer) confirms the following at the time of the event: Driver brake pedal torque was at sensor saturation (4,048 Nm — the maximum value the sensor can record) for approximately 1.75 seconds prior to impact. Brake hydraulic line pressure peaked at 12.144 MPa and remained above 7.8 MPa throughout that period. Throttle position was 0.0% for the entire recorded interval. All four wheel-speed sensors recorded 0.00 km/h for approximately 2.95 seconds prior to impact. Steering input was effectively neutral; no skid or loss of directional control occurred. The longitudinal accelerometer recorded a peak deceleration spike of -8.97 m/s² at the moment of impact — a value that requires forward velocity and is physically inconsistent with the wheel-speed data showing the vehicle stopped. The Pre-Collision System (PCS) main switch was ON throughout, with no faults, no obstruction flags, and no warning, pre-brake, brake-assist, or auto-brake activation recorded. The Parking Support Brake (PKSB) did not activate. No diagnostic trouble codes were set. In plain terms: with maximum brake pressure applied and zero throttle input, my vehicle continued to move forward into a clearly visible stationary obstacle. The brakes did not maintain authority over the powertrain's creep torque, and no advanced safety system intervened. This indicates a potential defect in the vehicle's brake-override / creep-suppression

The vehicle experienced a sudden, catastrophic brake-by-wire system failure while operating in one-pedal driving (Regeneration Boost mode). While decelerating at a low speed behind an 18-wheeler semi-truck, the vehicle failed to continue slowing down. I immediately depressed the physical brake pedal, but the vehicle completely ignored the foot pressure. The pedal and braking system became completely unresponsive, and the vehicle kept rolling forward uncommanded. Despite keeping the brake pedal fully depressed, the vehicle did not decelerate, resulting in a front-end low-speed collision with the metal rear bumper of the semi-truck. Because the collision occurred at a very low speed, the operator of the commercial truck was unaware of the impact and safely continued driving forward without stopping. The vehicle's brakes entirely refused to re-engage until I frantically pumped the physical brake pedal and cycled/pressed vehicle control power switches. This action seemingly forced the electronic braking modules to reboot or re-establish hydraulic pressure, allowing the vehicle to finally come to a stop. The vehicle was subsequently removed from the active roadway. This is a severe, life-threatening safety defect involving a total loss of electronic brake-by-wire and friction braking application without any mechanical override intervention, presenting an extreme hazard. The vehicle has been towed directly to an authorized Toyota dealership for a comprehensive electronic diagnostic pre-scan of the Skid Control ECU, Electronic Parking Brake system, and 12-volt auxiliary power systems to retrieve the stored DTC fault logs and freeze-frame data.

The contact owns a 2024 Toyota BZ4X. When pressing the accelerator toelerator pedal to go onto the freeway, the vehicle failed to accelerate and suddenly shifted to neutral. The contact would have to press the brake pedal, the contact would immediately shift the vehicle back to Drive and continue forward. While continuing to drive, the vehicle accelerated. There were no warning lights. The failure had occurred 3 times. When attempting to start the vehicle one day, the vehicle failed to start. The tow company came and provided a jump-start, and then advised the contact to drive carefully, but if it failed to go in reverse. The next morning, the tow company's mechanic came again and confirmed battery failure, then recommended the vehicle be taken to the dealer. The vehicle was towed to the dealer, who stated that the battery had not failed, but had not provided a diagnosis for the failure. The vehicle had not been repaired. The dealer had driven the vehicle back to the contact's home. After the vehicle was returned, the failure occurred again. The manufacturer was informed, and a claim had been filed. The failure mileage was 9,787.

The system produces a loud grinding/buzzing noise and has been diagnosed by authorized Toyota dealerships as requiring full compressor replacement. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? This failure directly impacts the vehicle’s defrost system, which is critical for maintaining windshield visibility. Reduced or failed defrost capability creates a safety hazard by impairing visibility while driving, increasing the risk of an accident. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue has been confirmed by multiple authorized Toyota dealerships, including Sheehy Toyota and Mike Calvert Toyota. Both independently diagnosed a failed A/C compressor requiring replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. The issue has been escalated to Toyota Corporate, and a case manager has reviewed the dealership findings. Documentation has been submitted to Toyota for warranty and goodwill consideration. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. A grinding/buzzing noise began when the A/C system was activated and was formally reported to Sheehy Toyota on October 27, 2025, while the vehicle was under warranty. The issue was not repaired at that time. The condition progressively worsened and resulted in confirmed compressor failure on March 25, 2026.

While driving on the highway under normal conditions with no road hazards present, the engine under cover (part #51441-42210) detached from my vehicle and the bumper clips broke, causing both the under cover and bumper to become damaged and requiring replacement. There was no collision, no debris, no pothole, or any other road hazard involved. The failure occurred solely under routine highway driving conditions. Upon taking the vehicle to my Toyota dealership, the service team acknowledged this is a known and recurring issue with the bZ4X model. Research and owner forums confirm multiple 2023 bZ4X owners have experienced the identical failure — undercarriage cover and bumper detachment under normal driving and rain conditions. Toyota dealers have repeatedly told affected owners ‘this happens all the time,’ yet Toyota refuses to cover repairs under warranty, claiming it is not a manufacturing defect. I believe this represents a systematic design or manufacturing defect in the 2023 Toyota bZ4X. The undercarriage cover and bumper mounting system fails under normal driving conditions, which poses a safety risk to the driver and other vehicles on the road. I am requesting a formal investigation into this issue and urge NHTSA to consider a recall to protect other bZ4X owners.

The contact called another dealer and the manufacturer, and was informed by both that the part was on national back order. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The failure mileage was approximately 55,000.

I LEASED A TOYOTA BZ4X THE PARKING BRAKE LIGHTS AND OTHER LIGHTS COME ON AND OFF WHILE I AM DRIVING AND FOB IS NOT TOUCHED. I TOOK CAR IN TO BE SERVICE FOR REGULAR MAINTENANCE AND WAS TOLD I HAD TO PAY $295 AND I AM UNDER WARRANTY. I WAS SOLD THE CAR BRAND NEW AND I DON'T THINK IT IS. I KEEP HEARING PEOPLE TALKING WHEN THE CAR IS POWERED OFF. I REPORTED THIS SEVERAL TIMES AND WAS IGNORED. MY CAR KEEPS TRYING TO CUT OFF WHILE I AM DRIVING AND THE CAR STARTS SHAKING, ALL THE LIGHTS COME ON THEN GO OFF. I WAS TOLD I COULD GET MY CAR SERVICE ANYWHERE FOR NO CHARGE UNDER WARRANTY. Inaccurate Battery Readings: The electric system appears to malfunction regularly. The displayed battery percentage and estimated mileage are inconsistent and unreliable. For example, the battery may indicate a higher percentage while simultaneously showing a drastically reduced range, or vice versa. 2. Lane Departure Warning Malfunction: The lane departure system activates erratically, sometimes without any lane deviation. At other times, it fails to engage when it should, which compromises the vehicle’s safety features. 3. Tire Pressure Monitoring System (TPMS) Alerts: Warning lights frequently come on indicating that the tires are losing pressure, even after verifying that tire pressures are within recommended levels. These warnings occur unpredictably and without cause. Despite having the vehicle inspected twice, I have not received a resolution or acknowledgment of any defects. Toyota has stated that nothing is wrong with the vehicle, but the issues are ongoing and continue to impact the car’s drivability and safety. Premature and abnormal tire wear on my 2023 Toyota bZ4X, confirmed by two independent tire shops, despite consistent Toyota dealer maintenance every 6,000 miles. Beaverton Toyota dealer dismissed the condition as “normal,” but both [XXX] and Discount Tire documented unsafe tire wear caused by alignment. --- Details of Problem: Vehicle serviced exclusively at Toyota dealer (Beaverton Toyota, OR) every 6,000 miles, with multipoint inspections and rotations. On 9/22/25, Toyota dealer inspected the vehicle and reported that alignment and tires were “normal.” On 9/25/25, [XXX] inspected the vehicle and documented severe outside shoulder wear on the right-side tires. They recommended immediate tire replacement for safety. Alignment was also adjusted at that time to bring values into balance (report attached). On 9/27/25, Discount Tire replaced all four tires at 27,562 miles. Their written notes confirm edges were worn on all four tires due to alignment (right side more prominent). They declared the tires unsafe for continued use and recommended full replacement. Safety Concern: This abnormal alignment-related wear created unsafe driving conditions well before the rated tire life (55,000 miles). Uneven edge wear and shoulder failure increase the risk of blowouts, hydroplaning, or sudden loss of control. The 12 volt battery does not maintain a charge. I have had it replaced twice. I purchased new January 2025; first battery replaced on March 20, 2025; second battery replaced August 13, 2025. I am currently having the same issue with the battery management system which controls temperature and battery usage. After sitting overnight, I drove my car approximately three miles parked and attempted to use the key fob to secure the vehicle, it failed. I had no problem or indication at the start of my trip. I had to secure the door with my physical key. I returned to the vehicle 15 minutes later and the key fob would not work at all. I accessed the vehicle with the physical key. The alarm was activated upon entry. I tried to use my key fob to get the vehicle to deactivate the alarm system but it would not work. Approximately five minutes later and after several attempts, the alarm stopped and the vehicle was showing ‘Ready’ but ‘No Key Detected’ was flashing on the information screen and vehicle would not change mode from park to reverse or drive. After several attempts of holding the key fob directly over the ‘START/STOP’ button, the car showed ‘ READY’ and I was able to change modes. The vehicle started flashing ‘NO KEY DETECTED’ continuously while I quickly drove back home. I parked in my driveway and made an appointment with Toyota for service. Later in the day, I attempted my key fob again, to no avail. I was able to get the key detected, display showed ‘Ready’ and mode changed. My car does not start (READY) until after several attempts. The dealership advised the battery failed a load test overnight and was replaced (August 15th) After the second replacement my car sat overnight. I drove a few miles to a charging station. I placed the car in ‘My Room Mode’ to have access to the climate control system and radio. The car started flashing ‘low voltage/battery’, I turned off the accessory mode, returned to the dealer, and awaiting resolution. Case filed with NCDS/CDSP.
