On I95 by the Secaucus Train station there is something sending a signal to my Tesla and other Teslas to disable all of its ADAS systems. All of the functionality related to ADAS gets disabled thus making the car very unsafe to drive. Due to this disability it also disables the camera systems in the car. So if an accident were to occur there would be no way to record.

On XXX at approximately 7:23 a.m., a 2024 Tesla Model Y was traveling westbound on [XXX] in [XXX] in the middle lane at approximately 71 mph with Full Self-Driving (FSD) engaged. While traveling in traffic, the vehicle suddenly initiated a lane change toward the right lane without providing a warning or sufficient opportunity for the driver to respond. While the Tesla was executing the lane change, a vehicle traveling behind it collided with the left side of the Tesla. Following the impact, the driver manually took control and brought the Tesla to a complete stop on the right side of the highway. The system suspected of malfunctioning is Tesla’s FSD driver-assistance system, particularly its automated lane-selection and lane-change functions. The exact cause has not been determined. The vehicle and its electronic data were preserved. Currently, non-inspection was done. The unexpected maneuver placed the Tesla and surrounding motorists at risk because it occurred without warning at highway speed in active traffic and resulted in a collision. The driver had to intervene manually after the impact to control and stop the vehicle safely. At the time of this report, the suspected malfunction has not been reproduced or independently confirmed by a Tesla service center or independent repair facility. No technical determination of the cause has been received. A police officer arrived immediatly after the accident. An inspection was performed by Insurance company over a video. Before the unexpected lane change, there were no warning lamps, messages, audible alerts, or other symptoms indicating that he vehicle was about to initiate the maneuver or malfunction. Two occupants escaped through the windows. The vehicle was submerged and is a total loss. Lake County Sheriff report #XXX. The vehicle's incident data export confirms FSD was in an active state during the approach. The vehicle is preserved in storage pending inspection. An incident report has been filed with Tesla. i was in FSD mode and my car began parallel parking.As the car was in reverse it automatically accelerated and crashed into the car behind me. I did not step on the accelerator pedal, nor the brake and my hands were not on the steering wheel. I in no way caused this accident. What is most surprising is that there was no warning response. Normally if I get too close to any object a color warning shows up on the screen, if I decide to make a change while driving the screen comes up with 4 options as to why I took control but in this case with the car hitting an object there was no response at all.Teslas standard answer is always that it's my fault. I know what happened and it was not my fault.I now have a repair estimate for $1,897.98 which I must be responsible for

In May 2025, my 2021 Tesla Model Y, with approximately 50,600 miles, developed a squeaking/groaning noise from the front suspension, especially while turning. I brought the vehicle to a Tesla Service Center because of the noise. Tesla inspected and road-tested the vehicle and confirmed the squeaking/groaning from the front suspension. According to the Tesla service invoice, the noise was caused by worn front suspension components. Tesla replaced both front lower lateral links, both front lower compliance links, and both front upper control arms. Tesla also replaced the associated lateral-link bolts/hardware and performed a four-wheel alignment. There were no warning lights or dashboard messages that I recall. The suspension noise was the symptom that caused me to seek service. The problem was confirmed by Tesla during its inspection and road test. I paid for the repair myself. I am reporting this because I recently learned that NHTSA is investigating front lower lateral-link failures in 2021–2023 Tesla Model Y vehicles under Preliminary Evaluation PE26006. My vehicle is within the model years under investigation, and Tesla replaced both of my front lower lateral links after confirming my front suspension symptoms. I do not know whether my lateral links would have eventually detached if they had not been replaced. The original components were removed by Tesla during the repair and are not available for inspection.

On the morning of 8-17, 2026, the vehicle experienced a total electrical power loss and high-voltage system deadlock immediately following an official Tesla Over-The-Air (OTA) software update pushed the previous night. Prior to this remote software update, the vehicle had zero operational issues or warning signs. Right after the update completed, the vehicle became completely unresponsive and undrivable. This issue directly compromised safety by causing a sudden, unpredictable loss of vehicle power and propulsion capability, creating a severe risk of stranded vehicle hazards and secondary collisions. The vehicle was brought to an official Tesla Service Center (Whitestone/Flushing, NY) for diagnosis. The manufacturer’s service center inspected the vehicle and confirmed a high-voltage Power Conversion System (PCS) deadlock failure (Repair Estimate ID issued). The failed PCS component remains at the Tesla Service Center awaiting replacement and is fully available for inspection upon request.

This is what happened: While Full Self-Driving (FSD) was engaged, the vehicle approached an intersection and appeared unable to correctly follow the roadway. Instead of following the intended road path, the vehicle continued forward, drove over the curb, and struck a road sign. The impact knocked the sign down, and the sign subsequently struck the vehicle, including the roof glass. The roof glass shattered and small pieces of glass entered the cabin, exposing both the driver and front passenger to a safety risk. Vehicle data obtained after the incident shows that the vehicle was in ACTIVE_FSD leading up to the collision. FSD changed from ACTIVE to UNAVAILABLE approximately 1.5 seconds before the crash algorithm wake-up. Significant braking occurred only very close to the collision, and no Automatic Emergency Braking intervention was recorded during the incident window. The data also shows multiple substantial steering movements shortly before the collision. Accelerator-pedal data does not establish driver acceleration as the cause of the collision. The available samples show an accelerator position of 2.0% shortly before the collision and 0.0% in the next recorded sample. The driver applied the brake shortly before impact. There were no prior warning lamps or messages indicating that FSD was about to become unavailable before the vehicle departed the roadway. The vehicle has been inspected/repaired following the collision, and vehicle data relating to the incident has been obtained and preserved. Police responded to the collision, and the incident was also reported to insurance. A police report was filed, and the case number is 2026-07160174. I have not yet received the formal CR-4 crash report. Following the collision, I experienced significant psychological distress and trauma-related stress symptoms as a result of the incident.

The specific component listed for replacement is STEERING GEAR - LEFT HAND DRIVE, Part Number: 1188832-00-B. This is the identical engineering architecture and part family (1188832) targeted in NHTSA Safety Recall 25V092 for an overvoltage breakdown of the internal EPAS printed circuit board. While my vehicle falls outside the current 2023 model year production window defined by Tesla for 25V092, it uses the exact same component architecture and suffered the exact same failure mode. This represents a clear pattern of failure and component commonality expanding into earlier model years (2021). I am submitting my official itemized Tesla repair estimate of $3,072.50 as physical documentation. I request that the NHTSA review this component match and investigate an expansion of Recall 25V092 to protect other owners of earlier model years from this sudden loss of steering control.

On XXX, while driving southbound on XXX from [XXX] toward [XXX], my vehicle’s center display (MCU) crashed and rebooted at highway speed, leaving me with no screen for approximately 5–10 minutes. Sequence of events: 1.Earlier in the drive, while using Full Self-Driving (Supervised) v14 Lite, the vehicle exhibited phantom braking and speed oscillation (repeatedly cycling between approximately 73, 70, and 72 mph without cause). I disengaged FSD and switched to standard Autopilot. 2.During this portion of the drive, three alert codes appeared: UI_a111, APP_w207, and DI_a175 (the latter twice). 3.I stopped to Supercharge in [XXX] at 1:17 PM (33-minute session ending approximately 1:50 PM), then resumed driving on XXX [XXX]. 4.Shortly after resuming the drive, at approximately 1:53 PM, I tapped on one of the displayed alerts on the touchscreen to view details. The screen opened an in-car help/browser page, then the entire MCU crashed and went dark. 5.The display remained black for approximately 5–10 minutes while I was traveling at highway speed. During this time I had NO speedometer, NO navigation/GPS, NO touch controls, and NO visibility into vehicle alerts or state. In a Tesla Model Y, the center display is the ONLY instrument display — there is no separate gauge cluster. 6.The MCU eventually rebooted on its own and the display returned. The incident time is corroborated by a text message my wife sent me from inside the vehicle at 1:53 PM at my request, specifically to create a contemporaneous timestamped record. Safety concern: A driver-initiated tap on a safety alert — an action the interface invites — triggered a total display failure at highway speed. Loss of the sole speedometer and all vehicle status information while traveling at approximately 70+ mph is an unreasonable safety risk. SAFETY RISK: At highway speed on Aviation Parkway, the car suffered an unprompted, violent directional loss, zig-zagging uncontrollably in the lane and risking a multi-vehicle crash.
