- #11714614January 30, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING, LANE DEPARTURE: BLIND SPOT DETECTION
The system lost its ability to audibly warn me of various conditions that were a risk to everybody in the car. Examples of alerts we could not here were: blind spot collision warning, cross traffic while backing up, driver attention alert when hands are not on the steering wheel, and audible blinker sounds. After the loss of the audible alerts, I nearly collided with vehicles that were in my blind spot because the alert was no longer present.

The horn on my 2020 Hyundai Ioniq has stopped working. I checked the fuses and they are all good. Hyundai currently has several other models with recalls or at least known to fail. Yes my vehicle is available for inspection and no I have not had it looked at yet… dealer wants to keep it for 3-5 days to diagnose and further, given the nature of the issue, I don’t feel I should be responsible for any fees involved in doing so. It poses me a major safety concern as I cannot warn other cars of potential collision etc. Unfortunately, this problem does not have any type of warning message to let you know. I had to find out by trying to use my horn when someone almost hit me. This appears to me as a Hyundai manufacturing issue that also affects the Ionic line.

When the engine is turned off, and the car is not in park, it rolls. When the engine is turned off the car should automatically switch to P to prevent it from rolling. Our car was left in R with the engine off. And instead of switching to P automatically, it just rolled away and hit the neighbors garage.

January 2022. The car was brought to dealership because the transmission was slipping in 3rd gear. Service light was on. On my way the problem fixed itself. I explained that it happened only in the extreme cold here in michigan. They documented for warranty. Said it may be due to clutch overheating on slippery roads. IT WAS NOT. The cars was not used for 2 winters as we were in Florida. Dec. 2025 it happened again and expressed my concerns that if this happened in traffic it could be dangerous. Both the dealership and Hyundai washed there hands of it even though I was assured that it would be covered and addressed it it happened again. The service light came on but the problem fixed itself . I do not believe this is a mechanical issue, it sure would have failed by now. Software/electronic.

My 2018 Hyundai Ioniq Hybrid has a safety-critical transmission defect. The vehicle slips violently from 1st to 2nd gear, loses acceleration, and causes unpredictable hesitation. This creates a danger when merging onto highways or crossing intersections. Hyundai refused goodwill assistance, denied escalation, and directed me to BBB Auto Line, but BBB does not accept 2018 vehicles. This defect matches known DCT failures in Hyundai hybrid models and presents a significant safety risk, especially when transporting my two young children ( we lost mother during the childbirth .

What component or system failed or malfunctioned, and is it available for inspection upon request? Braking and acceleration control, yes How was your safety or the safety of others put at risk? The car continued to move/accelerate after depressing the brake pedal, and upon applying additional force continued to move forward, this could lead to a situation where a diver is unable to stop their car when they need to. Has the problem been reproduced or confirmed by a dealer or independent service center? No Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by a Hyundai service center on 10/09/2025 (For a similar concern and related damage) and the event occurred on 10/12/2025 Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There was no warning lights, symptoms (other than the prior occurrence of this issue). The issue was first noticed on 10/3/2025 when the vehicle accelerated while the driver was depressing the brake and damaged the undercarraige.

While attempting to park and depressing the brake the car made a sudden acceleration without input and hard braking did not slow the car. The car then impacted the curb. On another occasion while stopped at a red light the car began to move forward while the break was depressed. Pressing the break with additional force did not stop the vehicle and it continued to roll forward.

The Electronic Parking Brake (EPB) on my 2022 Hyundai Ioniq Hybrid has a recurring safety defect. The EPB warning light continues to come on and off intermittently even after EPB components were replaced. Diagnostic Trouble Codes C2416 and C2417 have been detected multiple times. This issue first occurred at around 10,000 miles, while the vehicle was still under factory warranty. At that time, the dealer only performed a cleaning procedure, which did not resolve the problem. Over the years, I returned to the dealer multiple times, but they refused to perform repairs unless the warning light was present during diagnosis. Frequently, the light turned off before inspection, leaving the defect unresolved. At approximately 87,000 miles, the dealer finally replaced EPB components at my expense (out-of-warranty repair). However, even after this replacement, the EPB warning light has continued to reappear, proving that the underlying safety defect was never fixed. This is a serious and ongoing safety concern. The EPB is a critical braking system component, and its failure could prevent the vehicle from safely parking or holding on a slope. Because this problem began during the warranty period, was never properly resolved, and persists even after paid repair and part replacement, I believe this represents a manufacturing defect that requires investigation and potential recall.

Engine failure occurred on interstate. Car was jerking and smoke coming from the hood and exhaust while engine was giving out. Hyundai dealership confirmed something blew up within the engine and blew a hole out the side of it. Check engine light came on about 2 weeks prior notified Hyundai dealership as soon as this happened and they stated I had to wait 2 weeks to have the vehicle checked out.

The contact owns a 2022 Hyundai Ioniq Hybrid. The contact stated that while operating the vehicle with the Smart Cruise Control activated, the EPB (Electronic Parking Brake) warning light activated. Additionally, while stopped, the EPB erroneously deactivated, causing the vehicle to drive forward, and the driver had to manually depress the brake pedal to stop the vehicle. The contact stated that the vehicle almost struck a pedestrian during the failure. The vehicle was taken back to the dealer who was unable to determine the cause of the failure. The contact stated that on another occasion, while driving at an undisclosed speed, the vehicle suddenly lost automotive power and was towed to the local dealer who diagnosed that the transmission was faulty and needed to be replaced. The vehicle was not yet repaired. The failure mileage was 40,000.

1. Component/System Failure: The airbag system failed to deploy during a collision. Yes, the vehicle is available for inspection upon request. 2. Safety Risk: On August 5th, while driving on the highway, my vehicle was involved in head collision. Despite the severity of the impact, the airbags did not deploy. This malfunction exposed me to significant physical danger, as I was left unprotected during the crash. 3. Problem Confirmation: No,I was not aware of the malfunction of the airbags system. 4. Inspection Status: Not yet, I contacted Hyundai America to investigate the malfunction of the airbags the insurance company did their assessment; they determined the car to be a total loss due to the extent of the damage and the car is available for inspection. 5. Warning Signs: There were no warning lamps, error messages, or other symptoms indicating a malfunction in the airbag system prior to the accident. The system appeared to be functioning normally until the moment of failure.

The instrument cluster (digital speed display) on my 2019 Hyundai Ioniq Hybrid Limited has repeatedly turned off while driving. This issue happens randomly, especially in warm weather or after the car has been sitting in the sun. Sometimes the cluster reboots (I hear the start-up chime, the display shows the headlight icon, etc.), other times it remains blank for several minutes. This affects the ability to see speed, gear, and warning indicators, posing a serious safety risk. I’ve had to pull over multiple times because I couldn’t see how fast I was driving. Turning the car off and on sometimes resets it, but the issue returns. I am not the only one experiencing this. Other Hyundai owners across multiple models and years have documented the same or similar problems, including: 2024–2025 Hyundai Santa Fe (cluster and HUD flicker, total blackout) 2025 Hyundai Tucson Hybrid Limited (cluster goes blank within 3 days of ownership) 2022 Hyundai Santa Cruz (cluster resets while driving) Hyundai Ioniq 5 (cluster flickers, reboots mid-drive, sometimes blanks) 2022–2023 Hyundai Palisade (recall #217 for illegible or blank cluster) These reports appear across NHTSA, Reddit, IoniqForum, TucsonForum, and Hyundai Santa Fe and Genesis forums. Some owners had to replace their entire cluster or HUD; others received full vehicle replacements after failed repairs. Hyundai should investigate this as a pattern defect in digital instrument clusters across multiple product lines. I am submitting this to protect future drivers and urge NHTSA to act.

While driving the transmission started driving rough and the car was stuck in ev mode. Wouldnt even go back to hybrid mode. I kept driving until all my battery range was exhausted and lost acceleration and had to pull over the side of the highway. It was late at night so there was no one around or behind me, otherwise I would have been hit. Once stopped the car would not move at all so I called a tow to get me off the highway. I was away from home and had to tow it 120 miles back to the local dealer. The engine clutch actuator failed and ruined my transmission and left me stranded in the middle of the night!

The contact owns a 2020 Hyundai Ioniq Hybrid. The contact stated that while idle, releasing the brake and pressing the accelerator, the vehicle would jolt forward before accelerating. The failure would occur intermittently and had become progressively more pronounced. There were no warning lights illuminated. During the most recent occasion, the vehicle jolted forward so aggressively that it caused the contact to have whiplash. The contact had to refrain from work due to the whiplash symptoms sustained, such as neck pain and nausea. The vehicle was taken to the dealer twice, and the dealer was not able to diagnose the cause of the failure. The dealer was able to experience the failure described when inspecting the vehicle. The vehicle was not repaired. The manufacturer was informed of the failure and confirmed the vehicle was not covered under warranty or recall. The manufacturer filed a complaint. The failure mileage was approximately 60,000.

The "Auto Hold" feature on my Hyundai Ioniq malfunctioned, displaying a dashboard warning: "AUTO HOLD conditions not met. Press brake pedal." This message randomly appears while driving or stopping, and it deactivates the Auto Hold and Electronic Parking Brake (EPB) functions. In some cases, the car rolls after stopping because the system fails to hold the vehicle. This issue compromises vehicle safety, especially on inclines or during stop-and-go traffic. It puts me, passengers, and other drivers at risk. The problem has occurred multiple times and is unpredictable. It appears to be linked to a faulty brake switch or sensor, as noted in Hyundai’s TSB 23-BR-001H. I discovered this issue is affecting many other Hyundai Ioniq owners, as seen in this discussion: [XXX] The dealer has not yet inspected the vehicle, but the symptoms match the TSB. No accident has occurred, but the safety risk is serious due to brake failure behavior. There were no visible warning lights before the issue started, just the "Auto Hold" message when the failure happens. I bought the car from a used car dealer when it had 39,000 miles. Now it has around 74,000 miles. I’m not sure if it’s still under warranty. I haven’t taken it to the dealer yet, but the symptoms match exactly what other people are reporting online. - #11672051July 8, 2025ELECTRICAL SYSTEM, HYBRID PROPULSION SYSTEM
The contact owns a 2017 Hyundai Ioniq Hybrid. The contact stated while at a complete stop at a red traffic light, the vehicle stalled. The vehicle failed to restart and was towed to an independent mechanic, where it was diagnosed that the power relay assembly (PRA) located underneath the rear seat had failed and needed to be replaced; however, the part was not available. The vehicle was not repaired, but the mechanic overrode the computer system. The mechanic related the failure to NHTSA Campaign Number: 22V459000 (Hybrid Propulsion System). The contact stated that the VIN was previously included in the recall; however, the recall was marked as complete after the vehicle passed the inspection test. The contact stated while driving 20-25 MPH, the failure recurred. The vehicle was towed to the dealer, where the contact was informed that the vehicle could not be repaired under the recall. The vehicle was then towed to a second dealer, Keyes Hyundai of Mission Hills(10240 Sepulveda Blvd, Mission Hills, CA 91345), where it was diagnosed that the power relay assembly (PRA) located underneath the rear seat and the 12-Volt battery had failed and needed to be replaced; however, the part was on backorder. The manufacturer was notified of the failure and advised the contact that a goodwill discount could be provided at the dealer's discretion. A case was filed. The contact stated that after the manufacturer contacted the dealer, the dealer doubled the cost of the repair and provided a goodwill discount. The vehicle was not repaired due to the cost. The failure mileage was approximately 153,000.
