- #11768641October 2, 2026LANE DEPARTURE: ASSIST, ELECTRICAL SYSTEM, VISIBILITY/WIPER
Components: rear vision camera display, front camera module/lane keep assist, and the center infotainment/navigation screen. The vehicle is currently with the dealership. Failures: The rear camera image intermittently distorts into a severe fisheye view, so I cannot judge distance or what is behind the truck when backing. Lane keep assist intermittently will not activate. The center screen freezes, becomes unresponsive, makes phantom inputs, overheats, goes black, and reboots on its own, cutting off navigation while driving. Safety risk: A distorted backup camera creates a risk of striking pedestrians, children, or objects when reversing a full-size pickup. Losing lane keep assist removes a driver-assistance safety function. The screen shutting off during active navigation has caused me to lose directions while driving in unfamiliar areas, which is distracting and has led me to get lost. Dealer confirmation: Reported to [XXX] at Milton Chevrolet ([XXX]) the day after delivery at 30 miles. Repair Order #XXX(XXX) confirmed the lane assist failure and found DTCs U3000 and U0131 stored in the Front Camera Module. The dealer cleared the codes and moved a toll transponder; no part was replaced. The touchscreen issue was not duplicated during a 10-minute test. The camera and screen failures continued, and I recorded multiple videos of them. Manufacturer: GM reviewed my repurchase request under Case #XXX and denied it on XXX. The vehicle has not been inspected by GM, and the defects remain unrepaired. Warning signs: Symptoms began immediately after delivery on XXX. No specific warning lamp was observed; the lane assist button light failed to illuminate when pressed. Current mileage is approximately 1,200. Rear window defrost caught fire. No noticeable damage to vehicle. On my twenty fifteen chevy silverado. Smoke filled the cab of the vehicle. There were no warning or messages to alert to this happening. I reported the dangerous incident to a dealer who told me to contact corporate. He said this is an issue but it was called a customer satisfaction program N192265660 rear window thermal event which yes it was my year but the safety issue which I was never aware of expired. He then told me to report it to NHSTA.gov. This is very dangerous and certainly should be a recall to befixed. Luckily I had started the vehicle in my driveway and was getting into it when it happened. It could have happened while I was driving. That was a known dangerous issue this should still be fixed without an expiration date. It is still dangerous.

- #11768230October 1, 2026POWER TRAIN, STRUCTURE:BODY, FORWARD COLLISION AVOIDANCE: WARNINGS
The vehicle is experiencing gear shifting failure. Specifically, when the transmission temperature reaches over 160 degrees, it shifts into limp mode and loses power, which creates a significant collision risk. Additionally, the forward collision warning system is not working at all.

- #11767934September 30, 2026VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: WARNINGS
On XXX at ~3:00 p.m., I was driving my 2025 Chevrolet Silverado 1500 High Country (VIN [XXX]) in stop-and-go highway traffic with Super Cruise engaged. The vehicle ahead was a truck towing a low, empty flatbed trailer. My truck came to a complete stop behind the trailer. While stopped, with no accelerator input from me, the vehicle accelerated forward on its own and struck the rear of the flatbed before I could intervene. My foot was not on the accelerator. It appears the forward sensors may not have detected the low, unloaded trailer as an obstacle, so when the system resumed from the stop it accelerated into it. No injuries. The other vehicle had a steel rear plate and was undamaged; damage to my truck is estimated at $3,549.68. A vehicle that accelerates on its own from a complete stop into a stationary object ahead is a serious hazard. This was low-speed property damage only, but the same failure at higher speed, or with a pedestrian, cyclist, or child ahead, could cause death. I have an infant, and the thought of this happening with my child aboard is alarming. I am concerned that low-profile or unloaded trailers may not be reliably detected by the forward sensing system — a systemic limitation, not a one-time glitch. I ask NHTSA to review whether other GM/Super Cruise vehicles have experienced unintended acceleration from a stop or failure to detect low trailers. I notified GM (Chevrolet claim #XXX) and requested the vehicle's Event Data Recorder and Super Cruise/OnStar telematics be preserved. I am filing this so there is an independent safety record. Two steering issues on the same 2026 Silverado 2500 HD LTZ Trail Boss Z71, 6.6L gas, truck has about 11,000 miles as of 09/29/2026. 1. Mechanical clunk heard and felt through the steering wheel over bumps and when the wheel is at full lock. Assist is still present; the wheel does not spin free. This matches wear/lash in the upper intermediate steering shaft (the collapsible shaft between the column and the steering gear), not the tilt mechanism. Fluid is not low and there is no visible leak. 2. Separate hissing / hydraulic-flow noise heard inside the cab from the column area when tension is applied to the wheel. There is no hydraulic fluid in the column; the noise is conducted up the shaft from the hydraulic steering gear / Digital Variable Assist (NV8) system. I am reporting both so they stay on one VIN record. 3. Both symptoms were present when I purchased the truck new on 11/25/2025 and Chevrolet dealer checked it before delivery, they said it was normal. On my first service, the truck was taken to a GMC dealer where they also stated this was normal. Now both symptoms have gotten worse and the hiss has gotten louder.

- #11767650September 29, 2026POWER TRAIN, SERVICE BRAKES
At 124,000, the transmission started slipping and not grabbing gears. Coleman Taylor Transmissions ended up having to do a full rebuild which costs about $4000. At around 20,000 miles, the vehicle started to shutter going 45 mph. The vehicle still does shake violently at 62-75 mph causing a safety concern. The brakes wont go if Ive been standing still for a long time like in a drive through or backing down a boat ramp. You have to stand on the brakes with both feet to get the vehicle to stop and thats only going 5 mph. We still own this vehicle and still have issues with it.

- #11767704September 29, 2026FUEL SYSTEM, DIESEL, ENGINE
The contact owns a 2026 Chevrolet Silverado 2500. While driving 65 MPH, the vehicle lost motive power and the steering wheel seized, and the brakes failed to stop the vehicle. White smoke with a burning odor was coming from the engine compartment. The vehicle was on an incline, which helped the vehicle to decelerate. The contact pulled off the road and came to a stop with smoke billowing under the hood. A Sheriff driving by called the fire department. The fire fighters arrived and determined that the smoke had become extinguished. The vehicle was towed to the dealer, who determined that the fuel line had ruptured and there was diesel fuel collecting inside the engine compartment. The contact was informed that the fuel system had a catastrophic failure and the engine needed to be disassembled for further diagnostic tests. The manufacturer was notified of the failure by phone and a case was filed. The failure mileage was approximately 9,100.

This vehicle's engine failed on a high-volume roadway in [XXX] which resulted in a significant safety issue. The vehicle was taken to a certified Chevrolet dealership in Scottsdale (Van Chevrolet) whereby the engine in this 5.3L V8 was replaced due to catastrophic engine failure with the lifters being the suspected cause of the failure. Aside from the $10,400 cost of the remanufactured engine replacement, I have had considerable discussions (live and written) with GM Arizona and Corporate representatives outlining the issue(s) and filing formal complaints. These have gone relatively unheeded... Unfortunately, this is the second 5.3L V8 engine failure in one of our GM vehicles as I will be filing yet another formal complaint under separate cover for a 2021 Tahoe 5.3L V8 which had an engine failure requiring new lifters. I am not done yet as I have an appointment tomorrow for what appears to be, a third Chevrolet vehicle with a 5.3L V8 engine lifter failure. Depending on the outcome of this service appointment, I may be filing a third safety complaint regarding the GM 5.3L V8 engine. In my opinion, the NHTSA should absolutely open an investigation into this engine in the same manner that you have a formal investigation/recall in effect for the 6.2L GM engine as based on two personal experiences, the safety of the driver and passengers in these vehicles is compromised due to what appears to be significant engineering and/or manufacturing issues on the part of General Motors. Thank you for your consideration and I am free to discuss in further detail at [XXX] or [XXX]. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I have received a voluntary recall notice on my 2023 Chevrolet Silverado. The recall states that the vehicle may have a condition which can cause the Fuel Pump power control module to fail. If this happens fuel flow to the engine could be interrupted causing the motor to shut off. Would this be considered a safety concern if the engine shuts down on th highway?

I opened my passenger front door to load and the tailgate opens and closes by itself but doesn’t and remains open. I physically shut the tailgate and it opened again by itself. I started my truck to see if it would stop. It didn’t stop until I put my truck in reverse to back out of the garage. After I backed out and the tailgate stopped open. I closed it and the tailgate will not open by pressing the button on the tailgate. It will open if I use my key fob.

Driving on the interstate and the truck had a horrible vibration. It smoothed out and it did it again. I got off at the next exit and the engine was making a horrible knocking noise. I had it towed to my Chevy dealer for inspection. They drained the oil and found excessive metal in the oil. They decided that the engine needs to be replaced. The truck only has 4524 miles on it and it was purchased in June of 2026.
