FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, SERVICE BRAKES
SEE NHTSA Safety Recall Report 25V-12- It appears it should apply to both 2025 FRONT WHEEL DRIVE Chevy Equinox EVs with ADAPTIVE CRUISE CONTROL (and without Super Cruise) in addition to 2025 ALL WHEEL DRIVE Equinox EVs with ACC (also without Super Cruise). This is very dangerous. Here's what happened: My 8 month old 2025 Chevrolet Equinox EV FRONT WHEEL DRIVE w/ACC(no Super Cruise) has had 2 instances where a very large "thunk" sound and vibration emanating from the rear of the car while stopped at an intersection, was immediately followed with sudden brake failure and sudden forward acceleration. The ACC was on in both instances. The first time this happened was alarming but since the was nothing in front of me there was no damage and the problem seemed to resolve itself thereafter. The second time was while waiting at a red light about 10 feet behind another vehicle. My car shot forward and into the other car, which was then pushed into the car ahead of it. Both my car and the car ahead of me had very expensive damages, but fortunately, no injuries. What I experienced sounds exactly like the recall problem described in NHTSA Safety Recall Report 25V-12 which has thus far only been applied to the AWD version of the 2025 Equinox EV, not the FWD version that I have. I believe this needs rapid consideration due to the danger of of sudden acceleration resulting in serious property damage, injury, or death. It appears to me that other instances of this problem have been described in various online forums and YouTubes.

CASE NUMBER 11715650 , check engine light went on , and the vehicle reduced speed, I took car into dealership, they said the engine had an ice problem and a repair would be needed, they said that this was a flaw in design, and it should be part of a recall, and that this is a common problem with this vehicle. the cost of a repair would be around $1000.00 . T fill this should be covered by the manufacture because of a design flaw.

Blind spot indicator malfunction. Seat sensors alert for no reason. Cruise control stops working. Vehicle automatically brakes for no reason. Information screen malfunction and goes black at random.

ENGINE AND ENGINE COOLING
The contact owns a 2021 Chevrolet Equinox. The contact stated that while driving at 65 MPH, the check engine warning light illuminated, and the vehicle lost motive power. The contact pulled over to the side of the road and was able to restart the vehicle. The contact resumed driving, but the vehicle failed to exceed 55 MPH on the Expressway. The vehicle was taken to the dealer, where it was diagnosed with a factory defect. The dealer determined that the turbo wastegate solenoid, the charge air cooler, and the TCV wrap needed to be replaced with an aftermarket part. The parts were ordered; however, the vehicle was not yet repaired. The manufacturer was not contacted. The failure mileage was approximately 52,000.

SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: WARNINGS
Hello, since I purchased the vehicle on October 1, 2025, there have been issues regarding collison system and adaptive cruise control. When driving down the road it will try to come to a complete stop with no reasoning. It happens more often when it is rainy and night time. I believe the collision system is conufsed with oncoming traffic and it thinks they are on my side of the road but they are not. I also get alerts with side mirrors and shaking of my seat when no one is beside me. It triggers to say someone or a vehicle is approaching from the side, but there is no one or nothing there. The issue that is most troubling is I almost hit someone in the rear end on 02-03-2026. I went to slow down for a traffic light, and it started slowing down, but then got a surge and would not allow me to stop any more. It kept going. I had to remove my foot from the brake and hit it again harder. It has happened a few times, but not in a situation like this. Thankfully I was following a safe distance or I may have hit the vehicle stopped in front of me. Similar situation happened a few times, one when I was pulling into an Applebees to eat, the brake did not want to stop the vehicle. It was like a surge of power again. This time was not as bad as no one or a vehicle was not in front of me. I have also noticed it a few times driving and when I go to stop the surge, but it seems to be escalating. I do not have the auto start stop on, I switch it off as soon as I start the vehicle. I do not have the auto hold on when it happens either. I have an appointment on the 16th of February to hopefully get it looked at, but will see.

FORWARD COLLISION AVOIDANCE, LANE DEPARTURE: BLIND SPOT DETECTION
The contact owns a 2026 Chevrolet Equinox. The contact stated that in the first month after the vehicle was purchased, while driving at an undisclosed speed, the Forward Collision Avoidance system failed to function as intended. The contact stated that the alert system warned about vehicles or objects too close, while there were no objects or vehicles around. The blind spot detector failed to function as intended. The warning light on the driver’s side mirror was illuminated; however, there was no vehicle on the side of the vehicle. The contact stated that on a separate occasion, while driving at an undisclosed speed, the vehicle experienced phantom braking. Most recently, while driving at 30 MPH, the brake pedal was depressed; however, the vehicle accelerated independently. The contact used force while depressing the brake pedal to stop the vehicle. The contact stated that the vehicle in front was almost rear-ended. The contact stated that the failure was more prevalent while raining. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 15.

ELECTRICAL SYSTEM, UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: WARNINGS
Just after Thanksgiving, my vehicle displayed a “Service Emergency Calling” message. I brought it to the dealership and they documented the concern, referencing GM bulletin PIP6077B, but no repairs were performed. On December 30th, while parking, I shifted into reverse and the vehicle would not reverse and instead moved forward. A “Service Transmission” warning appeared and the vehicle was towed. The dealer cleared the codes and released the vehicle without performing any diagnostics. About two weeks later, when starting the vehicle after work, several warnings appeared, including “Service Safety Restraint System,” “Forward Collision System Unavailable,” and a rear-seat alert despite no passengers. While driving, lane keep assist, lane departure warning, and blind zone steering assist were not functioning. The next morning, the vehicle suffered a complete electrical failure. It would not start, the screens stayed off, and the doors would not lock or unlock. The vehicle was towed again. The dealer replaced the battery, claimed it had simply died, and attempted to return the vehicle to me. I refused because none of the prior warnings were diagnosed, and I was told the codes “erase when the battery is changed.” After I demanded full electrical and transmission diagnostics, including a parasitic draw test, the dealership informed me the vehicle was unsafe to drive. They stated the OnStar/telematics module was draining the battery and the replacement part was backordered with no estimated arrival date. I was told multiple customers are waiting for the same part, which means this is NOT an isolated event and needs way more attention than it's being given. This defect results in loss of essential safety systems and complete vehicle shutdown, creating a significant safety risk.

VISIBILITY/WIPER
The angle of the windshield reflects the dashboard so much that you can't see out the windshield when heading anywhere towards the sun. It also reflects the dash at night inside the car. It's a significant safety hazard and a driving hazard the car creates for itself. This is a design defect, according to all service departments at GM I have visited to address the issue. Update: The dash cover remedy GM provided did not help; in fact, it's more annoying now than before and distracting.

Codes p2227 and code p0299 are coming up machine says this is a known issue with 2021 to 2023 Chevy equinox and there is a lot of water in intake and engine is in limp mode 1.5L engine, are known to experience issues with moisture accumulation in the intake system, specifically within the charge air cooler (CAC). In cold temperatures, this moisture can freeze, causing reduced engine power, stalling, and illuminated check engine lights (DTCs P0299, P2227, P0300). Key Details Regarding the Problem: Cause: Moisture from the PCV system and ambient humidity builds up in the CAC, which can freeze during cold weather. Affected Models: 2018-2022 Chevy Equinox and other GM vehicles with small-displacement turbocharged engines. Symptoms: Reduced power, stalling, rough idle, and engine light in cold, humid, or snowy conditions. I would like for the vin number check to be expanded if my car is experiencing this issue under a vin that is not coming up for recalls or a factory warranty then other People are having the same issues.

FUEL/PROPULSION SYSTEM
My vehicle keeps jerking/stalling/choking but continues to drive. I went to AutZone (01/28/2026) and they ran a check on it. The report states it has to with a Fuel Tank Pressure Sensor and that an Evaporative Emission (EVAP) System Large Leak was detected. I need to know in what direction do I need to go to get assistance in this matter. Do I take to the dealer manufacturer to fix the problem or do I contact a lawyer? I have another complaint that was filed with the NHTSA #11711799 on January 18/19, 2026 and no resolution was made. That complaint is for a problem with the Mass Airflow Sensor pertaining to the fuel system. Your assistance in this matter is greatly appreciated.

The contact owns a 2022 Chevrolet Equinox. While driving at approximately 70 MPH, the check engine warning light illuminated with the message “Reduce Engine Power – Drive with Care” displayed, and the vehicle was unable to accelerate as needed. The vehicle was driven to a local dealer, where the failure was diagnosed as an active shutter system failure, not allowing air into the turbocharger. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 130,736.

On 1/22/25 took my car in for warranty repairs of the air induction system. The dealership claimed they fixed it, On 2/17/25 went back letting the dealership know the issue was not fixed but the told me that they didn’t see anything wrong. yet a year later to the date I am having the same issue visit the dealership for them to let the car thaw and cleared code for my check engine light to pop back on and reduce engine speed once again

VISIBILITY/WIPER
Windshield glare is horrible. It can cause distortion of objects. It can cause dizziness. Especially on bright sunlight and when many lights from oncoming traffic reflect off the windshield. I have had to pull over a few times. The dashboard shininess is a real problem. The dealerships don’t want to order the free dash mat unless they “diagnose” it even though it is a known problem and they ignored my concerns when I went in to the dealership even though a free dash mat is offered by GM The fix for this in reality is a new dashboard that will end this problem but that is too expensive for GM. They all say to wear polarized glasses to help. Wrong. That’s like putting a bandaid over a gunshot wound. If they aren’t willing to fix this error I want them to buy me out cuz it’s not a small problem that could result in injury or death if not addressed. The 2026 models have a new dashboard that corrects the problem. This problem has caused me not feel safe and avoid driving sometimes and this is not acceptable to me.

IN THE EQUINOX LT, VIN [XXX] , THE TELEMETRY MODULE HAS AN ISSUE PER THE DEALERSHIP. THE VEHICLE LOSES POWER DUE TO THE TELEMETRY MODULE ISSUE. THE PART IS ON ORDER AND AS OF JANUARY 16, 2026. IT WILL TAKE 4 TO 6 WEEKS TO ARRIVE. IN THE MEAN TIME I HAVE HAD TO CALL ROADSIDE ASSISTANCE TWICE IN TWO WEEKS, AND THE VEHICLE HAS BEEN JUMPED 3 TIMES IN THE PAST TWO WEEKS. HOWEVER, THE VEHICLE HAS BEEN TO THE DEALERSHIP 4 TIMES. ONCE PRIOR TO THE DIAGNOSIS, AND THREE TIMES SINCE THE DIAGNOSIS. THE VEHICLE HAS APPROXIMATELY 16, 600 MILES, AND IS DRIVEN TO AND FROM WORK. THE TELEMETRY MODULE IS THE CAUSE OF THE OVER ALL ISSUE, PER THE DEALERSHIP. THE BATTERY CONTINUES TO GO DEAD ON VEHICLE [XXX] , 2025 CHEVROLET EQUINOX. While driving my Equinoix EV I lost propulsion and the dashboard started flashing. I could coast to curb lane. When fully stopped I turned the car off and on again and propulsion resumed. I have taken it in to the dealership and they said they see no error message and I should continue driving to see if it reoccurs. They suggested I keep cloth to wipe off windshield where sensors are in case it was obscured. This happened in clear weather.

Light from recessed LED headlights becomes obscured by packed snow when traveling in a snowstorm. LED lamp does not emit enough heat to keep clear. Loss of light is gradual and can reach an unsafe level almost without notice.
