ENGINE
My truck showed “Hybrid Engine Malfunction contact dealer” upon start up, then “Reduced Engine Performance Check Engine” warnings. I was not warned that I should not drive at all so I took my son to school. As I was driving home up a hill around 35 mph the temperature gauge began to climb fast towards the upper limit. At the upper red limit the car warned “Engine Overheating Stop Immediately” and I pulled over. The engine began smoking as I pulled over. I have an aftermarket Air Intake for the twin turbo and an aftermarket cat-back exhaust system but neither of these parts could have caused dramatic overheating of the main engine and failure of the hybrid system. Events happened over the span of about 15 min driving from start up to breakdown.

POWER TRAIN
Vehicle Information: 2025 Toyota Tundra CrewMax 1794 Edition 4x4 (i-FORCE MAX Hybrid) VIN: [XXX] Component: Power Train / Automatic Transmission / Hybrid Propulsion System Description of Defect: I have discovered an active fluid leak originating from the transmission/bell housing area on my vehicle. There are currently no dashboard warning lights illuminated, and the transmission is not yet slipping or shifting roughly. However, the vehicle is actively dripping fluid onto the ground and undercarriage components from the powertrain assembly. Safety Risk: A transmission fluid leak on a high-pressure 10-speed automatic transmission presents an immediate safety hazard. If the transmission fluid level drops too low while driving at highway speeds, it can cause sudden power loss, transmission lock-up, or catastrophic mechanical failure, increasing the risk of a crash. Furthermore, transmission fluid leaking onto hot exhaust or hybrid electrical components poses a severe vehicle fire risk. Subject: Safety Complaint – 2025 Toyota Tundra – Sudden Acceleration Delay – T-SB-0032-26 – Two Near-Accidents. I am the owner of a 2025 Toyota Tundra and am filing this complaint to report a serious safety concern involving delayed acceleration from a stop or near-stop condition. My vehicle has experienced this problem on two separate occasions. When I attempted to accelerate, the truck did not respond as expected, creating a dangerous situation and nearly causing a car accident on both occasions. The delay in acceleration left me unable to move forward as quickly as required by the traffic conditions. I am concerned that this issue could result in a serious collision, injury, or loss of life. I understand that Toyota issued Technical Service Bulletin T-SB-0032-26, titled “Hesitation from a Rolling Stop,” addressing hesitation from a rolling stop or quick stop on certain Toyota Tundra vehicles. My experience appears consistent with the condition described in this bulletin. I took my truck to Toyota Service in [XXX], on two separate occasions to have this issue inspected. On both occasions, the service department declined to identify or address a problem, stating that there was nothing wrong with the vehicle. As a result, I remain concerned that the safety issue has not been properly investigated or resolved. I believe this matter warrants NHTSA's attention because an unexpected delay in acceleration can create a dangerous situation when entering traffic, crossing intersections, or responding to changing road conditions. I respectfully request that NHTSA Review this complaint and investigate the delayed-acceleration issue affecting my 2025 Toyota Tundra. Examine whether the condition described in Toyota Technical Service Bulletin T-SB-0032-26 represents a safety-related defect. Investigate whether affected vehicles require a mandatory safety recall or another appropriate corrective action. I appreciate the prompt solution to this issue ASAP. ELECTRICAL SYSTEM
Here's your description, about 1,750 characters, so it fits the 2,000 limit: --- My 2025 Toyota Tundra has two intermittent defects that have not been repaired despite multiple visits to Berglund Toyota in [XXX] 1. Driver's door will not unlock. Intermittently, when I press unlock on the key fob, all doors unlock except the driver's door. I have video showing me standing outside the truck with the fob in hand while the driver's door stays locked and the other doors open normally. The dealer disassembled the door and checked the wiring and connections but could not duplicate the problem and found no codes. A Toyota field technician also inspected the truck. No parts have been replaced because the failure could not be reproduced in the shop. When this happens I cannot get into the driver's seat of my vehicle, which is a safety concern, especially in an emergency or bad weather. 2. Tailgate fails in two ways. Sometimes when raised it will not latch closed. Other times it becomes stuck in the down position and cannot be raised, even manually. On one occasion this happened in a store parking lot, and I had to secure my load with a cargo net across the open bed to drive home. The truck has been to the dealer for the tailgate more than once. The dealer has ordered latch parts, but it has not been confirmed that this will correct the stuck-down condition. Both problems are intermittent and can occur at any time. My wife and I are in our [XXX] and rely on this vehicle. Toyota has been notified in writing. Other 2025 Tundra owners have reported the driver's door locking when the tailgate is operated, which may indicate the two problems are related. Root cause: UNKNOWN. No warning lights or codes appeared before the door failures. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

ELECTRICAL SYSTEM, UNKNOWN OR OTHER
Two issues happening at the same time. “Key not detected” and “TPMS System Malfunction”. Key not detected: Happens at various times, either at startup or while driving, an error message that says “Key not detected “ comes on the dash and a loud beeping. My concern is that I may become stranded somewhere if the system cannot detect the key when trying to start the truck. The issue has been confirmed by both the dealer and Toyota and has not been fixed to date. TPMS System Malfunction: At various times while driving, a warning message is displayed on the dashboard that states there is a malfunction with the tire pressure monitoring system and that it should be checked by the dealership. My concern is that the TPMS system is unable to monitor the tire pressure and that could cause a dangerous situation. The issue has been confirmed by both the dealer and Toyota and has not been fixed to date. The truck has been back to the dealership 5 times for these issues and has been there for four days as of this writing.

UNKNOWN OR OTHER
Possible issue with the safety of others: My tailgate for my 2025 Tundra crewmax came opened (unknown to me) and contents of my Truck bed dumped onto the road. The incident as it happened. I was backed into a parking spot, truck was off, when I got in my remote was in my pocket and the truck would not start. So I pulled the remote out of my pocket and put it in the console. The truck would then start. I wasnt observant to the dashboard warnings if they were there. While driving home I heard all my bed contents fall out. Fortunatly I noticed it before it affected anyone. The best I can figure is that other objects in my pocket may have pushed the tailgate button. Since I was backed in and the tailgate lowering is so quiet, I didnt hear it or see it. I feel unsafe now to carry anything in my truck bed without backup protection. I would rather have a manual tailgate and totally disable the auto function but as I understand it, only the auto close mode can be disabled. I have not presented this to the dealership yet nor have I tried to duplicate the issue yet.

STRUCTURE:BODY
On 06/02/2026, at approximately 20,000 miles, the driver's front interior door handle on my 2025 Toyota Tundra TRD Pro failed. An internal component connecting the handle to the latch mechanism broke, and the door could not be opened from inside. I was unable to exit the vehicle through the driver's door using the interior handle. I had to lower the window and open the door using the exterior handle to exit the vehicle. The vehicle was repaired under warranty at Loyalty Toyota (Chester, VA), repair order 7245169/1. The dealer initially suggested the failure might be classified as improper use. They later informed me that Toyota considers it a known manufacturing defect, and the handle was replaced. On 09/06, at approximately 25,000 miles, the driver-side rear interior door handle failed in the same manner. The vehicle is currently at the same dealer awaiting parts, repair order 7255879/1. I asked the dealer to replace the remaining interior door handles because the failure is a known defect. The dealer said Toyota will replace each handle only after it fails. This is a safety concern. The interior door handles are the primary way for occupants to exit the vehicle. If a handle fails during a crash, fire, submersion, or other emergency, an occupant may be unable to open the door and could be trapped. Passengers, including children or anyone unfamiliar with the vehicle, are unlikely to know to use the window and exterior handle as a workaround. Because the failures occur without warning, there is no way to know which remaining handle will fail next. Other owners of 2025 Tundra's report the same failure on owner forums, and similar interior handle failures have been reported on the 2025 Tacoma. I am asking NHTSA to review whether this is a defect pattern affecting 2025 Tundra and Tacoma interior door handles that warrants investigation or a recall.

STEERING, LANE DEPARTURE: ASSIST
When using the cruise control and lane keeping system the truck will jerk into oncoming traffic and try to swerve off to turning lanes. I took it to Toyota and they couldn't find any problem. no warning lights or any indication of a problem shows on the dash. At this point the system is unusable. I have seen many post of similar incidents on the internet.

EXTERIOR LIGHTING
The contact owns a 2025 Toyota Tundra. The contact stated that while driving 6 MPH and parking the vehicle, the brake pedal was depressed, and the brake lights became inoperable. Additionally, the rear cross traffic warning light was failing. An unknown warning light was illuminated with an audible alert, and the temperature warning light was displayed for a short while before turning off. There was an abnormal plastic burning odor during the failure. The contact received notification of NHTSA Campaign Number: 25V322000 (Exterior Lighting); and associated the failure with the recall. However, the part to do the recall repair was not yet available. The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was contacted by phone and made aware of the failure. The failure mileage was approximately 238.

SUSPENSION
The alignment has consistently been an issue since I purchased the truck in March 2025. I've had the wheels aligned 3 times since I purchased it. I took it in for the 25000 mile service and it's out of alignment again. I've never had a vehicle go out of alignment let alone 4 times in 18 months.

ENGINE, POWER TRAIN
On July 20, the vehicle experienced a catastrophic mechanical failure resulting in a total engine seizure while operating. The failure caused an immediate, sudden loss of motive power, forcing the vehicle to become completely immobilized on the Interstate highway. This unexpected loss of propulsion created an immediate and severe highway crash hazard for myself and surrounding traffic. The vehicle was towed to an authorized Toyota dealership on July 20 under the 5-year/60,000-mile factory powertrain warranty. The vehicle has remained completely out of service since that date. The dealership has tentatively scheduled the engine repair and replacement to begin on August 24 or 25, meaning the vehicle will have been non-operational and out of service for over 35 consecutive days before repairs even commence.

VISIBILITY:DEFROSTER/DEFOGGER/HVAC SYSTEM
The contact owns a 2025 Toyota Tundra. The contact stated that while attempting to defrost the windshield on several occasions during the winter months, the windshield defroster failed to operate properly. The vehicle was taken to a dealer on two separate occasions and remained there for weeks. However, the dealers were unable to determine the cause of the failure and were awaiting approval from the Corporate Office to proceed with the diagnostic test. The vehicle was not yet repaired. The manufacturer was notified of the failure. The failure mileage was approximately 17,000.

ELECTRICAL SYSTEM
I am reporting a severe thermal and electrical safety hazard regarding the OEM wireless charging pad in a 2025 Toyota Tundra. During standard vehicle operation, the integrated charging pad overheats to extreme temperatures. On a recent drive, the heat generated by the vehicle's charging pad was so intense that it physically melted, blistered, and deformed a MagSafe-compatible protective case on a Samsung Galaxy S25 Ultra. This safety issue has been brought to an authorized dealership, Toyota of Hollywood (Repair Order #[XXX]), for a second consecutive time. The dealership officially documented the property damage in writing on the invoice, stating: 'Phone got melted from how hot it gets.' However, they refused to perform repairs or disable the system, explicitly noting: 'Vehicle was compared to another and both doing the same.' By admitting that multiple new vehicles exhibit this exact overheating behavior, the manufacturer is confirming a systemic and unaddressed design or manufacturing defect. An in-dash electrical component capable of melting plastic poses an immediate cabin fire hazard, risks triggering thermal runaway in the mobile device's lithium-ion battery, and directly threatens occupant safety. Toyota Corporate has opened Case #[XXX], but no remedy or safety recall has been provided." INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Heads up display(HUD) and windshield incompatibility issues from factory making HUD display blurry. Very distracting while driving because you’re too busy trying to see clearly and it takes away time from playing attention to the road. Lots of reports online about blurry HUD with 3rd generation toyota tundra from the beginning model year all the way up to most recent model year- it seems like they’re not doing anything about it. People online have reported getting dizzy while driving when trying to focus on the HUD- possible cause of accidents? Please look into this!

SERVICE BRAKES, ENGINE, VEHICLE SPEED CONTROL
Toyota Tundra 2025 Limited stalled at highway speeds. The vehicle flashed "Parking Brake Support" message, but dealer says it has to do with an engine failure. Vehicle came to a grinding stop and would only advance in short spurts before dying on the side of the road. I was in the left lane of a major highway during rush hour. Luckily, traffic was behind me, and I was able to limp to the shoulder before traffic arrived. I noticed nothing prior. A loose gas cap may have prompted the system to send failure message. I haven't heard yet why it stalled. But my safety was definitely put at risk, and I was lucky to not be rearended

ENGINE, FUEL/PROPULSION SYSTEM
Have owned the 2025 Toyota Tundra for about 18 months. Multiple instances of delayed acceleration when stopped or almost stopped, sometimes as much as 3 or 4 seconds. Last time was in early June, 2026. This is very dangerous because when I pull out into an intersection it will start to go but then lose all power for a few seconds with traffic coming behind me. This is something Toyota needs to look into.
