The automated wipers will stop working when it is raining making it very difficult to see the road and traffic, and they will start working when the windshield is dry. The speed that the wipers work at is also inconsistent - speeding up and slowing down even though the rate of precipitation has not changed. This happens randomly, and after learning that other Tesla Y owners have also had this happen to them, I suspect that this is a common issue in the Tesla Y’s wiper system / software. This has been ongoing since we bought our car in Nov 2021.

- #11591807May 31, 2024FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
This Tesla engages in "phantom braking" on a daily basis. I will be driving along at 70 mph on Adaptive Cruise Control, no other cars or otherwise around, and the brakes will suddenly slam on reducing speed to 40-50mph. It is clearly a sensor type issue. Seems to happen when a turn lane is sensed? a side road? who knows?? This has been a know issue for a very long time. it's dangerous as it will lead to rear end collision if anyone is behind. this happens without warning, almost daily, and I am not the only one experiencing this. Tesla people have no solutions. I do not routinely report things, but I feel very strongly that this is a major safety issue, so much that I am considering trading in the car for something else.

The contact owns a 2024 Tesla Model Y. The contact stated that the vehicle was experiencing abnormal battery drainage and was not operating based on the designed mileage. Additionally, the contact stated that the vehicle was overheating, and the failure persisted after the vehicle had been parked and turned off. The contact also stated that the front passenger's side window would roll down unintentionally while driving. There was no warning light illuminated. The vehicle was taken to a Tesla Service Center several times; however, the failure could not be replicated. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 6.

•Water leak from under the dash (potential electrical hazard) (AVAILABLE FOR INSPECTION) •Window regulator malfunctions (AVAILABLE FOR INSPECTION) •Front suspension noise (potential handling issue) (AVAILABLE FOR INSPECTION) •Persistent rattle or squeak (AVAILABLE FOR INSPECTION) •Autopilot failures and power steering issues (including excessive vibration and unexpected braking) (AVAILABLE FOR INSPECTION) •Constant noise and vibration (AVAILABLE FOR INSPECTION) *Autopilot failures and power steering issues (including excessive vibration and unexpected braking): This can lead to a loss of control of the vehicle, increasing the risk of an accident. *Water leak from under the dash (potential electrical hazard): If water damages electrical components, it could cause the car to stall or malfunction while driving. *Front suspension noise (potential handling issue): A failing suspension system could compromise the vehicle's handling, making it more difficult to control during maneuvers or emergencies. **has been submitted for repairs at a Tesla maintenance center on multiple occasions." This suggests Tesla has acknowledged the issues and attempted to fix them. ***For Concerning Behavior: Autopilot Failures: The Autopilot system unexpectedly disengaged or made erratic maneuvers while engaged. False Object Detection: The Autopilot camera system falsely detected objects on the road, leading to unnecessary braking or swerving ***For Warning Lights: It is particularly concerning that these issues were not accompanied by any warning lights on the dashboard.

I was trapped in my car when the 12V starter battery died without warning and everything in the car shut down. I was very fortunate that I had my cell phone with me to call my friend who had just left. However, he was unable to help me open the car from the outside. I spent several minutes going through several questions in an attempt to make an emergency request for roadside assistance on the Tesla app in hopes they could talk me through getting out. I finally received a text stating my request was 'being reviewed'! I have claustrophobia so by now I'm not breathing comfortably. I responded that I was trapped in my car and needed immediate help. They responded telling me where an emergency release was. A release which every Tesla owner that I know has told me they had NO knowledge of. When I was towed to Tesla, the service adviser told me ' claustrophobic' people probably shouldn't buy a Tesla! Well... he was partially correct...only I would go further to say 'nobody' should be a Tesla... it is a death trap. What if I had put my 3 year old grandchild in her car seat and shut the door to get in the driver side? She would not be able to pull an emergency lever! I was sitting on a fully charged Lithium-Ion battery yet I was unable to open a door...so how about a safety bypass to this battery ?? Seems to me the default with a dead battery should be that the doors unlock...not lock! This is certainly NOT a 'smart' car....it is a potential death trap for which there are countless other reports in the news, yet Tesla and your organization has yet to take action.

While driving on a freeway during very heavy rain, the windshield wipers were working in auto mode. All of a sudden the wipers stopped working. Even though the visibility was almost non existent due to windshield wipers not working, I managed safely to pull over to the curb of the freeway. Upon further investigation, I realized that the auto mode of windshield wipers was not working, but manual mode was working. With the manual mode I was able to get home safely. At home in my driveway I tried the auto mode again and it worked for a few seconds and stopped. Two days later my wife contacted Tesla about the issue and was informed by service representative that it`s a known problem to Tesla for the vehicle, but as of now they do not have a fix for the problem and there is no known expected time for the fix.

- #11591439May 30, 2024WHEELS
My Tesla model Y performance 2022 was purchased on September 26, 2022. I found that the left rear wheel was leaking in July 2023. After other tire repair shops failed to find the cause of the leak, I went to the Alhambra Tesla service center for further inspection. They found that my wheels rims had cracks that caused the tire to leak very slowly. These cracks are invisible to the naked eye (I still have this wheel rim at home). During my negotiation with this Tesla service center, they told me that it was my use that caused the wheels rims to crack, and they would not be responsible for the wheels rims. I disagreed with their decision and bought a new WHEEL RIM at my own expense. I use this car for work. I have never driven this car for strenuous exercise or to steep mountains. This is my vehicle usage scenario. In February 2024, the same situation occurred on my right rear wheel. My wheel began to leak slowly and I couldn't find the reason. After multiple inspections, Tesla service center found that there was a very small crack in the right rear wheel rim. The conclusion of TESLA SERVICE CENTER is still that my use caused the crack of the wheel rims. TESLA MODEL Y PERFORMANCE is a high-performance sports car. The wheels rims are specially made 22 inches and are produced by the original factory. There have been many cases of wheels rims cracking in 18 months. I hope to attract your attention and conduct product quality inspections, which is very important for safety.

- #11591262May 29, 2024VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Was traveling along the [XXX] about 5 miles north of the [XXX]. Vehicle was in traffic awareness cruise control when it suddenly applied the brakes. Vehicle did not sound any warnings. Vehicle did not display any warnings. Vehicle made no attempt to warn driver of a braking event. Vehicle traffic awareness cruise control was set to 79 or 78 mph in a 70 mile per hour zone. I was able to push the accelerator pedal to regain speed but had already dropped to about 65 mph. Vehicle did not pass under a shaded section ( I. E. no bridge or trees or big rigs) There was change in color of the road top. My foot was resting just in front of the accelerator pedal. Fortunately for us, there was no vehicles directly behind us. This incident is the 3 of this kind in this vehicle. This has me spooked to use any of the convenience software. I already do not like using the autopilot feature because of it erratic behavior. It feels very unsafe and unpredictable. We are unable to predict when this car will act on its own and feel it will cause us harm one day with actions like this. We are glad that no one was following us closely because they might have hit us. We are also upset that the vehicle never stated why it braked suddenly. Windshield wipers put in manual maximum sensitivity mode. During the trip they stopped working multiple times. They just froze. When a manual one time wipe button was pressed wipers menu showed “II” manual sensitivity and started working. Selected back manual III sensitivity. Wipers worked for a while and stopped again. SW version: 2024.14.6. Autopilot was not engaged.

While driving with the adaptive cruise control on, the vehicle suddenly braked. There was a large truck approaching from the other direction, but it was in its lane. There was no present danger. The hard breaking could have caused the following car to rear end us. It happened more than once, but this was the most dramatic braking.

During two hour trip I experienced at least five incidents of so called Fantom Braking, when car engaged in Autopilot or adaptive cruise control brakes suddenly for no apparent reason. At least two time during this trip, the braking was really hard. This is very dangerous conditions and could result in serious accident if someone closely follow the vehicle. I had to keep my foot on the gas pedal during this trip to act if necessary. I have experienced these conditions before and reported it to Tesla. They basically ignored this complaint.

FSD Stops in front of oncoming traffic. The car stops at a two-way stop sign. It pulls out into the intersection and detects traffic heading my way. It stops right in the middle of the oncoming traffic and just sits there. The oncoming traffic was moving about 50MPH. I had to hit the accelerator to get out of the way. FSD SEEMS TO WORK WELL 99% OF THE TIME. THE OTHER 1% MAY GET YOU KILLED. FULL SELF-DRIVING MODE. Tesla sent me a 30-day trial.

I just got T-Boned in my 2022 Model Y performance. I got out of the vehicle which wasn't in terrible condition, in fact the left side of the vehicle was still perfect and the doors worked fine. I had my dog with me and everyone was okay, but I opened my door and the back passenger door to grab a jacket and check on my dog. I shut the doors to make sure he didn't run out and was a bit out of it because of the accident impact so I took a moment to compose myself.. All of the car's lights flickered and then the electric cut off leaving the doors completely locked and inoperable even though they had just been used moments before and were not damaged in the accident. I had to climb in through a shattered glass window to get my dog as all the doors wouldn't open from the outside and I was unable to access the glove box, nor pry it open because the electricity was out and the impact caused that door to make the side panel to unlock the glove box manually accessible. If the car DID catch fire or an occupant were bleeding out: Anyone inside, unconscious, would be unable to use the manually release and would be burned alive while people pull on doors that won't open that were undamaged in the accident and working 60 seconds before. From what I have read the only solution would be to jump the car using the 12V battery in the Frunk of which the Tow company and Emergency Responders were not familiar with.

Full self driving is not recognizing traffic signals. This results in the vehicle proceeding through red lights, and stopping at green lights. Without properly seeing/processing traffic signals, it approaches every intersection as if there are no traffic lights. Service center has had the car twice, without being able to remedy the problem. The 3rd appointment was cancelled by Tesla. I have called the corporate office and spoken with a service rep. He was unable to help provide resolution, saying only that my local rep would reach out. I have not been contacted. I've tried every other possible solution. Tesla doesn't want to fix it, or even acknowledge the problem, even though they've done a test drive with me and seen the issue with their own eyes.

What component or system failed or malfunctioned, and is it available for inspection upon request? >>> The passenger detection hardware underneath the front passenger seat had faulty wiring. The repaired hardware completed by Tesla this week is available for inspection upon request. I did take amateur photos of my seat hardware before-and-after repair. How was your safety or the safety of others put at risk? >>> MY PASSENGER'S AIRBAG COULD RANDOMLY FAIL IN THE MIDDLE OF A DRIVE. Anywhere from 10 seconds to 10 minutes while driving with a damp passenger in the front passenger seat (e.g., a 100 lb. person after swim practice), a red airbag indicator will assert with error code rcm2_a636 ("Front passenger safety restraint system fault"). The indicator would stay asserted for days, then start toggling on-and-off randomly. According to Tesla Model Y documentation, "If the airbag system detects a fault in any of the previously mentioned components, the airbag warning indicator stays on. In this case contact Tesla service immediately. Do not drive the vehicle until the airbag system is inspected by Tesla." (reference: [XXX] ) Has the problem been reproduced or confirmed by a dealer or independent service center? >>> YES! I was able to reproduce the problem WITHIN MINUTES with the loaner Model 3 given to me by Tesla (sorry, I did not get that vehicle's VIN but I wager Tesla Ridgepoint Austin TX has it). SURELY THERE ARE TENS OF THOUSANDS OF VEHICLES WITH THIS PROBLEM! Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? >>> Yes, but only after three attempts by Tesla. Each attempt was incremental (first: cable shielding, second: upholstery, third: replace the entire seat). Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? >>> No. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I’ve received repeated errors of front right safety restraint system needs service with an indication of the airbags being disabled. Tesla has attempted to fix this on two occasions by replacing various wiring and seat belt parts, but the problem returns each time.
