This incident appears to relate to NHTSA ongoing investigation at Tesla's low voltage battery system failure causing door handle malfunction. (September 26, 2025) 2021 Model Y front and rear doors (all four) was locked down with a seven years old child inside the car at home garage driveway. Mom unable to open up doors from exterior. A few hours later, left driver door was able to open but other passengers door remained locked down. (October 2, 2025) Sent Model Y to Tesla service center and identified low voltage battery and vehicle controller failure. Tesla technician replaced those electrical components with service fee charged.

On [XXX] at ([XXX]]) my Tesla Model Y (VIN: [XXX] ) was **parked in Park/Hold mode** in the [XXX] The **GPS navigation was set to an address in the center of the park**. the address on the address log of the car navigation system. The **TESLA GPS navigation destination was set to a point marked in the middle of the park** (not a parking spot or road). **Without any driver input** (no accelerator pedal pressed, no hands on wheel), the vehicle **suddenly shifted into Drive, jumped the curb, and accelerated approximately more than 30 feet] into the park grass**. This caused **damage to wheels, and all the passenger side of the vehicle to impact with a large rock placed in the park **Zero pedal input** — confirmed by driver and will be verified in Tesla server logs. No injuries. No other vehicles involved. **Root cause**: **Full Self-Driving (FSD) software / navigation system** incorrectly engaged **Lane Keeping Assistance** while parked, triggering **unintended acceleration** via **Speed Control** logic. The car was on software update and suddenly disappeared. The car **autonomously drove itself** toward the GPS point **from a complete stop**. Police report filed to LAPD (Case #[XXX]). Tesla Vehicle Data Report requested. **Request NHTSA investigation** into **FSD navigation-induced unintended acceleration from Park**. I was driving the Tesla and coming up my circular drive(note I was driving uphill). The car suddenly accelerated and went out of control. I pumped the brake and took my foot off the accelerator but the car kept going and crashed into our house (sunroom) at end of the driveway at high speed > we went through the casement windows and brick foundation and would have gone through the other side window and down a hill and probably been killed if it were not for a heavy cast iron table that brought us to a stop. The airbags never opened.The car was totaled(the insurance company declared it a total lost and refused to repair and paid us a small amount , less than we paid for the car. This was the second repeat incident, the first one very similar was reported to NHTSA by my erhusband ,reported to NHTSA 0n 1/20/2025, 6.19 pm; complaint number 11637163 .after the first crash we tried to get Tesla to pick up the car . they refused. Instead they said they would check it remotely. We gave them the opportunity to inspect and repair the car between the first and second incident . They ignored us , do to their negligence we were almost killed. Please open an investigation. We have names and emails to show who we complained to.

On October 2, my Model Y battery was replaced with a defective cell. Tesla service center replaced the battery. The vehicle could not charge or operate, making it unable to be driven. Today, November 2, while driving my car, my Tesla ABS system and other battery components experienced total failure while driving. For a brief period of time, I was unable to brake completely and my vehicle felt like it was skidding. At the time this occurred, my 1 year old daughter and pregnant wife were in the vehicle. This could have been much worse.

The contact owns a 2021 Tesla Model Y. The contact stated that while driving at an undisclosed speed, there was a loud clunking sound coming from the rear driver’s side of the vehicle. There was no warning light illuminated. After the failure had occurred, the mechanic who towed the vehicle informed the contact that the failure could be related to a detached axle. The vehicle was towed to the dealer, where it was diagnosed and determined that the front suspension lateral link had failed and needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. After researching, the contact discovered NHTSA Campaign Number: 21V835000 (Suspension). The contact expressed concerns that the vehicle should be included in the recall due to the similarities between the reported failure and the recall summary. The approximate failure mileage was 45,483.

Tesla removed the speed limit offset feature in FSD version 14.1.4 and now only uses speed profiles called Sloth, Chill, Standard, Hurry, and Mad Max, and this has made my car extremely unsafe to drive. The problem is that these profiles make the car go either way too slow or way too fast and I cannot adjust it to match actual traffic conditions anymore. To make matters worse, the speed limit detection is broken and frequently shows the wrong speed limit for the road I am on. When I use Sloth, the car never goes over the detected speed limit no matter what, so if the system thinks I am in a 35 mph zone when I am actually on a 55 mph highway, my car crawls along at 35 while traffic is flying past at 60-70 mph. I have people tailgating me, honking, flashing their lights, swerving around me dangerously, and I have almost been rear-ended multiple times because my car is going so much slower than everyone else. Without the ability to set a maximum speed offset, I am stuck going whatever speed the broken detection system thinks is right. If I switch to Standard, Hurry or Mad Max to try to keep up with traffic, sometimes the car goes way too fast for the actual conditions or blows through school zones and residential areas at unsafe speeds. I cannot fine-tune the speed anymore to drive safely with the flow of traffic. I am forced to either be a rolling roadblock and risk getting hit from behind, or use an aggressive profile that might go dangerously fast. This happens every single time I use FSD now and the combination of broken speed limit detection plus no manual offset control has created a seriously unsafe situation. I feel endangered every time I drive and I am genuinely worried about causing or being in an accident.

Our tesla y is under warranty but despite a baby seat and an adult sitting in the back seats the rear AC wont turn on automatically and thus providing extreme danger to my toddler. Tesla tried to fix but refuses to continue further citing a software update is needed. THIS IS PUTTING BABIES IN TEXAS HEAT IN EXTREME DANGER AS THEY CANT TELL THE PARENTS THE REAR AC IS NOT WORKING. WHILE MOST TESLAS DO NOT HAVE THIS ISSUE, OUR BATCH OF TESLA HAS THIS PROBLEM YET TESLA REFUSES TO HONOR THEIR WARRANTY CARING MORE ABOUT DOGS WITH THEIR DOG MODE BUT GIVING NO SUCH OPTION TO BABIES(WOULD YOU BELIEVE NO WAY TO KEEP REAR AC on all the automatically unless seat sensor detects baby seat or adult which both are broken in our car

I recently purchased a new Tesla Model Y (Juniper refresh). The vehicle has a loud clunking/creaking noise coming from the front suspension at low speeds, especially when driving over small bumps, turning into driveways, or when the road surface is uneven. The noise is noticeable inside the cabin and feels like a mechanical component is shifting. I brought the vehicle to Tesla Service. They confirmed the noise and told me it is not caused by the damper assembly but is a known issue that Tesla’s engineering team is currently investigating. However, no permanent fix is currently available, and the issue has not been resolved. I am concerned that this may be related to a suspension component failure or premature wear that could impact steering stability or control over time. Since this affects a brand-new vehicle and others have reported similar symptoms online, I believe this should be monitored as a potential safety defect. I am requesting that NHTSA track this issue to ensure Tesla provides a proper correction and repair campaign if necessary.

Sudden loss of forward and side visibility made it impossible to see lane markings, surrounding traffic, or mirrors, preventing a safe lane change or pull-over to the shoulder. This created a risk of collision with vehicles ahead or beside me. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes but they are not covering it. This is a very common issue reported by many owners Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. The vehicle has not yet been inspected by Tesla, law enforcement, insurance, or third parties. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps or messages appeared. The fogging occurred suddenly while driving on [date, approx. time], immediately after the cabin heat pump failed to clear the glass and visibility dropped to near zero.

Recently Tesla released it's latest version of FSD, v14.1.4. One of the key features is the revamping of the driving modes: sloth, chill, standard, and mad max. Tesla also removed the ability to manually increase or decrease the speed of the car. This is a problem. Let me illustrate it with a real-life example. Yesterday I was driving of a 40 mph road and the car correctly set the speed. I was using the standard mode so the car allowed an overage of around 10% so the actual speed was 45 mph. In our area this is typical for most drivers. As we entered a village, the posted speed limit was changed to 30 mph. The panel was perfectly visible and readable. The car did not register it and continued at 45 mph. I changed to mode to "sloth" and the car decreased its speed to 40 mph but I was still 10 mph above the legal speed limit. I came across a second 30 mph panel and again the car "ignored" it. Because I did not have the ability to manually reduce the speed of the car, I had to disengage FSD and drive the car by myself. Tesla claims FSD must be supervised. Fine. But removing the ability to manually change the speed of the car should be a requirement not a design preference.

Description of the problem: The front trunk (“frunk”) of my 2026 Tesla Model Y Juniper is missing the interior emergency release button and light. This component, which was present in earlier Model Y versions, allows a person trapped inside the frunk to open it from the inside. In my vehicle, there is no visible button, glow-in-the-dark handle, or wiring harness for this feature. What component or system failed or malfunctioned, and is it available for inspection upon request? The malfunction involves the frunk emergency release mechanism (illumination and release button assembly). The component appears to have been omitted during manufacturing. The frunk and all relevant panels are available for inspection upon request. How was your safety or the safety of others put at risk? Without an interior release, a person—especially a child—could become trapped in the front trunk with no way to exit, posing a risk of suffocation or heat-related injury. The frunk is large enough for a small child to enter and can be closed from outside the vehicle. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue was confirmed by Tesla Service, which stated the vehicle was “built as designed” without the interior emergency release. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Yes. The vehicle was inspected by a Tesla Service Center representative. No other inspections have been performed. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No. There were no warning lights or error messages. The absence of the emergency release was discovered immediately after delivery. Assessment: The removal of this safety mechanism may violate the intent of FMVSS 401and presents a potential safety hazard.

New update to ASDS software allows the vehicle to do 15-25+ over the speed limit and no ability to control. This is unsafe and Tesla took away the ability to control speed. Either engaged or not, this is completely unacceptable and they seem set on this being the only way to go.

One day I was exiting the vehicle, and as I swung my leg out to get out of the car the outboard panel broke. Nothing caught on it, it just broke as my weight shifted. As the technician came out to inspect it for repair, it was claimed that this was a common repair. The seat controls are on this panel, so when this happens, this means wires are loose, while the seat automatically moves for use. This changes how the seat positions itself, and the panel interferes with the door closing properly, the seat position.

Tesla FSD v14.1.3 feels unsafe for the typical “average” driver because it no longer allows users to set a maximum speed limit offset when using the Standard driving profile. In FSD v13, drivers could specify a max speed offset—such as 10% or 15% above the posted limit—when FSD was enabled. However, this option is no longer available in FSD v14. As a result, FSD v14 in the Standard profile often speeds on highways and makes aggressive lane changes to pass slower vehicles. This behavior feels unsafe and stressful for most everyday drivers. Tesla should restore the ability for each driver to set their own comfortable max speed limit and reduce the aggressiveness of lane changes.
