While driving on the expressway, I had the cruise control set at about 72 mph. The car to my right went to change lanes but then saw me and rectified; my car automatically applied the brakes hard without me even touching anything. I was lucky that the car behind me was far enough that it didn’t hit me. However, this is a safety hazard for me and others as I couldn’t control the breaks at all. It also caught me by surprise since I did t know it was going to do that.

At the end of my shift the "Full Self Driving" started working again, also without explanation. This time its driving me home, gets to the neighborhood I drive in and out of multiple times a day for the entirety of my "FSD Beta" and only address the car has "lived" at. This time it SLAMS on the brakes with someone behind me (scary) then quickly takes the wrong left turn, then another wrong left turn, then starts the ACCELERATE in a neighborhood/school zone directly towards a cul-de-sac with 2 story houses, speeds up more, then abandons all the "Full Self Driving" while I'm flying towards a power transformer box and curb when I SLAM the brakes (I gave her all she's got, she's heavy!) luckily I stopped with just a couple feet to spare. Then I drive back the direction it was supposed to go which also for the record it was showing a different path on the map, the entrance it always is. So the map was showing the blue line for GPS to turn into the second entrance, this time was the first time its ever taken the first entrance and I've literally never driven down the road it almost crashed me on but its been there for 6 months or longer. So just to clarify Tesla has FAILED at least 10 rounds at fixing the car and they cannot offer a solution. This car is a LEMON! Do something, you're the government I shouldn't be having a car try to kill me five times a day for several months without a recall. This clown car even plays the wrong songs and song titles! This is a coverup either the car is faulty and Tesla refuses to fix it or its hacked and being used for attempted murder. Either disgusts me and I was Tesla/Elon's biggest fan for over a decade, then I bought his car. The rest is history... "Full Self Driving" without explanation stopped working while I was using it. I tried to park the car, walk away, lock it; I even rebooted the car by holding both scroll wheels on the steering wheel as I've been directed in the past. Nothing worked. I noticed during my shift that "Auto Pilot" still worked on the highway so I was using it for a couple miles successfully. Then when the time for the exit came the car took the exit normally but then when it switched to "FSD" the car stopped steering and drove me directly towards a drainage ditch next to the off ramp and all steering assist and lane keeping was turned off all at the same time (while traveling at 50mph on I4). I was really scared luckily I never took my hands off the wheel but anyone with lesser reaction time would have crashed and possibly died. I got this on dashcam, pulled over to calm down (McDonalds near MCO), then drove back to prove it happened with the iPhone as Tesla has directed me to do on many occasions to "prove it". So I got it all on video you can see on this link here since NHTSA only allows low quality short videos less than 10mb like its the 1990's. The more I report Tesla and tell my riders about the child slaves in the Congo mining the cobalt in the batteries the more frequently the car tries to kill me. Park collision warning is defective. I have learned that Tesla has disabled Park Assist on this vehicle and on Model 3/Y manufactured after October 2022 despite still advertising this functionality in the current user manual. This is because they have transitioned from ultrasonic sensors to a purely camera based system. However, their camera based software has not been enabled for park assist. As a result of this, I had a minor impact while parking due to expecting an audible 'beep' or screen indication of distance from objects, as I have had in my prior tesla. However, I received no such warning. Also, Tesla poorly communicated to users, many of whom have had this functionality in prior teslas, that this functionality has been disabled. In speaking with Tesla Service, I was informed that Tesla has disabled park assist until a software update will be available. However, until the software update is made available, this is a safety defect that should be communicated through a recall to all owners.

While driving on cruise control, the car slammed on the brakes out of nowhere. It was terrifying and we were almost hit from behind. There was absolutely no obstruction or any reason for the car to slam on the brakes. We do not feel safe having our young children in the car. We have notified tesla of the issue many times and they refuse to fix it. This car is unsafe and someone, a Tesla driver or members of the public on the road with this car, is going to be killed by the phantom braking . What is the NHTSA waiting for? Please force a recall. Clearly Tesla doesn’t care.

We were driving on a three lane on the expressway with the autopilot engaged. all of the sudden and without any warning the car engage on the breaks. We didnt have any cars in front or side we did have a car behind us. This was an extreamly scare situation as it took us by surprised. This happened two times while engages on autopilot. We didnt use anymore after. I searched the web and there are thousands of complaint about it without a resolution. Tesla is no help following up. I hope you can do something about it.

I experienced the "Phantom Braking" twice during the three and a half hour trip. I reached out to Tesla but only received a canned response explaining how AutoPilot is engaged and disengaged.

I was driving on a large, multi-lane, divided freeway (5 south Interstate just before Anaheim, CA in Orange County, California) and using adaptive cruise control (not autopilot) to maintain my speed in the mid 70s MPH. It was around 9pm PT and there was very light traffic. The next car in front of me was at least 100 feet ahead of me and there weren't any cars on the left or right of me. All of a sudden the car started decelerating really quickly, and by the time I pressed the accelerator to counteract the deceleration, the car was at roughly 60 MPH. There wasn't any indication as to why the car decelerated so suddenly, and there wasn't anything that I could see that might have caused the cruise control to react that way. Thankfully there wasn't a car close behind me - if there had been, it's possible that the trailing car could have hit me and caused an accident at high speeds. I've read online that there are quite a lot of reports of "Phantom braking" from Teslas, and from the descriptions I've read, my experience seems to be exactly the same. I hope that NHTSA and Tesla can work together to get to the bottom of this and prevent these types of scary incidents from ever happening again.

Heatpump failed which caused a lack of cabin heat and in warmer weather would have caused charging issues due to lack of cooling possibly leaving me stranded. This had a warning on screen about loss of cabin conditioning. This happened over a mountain pass and caused the cabin temp to drop to 25F with two kids in car seats. This could be a really bad situation, luckily we had blankets and such to keep warm for the last few hours of the drive. I believe this was caused by an earlier TSB with sensor replacements and a software change done earlier in the life of my car for heatpump issues. This most likely caused wear on the pump and caused it to fail prematurely just out of warranty. The system was replaced by Tesla at significant cost and confirmed metal shavings in the system from the heatpump. I requested to keep the parts and still have them if further investigation is necessary.

Driving both east and west on 429 near the Wekiva river results in the car coming to a complete 0mph stop in a 70mph two lane highway with barriers on both sides and no signs or lights. Then the car starts changing lanes on the highway going about 15mph while cars are whipping past me at 70+mph with the horn on. The last 8 or 10 updates have not solved this issue I've been very alarmed and done everything in my power to avoid using the software and have demanded a refund many times, this is simply a deadly scam. The FSD contract is a license to kill and silence your attempts to alert the public like a good Samaritan; it must be shut down until proven safe. Each time they say they fixed it, each time I go out and risk my life and they lie, and lie and lie and now the stories of people dying from this exact scenario are starting to pile up (pun intended). The car is STILL confusing its sea level or altitude positioning on the GPS for the round about with stop signs below the sunpass highway. Fact of the matter is I have over 15,000 GB's of footage and there are hundreds if not thousands of scenarios where the car needs to be corrected. I am a professional driver and have software development experience but we cannot allow a private corporation to use human American citizens as lab rats and throw them aside when their software that is not safe yet kills the very people trying to improve the system. Not to mention innocent by standards who have never seen the inside of a Tesla nevermind an arbitration contract. This is all completely unethical. Windows havent been fixed after recall despite recalibrating windows, 2 software updates and restarting the car all as recommended. Cheap paint scratches, chips. Plastic snaps, upholstery loosens. While driving on the freeway into a tunnel during bright conditions while on Autopilot, the car unexpectedly braked heavily on a straight road, even though there were no debris or vehicles in front. Had to take immediate action since cars were driving around me. Similar event occurred when driving at night, luckily no cars were around me. During rain, automatic wipers did not activate. Required 2 actions to deploy wipers: pressing button on the turn-signal stock, then finding the menu on the screen to set wiper speed while driving in dangerous conditions. Another option is to use the voice command, however at times it takes 2-4 attempts to activate.

I had engaged Tesla’s “Full Self Driving” feature in a 25 MPH zone. As the speed limit changed to 35 MPH the vehicle correctly adjusted to 35 MPH. However, right after that I entered a school zone marked by a flashing yellow light. I used the brakes to slow down to 18 MPH, which disengaged the “Full Self Driving” feature because the vehicle did not adjust on its own. Once I was at 18 MPH I re-engaged “Full Self Driving” and the vehicle immediately started accelerating to 35 MPH again, in spite of the fact that I was in a school zone still. It seems the ”Full Self Driving” feature cannot detect school zones, even when marked by flashing yellow lights. The onboard visualizations appears to have detected them as stop lights.

On January 13th, 2023 we were waiting for green light on an intersection, 2nd in place. The car in front of us started moving on green light for a left turn, and suddenly it slowed down as a result of another car coming in the opposite direction. Our Tesla Model Y didn't recognized the situation and crashed into it. The automatic emergency braking didn't actuate and also the Forward collision warning didn't alert. The low speed collision avoidance also failed to act. A police report was filed because of the accident. Previously, On January 6th, 2023, a service was requested because of the vision system is not properly detecting objects / situations that can result in collision. Tesla canceled the service appointment. After contacting the dealer on January 13th, they acknowledged verbally that the car is missing sensors that were previously installed in older models.

There was a spontaneous blowout of a rear tire made by Continental. Thankfully I was not hurt while driving at 75 on a highway. I had to tow and replace the tire at discount. There was blowout from sidewall with no evidence of other damage or cause of it. I have kept the tire as evidence. I noticed there is a safety recall on very similar Continental tires. I am attaching the pictures and video of the tire and the note from Discount tire professionals that nothing was found inside the tire. I wonder if these continental tires also need to be on recall list.

On multiple occastions my car has suddenly applied the brakes when using cruise control. The braking ranges from mild to severe. This hasn't resulted in an accident but there has been a close call when a car was following closely. I have brought this to Tesla's attention and there response was along the lines of there is nothing that can be done to fix it and it should get better over time. It has not got better.
