As captured in images uploaded herein, the subject Infinit QX80's advanced safety system suite malfunctioned completely and without warning at 948 miles. The vehicle dash captures the numerous systems that simultaneously fail including: collision avoidance, steering assistance, lane assistance, blind spot monitoring - and all systems that rely on the advanced camera suite. Importantly, the first failure happened at highway speed without warning and resulted in the steering assist failing on a banked curve and directly placed a family of six within inches of a high-speed collision with an 18-wheeler. This same, intermittent recuring malfunction has happened TEN additonal times including and after SEVEN authorized dealership repair attempts. The dealer has replaced cameras, the entire dash, rebooted the entire software system, and then undertook a "hail mary" to hard reset the car by unhooking the battery and rebooting the vehicles operating system. The vehicle continues to malfunction. The dealership cannot replicate the code because the car does not recognize that there has been an error. The failure to store the failure codes means the dash resets every time the car is shut off and restarted. That makes diagnosis nearly impossible and the dealer just keeps replacing random parts hoping to luck out and fix the underlying electrical, software, wiring, or camera issue. Nissan has refused to acknowledge this serious safety issue and will not repurchase the vehicle. We have four daughters under 12 years old and are forced to keep driving this unsafe car until someone helps or something tragic happens. PLEASE HELP US! I can provide the repair orders from the dealership.

I am still having the same issues as the last time I reported this but new issues have occurred as well. The self driving is still disengaging with little or no warning and pulling the vehicle sharply to the left or right causing safety concerns with almost hitting another car or the center median wall on the highway. The stereo is still going black and turning the screen off. When you attempt to remedy, the buttons and screen are all frozen. When I power it off the AC goes off. The other issue is the crash warning is popping up when I'm trying to take off from a stop light, causing the wipers to start and the AC to turn off. One instant the braking applied like I was about to hit something but no cars were in front of me at the light. The car also turned on like it was in auxiliary mode after I got out and walked around to the passenger door to get my child out. These are causing lots of stress and numerous trips to the dealership to attempt to remedy. My vehicle has been at the dealership since Sept 29, 2026 making this the 3rd time and over a month total of not having access to it.

National Highway Traffic Safety Administration (NHTSA) U.S. Department of Transportation 1200 New Jersey Avenue SE West Department, Washington, DC 20590 SUBJECT: SAFETY COMPLAINT – STRUCTURAL PAINT DELAMINATION AND DEBRIS HAZARD • Vehicle: 2016 Infiniti QX80 • Paint Color: Pearl White (Paint Code: QAA) • Affected Area: Roof, hood, and upper body panels To Whom It May Concern, I am filing a formal safety complaint regarding a severe manufacturing defect on my 2016 Infiniti QX80 featuring the factory Pearl White (QAA) paint finish. The factory paint and primer have experienced a complete adhesion failure, causing the paint to shear off in massive, thick chunks down to the bare, unprimed sheet metal. This issue is not a cosmetic concern; it is an active highway safety hazard. The paint layers are delaminating at highway speeds, causing hard, sharp chunks of paint debris to violently fly off my vehicle’s roof. This poses an immediate distraction and danger to trailing motorists who are forced to swerve or suffer windshield impacts from the flying debris. Furthermore, the rapid deterioration of the protective coating risks structural corrosion of the roof panel over time. This is a well-documented, widespread manufacturing defect concerning Nissan/Infiniti’s factory white paint formulations, where an improper chemical bond between the primer and the metal substrate causes a total delamination failure. I request that the NHTSA review this defect for a formal safety investigation, as the debris shedding from these large SUVs at high speeds presents an unaddressed risk to public roadway safety.
