The contact owns a 2021 Toyota Rav4 equipped with Firestone Tires, Tire Line: Destination LE2, Tire Size: 225/65/R17, DOT Number: 8X2Y-DE8. The contact stated that while driving at various speeds the vehicle experienced abnormal shaking and vibration. The vehicle was taken to the local service center who discovered that all four tires were out of round and had experienced tread separation. The cause of the failure was not determined. The manufacturer and local dealer were not yet contacted. The failure mileage was 36,000.

My wife and I were traveling down SR74 mp 12 and rear ended a cow at 68 miles per hour. The airbags did not deploy from impact and yet the car is totaled the hood started through the safety glass of the windshield.

STEERING, LANE DEPARTURE: ASSIST
On February 14, 2024, I was driving to pick up a friend who needed a ride to an appointment. I had the lane correction feature on while I was driving on the interstate. When I exited the interstate, I traveled on a two-lane road that becomes narrow as it travels along a downhill slope. This road has a steep ravine on the right side of the road caused by rainwater runoff. There are reflector signs cemented into the road along the steep drop off. Because there was no oncoming traffic, I crossed the center line avoiding getting too close to the edge. When I did so, the lane correction feature activated, pulling the car hard into the signs along the road. As a result, the entire right side of the car received extensive damage. I was fortunate not to have lost control of the vehicle and gone through the signs into the ditch. My first reaction was to think that I had lost a wheel or had a tire blow out for the car to have jerked so hard to the right. When I stopped, I saw that this was not the case. I have since read that there have been many instances where the auto lane correction has caused other accidents like mine, some of which have been more severe. It is unfortunate that I have the inconvenience of getting the damages fixed and an accident being reflected in my insurance as a result. However, I feel fortunate that I was driving the car and not one of my children who may have lost control. I am glad the signs were securely installed to keep me from rolling into the ditch. I feel most blessed that nobody was injured. What if I had swerved to avoid a person walking on the road instead of the signs posted there? There was not a police report taken on this accident. It was reported to the dealer and insurance company who referred me to Caliber Collision for repairs where it is currently. There were no warning lights prior to the incident.

FUEL/PROPULSION SYSTEM
A strong fuel odor was evident in the cabin, and also under the hood after operation. Vehicle use was low, approx. 6K miles from new. My wife reported this to the dealer service department. They said nothing was wrong with the vehicle. Approx. 2K miles later, I reported the same problem to the dealer. Service rep said vehicle was unsafe to drive due to over torqued and leaking fuel line. They repaired it for no charge. No fuel vapors now, but if this is a production quality issue that others have reported, only to be told nothing was wrong, I think a major fuel-combustion safety issue may be overlooked. I am submitting documentation. Also, I am concerned about health effects of sustained fuel vapor exposure over months of driving. My wife rear-ended a stopped vehicle, possibly due to cognitive impairment from fuel vapor exposure. No warning indicators were ever triggered. Thank you.

Was driving to my daughters in the rain. As was pulling in my rav 4 started jumping and sputtering and cut off with a message of “engine stopped, power steering low”. Tried to crank it and same thing. Let it set over night and cranked fine. Took to dealership and they said could not find anything wrong. Have googled and tons of this same report on there but no solution. Needs to be recalled and fixed. Car just cuts off while driving

1. most recent. 03/01/2024 sunroof spontaneously exploded during my drive to work. broken glass fell on me as I drove. No bad weather. nothing hit it. just spontaneous explosion. 2. older claim. about a year of having purchased the car the front windshield cracked while parked at work. like the other incident no accident. I had that fixed. but the wipers do not work properly. Toyota charged me to have them check but did not fix it. to this day they only work at medium or maximum speed. wipers do not work on low or intermittent. after doing some research on the internet and YouTube it seems that this is becoming common with this model car.

While driving down the highway at the speed limit in our town on a clear evening with no weather, the sunroof of our 2023 Toyota RAV 4 exploded. There was a loud bang and it shattered as if the glass had been under significant stress or pressure. The glass bowed up in the middle and a section the size of a small saucer dropped out. There was a rigged break along the front edge of the sun roof. There were no bumps or holes in the road - it was smooth. There were no other vehicles nearby - no one threw a rock at our car. The noise was so loud that it was disruptive and shocking and thankfully I was not in heavy traffic or an accident might have occurred. I researched the web and found numerous instances of this sunroof explosion occurring. Please get Toyota to fix this before someone is serious injured.

The front passenger door started to "not unlock" when using the keys or door buttons to unlock it, you would have to manually unlock the passenger door by reaching across from the driver's side of the car. For about a week, that was happening, then that same door started to "not lock" using the key fob, handle or door buttons, so both locking and unlocking could only be done manually. Then the most concerning incident, while driving and while a passenger was seated in the front seat, upon rounding a corner (moving less than 20 mph) in a very busy town full of parked cars and pedestrians, the closed front passenger door flung open while the vehicle was turning and moving, almost striking a pedestrian and parked car. The passenger was able to safely close the door, but this would have been impossible had there only been a driver in the car at the time. The door had been shut properly, and there were no indicator lights that the door was not fully closed. The door also "auto-locks" when moving, so there were multiple failures of the lock/door system when this occurred. The car was on the road more than 5 minutes when this happened with absolutely no warnings until the door flung open and the car made lots of warning noise at this point. We called the dealer and made an appointment and on the day of the appointment the locks magically started working again. The dealer just charged us $90 to perform a "lock diagnostic" to which they said there are no issues found. Well, let me just say, I feel a door randomly opening in a moving vehicle is a major safety problem and it is a grave concern for me to drive or ride in this car when I am not sure when or if this is going to reoccur at any given moment.

There was a stalled vehicle in a traffic lane. The vehicle ahead of me was a large that hit the stalled vehicle. I couldn’t see what he hit but he came to a dead stop and I couldn’t avoid him. My problem is that the airbag under the dashboard on the driver side hit my shin with such force that it caused an abrasion that didn’t seem bad at first but developed into a wound requiring a large hematoma and surgery to remove it and to have skin graft.

Check engine sing comes and all dash board alarms lights on and bleep and while driving it shuts off while vehicle is on motion. This vehicle operates as yellow taxi and this issues is serious concern about passengers safety on board . The vehicle took to bay ridge Toyota dealer and the technician was unable troubleshoot and solve the concern Dealer charged 200 dollars for diagnostic and car is still under warranty.

I was turning left pulling into my carport with my foot lightly on brake and the car suddenly accelerated forward !!! I slammed the break hard and was able to stop the car from hitting the concrete wall because my front tire hit the wall to the left and the car hit the cement parking bumper in my carport!! It scared me so bad I am afraid to drive the car in fear I could hurt someone. The dealer could not replicate the problem and sent me home with the car. I am not happy with Toyota !! The car is 8 months old with 7,000 miles on it!!

I don’t know if this is a safety issue, but it is an issue with my car and others with this type of vehicle. The lift gate does not always open. It makes no difference if you use the inside button the outside button or the key fob. It will either open about an inch just enough to unlock it or it will open about 6 inches and then it will stop and beep it has to be manually closed and try again anywhere from 1 to 5 times to get it open. There is no rhyme or reason as when it happens. I’ve taken it to the dealer two times and they tell me they have to replicate the problem at the dealership but of course it doesn’t happen there. I’ve also written to Toyota company and all they tell me to do is reset liftgate. I have done this about five times it does not help. It’s obviously an electronic or something else problem that Toyota is not addressing. if I am contacting the wrong place and there was somewhere else to make consumer complaints. Please let me know.

Received a recall alert on February 8, 2024 indicating that my vehicle may have a passenger airbag that may not deploy as intended, increasing risk of injury. Toyota indicated that they do not have a remedy for this recall and that their estimate would be available in the 3rd quarter of 2024. I was at the dealership on 2/13 and asked if they could check the sensor for the airbag but I was told they could not at this time and I would have to wait until Toyota releases the remedy. I drive loved ones in my passenger seat. I now carry the risk on my own of injurying my passengers? This is very unacceptable knowing that they are putting risks of injuries to people.

The contact owns a 2021 Toyota RAV4. The contact stated that while driving at an undisclosed speed, the contact was involved in an accident. The contact's vehicle was crashed into by an oncoming vehicle. The contact stated that the vehicle had crashed into the front driver's side and the entire front of the vehicle was damaged. The contact stated that the driver's and passenger's front air bags did not deploy. The contact stated that only the driver's and passenger's side air bags were deployed. The vehicle was towed to a tow yard. A police report was filed. The contact sustained injuries and was transported to the local hospital by the paramedics. The vehicle had not been inspected by the insurance adjuster. The manufacturer was made aware of the failure and a case was filed. The contact was advised that the liability team would be handling the case. The failure mileage was approximately 15,000.

My 2021 Rav 4 Premium I Bought new @ 84 miles from Toyota Town of Stockton,Ca I noticed Once in November 2023 Gas gauge not read gas level properly. December at least twice I noticed issue progress from gas Gauge reading inconsistence to Gas tank failure to fill up or failure to Allow gas. [XXX] I am at Gas station, while trying to fill up Based on my tank level at half a tank My gas tank did not allow gas in I got Push back. I retried 2 more times And gas went in slowly but gas gauge after still read half tank. Local Toyota [XXX] Said they cannot duplicate problem. I cannot rely on accuracy of gauge Or tank I have had same problem gas gauge failure read & tank no accept gas issues In previous Toyota’s bought new. 2008 PRIUS Prius safety issue found by Local Toyota who switched out Gas gauge & possibly gas tank but Issue continued never could be fixed. Reported to Toyota corporation, [XXX] issue started Until I got ride of it 2021. When Prius Deceleration happened for first time On my way to Toyota. I was drove from work I could guide steering but power dropped, deceleration stayed active. So thankful no accident. Toyota Tech said I had gas. They did whatever problem Paused but came back! I HAD TO GET RIDE OF PRIUS SAFETY AT STAKE! [XXX] The Prius decelerated hazards On Prius system on a red Triangle On my computer saying need gas But, it had plenty gas and Technician Found it had gas, that was not cause Of problem. 2020 Hybrid RAV 4 delivered by Roseville Toyota of CA When they delivered SUV on less than full tank to my home. Waited 2 days for insurance Fully ensure car. 3rd day first day I Drove car and I went to gas station Twice to try fill up, but Gas gauge no respond to input of gas In tank. I did a video address issues With Roseville Toyota and Toyota Corporation. I returned car, I did Not know Toyota Corporation Rejected Contract until I spoke to Roseville Toyota in person. 6/15/23 @ 18,357 miles the car was given a four wheel alignment at the owners request. Due to loud tire noise and visible cupping on the right rear inside edge, it was clear the car's wheel alignment was wrong. The dealer's before and after alignment is attached. When presented the before and after, I noted RR Camber was.-2.3 vs. a range of -1.8 to -0.3 degrees. The reading was unchanged from initial. They advised me the camber cannot be adjusted and is "fixed" from the Factory. The form also read "One of more value are not within specification. Tire wear, handling and safety problems may result". But they had no remedy. I escalated at Dealer. Dealer opened a ticket with Toyota to look at my car. Of note, my Sister in Law's RAV4car has the same issue. I felt I had made my concerns clear and Toyota Corp would address. 2/1/24 At 30,345 the problem is still unresolved and dealer gave me Toyota Corp number and I opened case 240201000061. 4 calls later I am told to bring my car to dealer 2/20/24. Research shows TSB 0042-14 for earlier RAV4. My safety problem is not new to Toyota, it is just a later model. When I told Toyota Corp their response from 6/15/23 to 2/1/24 is unacceptable, they took no ownership. When I asked if I should drive an unsafe car they said it is my choice. When I advised them of the language from the dealer said the tires were unsafe, they said the Dealer is independent from Toyota. In short, the dealer is "Independent". Toyota has a problem with their design using a static camber strut which causes premature wear which can lead to a blow out as shown by forums. Toyota is only interested in getting the car to 36,000 miles and little focus on safe design. I was surprised not to see this already listed. Perhaps it exists under another year? A company Hard Race has made adjustable rear Toe Arms for the RAV4. The dealer confirmed same.

Driving home at [XXX] on [XXX] from Winters CA to Davis CA with a temperature of 55 degrees F and some rain in the area. The sunroof exploded. A large bang happened. Pulled over nothing hit the sunroof, and the glass was protruding outward (not inward which would have happened if the glass was hit from an object from above). Vehicle will shut off while in “drive” gear after a complete stop and will report on the dash screen “ Engine stopped. Steering power low.” All other alert lights will be on and unable to change the gear into neutral to move the vehicle into a safe location. This vehicle has already done this exact issue twice and it’s currently less than 10,000 miles on the vehicle. First incident was 2k mileage in a safe environment however the next incident was at 6880 miles. Vehicle shut down in the middle of a highway and I was unable to place the car into neutral gear to move the car to safety. Dealership has not been successful to find the source of the issue.
