While driving on the freeway with outside temperatures of about 89/90F, at about 67-68 mph, our passenger window randomly exploded and shattered. There was no prior damage to the window or any indication that a rock, projectile or other debris had hit the window. Prior to this incident, we had observed that the passenger window would rattle inside the door pocket/window frame when the car was in motion. When the window shattered, it did so instantly without any warning, as if it had exploded, startling us. As we continued driving, the outside air started pulling apart the shattered window, causing the broken window glass to fall apart into thousands of little pieces that ended up all over the passenger, driver, and back seats, which were also occupied with children [XXX] and [XXX] old). This was a very scary and traumatic experience for us, as things could have taken a turn for the worse and we could have been seriously injured. Luckily, we had nothing more serious than minor cuts in the aftermath. However, our family and especially our children were very affected by this incident and our car suffered damage to the outside body and some more minor damage on the inside from the glass flying off from the shattered window. We believe the rattling of the window caused it to eventually explode and shatter. We believe the rattling is due to some kind of manufacturing error: either improper installation or possibly low quality glass material or incorrect size to match the door frame or a missing part that should hold the window securely. We would like our car inspected for any further damage not visible to the eye (such as glass inside the door pocket and the safety of the door window retainer of the passenger door as well as a check of the integrity of the rest of the car’s windows to check that they are property installed and secured in place to prevent rattling. The driver side window in the car continues to rattle and we fear that this incident may occur again. INFORMATION REDACTED PURSUANT TO THE FREEDOM

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, ELECTRICAL SYSTEM
Cruise control suddenly quit working, alarms for hybrid system malfunction, cruise control malfunction. This is a known issue for Toyota, high voltage leaking out because of a corroded ground. This repair is going to cost around $7,000 out of pocket.. My car is 3 years old.

On [XXX], I opened the unblemished retractable glass roof of my 2022 RAV4 XSE. The rear windows were rolled down approximately 4 inches. No other cars/people were in front, next to, or behind me. As I accelerated to approximately 35 mph, I heard an explosive blast and peered upward. I noticed that the retractable glass roof panel had completely shattered. Shards of the roof rained on me as I drove home. Once home, I closed the roof just enough to secure my vehicle. The motor /closing mechanisms were operable. Glass shards were present in the tracks as well as on the top of the intact remainder of the glass roof. Glass shards were present in both the front and back seats of the vehicle. Upon inspection, there were NO visible pits/holes in the shattered roof; however, the roof was almost completely displaced from the edges of the moonroof’s seal/frame (see pictures). I contacted my Insurance to file a claim, and my SUV was towed to Toyota. 6/24 Toyota’s estimator left a voicemail stating that the glass roof was being assessed. I left a message asking for the cause of the damage. On 6/25, Toyota called and stated that the roof panel was being replaced. The estimator was unable to determine the cause of the breakage and mentioned that this was the first time they had to replace a panoramic sunroof. No other outside agencies (only Toyota Carlsbad Collision) inspected and/or addressed the damaged glass roof. A faulty design and /or improper manufacturing of the glass was never addressed or considered. 6/27 Toyota billed my insurance $1971.52 minus the $500 deductible I was required to pay per the terms of my insurance. *** It should be noted that there are a number of RAV4 cases involving sudden glass roof explosions. These lawsuits allege that glass sunroofs of RAV4s shatter under normal driving conditions. In fact, on May 3, 2024, a class action suit brought before the US District Court in California, rejected Toyota’s rebuttal, and the lawsuit is moving forward. Driving NB when a Toyota Corolla drove into my lane of traffic hitting me head on. No air bag deployed in my RAV but all air bags deployed in the Corolla. No air bag light was on prior to the accident. My vehicle data recorder was copied by a Bosch technician and forwarded to Toyota Corp. The body shop manager stated that my vehicle was the first he had encountered with the extent of front damage where the air bag(s) did not deploy

Manufacturer Recall Number23TB15 NHTSA Recall Number23V865 There is currently a recall on the airbags for my vehicle. An alert popped up on my dash saying there is a error with the airbags. When I went to take it to the dealership, they refused to check if the sensor was related to the airbag recall for free. They said they would check it out, but if the sensor wasn't related to the recall then I would be charge just for inspecting it. I don't understand why they would not check it for free regardless since there is a recall out on the car.

I was at a complete stop and was rear ended. The person who hit me was going over 45mph, and the impact caused me to hit the car in front of me, which crushed my car at both ends. My (driver) airbag did not deploy, and my head struck the steering wheel. It is available for inspection, and has not been confirmed by a dealer - it is at the insurance company’s tow lot. The police officer at the scene was surprised that my airbag did not deploy. There were no warning lights, messages, or any other indication that anything was malfunctioning/in need of service.

Toyota OEM windshield glass has SEVERE distortions/waviness in glass. I have had my windshield replaced 3 times now with Toyota OEM glass and the issue is still not resolved all 3 have had this distortion/wavyness in the glass. I don’t want aftermarket glass in a brand new car especially with all the safety features that relies on glass clarity, but Toyota is making it hard not to choose that option. Toyota needs to do better on quality control of their glass. The distortion is happening in the line of sight for the driver causing dizziness and issues while driving, as well as BAD wavyness around the edges. Ive done some research into this as well and I am not the only Toyota driver having this issue with their glass. I’ve never owned a vehicle that did this and it’s very disappointing and frustrating that I am not able to have a clear windshield in my brand new car. I called Toyota of America but they basically pushed me off onto the dealership. Someone needs to contact the MFG and make them aware of this problem so I can get glass that is normal.

I had another incident this week where I was stranded in a parking lot and had to call Triple A to get my car running properly again. They tested the battery and belts and all were good. When trying to start my car, it locked and said: "ENGINE STOPPED, POWER STEERING LOW". Not only was my car locked, but it was shaking and making scary noises. The same thing happened to me at a drive-thru a year ago. My car is a Toyota Rav 4 LE 2022 with 20K miles on it and is too new to be having these problems. Through looking up this issue on Google, I have seen that there are a good amount of Rav 4s that are newer dealing with this same issue.

I believe Toyota is falsifying the MPG rating to make it appear they are getting better gas mileage than they are actually getting. On a new Toyota Rav4 bought last year, the speedometer always reads 3 miles faster that my actual speed. This is confirmed by my stand-alone GPS unit and my iPhone as well. I have since checked out other new Toyotas and found the same error. This error favors Toyota because if make the onboard computer report that you are achieving better MPG that you are. This error is also recording more fictitious miles driven, on your car, than you actually have driven and causing you to get servicing sooner as well as running out your warranty quicker than it should. However, I have since traded in this vehicle for a different vehicle that I feel is more honest and accurate with their information.

We’ve had the car for three months had it to the shop of the Toyota place seven times for the electrical issue the car will shut off going down the road when you give a gas when you let off the gas, it will start back up spitting sputter, the instrument cluster The gauges will stick in one spot and not budge and you lose all power to the vehicle to drive

Sunroof shattered suddenly while driving down a freeway. It sounded like an explosion, and my passenger riding and I were concerned it was gunshots when we slightly moved the sun shade cover, we realized it was the exploded sunroof. There have been no prior incidents or found debris anywhere that could have caused the sudden explosion of the glass. We had some glass fell on us , and if not for the sun cover blocking us from harm, we could have been seriously injured .

The brakes failed, without warning, while parking; causing me to hit a pole/wall in my employer’s parking garage on April 12, 2024. I was injured, and obtained care from UChicago Medical Center. Toyota assigned an independent investigator to assess my vehicle; which was completed on April 29, 2024. Per Toyota, no brake issues were noted. My vehicle remained at Oak Lawn Toyota from April 12, 2024- May 31, 2024.

Hello, I recently purchased a RAV4 2021 Limited and immidiateky noticed the side mirrors are shaking while the car is on the move to an extent where it is hard to keep track of the other vehicles from the mirrors. When I looked up this is a toyota design defect for which they have a service bulletin. I believe this should be a safety concern as the mirrors are integral part of driving and excessive shaking limits the usuability as well as strains the driver’s eyes.

SERVICE BRAKES
The contact owns a 2024 Toyota RAV4. The contact stated that while driving at an undisclosed speed, the brake pedal was depressed, however, the vehicle did not stop as intended. The contact stated that the failure occurred while the contact was trying to avoid a crash. The contact stated that while trying to prevent the oncoming vehicle from crashing into the contact's vehicle, the contact crashed into a curb. The contact stated that while driving off the curb, the brake pedal was depressed and the steering wheel was steered to the left, the vehicle did not stop. The contact stated that the vehicle crashed into a pole and then the vehicle stopped. The vehicle was damaged on the passenger's side from the front bumper to the passenger's front door. The driver's front door rear view mirror was damaged. In addition, the transmission fluid leaked and the passenger's front wheel was detached. The fire department and Police department arrived on the scene. The contact sustained injuries to the right lower leg and left wrist. The contact’s left wrist was burned due to the air bag deployment. The contact received medical treatment for the injuries at the scene. A police report was filed. The vehicle was towed to an independent mechanic. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 5,500.

The incredibly frightening incident occurred 2 times within 3 days. My 2023 RAV unexpectedly and very forcibly accelerated on its own when I was pulling into a parking space. Luckily I am in the habit of parking my RAV far away from other vehicles in parking lots when the option exists. The first ‘acceleration and/or braking’ malfunction occurred 5/31/24 in a Walmart parking lot. When pulling into a parking spot my right foot was fully off the accelerator (I use my right foot only for the accelerator and brake pedals) and was gradually depressing the parking brake pedal to come to a complete stop. Instead my RAV suddenly and forcibly accelerated forward through the empty parking spot in front of me and beyond!! I would have absolutely smashed into the car and or person if one were there!! As the RAV shot forward I began desperately pushing full force on the brake pedal; the RAV did not stop! so, I began full force repeatedly pushing and pumping the brake pedal. My RAV eventually came to a halt as the tires may have locked up which it skidded about 12-18 inches. By God’s grace no one was hurt! On 6/2/24 ‘it’ happened again!! Pulling into a parking spot at an outdoor cafe, as I was gradually depressing the brake pedal to come to a complete stop my RAV instead quickly and forcibly accelerated forward!! I thank God that I did not pull into the other spot (my sister took that one) which was just a few feet away from around 12 people and families enjoying their outside lunch dining!!! This 2nd horrifying incident my RAV took longer to come to a complete stop. I fully and forcefully pushed on the brake pedal but my RAV kept going!! I continued repeatedly releasing and fully depressing the brake pedal but it kept going! The next thing my RAV was approaching was someone’s backyard and house!! I forcibly pushed the brake pedal one more time and also threw the RAV shifter into “park” finally the RAV stopped! I am so shaken up by these traumatic malfunction incidents.

The Multimedia dashboard is completely blank. Not able to use sound, gps, music, etc. Cant set time or any adjustments to vehicle. This seems to be a manufacturing issue. Dealer said there is nothing they can do about it. Was told will need to pay $1600 to replace it. Don't know how long a new won will last. Can't keep spending $1600 everytime the electrical system Malfunctions. Please help.

The red brake light came on and I slammed on the brake but the damage was done. The front bumper was cracked on the left side and dented on the right side. There whole front end was scraped and scratched. The collision sensor is damaged

STRUCTURE:BODY:HATCHBACK/LIFTGATE:HINGE AND ATTACHMENTS
The contact owns a 2021 Toyota Rav4. The contact stated that the liftgate was making an abnormal rubbing sound while being closed manually or automatically. The vehicle was taken to an independent mechanic where it was discovered that fluid was leaking from the left liftgate bar which had caused the failure. The mechanic reached out to the parts department and was provided an estimate for the replacement parts. Neither the dealer nor the manufacturer was notified of the failure. The vehicle was not repaired. The failure mileage was approximately 44,000.
