I am receiving codes, GTW_w074 Steering Assist reduced, and UL_a020 Steering Assist reduced. In reality, steering is unavailable without extreme effort. This makes the car undrivable. I had the same problem in early February. The car was towed to Tesla for service and repaired. Now one month later the problem has returned.

- #11577950March 18, 2024SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Driving on a two lane highway at 70 mph, the vehicle has applied the breaks extremely abruptly without reason, three times in the past week. Slamming on the breaks suddenly reduced our speed from 70 to 45 mph before I could return to the speed limit. I came very close to being rear-ended last time. The drivers behind me were angry as well, assuming that I was “break-checking” them. I am extremely concerned for my family’s safety.

Charging system has become defective over last month. Two weeks ago, gave error that charging cover was open when it wasn't. Couldn't get it to charge for a while. Eventually, it worked. Yesterday, would not charge at all on any charger. Being towed to service center tomorrow.

We bought a 2022 Tesla Y performance in August 2023. Love the car but customer service in the service center is and has been beyond horrible and frustrating. We had a back seat (driver side) panel that was not snapped in properly. In November 2023 we took it to a service center so they could fix it. They said they needed to order the part and they would send someone to come to our house to fix it. On 2 different occasions someone came to our house to fix it. The first service tech replaced the bottom piece but it wasn’t snapping in securely so he said they need to order the middle piece. The 2nd service tech came to the house and said they did not send him with the correct part and he will schedule an appt for us to come to the service center. We scheduled an appt to go to ant other service center (an hour away) and they had our car for a couple hours and gave us a loaner. When my husband went to pick it up they did not fix the panel and it was not snapped in correctly. We have a picture of it. They said they would have to order the part and schedule us to come back to fix it. We went back and they said it would be 2 hours and my husband was already waiting an hour. They did not fix it and we left. Very rude customer service.

Vehicle screen has phantom touches. Extremely unsafe as the vehicle will turn volume to max, turn heat/air up and down, and make other changes on its own and while driving. Almost caused several accidents. Tesla had made me wait for a month before they will fix.

- #11577438March 14, 2024FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, SERVICE BRAKES, VEHICLE SPEED CONTROL
Phantom braking has occurred on 4 different occasions nearly causing a rear end collision during one of them. On all occasions it occurred with no other car in front of us. Tesla Service was contacted through their app for service and we were told this is a known issue and to simply apply pressure to the accelerator. No warning lights or other was given before the incident. The problem has not been reproduced or confirmed by Tesla or independent service center?

I am writing to express my serious concerns regarding the recent enforcement that leads to Tesla's Auto Steer being disabled after three warnings, deemed "strikes," within a certain period. This policy, which I understand may have been implemented with safety intentions, paradoxically compromises my safety and potentially that of others. My experience with the system has shown that the warnings are overly sensitive, activating even while I am fully attentive and engaged with the driving process. The result is the Auto Steer feature—a tool I rely on for enhancing my driving safety—being disabled for an entire week after these strikes. This blanket punishment is applied universally across all driver profiles, disregarding individual driving contexts and histories. The logic behind this stringent measure seems counterintuitive. The Auto Steer function, in my experience, significantly increases safety by aiding in maintaining lane integrity, adjusting speed, and managing distance from other vehicles. Disabling it not only diminishes my vehicle's capability but also impacts my confidence in the vehicle's safety features, which I have come to depend on. I urge the NHTSA to reconsider this mandate and recommend adjustments. Possible improvements could include making the warning alerts more prominent to aid drivers in real-time, allowing for a more interactive and educational approach to enhancing driving habits. Alternatively, revisiting the policy to provide drivers with more control and flexibility over the use of Auto Steer could be beneficial, ensuring that it serves its intended purpose of augmenting safety without unwarranted restrictions. The overarching goal should be to enhance safety while also respecting the drivers' ability to utilize technology responsibly. I hope for a revision of this policy that aligns more closely with the practical needs and safety interests of Tesla drivers. Thank you for your attention to this matter.

Unknown. Drove car in front of garage. Car came to a full stop. About to put car into reverse from drive in order to back into the garage, when car suddenly accelerated forward. Turned wheel sharply to the left in order avoid hitting neighbors fences. Vehicle traveled in an arch and collided with a tree next to an fence and stopped.

On [XXX], I was at [XXX] waiting for green light, as soon as the light changed. I hit the drive but the car suddenly lost control and speed up and I couldn’t stop. I try my best not to hit any people or any cars and ended up hitting the board on the right. I never smoke or drink alcohol. never did any drugs, I didn’t hold phone. It was absolutely an uncontrollable incident very unfortunate but totally out my control I wish this never have happened as I have good driving record. Thank you, your honor ! - #11577012March 12, 2024AIR BAGS, ELECTRICAL SYSTEM
(1). Electrical wire related to passenger airbag function is allowed for rodent chewing. I was told by technician that if this wire chewed by rodent or broken, passenger air bag will not function at all when there is an impact or an accident. I'm not really sure whether the driver will be even alerted whenever this wire is broken. This is simply a design fault because such an important wire should not be placed in a vulnerable location where rodent can chew it. I have never come across this in any car in my life so far. (2). If an airbag does not function, a passenger could get hurt or die (3). I asked the service team about the design fault and filled their service visit survey as well, but no one answered me so far. (4). Service technicians inspected the broken electrical wire, repaired it, but there is no guarantee that rodent can not chew it again because repaired wire is at the same place as before. (5). There is no prior warning for me. Service technicians came over to replace the passenger seat. Once they replaced it, the senior technician tried to check all the system using his laptop and it appeared that he noticed a fault on his laptop screen. So, he opened the frunk and found the broken wire. (6). Instead of replacing this part with OEM part he repaired it and even charged me for the cost of repair though my car is under warranty. This is the design fault and this wire can be chewed again and manufacture can keep charging the customers till the life of the car.

This is in regards to wipers getting frozen and major snow accumulation under them under heavy snow fall, something that can be mitigated with vehicles that are manufactured with Heated Wiper Park. This issue is dangerous if you are travelling on a highway or in places where you cannot stop to clear the wipers. I have contacted Tesla service and talked to them about potential solutions but they kept coming back with the statement that “this feature is working the way it was designed to.” Currently, if wipers are on or in auto mode, the wipers do not go over the wiper park heater, something that could be modified through a software update when ambiant temperature drops below a certain point or if snow fall is detected through their vision system and automatically activate their Heated Wiper Park, because not only is it uncomfortable to have the defrost blasting hot air on your face, but it also is extremely inefficient for electric cars to be doing that.

This is not a defect but rather something I consider could be dangerous. On slippery conditions example, during heavy snow fall, or when there is slush or if there is an icy surface, I find that the AWD system in my vehicle which is all controlled via software that the manufacturer wrote (because it is a dual electric motor setup, and there is no mechanical connection between the front and rear axle), tends to either send too much power or tried to regen too much to/from the front motor when the steering wheel is turned (something they can detect), and it tends to cause unexpected understeering which if you are not accustomed either to driving an electric car or Tesla’s AWD system could end up in a potential accident. I am writing to you because I know that my voice will go unheard since I do not have access to any of their feedback challenge. Again I do not consider that a potential dange, nothing imminent or defective with the vehicle. The fix in my opinion is a software update that could impliment road surface detection via their vision, in combination the steering angle sensor wheel slippage detection, to detect that the trajectory of the vehicle is not going as desired and mitigate that through multiple techniques of traction control, I know that with a RWD Tesla they do use the front brakes in certain conditions to force the wheels to dig in the snow and try to find traction from experience even though there is no front motor.

- #11576176March 8, 2024FORWARD COLLISION AVOIDANCE: WARNINGS
Tesla is using camera-only detection for Forward Collision Warning system and this leads to numerous false positive alerts. I'm filing a complaint because this did occur today (March 7, 2024), but this has happened countless times in my 2 years of ownership. This particular incident was triggered by cars parked along a curved street in my neighbor, while I was traveling approximately 20 mph with no other vehicles driving around me. These occur from simply driving next to parked cars or slowly approaching vehicles at a traffic light. When these faulty alerts occur, a loud audible beeping is heard and red alerts are displayed on the screen. These faulty alerts are dangerous distractions, causing panic, fear, and uncertainty in the driver, causing them to take their attention off of the road while they assess what just occurred. This is a critical flaw in the functionality of Tesla vehicles that the company has refused to address.

- #11575187March 3, 2024AIR BAGS, ELECTRICAL SYSTEM, STEERING
My vehicle encountered a severe loss of control, resulting in a collision with a concrete wall on I-90 west towards Seattle at 19,877 miles. This critical event, leading to non-deployment of airbags and subsequent hospitalization, occurred shortly after two prior services addressing a Rear Drive Motor unit malfunction, first at 14,768 miles and again at 14,820 miles. These incidents underscore a troubling pattern of safety concerns. Inspection Details: Front Suspension: Replacement of Stabilizer Bar Link (1188391-00-B). Steering Gear: Replacement of Steering Gear Assembly (1044831-00-H), essential for control. Sub-Frame & Steering Components: Reinstallation of the sub-frame and replacement of the tie rod end (1044841-00-E), indicating significant structural and steering issues. Safety Risks: The sequence of malfunctions, particularly the recurring issues with the Rear Drive Motor unit followed by a severe loss of control and airbag system failure, presents an extreme safety risk. The absence of warnings prior to these failures compounds the danger, suggesting systemic safety and reliability flaws. Conclusions & Reporting: The vehicle's history of critical malfunctions within a short mileage interval demands an in-depth examination by the manufacturer and safety regulatory authorities. The objective is to uncover root causes, evaluate if these issues are widespread among similar models, and implement corrective actions promptly. This report aims to highlight the critical nature of the encountered safety issues, advocating for immediate interventions to safeguard vehicle occupants and the public.
