What component or system failed or malfunctioned, and is it available for inspection upon request? The "Actually Smart Summon" failed. It is available for inspection. How was your safety or the safety of others put at risk? The "Actually Smart Summon" function did not detect the post next to my vehicle as it was exiting the parking space. It immediately turn right into the post instead of pulling straight out first. I had very little reaction time. If there had been a person in the position of the post, this could have caused physical harm. My vehicle sustained damage to the front passenger door, its side mirror, and to the PPF material over these components. Has the problem been reproduced or confirmed by a dealer or independent service center? I don't know Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? When opening the feature on the app, it states: "Warning: Monitor vehicle's surroundings for people, animals, or unpredictable movement."

Tesla service identified LH half Shaft was damaged and blaming it's due to outside influence because impact to the undershiled. This car should be still under 4 years and 50K warranty but Tesla service refused warranty on the parts.

I purchased a 2024 TESLA model Y and received delivery of my Tesla vehicle on [XXX], at 4 PM EST. Upon receiving the vehicle, I noticed manufacturing defects on the tail lights, which I reported to Tesla Support immediately and scheduled an appointment on September 29. Unfortunately, no appointments were available at that time, and my service date was postponed to October 28, 2024. On [XXX], while traveling at low speeds, the rear passenger tire of my vehicle unexpectedly exploded. They are the factory-installed Continental tires 255/45/19. The car had to be towed to Tesla in Coral Gables, where they installed a temporary tire and quoted me $517.35 for replacement—despite the vehicle being less than a month old. Since delivery, I’ve also observed that my tail lights have accumulated water, indicating a potential safety hazard and manufacturing defect. I have contacted Tesla Support on multiple occasions, but they have not provided clear guidance on whether a loaner vehicle will be provided while these issues are addressed. The lack of timely and effective support for these escalating defects has been extremely frustrating and concerning. Primary Concerns: Safety hazards related to the tire explosion and water accumulation in the tail lights. Delays in service appointment scheduling. Lack of clarity and support from Tesla regarding service procedures, such as whether a loaner vehicle will be provided. Given these issues, I am requesting assistance from the NHTSA in addressing the quality and safety of this vehicle, as these defects and delays are affecting my ability to operate it safely and reliably. I was driving in the dark down a straight road traveling around 60mph when the car abruptly slammed on the breaks and came to an almost complete stop. I was not using self driving, I was in control of the car but when this occurred I did not release the gas. Out of Instict I tried to get the car moving again and pressed the gas but the car did not respond so I pressed the brake and then the gas and the car started driving normally again. Thankfully there was nobody in the car with me and no cars behind me. The braking was abrupt enough that my neck hurts from the jerking forward of my body.

The contact owns a 2023 Tesla Model Y. The contact stated that while driving at approximately 30 MPH, the front driver-side seat inadvertently adjusted the seat repeatedly; the contact had to pull over and allow the vehicle temperature to cool down for several minutes for the vehicle returned to normal operation. No warning light was illuminated however the "Restoring Driver Position Settings" warning message was displayed on the instrument panel. The contact stated that his daughter was in a booster seat, seated in the rear driver-side seat and was repeatedly hit by the driver-side seat; the impact of the driver-side seat left no bruises however, the contact along with his wife, his daughter, and his son all experienced emotional distress due to the incident. No medical attention was sought. The vehicle was taken back to the place of residence. A dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was 14,500.

The driver-side rear seatbelt latch has malfunctioned. The spring mechanism appears broken and now will not lock / click the seatbelt in place when inserted. This is a major safety concern that has been documented numerous times specifically for the Model Y. The Tesla mobile service center will come out and diagnose which can take 1-2 weeks for appointment. But when the broke latch is replaced, it will cost +$80 per latch that is replaced.

Tesla’s “Autosteer” function inexplicably and suddenly shifted lanes and sideswiped the other car that was passing me on the right. This defect of the “autosteer” function took control of the steering of the car, even with my two hands on the wheel. I do not know why the Tesla suddenly shifted in the intersection or, more importantly, why it did not detect the other car. I did see the other car approaching before the Tesla swerved into it. I wanted to remain in the left lane because it was the correct and safest thing to do. There were no obstacles ahead of me, and the left lane continued normally.

Tesla released a feature called Obstacle Aware Acceleration. The Owner's Manual states: "Obstacle-Aware Acceleration is designed to reduce the impact of a collision by reducing motor torque and in some cases applying the brakes, if Model Y detects an object in its driving path. The touchscreen displays a visual warning and sounds a chime when the brakes are automatically applied. For example, Model Y, while parked in front of a closed garage door with Drive engaged, detects that you have pressed hard on the accelerator pedal. Although Model Y still accelerates and hits the garage door, the reduced torque may result in less damage. Obstacle-Aware Acceleration is designed to operate only when all of these conditions are simultaneously met: Drive or Reverse is engaged. Model Y is stopped or traveling less than 10 mph (16 km/h). Model Y detects an object in its immediate driving path." However, my vehicle (and at least some other Model Y Performance models) do not behave in this manner. Rather than reducing motor torque when all of the above conditions have been met, it reduces torque every time the vehicle accelerates from a stop. This has been replicated by my local Tesla Service Center, and they confirmed the vehicle did not detect any objects in it's path. This is dangerous because the vehicle accelerates so slowly from a stop that I may not be able to avoid an accident if the vehicle doesn't accelerate as expected.

When addressing this with the local service center, they said they've reported this to Tesla engineers as they're prematurely failing at "an alarming rate". There is no advanced warnings. The vehicle just leaves you stranded.

#Issue Description I experienced an issue with Tesla’s Full Self-Driving (FSD) system when a trailer hitch was attached to my vehicle. From what I observed, the FSD system appeared to detect the trailer hitch as an object in close proximity to the vehicle. This caused the vehicle to accelerate rapidly to the FSD maximum speed of 85 MPH, regardless of the road conditions or posted speed limits. This issue occurred when the lane markings were visible, but the hitch obstructed the rear camera’s view. On the FSD display, this obstruction was represented as a persistent “cloud” following the vehicle. I attempted to adjust the maximum speed limit manually to mitigate the acceleration, but this did not prevent the behavior. Each time I engaged FSD, the car would accelerate rapidly, even in situations where such high speeds were not appropriate for the road conditions. # Steps Taken to Resolve the Issue I covered the rear camera, which allowed FSD to function normally and prevented the rapid acceleration. The “cloud” visualization on the FSD display also disappeared once the camera was covered. # Trailer Mode Tesla offers a “Trailer Mode,” but I did not enable it since I was only using a hitch without towing a trailer. I was driving out to Pittsburgh to drop my daughters off at school, and my rear driver-side tire rapidly went from 40 PSI to 20. I almost lost control of the vehicle at one point. It turns out my 9 month old Tesla Model Y performance had a cracked rim. I have had cars for almost 40 years and never had a cracked rim until now. Something is seriously wrong with the 21 inch rims Tesla is making for their model Y performance models. I was in the Tesla Dealer today and 2 other people had the exact same problem, and their is a ton of info online about Tesla cracked rims. I had to pay $1,018 for a new rim that was 9 months old. (So did the 2 other people today) Something seriously is wrong with Tesla Model Y Performance rims. Their manufacturing process clearly is flawed for this to be such a persistent problem.

Twice, while in full self-driving mode on I-395, my Tesla has made the potentially fatal mistake of trying to turn the vehicle directly into the closed gates of the so-called "hot lanes" (which separate the north and sound bound lanes) and into on-coming highway traffic on those lanes. This is a serious safety issue that must be fixed IMMEDIATELY. This happened most recently on Sunday October 20, 2024, when I was driving south-bound on I-395 with my wife and son in the car. I was in the far left lane with "full self driving" activated. The hot lanes were to our left with all traffic on those lanes driving northbound. The gates to the hot lanes were closed to southbound traffic to prevent someone from driving directly into oncoming traffic. Shortly before we were about to pass those closed gates, the Tesla left indicator light came on and the car began to turn. The map showed the car was trying to turn us directly into the closed gates and oncoming traffic. Fortunately, I was paying close attention to this potential problem because the identical thing had happened previously, during the prior 30 day period when full self-driving was made available for free to all Tesla owners in April 2024. On that weekday evening, I was my driving my son northbound on I-395 after his basketball practice. The hot lanes were running southbound. Once again, as I approached the closed hot lane gates, the left indicator light came on as the car prepared to drive us through the closed gates and into the on-coming hot lane traffic. I am not a Tesla critic. I really like the car, but please address this issue ASAP before somebody gets killed.

I have had this car since May 2024 and the current mileage on the vehicle is 7,130 miles. In the past few weeks, I have had to completely replace both rear OEM aluminum alloy wheels due to cracks caused by normal road driving, which caused both rear tires to go flat. Per Tesla, this costly replacement is not covered under their warranty. I am now learning that this is a common issue with these particular wheels on this particular model (Model Y Performance). It appears the aluminum alloy wheels are simply not sturdy enough to bear the weight of the vehicle and the required tires simply do not provide sufficient cushioning to prevent wheel cracks resulting from normal wear and tear/road driving. Many customers have had to replace the OEM wheels with aftermarket wheels made of forged steel. These customers report zero issues with the much stronger wheels. This appears to be a product design or manufacturing problem, which is creating a significant consumer/public health risk.
